      • Energy Recycling Voltage Scaler Based on Reconfigurable Capacitive Array

        This paper introduces a novel architecture of voltage scaler based on a reconfigurable capacitive array. While its reconfigurable structure allows it to scale the output voltage to any target level, its energy recycling process permits the voltage scaler to supply the voltage even after the input voltage is turned off. This architecture stores the energy in the capacitor array during the voltage scaling stage, then it reconfigures these charges step-wise to boost the lost voltage level during the energy recycling stage. Using the proposed architecture, an example voltage scaler was implemented using 65 nm CMOS process, which generates an output voltage of a range 500 mV to 1.16 V from an input voltage of 500 mV. Simulations were conducted with a 32-bit adder circuit as a load, which showed a reduction in energy consumption by 45.8% compared to a conventional converter.

        Evaluation of canalis basilaris medianus using cone-beam computed tomography

        The aim of this report is to present two cases of canalis basilaris medianus as identified on cone-beam computed tomography (CBCT) in the base of the skull. The CBCT data sets were sent for radiographic consultation. In both cases, multi-planar views revealed an osseous defect in the base of the skull in the clivus region, the sagittal view showed a unilateral, well-defined, non-corticated, track-like low-attenuation osseous defect in the clivus. The appearance of the defect was highly reminiscent of a fracture of the clivus. The borders of osseous defect were smooth, and no other radiographic signs suggestive of osteolytic destructive processes were noted. Based on the overall radiographic examination, a radiographic impression of canalis basilaris medianus was made. Canalis basilaris medianus is a rare anatomical variant and is generally observed on the clivus. Due to its potential association with meningitis, it should be recognized and reported to avoid potential complications.

        Hierarchical composite catalysts of MCM-41 on zeolite Beta for conversion of heavy reformate to xylenes

        Hierarchical composites of MCM-41 on zeolite Beta were synthesized by in-situ hydrothermal technique,characterized and their catalytic performance for converting heavy reformate into valuable xylenes wasevaluated. Systematic optimization of the hierarchical pore generation was carried out by varying thestrength of the alkaline solution (0.10 0.45 M NaOH) during disintegration of zeolite Beta. Thenanocomposites obtained were extruded with alumina binder and impregnated with 4.0 wt% Mo. Thecharacterization results indicate the transition of mesophase occurs from distinct disordered phase toordered/disordered and then to ordered mesophase with increasing alkaline concentration. The xyleneyield over the hierarchical composite zeolite was 1.4 times more than the parent zeolite Beta. Incorporation of a well-dispersed mild hydrogenation function (4 wt.% Mo) was advantageous inimproving the xylene yield and selectivity. The formulation prepared with moderately alkaline (0.2 MNaOH) solution exhibited optimum bimodal pore structure and improved dealkylation and transalkylationconversion resulting in a xylene yield of 33.0 wt.% and 1.7 times enhancement in xyleneselectivity over parent zeolite Beta. A 30 h stability test showed steady performance and xylene yields. Hence, a potential application of hierarchical zeolite Beta and MCM-41 composite catalysts for theconversion of heavy aromatics to produce xylenes has been successfully demonstrated.

        Frailty Status Is a More Robust Predictor Than Age of Spinal Tumor Surgery Outcomes: A NSQIP Analysis of 4,662 Patients

