        Trends in knee surgery research in the official journal of the Korean Knee Society during the period 1999–2018: a bibliometric review

        Background: We applied bibliometric tools to original articles published in the official journal of the Korean Knee Society between 1999 and 2018 to identify their characteristics related to knee surgery and to examine the changes in research trends in the last 20 years. Methods: Over a 20-year period, 579 original articles were published in the journal Knee Surgery and Related Research (KSRR). We analyzed the title, keywords, and abstract of the article to analyze the research topics and assigned original articles to seven surgical categories as follows: total knee arthroplasty (TKA), unicompartmental knee arthroplasty (UKA), high tibial osteotomy (HTO), arthroscopy, surgery for cruciate ligament, revision surgery, and other surgery. To analyze the trends in research, we divided the study period into two equal parts of 10 years each, examined the percentage of articles in each decade, and analyzed topic trends using the growth rate. Results: Among the original articles, 86 on the topic of non-surgery were excluded, and 493 original articles related surgical research were included. Articles related to surgery accounted for 85.2% of the total original articles published annually. By period, this was 85.6% in the first half and 84.8% in the late half (p = 0.76). A total of 493 original articles related to surgery, with the largest number of TKA-related research at 52.1%. In the study period, the largest increase in the percentage of articles was on the topic of HTO surgery, by 149%. The topics of UKA and revision surgery increased by 95.3% and 33.9%. The topic of TKA increased by 5.9% and the topic of surgery for cruciate ligament decreased by 18.7%. The topic of arthroscopy showed the largest decrease, by 47.6%. Conclusions: The bibliometric findings of this study suggest that the majority of surgery-related original articles published in KSRR during the last 20 years involved research about TKA surgery, and the greatest relative increase over the study period involved research about HTO surgery. The authors expect that the analysis of characteristics and research trends of original articles published in KSRR will provide useful information about KSRR for future researchers.

        Information and communication technology(ICT) around the world in the 21st century presents a new vision of agriculture. Time, place, and the high-tech industry to overcome barriers to the fusion of the so-called “smart agriculture” is changing the landscape of agriculture. Precision Agriculture’s core container production for the mushroom cultivation temperature, humidity, irradiation, self-regulation, such as carbon dioxide, the optimal environment for mushroom cultivation was implemented. Auricularia auricula-judae, known as the Jew's ear, wood ear, jelly ear or by a number of other common names, is a species of edible Auriculariales fungus found worldwide. The fruiting body is distinguished by its noticeably ear-like shape and brown colouration. The fungus can be found throughout the year in temperate regions worldwide, where it grows upon both dead and living wood. We controlled different light source (Blue-Red-White combined LED, blue LED, red LED and fluorescent light) with different intensity of LED irradiation (1.5, 10.5 and 20.5 μmol/m2s for LEDs) to compare growth and development. Lights were treated with 12 hour on/ 12hour off cycle maintained in a controlled room with 19 ~ 21oC temperature, 85~95% humidity, and 1000ppm CO2 atmosphere concentration for 30 days. Growth and development differed from the LED color source and intensity of LED irradiation. Growth and development was most effective in 1.5 μmol/m2s irradiation for blue LED. All LED light sources showed less growth and development in highest intensity of irradiation, which indicates that higher than 20.5 μmol/m2s for LED is not effective. After harvesting fruit bodies, we measured their weight, length, width, thickness and chromaticity of fruit bodies. 1.5 μmol/m2s blue LED group was the best result of harvest with average individual weight (5.38g), length (65.37mm), width (56.87mm) and thickness (13.51mm) with fine chromaticity. This results show us that 1.5 μmol/m2s blue LED was the best effect on growth and development of Auricularia auricula-judae mushroom’s ICT system container type environment.

