        Clinical and Histologic Features of Patients with Biopsy-Proven Metabolic Dysfunction-Associated Fatty Liver Disease

        Background/Aims: Fatty liver disease is defined as a cluster of diseases with heterogeneous etiologies, and its definition continues to evolve. The novel conceptional criteria for metabolic dysfunction-associated fatty liver disease (MAFLD) were proposed in 2020 to avoid the exclusion of a certain subpopulation, but their evaluations have been limited. We aimed to examine and compare the clinical as well as histologic features of MAFLD versus nonalcoholic fatty liver disease (NAFLD) in patients with biopsy-proven hepatic steatosis. Methods: From January 2009 to December 2019, 175 patients with histology-proven hepatic steatosis and 10 with cryptogenic cirrhosis who were treated at National Taiwan University Hospital, Taipei, Taiwan, were enrolled. Patients were classified into different groups according to the diagnostic criteria of MAFLD and NAFLD. The clinical and histologic features were then analyzed and compared. Results: In total, 76 patients (41.1%) were diagnosed with both MAFLD and NAFLD, 81 patients (43.8%) were diagnosed with MAFLD alone, nine patients (4.9%) were diagnosed with NAFLD alone, and 19 patients (10.3%) were diagnosed with neither. Those with MAFLD alone exhibited a higher degree of disease severity regarding histology and laboratory data than those with NAFLD alone. Advanced fibrosis was associated with the presences of hepatitis B virus infection and metabolic diseases. Conclusions: The novel diagnostic criteria for MAFLD include an additional 38.9% of patients with hepatic steatosis and can better help identify those with a high degree of disease severity for early intervention than can the previous NAFLD criteria.         This paper proposes a new method that utilized Android mobile phone to control the RGV (Rail Guided Vehicle)running through WIFI network to overcome the disadvantages of the traditional remote control can only control one-way communication for RGV in the Logistics and warehousing and realize two-way communication between the RGV and remote controller . The system is based on Android platform, and established a remote terminal system by all Android support resources. In this paper, it described the structure, function, software flow of the RGV remote terminal system and communication protocol between Android client and the RGV. The design is based on the Modbus TCP / IP protocol, to achieve a two-way communication between Android phone and the RGV.

        In this paper, we introduce an iterative scheme for finding a common element of the set of fixed points of a nonexpansive mapping, the set of solutions of the variational inequality for an inverse-strongly monotone mapping and the set of solutions of an equilibrium problem in a Hilbert space. We show that the iterative sequence converges strongly to a common element of the three sets. The results of this paper extend and improve the corresponding results announced by many others.

        Intracellular Polysaccharide and its Antioxidant Activity by Pleurotus citrinopileatus SM-01

        The extraction parameters of intracellular polysaccharide (IPS) from Pleurotus citrinopileatus SM-01mycelia were optimized, and the in vitro and in vivo antioxidant activities of IPS were investigated. The optimum conditions of IPS extraction were predicted to be an ultrasonic treatment time of 664.09 s, precipitation time of 23.03h and pH 7.36, and IPS yield was estimated at 16.13%. The in vitro inhibition effects of IPS at a dosage of 5 g/L on the superoxide anion, 1,1-diphenyl-2-picrylhydrazyl (DPPH) and hydroxyl radicals were 73.96±4.62%, 69.2±4.37%,and 50.75±4.39%, respectively, which were 72.56±5.08%, 22.83±1.94%, and 43.93±3.26% higher than that of butylated hydroxytoluene (BHT), respectively. The reducing power of IPS was 0.9±0.07, 69.81±5.24% higher than that of BHT. The activities of glutathione peroxidase (GSH-Px), superoxide dismutase (SOD), catalase (CAT) and alanine aminotransferase (ALT) in mice blood were 241.38±23.19, 454.95±42.39, 60.32±5.16, and 32.39±2.54 U/mL,respectively, and the malonaldehyde (MDA) level was 9.54±0.72 nmol/mL. The results provided a reference for the large-scale extraction of IPS by P. citrinopileatus SM-01 in industrial fermentation, suggesting that the IPS can be used as a potential antioxidant, which enhances adaptive immune responses.

      • The effect of Zn(2+) on Pelodiscus sinensis creatine kinase: unfolding and aggregation studies.

        <P>We studied the effects of Zn(2+) on creatine kinase from the Chinese soft-shelled turtle, Pelodiscus sinensis (PSCK). Zn(2+) inactivated the activity of PSCK (IC(50)?=?.079??.004?mM) following first-order kinetics consistent with multiple phases. The spectrofluorimetry results showed that Zn(2+) induced significant tertiary structural changes of PSCK with exposure to hydrophobic surfaces and that Zn(2+) directly induced PSCK aggregation. The addition of osmolytes such as glycine, proline, and liquaemin successfully blocked PSCK aggregation, recovering the conformation and activity of PSCK. We measured the ORF gene sequence of PSCK by rapid amplification of cDNA end and simulated the 3D structure of PSCK. The results of molecular dynamics simulations showed that eight Zn(2+) bind to PSCK and one Zn(2+) is predicted to bind in a plausible active site of creatine and ATP. The interaction of Zn(2+) with the active site could mostly block the activity of PSCK. Our study provides important insight into the action of Zn(2+) on PSCK as well as more insights into the PSCK folding and ligand-binding mechanisms, which could provide important insight into the metabolic enzymes of P. sinensis.</P>