        Objective: The present study aimed to evaluate the effect of baseline frailty status (as measured by modified frailty index-5 [mFI-5]) versus age on postoperative outcomes of patients undergoing surgery for spinal tumors using data from a large national registry. Methods: The National Surgical Quality Improvement Program database was used to collect spinal tumor resection patients’ data from 2015 to 2019 (n = 4,662). Univariate and multivariate analyses for age and mFI-5 were performed for the following outcomes: 30-day mortality, major complications, unplanned reoperation, unplanned readmission, hospital length of stay (LOS), and discharge to a nonhome destination. Receiver operating characteristic (ROC) curve analysis was used to evaluate the discriminative performance of age versus mFI-5. Results: Both univariate and multivariate analyses demonstrated that mFI-5 was a more robust predictor of worse postoperative outcomes as compared to age. Furthermore, based on categorical analysis of frailty tiers, increasing frailty was significantly associated with increased risk of adverse outcomes. ‘Severely frail’ patients were found to have the highest risk, with odds ratio 16.4 (95% confidence interval [CI],11.21–35.44) for 30-day mortality, 3.02 (95% CI, 1.97–4.56) for major complications, and 2.94 (95% CI, 2.32–4.21) for LOS. In ROC curve analysis, mFI-5 score (area under the curve [AUC] = 0.743) achieved superior discrimination compared to age (AUC = 0.594) for mortality. Conclusion: Increasing frailty, as measured by mFI-5, is a more robust predictor as compared to age, for poor postoperative outcomes in spinal tumor surgery patients. The mFI-5 may be clinically used for preoperative risk stratification of spinal tumor patients.

        Low-Power Voltage Converter Using Energy Recycling Capacitor Array

        This paper presents a low-power voltage converter based on a reconfigurable capacitor array. Its energy recycling capacitor array stores the energy during a charge stage and supplies the voltage during an energy recycle stage even after the power source is disconnected. The converter reconfigures the capacitor array step-wise to boost the lost voltage level during the energy recycle stage. Its energy saving is particularly effective when most of the energy remaining in the charge capacitors is wasted by the leakage current during a longer sleep period. Simulations have been conducted using a voltage source of 500 mV to supply a $V_{DD}$ of around 800 mV to a load circuit consisting of four 32-bit adders in a 65-nm CMOS process. Results demonstrate energy recycling efficiency of 85.86% and overall energy saving of 40.14% compared to a conventional converter, when the load circuit is shortly active followed by a long sleep period.

        The diabetes is a disease that can become a serious disorder for a lifetime. It kills more than a million people every year. This disease can afect anyone. Diabetes occurs when the body is unable to process all the sugar (glucose) in the bloodstream; its complications can move to heart issues, caress, vision loss, and kidney stoppage and leg amputation problems. Many people with diabetes inject their body daily and feel that their work is done. Diabetes is an incurable disease because of poor health. The diabetes be able to be divided keen on2 types. The type-1 diabetes is hereditary. It is not easy to cure. People with type 2 diabetes can greatly reduce their risk of developing diabetes by following a proper, proper lifestyle. In addition it helps reduce the risk of diabetes. The proposed model of managing diabetics explains this disease as a specifc lifestyle. The existence of an efective system for the treatment of diabetes, according to the tasks currently set out, provides for the achievement of goals. The proposed edge based machine learning approach was achieved 85% of results compared with the Blood glucose level prediction, Adaptive multivariable closed-loop control, neural model of blood glucose level and Detecting Undiagnosed Diabetes.

        Risk Analysis Index and Its Recalibrated Version Predict Postoperative Outcomes Better Than 5-Factor Modified Frailty Index in Traumatic Spinal Injury

        Objective: To assess the discriminative ability of the Risk Analysis Index-administrative (RAI-A) and its recalibrated version (RAI-Rev), compared to the 5-factor modified frailty index (mFI-5), in predicting postoperative outcomes in patients undergoing surgical intervention for traumatic spine injuries (TSIs). Methods: The Current Procedural Terminology (CPT) and International Classification of Disease-9 (ICD-9) and ICD-10 codes were used to identify patients ≥ 18 years who underwent surgical intervention for TSI from National Surgical Quality Improvement Program (ACS-NSQIP) database 2015–2019 (n = 6,571). Multivariate analysis and receiver operating characteristic (ROC) curve analysis were conducted to evaluate the comparative discriminative ability of RAI-Rev, RAI-A, and mFI-5 for 30-day postoperative outcomes. Results: Multivariate regression analysis showed that with all 3 frailty scores, increasing frailty tiers resulted in worse postoperative outcomes, and patients identified as frail and severely frail using RAI-Rev and RAI-A had the highest odds of poor outcomes. In the ROC curve/ C-statistics analysis for prediction of 30-day mortality and morbidity, both RAI-Rev and RAI-A outperformed mFI-5, and for many outcomes, RAI-Rev showed better discriminative performance compared to RAI-A, including mortality (p = 0.0043, DeLong test), extended length of stay (p = 0.0042), readmission (p < 0.0001), reoperation (p = 0.0175), and nonhome discharge (p < 0.0001). Conclusion: Both RAI-Rev and RAI-A performed better than mFI-5, and RAI-Rev was superior to RAI-A in predicting postoperative mortality and morbidity in TSI patients. RAIbased frailty indices can be used in preoperative risk assessment of spinal trauma patients.

