        Efficacy of Epigallocatechin-3-Gallate and Amla (Emblica officinalis) Extract for the Treatment of Diabetic-Uremic Patients

        Uremic patients with diabetes suffer from high levels of oxidative stress due to regular hemodialysis therapy (neutrophil activation induced by hemo-incompatibility between the hemodialyser and blood) and complications associated with diabetes. Several plasma biomarkers were screened in 13 uremic diabetic patients after receiving the mixture of (–)-epigallocatechin gallate (EGCG), a major component of green tea extract, and Amla extract (AE), from Emblica officinalis, the Indian gooseberry, for 3 months. We found that oral administration of a 1:1 mixture of EGCG and AE for 3 months significantly improved antioxidant defense as well as diabetic and atherogenic indices in uremic patients with diabetes. Furthermore, no significant changes in hepatic function, renal function, or inflammatory responses were observed. These results suggest that a 1:1 combination of EGCG and AE is a safe and effective treatment for uremic patients with diabetes.

        Multimorbidity Pattern and Risk for Mortality Among Patients With Dementia: A Nationwide Cohort Study Using Latent Class Analysis

        Objective Individuals with dementia are at a substantially elevated risk for mortality; however, few studies have examined multimorbidity patterns and determined the inter-relationship between these comorbidities in predicting mortality risk.Methods This is a prospective cohort study. Data from 6,556 patients who were diagnosed with dementia between 1997 and 2012 using the Taiwan National Health Insurance Research Database were analyzed. Latent class analysis was performed using 16 common chronic conditions to identify mortality risk among potentially different latent classes. Logistic regression was performed to determine the adjusted association of the determined latent classes with the 5-year mortality rate.Results With adjustment for age, a three-class model was identified, with 42.7% of participants classified as “low comorbidity class (cluster 1)”, 44.2% as “cardiometabolic multimorbidity class (cluster 2)”, and 13.1% as “FRINGED class (cluster 3, characterized by FRacture, Infection, NasoGastric feeding, and bleEDing over upper gastrointestinal tract).” The incidence of 5-year mortality was 17.6% in cluster 1, 26.7% in cluster 2, and 59.6% in cluster 3. Compared with cluster 1, the odds ratio for mortality was 9.828 (95% confidence interval [CI]=6.708–14.401; p<0.001) in cluster 2 and 1.582 (95% CI=1.281–1.953; p<0.001) in cluster 3.Conclusion Among patients with dementia, the risk for 5-year mortality was highest in the subpopulation characterized by fracture, urinary and pulmonary infection, upper gastrointestinal bleeding, and nasogastric intubation, rather than cancer or cardiometabolic comorbidities. These findings may improve decision-making and advance care planning for patients with dementia.

        An Optical-Based Method and System for the Web Thickness Measurement of Microdrills Considering Runout Compensation

        In this paper, an optical-based method and system for measuring web thickness of microdrills considering runout compensation is introduced. The presented measuring method is a nondestructive and noncontact means based on the use of an optical micrometer (OM) and a surface scanning laser confocal displacement meter (LCDM). The OM is used to determine the runout amount and the cross-sectional outer diameter of the microdrill, and the surface scanning LCDM is used to scan the cross-sectional profile of the microdrill to further determine its dual cross-sectional flute depths with runout-related deviation amounts being excluded. Then, the cross-sectional web thickness of the microdrill is calculated through the difference between the determined cross-sectional outer diameter and dual cross-sectional flute depths. An optical-based measuring system, for implementing the presented method, was constructed. Experiments meant to measure web thickness of microdrill samples were conducted. It showed that the presented measuring method, combined with the constructed measuring system, was feasible for the runout compensation of the web thickness measurement for certain microdrills.

        Cost Estimation Model Based on Building Information Modeling and Virtual Reality for Customizing Presold Homes

