        Risk Factors and Characteristics of the Recurrence of Juvenile Nasopharyngeal Angiofibroma: A 22-Year Experience With 123 Cases at a Tertiary Center

        Objectives. Despite the efficacy of surgical treatments, the high rate of recurrence in juvenile nasopharyngeal angiofibroma (JNA) after surgery remains an unresolved problem. The present study comprehensively analyzed the risk factors and characteristics of JNA recurrence, providing clinical guidance for reducing recurrence. Methods. A total of 123 patients who underwent surgery for JNA between 1997 and 2019 at a single hospital were analyzed retrospectively. Univariate and multivariate analyses were used to assess the clinical risk factors for the recurrence of JNA. The relapse-free survival and annual cumulative recurrence rates were analyzed for subgroups defined according to clinical parameters. Results. After screening, 78 of the 123 patients were included in the present study. The main risk factors associated with JNA recurrence included the year of diagnosis, tumor size, sphenoid bone invasion, Radkowski stage, surgical approach, and intraoperative bleeding. Importantly, the surgical approach and sphenoid bone invasion were independent prognostic factors affecting recurrence. Patients who underwent endoscopic surgery without sphenoid bone invasion exhibited longer relapse-free survival. In the present study, the overall cumulative recurrence rate of JNA was 38.7%, and recurrence occurred mainly in the first year after the initial surgery. Conclusion. Endoscopic surgery achieved better relapse-free survival in JNA patients, and patients with sphenoid bone invasion should be carefully explored to avoid residual JNA. The recurrence rate of JNA differed among subgroups defined based on clinical parameters and was highest in the first year after surgery. Computed tomography or magnetic resonance imaging, along with close follow-up, should be performed strictly within 1 year after the primary operation.

        Meta-Analysis of the Efficacy and Safety of Mirabegron Add-On Therapy to Solifenacin for Overactive Bladder

        Purpose: We performed a meta-analysis to evaluate the efficacy and safety of mirabegron add-on therapy to solifenacin for patients with overactive bladder (OAB). Methods: We conducted a systematic literature review to identify all randomized, double-blind, controlled trials (RCTs) of this combination (mirabegron and solifenacin) for OAB. Embase, MEDLINE, and the Cochrane Central Register of Controlled Trials were searched. A manual search was also performed to investigate relevant references from the retrieved studies. Results: Four publications describing 5 RCTs that compared combination therapy with solifenacin, including a total of 3,309 patients, were analyzed. The mean number of micturitions per 24 hours (mean difference [MD], -0.45; 95% confidence interval [CI], -0.65 to -0.26; P<0.00001), number of episodes of incontinence per 24 hours (MD, -0.71; 95% CI, -0.14 to -0.02; P=0.04), volume voided per micturition, and number of urgency episodes per 24 hours demonstrated that combination therapy was more effective than solifenacin therapy alone. Safety assessments, including common treatment-emergent adverse events (odds ratio, 1.09; 95% CI, 0.95–1.27; P=0.23) and discontinuations due to adverse events (P=0.30), demonstrated that the combination therapy was well tolerated. Conclusions: This meta-analysis suggests that mirabegron therapy as an add-on to solifenacin provides a satisfactory therapeutic effect for OAB symptoms with a low occurrence of side effects.

      • Mechanical properties of coconut filter fiber

        In this study, mechanical properties of single coconut filter (CF) and coconut husk (CH) fiber were compared under the same chemical treatment conditions. The fibers were subjected to various surface treatments as follow: CF and CH fibers were treated using 6%w/v NaOH solution at 95℃ for 3 hours, respectively, and then they were washed with water to remove NaOH solution sticking to the fiber surface and dried in oven for 24 hours at 70℃. After that they were soaked in 0.5%, 1%,3% acrylic acid for 0.5, 1 and 3 hours and then washed with distilled water and dried in oven at 70℃ for 72 hours. The tensile test is conducted according to ASTM standard (C1557-03). Tensile strength, Young's modulus (E) and strain at break (ε<SUB>b</SUB>) were evaluated. Mechanical properties of CF fiber are more than twice of those of CH fiber except the strain at break (ε<SUB>b</SUB>) is lower than that of CH. NaOH treated fibers (CF and CH) have higher tensile strength, E and ε<SUB>b</SUB> than those of untreated fibers. Based on the immersion time, the maximum tensile strength and E of CF occur at 0.5h immersion for all acrylic acid concentration. They show a decrease tendency when the immersion time increased. While the maximum tensile strength and E of CH occur at 3h immersion for all acrylic acid concentration and the lowest tensile strength and E at 1h acrylic acid immersion but higher than untreated. The maximum tensile strength and E of both CF and CH occur at 0.5% acrylic acid for all immersion time and show a decrease tendency when the acrylic acid concentration increased. Surface morphologies of the fiber were observed using SEM method and FTIR for characterizing chemically modified fiber.