      • Simvastatin-loaded solid lipid nanoparticles for enhanced anti-hyperlipidemic activity in hyperlipidemia animal model

        <P><B>Abstract</B></P> <P>The objective of current study was to develop solid lipid nanoparticles-loaded with simvastatin (SIM-SLNs) and investigate their <I>in vivo</I> anti-hyperlipidemic activity in poloxamer-induced hyperlipidemia model. Nano-template engineering technique was used to prepare SIM-SLNs with palmityl alcohol as lipid core and a mixture of Tween 40/Span 40/Myrj 52 to stabilize the core. The prepared SIM-SLNs were evaluated for physicochemical parameters including particle diameter, surface charge, morphology, incorporation efficiency, thermal behaviour and crystallinity. <I>In vitro</I> release profile of SIM-SLNs in simulated gastric and intestinal fluids was evaluated by using dialysis bag technique and anti-hyperlipidemic activity was assessed in hyperlipidemia rat model. SIM-SLNs revealed uniform particle size with spherical morphology, zeta potential of −24.9 mV and high incorporation efficiency (∼85%). Thermal behaviour and crystallinity studies demonstrated successful incorporation of SIM in the lipid core and its conversion to amorphous form. SIM-SLNs demonstrated a sustained SIM release from the lipid core of nanoparticles. SIM-SLNs significantly reduced the elevated serum lipids as indicated by ∼3.9 and ∼1.5-times decreased total cholesterol compared to those of untreated control and SIM dispersion treated hyperlipidemic rats. In conclusion, SIM-SLNs showed a great promise for improving the therapeutic outcomes of SIM via its effective oral delivery.</P> <P><B>Graphical abstract</B></P> <P>[DISPLAY OMISSION]</P>

      • IL-1β Polymorphism and Expression Associated with Decreased Risk of Gastric Carcinoma: a Case Control Study in the Ethnic Kashmiri Population, India

        The aim of this research was to investigate the possible association between gastric carcinoma (GC) and polymorphisms of the IL-$1{\beta}$ gene in the Kashmiri population using peripheral blood DNA from 150 gastric carcinoma cases and 250 population controls with detailed data for clinicopathological characteristics of the disease. Two SNPs in the IL-$1{\beta}$ gene were selected for this study. Expression of IL-$1{\beta}$ was studied in 50 gastric carcinoma cases using immunohistochemistry and RT-PCR and then correlated with genotype. The frequency of the IL-$1{\beta}$-511 C allele was significantly higher in the GC case group (53.3%) than in controls (45.4%) with an odds ratio (OR) of 0.73 and a P value of 0.03. Multivariate regression analysis showed associations of gastric carcinoma with mutant form of IL-$1{\beta}$-511 TT (OR 0.309; P value <0.001) and the CC genotype of IL-$1{\beta}$-31 (OR 0.313; P value of 0.002). Haplotype analysis of IL-$1{\beta}$-31 and IL-$1{\beta}$-511 showed decreased association of IL-$1{\beta}$-31 T with IL-$1{\beta}$-511 C with gastric carcinoma (OR 0.728; P value 0.03). Expression study of 50 samples by immunohistochemistry (IHC) and RT-PCR showed association with grade III and stage III+IV. After correlating the expression with polymorphism no association was found.