        황반원공과 망막앞막에서 25게이지 유리체수술과 관련된 의인성 망막열공

        목적: 황반원공과 특발성 망막앞막으로 25게이지 유리체수술을 시행한 환자에서 수술과 관련된 의인성 망막열공의 발생과 특징을 수술 기록과 수술 전 광각안저촬영 및 안저검사와 비교하여 알아보고자 한다. 대상과 방법: 2016년 1월부터 2019년 12월까지 본원에서 유리체절제술을 시행받은 황반원공 82안과 망막앞막 285안에 대해 후향적으로 의무기록을 분석하였다. 의무기록을 통해 수술 전 안저검사 및 광각안저촬영에서 관찰된 망막열공과 수술 중 관찰된 망막열공의 수와 위치를 비교하고, 망막원공, 망막격자변성 여부를 조사하였다. 후유리체박리 여부를 확인하여 각 인자와의 연관성을 알아보았다. 결과: 유리체절제술을 시행받은 367안 중 6안(1.6%)에서 의인성 망막열공이 발생하였다. 그 외 34안(9.3%)에서는 수술 전 망막열공이 확인되었다. 망막원공은 15안(4.1%), 망막격자변성은 32안(8.7%), 후유리체박리는 261안(71.1%)에서 있었으며, 의인성 망막열공과의 연관성은 확인할 수 없었다. 수술 전 망막열공 중 안저검사에서 발견된 것 외에 광각안저촬영에서 14개를 더 확인할 수 있었다. 결론: 25게이지 유리체절제술과 관련된 의인성 망막열공은 기존의 보고된 20게이지 유리체절제술보다 낮은 빈도로 발생하였고, 광각안저촬영으로 기존 연구 방식보다 더 정확하게 망막열공을 확인할 수 있었다. Purpose: To determine the incidence and characteristics of iatrogenic retinal breaks in patients who underwent 25-gauge vitreous surgery for macular hole and idiopathic epiretinal membrane, with a comparison and analyses of surgical records and preoperative data, including wide-angle fundus imaging and indirect ophthalmoscopic examination findings. Methods: The medical records were analyzed retrospectively for 82 macular hole eyes and 285 epiretinal membranes that underwent vitrectomy at our hospital from January 2016 to December 2019. The number and location of retinal breaks observed in preoperative fundus examination and wide-angle fundus imaging were compared with those indicated in the surgical records. The presence of retinal breaks and retinal lattice degeneration was investigated, and the relationship of each factor with respect to posterior vitreous detachment (PVD) was examined. Results: Of the 367 eyes that underwent vitrectomy, six eyes (1.6%) developed iatrogenic retinal breaks. Thirty-four eyes (9.3%) had retinal breaks before surgery. Retinal break was found in 15 (4.1%) eyes, retinal lattice degeneration was indicated in 32 eyes (8.7%), and PVD occurred in 261 eyes (71.1%). The association of iatrogenic retinal break and PVD was not confirmed. An additional 14 preoperative breaks, not seen in indirect ophthalmoscopic examination, were identified in wide-angle fundus images. Conclusions: It was found that 25-gauge vitrectomy-related iatrogenic retinal breaks occurred at a lower frequency than previously reported for 20-gauge vitrectomy procedures. Wide-angle fundus imaging was able to identify retinal breaks more accurately than conventional research methods.

        Preoperative vitamin D level does not affect the short-term functional outcome after total knee arthroplasty in elderly women

        Background: We aimed to evaluate the effect of vitamin D levels on the functional outcome of elderly women who underwent total knee arthroplasty (TKA). Methods: Seven hundred and four patients (1013 knees) who underwent primary TKA were included in our retrospective study. Preoperative vitamin D levels were measured and the relationship analyzed between these and age, weight, height, body mass index, and bone mineral density. Two hundred and twenty patients (220 knees) who received unilateral TKA and were followed up for more than 1 year after operation were divided into two groups: Group 1, serum 25-hydroxyvitamin D3 (25(OH)D3) level < 20 ng/ml; and Group 2, 25(OH)D3 level ≥ 20 ng/ ml. Both groups were evaluated for the relationship between vitamin D levels and postoperative Visual Analogue Scale (VAS) score, Knee Society Knee Score (KSKS), Knee Society Function Score (KSFS), and Western Ontario and McMaster Universities Arthritis Index (WOMAC) score. Results: The number of vitamin D-deficient patients (< 20 ng/ml of serum 25(OH)D3 level) was 556 (79.0%). In the correlation analysis, the vitamin D level was negatively correlated with weight only (p = 0.033). No significant differences were observed between the groups in terms of postoperative VAS score, KSKS, KSFS, and WOMAC score. Conclusions: Vitamin D deficiency was highly prevalent in patients who underwent TKA. Vitamin D levels negatively correlated with weight. Low vitamin D level was not a risk factor for unsatisfactory TKA outcome in elderly women.