        MiR-506 Promotes Natural Killer Cell Cytotoxicity against Human Hepatocellular Carcinoma Cells by Targeting STAT3

        Purpose: It is well documented that natural killer (NK) cytotoxicity against hepatocellular carcinoma (HCC) cells is impaired inHCC, which might account for the failure of anti-tumor immune response. miRNAs are considered as important regulators forthe development and functions of NK cells. However, the entire role of miR-506 in NK cells remains far from being addressed. Materials and Methods: The expressions of miR-506 and signal transducer and activator of transcription 3 (STAT3) mRNA in primaryNK cells from HCC patients and healthy controls were detected by quantitative real-time PCR. NK cell cytotoxicity was assessedby CFSE/7AAD cytotoxicity assay and lactate dehydrogenase assay. Luciferase reporter assay, RNA immunoprecipitationassay, and western blot were conducted to confirm the interaction between miR-506 and STAT3. Results: miR-506 expression was downregulated and STAT3 mRNA was upregulated in primary NK cells from HCC patients. PrimaryNK cells from HCC patients showed remarkably reduced cytotoxicity against SMMC7721 or HepG2 cells. NK cell cytotoxicitywas positively correlated with miR-506 expression and negatively correlated with STAT3 mRNA expression. Additionally, miR-506 overexpression enhanced NK cell cytotoxicity against HCC cells, while miR-506 inhibitor showed the reverse effect. Moreover,miR-506 could suppress STAT3 expression by directly targeting 3'-untranslated regions of STAT3. A negative correlation betweenmiR-506 and STAT3 mRNA expression in HCC patients was observed. Mechanistically, overexpressing STAT3 greatly reversedmiR-506-mediated promotion of NK cell cytotoxicity against HCC cells. Conclusion: miR-506 enhanced NK cell cytotoxicity against HCC cells by targeting STAT3, suggesting that modulating miR-506expression maybe a promising approach for enhancing NK cell-based antitumor therapies.

        Identification of Autoantigens in Pediatric Gastric Juices

        Purpose: This study aimed to investigate the presence of autoantigens in the gastric juices of children. Methods: Gastric juice and serum samples were obtained from 53 children <15 years of age who underwent gastric endoscopy. Among these, 8, 22, and 23 participants were in the age groups 0–5, 6–10, and 11–15 years, respectively. These samples were analyzed using two-dimensional electrophoresis (2-DE), immunoblot analysis, and matrix-assisted laser desorption ionization-time of-flight mass spectrometry. Furthermore, we reviewed the histopathological findings and urease test results and compared them with the results of 2-DE and immunoblot analysis. Results: There were no statistically significant differences in urease test positivity, grades of chronic gastritis, active gastritis, or Helicobacter pylori infiltration of the antrum and body among the three age groups. Three distinct patterns of gastric juice were observed on 2-DE. Pattern I was the most common, and pattern III was not observed below the age of 5 years. Histopathological findings were significantly different among active gastritis (p=0.037) and H. pylori infiltration (p=0.060) in the gastric body. The immunoblots showed large spots at an approximate pH of 3–4 and molecular weights of 31–45 kDa. These distinct, large positive spots were identified as gastric lipase and pepsin A and C. Conclusion: Three enzymes, which are normally secreted under acidic conditions were identified as autoantigens. Further investigation of the pathophysiology and function of autoantigens in the stomach is required.

        『東醫壽世保元』 少陰人 表病의 益氣 및 升陽 治法에 대한 考察

        『Dong-uisusebowon(『東醫壽世保元』)』, written by Ijema(李濟馬), is about the treatment and prevention of diseases according to the four constitutions, Soeum, Soyang, Taeeum and Taeyang. He presents a precise pathogenic mechanism along with specific treatment methods of the Ulgwang(鬱狂) Syndrome which are Seungbo(升補) and Ikgiseung-yang[益氣而升陽] in comparing Mang-yang(亡陽) and Ulgwang Syndromes. However, in the case of the Mang-yang Syndrome, he merely presents formulas without mentioning details of the treatment methods. In this study, the formulas of the Mang-yang Syndrome and Ulgwang Syndrome were thoroughly compared and analyzed, leading to the conclusion that the concept of the Ikgi(益氣) method matches that of the Bojung-ikgitang(補中益氣湯) of Idongwon(李東垣), consisted of ingredients such as Ginseng, Radix Astragali, Radix Angelicae Gigantis, Rhizoma Atractylodis Macrocephalae etc,, and the concept of Seung-yang matches that of Gyejitang(桂枝湯). Furthermore, we have examined the addition of Radix Aconti Lateralis Preparata[附子] to the Ikgiseung-yang[益氣而升陽] method to be aimed at restoring the Yanggi(陽氣). Lastly, through comparison of formulas according to the progress of the Mang-yang Syndrome and Ulgwang Syndrome, we have presented a more detailed explanation of the concept of each treatment methods: the Mang-yang Syndrome focuses more on Seung-yang(升陽), Ulgwang leans more towards Ikgi(益氣).
