        Risk of Diabetic Retinopathy between Sodium-Glucose Cotransporter-2 Inhibitors and Glucagon-Like Peptide-1 Receptor Agonists

        Background: To compare risk of diabetic retinopathy (DR) between patients taking sodium-glucose cotransporter-2 inhibitors (SGLT2is) and those taking glucagon-like peptide-1 receptor agonists (GLP1-RAs) in routine care.Methods: This retrospective cohort study emulating a target trial included patient data from the multi-institutional Chang Gung Research Database in Taiwan. Totally, 33,021 patients with type 2 diabetes mellitus using SGLT2is and GLP1-RAs between 2016 and 2019 were identified. 3,249 patients were excluded due to missing demographics, age <40 years, prior use of any study drug, a diagnosis of retinal disorders, a history of receiving vitreoretinal procedure, no baseline glycosylated hemoglobin, or no follow-up data. Baseline characteristics were balanced using inverse probability of treatment weighting with propensity scores. DR diagnoses and vitreoretinal interventions served as the primary outcomes. Occurrence of proliferative DR and DR receiving vitreoretinal interventions were regarded as vision-threatening DR.Results: There were 21,491 SGLT2i and 1,887 GLP1-RA users included for the analysis. Patients receiving SGLT2is and GLP-1 RAs exhibited comparable rate of any DR (subdistribution hazard ratio [SHR], 0.90; 95% confidence interval [CI], 0.79 to 1.03), whereas the rate of proliferative DR (SHR, 0.53; 95% CI, 0.42 to 0.68) was significantly lower in the SGLT2i group. Also, SGLT2i users showed significantly reduced risk of composite surgical outcome (SHR, 0.58; 95% CI, 0.48 to 0.70).Conclusion: Compared to those taking GLP1-RAs, patients receiving SGLT2is had a lower risk of proliferative DR and vitreoretinal interventions, although the rate of any DR was comparable between the SGLT2i and GLP1-RA groups. Thus, SGLT2is may be associated with a lower risk of vision-threatening DR but not DR development.

        Fabrication of alignment layer free flexible liquid crystal cells using thermal nanoimprint lithography

        In this article, thermal nanoimprint lithography (thermal-NIL) has been utilized to transfer the microgroove pattern onto the plastic substrate. Without coating alignment polymer, the microgrooves on the flexible substrates can align liquid crystals (LCs) directly. The flexible LC cell is shown to maintain comparable electro-optical properties while bending. The plastic film and the alignment layer integrated into an alignment substrate could effectively prevent cracks of the additional polymer alignment layer during the bending process. This method is applicable to the roll-to-roll process to increase the production efficiency.         A retrospective analysis of 20-year data of the surgical management of ulcerative colitis patients in Taiwan: a study of Taiwan Society of Inflammatory Bowel Disease

        Background/Aims: With the recent progress in medical treatment, surgery still plays a necessary and important role in treating ulcerative colitis (UC) patients. In this study, we analyzed the surgical results and outcomes of UC in Taiwan in the recent 20 years, via a multi-center study through the collaboration of Taiwan Society of IBD. Methods: A retrospective analysis of surgery data of UC patients from January 1, 1995, through December 31, 2014, in 6 Taiwan major medical centers was conducted. The patients’ demographic data, indications for surgery, and outcome details were recorded and analyzed. Results: The data of 87 UC patients who received surgical treatment were recorded. The median post-operative follow-up duration was 51.1 months and ranged from 0.4 to 300 months. The mean age at UC diagnosis was 45.3±16.0 years and that at operation was 48.5±15.2 years. The 3 leading indications for surgical intervention were uncontrolled bleeding (16.1%), perforation (13.8%), and intractability (12.6%). In total, 27.6% of surgeries were performed in an emergency setting. Total or subtotal colectomy with rectal preservation (41.4%) was the most common operation. There were 6 mortalities, all due to sepsis. Emergency operation and low pre-operative albumin level were significantly associated with poor survival (P =0.013 and 0.034, respectively). Conclusions: In the past 20 years, there was no significant change in the indications for surgery in UC patients. Emergency surgeries and low pre-operative albumin level were associated with poor survival. Therefore, an optimal timing of elective surgery for people with poorly controlled UC is paramount.