        MDM2-dependent Sirt1 degradation is a prerequisite for Sirt6-mediated cell death in head and neck cancers

        Sirt6 is involved in multiple biological processes, including aging, metabolism, and tumor suppression. Sirt1, another member of the sirtuin family, functionally overlaps with Sirt6, but its role in tumorigenesis is controversial. In this study, we focused on cell death in association with Sirt6/Sirt1 and reactive oxygen species (ROS) in head and neck squamous cell carcinomas (HNSCCs). Sirt6 induced cell death, as widely reported, but Sirt1 contributed to cell death only when it was suppressed by Sirt6 via regulation of MDM2. Sirt6 and Sirt6-mediated suppression of Sirt1 upregulated ROS, which further led to HNSCC cell death. These results provide insight into the molecular roles of Sirt6 and Sirt1 in tumorigenesis and could therefore contribute to the development of novel strategies to treat HNSCC.

        Endothelial dysfunction induces atherosclerosis: increased aggrecan expression promotes apoptosis in vascular smooth muscle cells

        Endothelial dysfunction-induced lipid retention is an early feature of atherosclerotic lesion formation. Apoptosis of vascular smooth muscle cells (VSMCs) is one of the major modulating factors of atherogenesis, which accelerates atherosclerosis progression by causing plaque destabilization and rupture. However, the mechanism underlying VSMC apoptosis mediated by endothelial dysfunction in relation to atherosclerosis remains elusive. In this study, we reveal differential expression of several genes related to lipid retention and apoptosis, in conjunction with atherosclerosis, by utilizing a genetic mouse model of endothelial nitric oxide synthase (eNOS) deficiency manifesting endothelial dysfunction. Moreover, eNOS deficiency led to the enhanced susceptibility against pro-apoptotic insult in VSMCs. In particular, the expression of aggrecan, a major proteoglycan, was elevated in aortic tissue of eNOS deficient mice compared to wild type mice, and administration of aggrecan induced apoptosis in VSMCs. This suggests that eNOS deficiency may elevate aggrecan expression, which promotes apoptosis in VSMC, thereby contributing to atherosclerosis progression. These results may facilitate the development of novel approaches for improving the diagnosis or treatment of atherosclerosis. [BMB Reports 2019; 52(2): 145-150]

      • 소아에서 역기하렌즈 착용의 장기 임상 결과

        Purpose: This study was designed to investigate the effectiveness and safety of reverse geometric lens (Ortho-K LK lens) in children wearing over 12 months. Methods: Fifty eight eyes of 29 Children were enrolled and studied from March 2003 to April 2004. Uncorrected visual acuity, spherical equivalent, corneal thickness, astigmatism, anterior chamber depth and corneal endothelial cell count were measured before fitting , and 1,2,4,12,25,50 weaks after lens fitting. Results: Uncorrected visual acuity was increased to log MAR 0.03 at I week and sustained. Manifested refractive error was decreased during this period. There was not any significant change in corneal thickness, corneal endothelial cell count and anterior chamber depth. Conclusion: Reverse geometric lenses were found to be effective to correct visual acuity and safe over long period in children.