        Presold homes refer to houses that are sold before construction has begun or while the construction is still underway. To facilitate the selection and purchase of houses by clients, construction companies usually use sample houses, models, or videos to show the appearance of new houses being constructed. Clients have numerous opportunities to participate in the spatial planning and spatial allocation of the houses because these houses have not yet been constructed, and clients can direct the style and materials used for interior partition walls and furnishings. However, when discussing housing styles with their clients, construction companiesoften depend on their imagination to create three-dimensional images depicting the interior layout of houses. Construction companies can neither immediately present new styles when clients request changes to partition walls or styles nor estimate the costs of the changes. Consequently, clients cannot immediately decide on such changes, which then leads to poor communication between the two parties. Accordingly, this study designed a system that integrates building information modeling (BIM) and virtual reality (VR). Through this system, clients can change and immediately view house styles in immersive VR and decide on whether the changes meet their demands. Furthermore, the designed system is linked to a unit price database for materials. When clients make alterations in the system, it automatically calculates the costs of the changes and then helps the client determine whether the changes are within their budgets. The results of this study indicate that the designed system can accurately and rapidly achieve cost estimation. Furthermore, the proposed system ensures consistency between BIM and virtual environment information, thereby promoting the application of BIM in the stages of a building life cycle subsequent to building design.

        <P><B>Abstract</B></P> <P>In this paper, we consider the solitons to the fully nonlinear type Yamabe flows. We prove that any k -Yamabe soliton with positive k -curvature must have constant <SUB> σ k </SUB> -curvature on closed locally conformally flat manifolds. We also prove that any quotient Yamabe soliton with positive k and l curvatures must have constant <SUB> σ k </SUB> <SUB> σ l </SUB> -curvature on closed locally conformally flat manifolds. Similar results are also proved on compact locally conformally flat manifolds with totally geodesic boundary.</P>

        A combination method of the theory and experiment in determination of cutting force coefficients in ball-end mill processes

        In this paper, the cutting force calculation of ball-end mill processing was modeled mathematically. All derivations of cutting forces were directly based on the tangential, radial, and axial cutting force components. In the developed mathematical model of cutting forces, the relationship of average cutting force and the feed per flute was characterized as a linear function. The cutting force coefficient model was formulated by a function of average cutting force and other parameters such as cutter geometry, cutting conditions, and so on. An experimental method was proposed based on the stable milling condition to estimate the cutting force coefficients for ball-end mill. This method could be applied for each pair of tool and workpiece. The developed cutting force model has been successfully verified experimentally with very promising results.

        Strengthen by Artificial Intelligence (AI) and complexly integrated sensors, an autonomous mobile robot (AMR) is extensively applied in coping with various human resources tasks in indoor office environments. However, implementing an AMR system from scratch needs a strong Electric Engineer background due to the complexity of robot-controlling. Besides, Communication between robot-server, sensor, and client-service also increases the difficulty and time-cost in AMR developing. In this paper, the AMR system we proposed aims to be implemented by people without a EE background, and we will achieve this by employing Robot Operating System. Besides, the AMR system should work independently but still capable of responding to human requests. We will demonstrate a serverless could structure that includes the client-side service and integrate the edge-computing, which in charge of the immediacy-demanding job.

        <P>Abstract</P><P>Hepatocellular carcinoma (HCC) is a highly prevalent disease in many Asian countries, accounting for 80% of victims worldwide. Screening programs improve the detection of early HCC and have a positive impact on survival, but the majority of HCC patients in Asia still present with advanced stage disease. The treatment outcomes of HCC are affected by multiple variables, including liver function, performance status of the patient, and tumor stage. Therefore, it is not easy to apply a multidisciplinary therapeutic approach for optimal management. At present, limited numbers of HCC patients are eligible for curative therapies such as surgery or ablation in Asia. Therefore, most patients are eligible for only palliative treatments. For optimal management, the treatment choice is guided by staging systems and treatment guidelines. Numerous staging systems have been proposed and treatment guidelines vary by region. According to the Barcelona Clinic Liver Cancer (BCLC) guideline based on evidence from randomized clinical trials, only transarterial chemoembolization (TACE) is recommended for intermediate stage HCC and sorafenib for advanced stage HCC. However, treatment guidelines from Asian countries have adopted several other therapeutic modalities such as a surgical approach, hepatic arterial infusion chemotherapy, external radiation, and their combinations based on clinical experiences for intermediate and advanced stage HCC. Although TACE is the main therapeutic modality in the intermediate stage, overall therapeutic outcomes depend on the tumor size. In the advanced stage, the prognosis depends on the tumor status, e.g. major vessel invasion or extrahepatic spread. Thus, a new staging system representing prognoses suitable for Asian HCC patients and a corresponding optimal treatment algorithm should be further investigated using evidence-based data, which will finally bring about an Asian consensus for the management of intermediate and advanced stage HCC.</P><P>Copyright © 2011 S.