        Semi-active leverage-type isolation system considering minimum structural energy

        Semi-active isolation systems based on leverage-type stiffness control strategies have been widely studied. The main concept behind this type of system is to adjust the stiffness in the isolator to match the fundamental period of the isolated system by using a simple leverage mechanism. Although this system achieves high performance under far-field earthquakes, it is unsuitable for near-fault strong ground motion. To overcome this problem, this study considers the potential energy effect in the control law of the semi-active isolation system. The minimal energy weighting (MEW) between the potential energy and kinetic energy was first optimized through a series of numerical simulations. Two MEW algorithms, namely generic and near-fault MEW control, were then developed to efficiently reduce the structural displacement responses. To demonstrate the performance of the proposed method, a two-degree-of-freedom structure was employed as a benchmark. Numerical results indicate that the dynamic response of the structure can be effectively dampened by the proposed MEW control under both far-field and near-fault earthquakes, whereas the structural responses resulting from conventional control methods may be greater than those for the purely passive control method. Moreover, according to experimental verifications, both the generic and near-fault MEW control modes yielded promising results under impulse-like earthquakes. The practicability of the proposed control algorithm was verified.

        The simple and easy way to manufacture counter electrode for dye-sensitized solar cells

        We previously developed poly-N-vinyl-2-pyrrolidone (PVP)-capped Pt nanoclusters on ITO glass via a simple ‘‘2-step dip coating process” as counter electrode for DSSC. This new counter electrode was examined by transmission electron microscopy (TEM), inductively coupled plasma-atomic emission spectroscopy (ICP-AES), electrochemical impedance spectroscopy (EIS), cyclic voltammetry (CV), and current–voltage curve (I–V curve). The TEM results revealed that PVP-capped Pt nanoclusters’ size is about 3 nm, and the amount of Pt deposited on ITO glass is about 5 ㎍/㎠. Comparing with sputtered Pt and Solaronix thermal cluster Pt-catalyst T/SP, the PVP-capped Pt counter electrode has lower amount of Pt deposited on TCO glass,more positive potential of tri-iodide reduction, and better performance for the charge-transfer resistance (RCT) and the cell efficiency (g).

        Allicin Inhibits Glutamate Release from Rat Cerebral Cortex Nerve Terminals Through Suppressing Ca2+ Influx and Protein Kinase C Activity

        Evidence indicates that indirect inhibitory regulation of glutamatergic transmission, via reducing glutamate release, may induce neuroprotection. The present work was designed to examine whether allicin, a major component of garlic with neuroprotective effects, affected the release of glutamate evoked by 4-aminopyridine in rat cerebrocortical nerve terminals (synaptosomes). Allicin caused a potent inhibition on the release of glutamate evoked by 4-aminopyridine, and this inhibitory effect was abolished in the presence of Ca2+-free medium and vesicular transporter inhibitor. Allicin decreased the 4-aminopyridine-evoked elevation of intrasynaptosomal Ca2+ levels, but had no effect on the synaptosomal plasma membrane potential. The allicin-mediated inhibition of glutamate release was prevented by the N- and P/Q-type channel blocker and the protein kinase C (PKC) inhibitor, but was not affected by the intracellular Ca2+-release inhibitors, mitogen-activated protein kinase inhibitor, and protein kinase A inhibitor. Western blotting data also showed that allicin significantly reduced the phosphorylation of PKC. Together, these data indicate that in rat cerebrocortical nerve terminals, allicin depresses glutamate release and appears to decrease N- and P/Q-type Ca2+ channel and PKC activity.