        급격히 진행된 비전형 급성 대상외망막염

        목적: 건강한 젊은 여성에서 나타난 급격한 진행을 보이는 단안 비전형 급성 대상외망막염을 보고하고자 한다. 증례요약: 특이 병력이 없는 32세 여자가 일주일 전 시작된 좌안의 시력저하로 내원하였다. 좌안 시력은 0.4였고 뇌자기공명영상, 혈액검사에서 특이 소견은 없었다. 안저검사에서 좌안 시신경유두 주변으로 고리 형태의 망막혼탁이 관찰되고, 빛간섭단층촬영에서 해당 부위의 외망막층의 손상, 시야검사에서 해당 부위의 시야 소실이 관찰되었다. 형광안저혈관조영 및 인도사이아닌그린 혈관조영을 시행한 후 소실다발흰점증후군으로 추정하고 프레드니솔론 복용을 시작하였다. 10일 후 시력이 안전 수동으로 감소하였고 고리 형태의 망막혼탁이 주변부로 확장된 소견이 관찰되었다. 3일간 고용량 스테로이드 정맥주사를 시행하였으나 호전이 없었고, 전방수를 이용한 중합효소연쇄반응에서 바이러스는 검출되지 않았고 항바이러스제에도 반응하지 않았다. 결론: 시신경유두 주변에서 시작된 외망막염이 빠른 속도로 진행하였고 시력이 소실되었다. 후극부에서 시작된 외망막염이 주변부 망막까지 급격히 진행하였다는 점에서 기존 보고들과 차이가 있다. 비전형 급성 대상외망막염으로 진단하고 이를 보고하고자 한다. Purpose: To report a case of unilateral progressive atypical acute zonal outer retinitis in a healthy young female. Case summary: A 32-year-old healthy woman presented with visual disturbance in the left eye for 1 week. Best corrected visual acuity was 0.4 in the left eye. No abnormal findings were detected on brain magnetic resonance imaging and a laboratory work-up. Zonal retinal opacity around the optic nerve disc was noticed in a fundus photo which was consistent with the outer retinal signal defect on optical coherence tomography and a defect in the central visual field from perimetry. Multiple evanescent white dot syndrome was suspected based on fluorescein angiography and indocyanine green angiography. Prednisolone was prescribed. Visual acuity was reduced to hand-motion after 10 days with extended zonal retinal opacity. No virus was detected by multiplex polymerase chain reaction of the aqueous humor. The lesion did not improve despite high-dose intravenous steroid and antiviral treatment. Conclusions: Visual function was lost with the progression of outer retinitis. This case differed from previous cases as the outer retinitis progressed rapidly from the posterior pole to the peripheral retina. It was named fulminant progressive atypical acute zonal outer retinitis.

        고도근시안의 황반원공 망막박리에서 내경계막판술 후 잔존한 망막하액 1예

        목적: 고도근시안의 황반원공 망막박리에서 내경계막판술 후 장기간 망막하액이 잔존하면서 불량한 시력 결과를 보이는 증례를 보고하고자 한다. 증례요약: 55세 남자가 한 달 전부터 발생한 우안 중심부 시야장애를 주소로 내원하였다. 우안 최대교정시력 0.32, 안축장 32.57 mm 였다. 안저검사에서 황반원공과 함께 후극부에 국한된 망막박리를 확인하였다. 유리체절제술, 망막앞막제거술, 내경계막제거술 및 뒤집힌 내경계막판술을 시행하고 유리체강을 실리콘기름으로 충전하였다. 수술 1일째 황반원공은 폐쇄되었으나 망막하액은 이후 14개월간 지속되었다. 수술 3개월째 시력은 0.16이었고, 실리콘기름을 제거하였다. 첫 수술 14개월 후 망막하액이 흡수되었고, 황반부에표범무늬변성이 뚜렷해지고, 타원체구역의 소실이 관찰되었고, 시력은 0.1로 감소하였다. 결론: 고도근시안의 황반원공 망막박리에서 뒤집힌 내경계막판술은 원공폐쇄율을 높이나 망막하액의 배출을 방해할 수 있다. 오래지속된 망막하액은 불량한 시력예후를 야기하는 망막의 영구적 손상과 연관되어 있을 수 있음을 고려하여 수술 방법을 결정하여야한다. Purpose: To report a case of poor visual prognosis complicated by residual subretinal fluid after use of the internal limiting membrane flap technique to treat macular hole retinal detachment in a patient with high myopia. Case summary: A 55-year-old male stated that he had experienced a transparent circle in the central visual field of the right eye for 1 month. His best-corrected visual acuity (BCVA) was 0.32 and the axial length of the right eye was 32.57 mm. Fundus examination revealed a macular hole with retinal detachment localized to the posterior pole. We performed vitrectomy, membrane peeling, internal limiting membrane peeling with inverted internal limiting membrane flap, and silicone oil injection. On day 1 after surgery, the macular hole was closed, but subretinal fluid was noticed in the macula. At 3 months after surgery, the BCVA was 0.16 and the silicone oil was removed. At 14 months after the first surgery, the subretinal fluid was completely absorbed, but leopard-pattern pigment degeneration became prominent and the macula exhibited ellipsoid zone disruption. The BCVA decreased to 0.1. Conclusions: In patients exhibiting macular hole retinal detachment in the context of high myopia, an inverted internal limiting membrane flap may increase the macular hole closure rate but disturb subretinal fluid drainage. As persistent subretinal fluid may cause permanent retinal damage with a poor visual prognosis, the surgical method must be carefully chosen.
