      • 동종 골수이식 후 만성 이식편대숙주질환과 동반되어 신증후군으로 발현된 IgA 신병증 1예

        동종 골수이식 후 발생하는 신기능부전은 원인이 다양하여 감별에 어려움이 있다. 만성 GVHD는 흉선의 기능저하에 따른 면역관용의 실패로 발생되는 자가면역현상에 의해 발생되는 것으로 알려져 있으며 드물게 신장을 침범하여 신증후군을 일으키기도 한다. CaA의 용량을 줄이거나 투약중지 후 발생한 만성 GVHD에 동반된 신증후군의 경우 조기에 신조직검사를 시행하여 감별진단하고 CsA의 용량을 증량하거나 재투약하여 좋은 치료 결과를 보일 것으로 기대된다. 저자들은 문헌고찰을 통해 동종 골수이식 후 발생된 신증후군이 유사한 임상양상을 보이며 발생기전에 만성 GVHD와 자가면역현상이 연관되어 있음을 확인하였으며 동종 골수이식 후 신증후군의 임상양상을 보이는 환자에서 병리조직학적으로 lgA 신병증을 진단하고 이러한 lgA 신병증이 발생기전에 만성 GVHD와 자가 면역현상이 연관되어 있을 가능성을 최초로 확인하였으며 CsA를 이용하여 성공적으로 치료한 경험을 하였기에 보고하는 바이다. Renal insufficiency is occasionally encountered in allogeneic bone marrow transplantation and its cause is difficult to ascertain. Chronic graft-versus-host disease (GVHD) related to thymic dysfunction is immune-mediated and involves autoreactivity of T-lymphocytes derived from donor marrow to recipient's major histocompatibility complex(MHC) minor antigens. The clinical mainifestations of chronic GVHD are similar to those of autoimmune disease but kidney involvement is rare. Few cases of nephrotic syndrome with membranous nephropathy or minimal change nephrotic syndrome have been reported to be associated with chronic GVHD in allogeneic bone marrow transplant recipient especially after cyclosporine A(CsA) withdrawal, and these cases have responded well to CsA. Therefore, it is prudent to differentiate the cause of post-transplant renal insufficiency using renal biopsy and to start CsA in a case of nephrotic syndrome associated with chronic GVHD as early as possible. We report a case who had a massive proteinuria during the post-allogeneic marrow transplantation period. The cause of nephrotic syndrome was diagnosed as IgA nephropathy by renal biopsy. The patient was successfully treated with reintroduction of CsA.

        Suppression of Leakage Currents in Photo-multiplication Photodetectors with Oxidation controlled Metal Interfacial Layer

        In this work, we investigate the use of annealed metal oxides in electron blocking layer (EBL) of photo-multiplication organicphotodetectors (PM-OPD), to obtain low leakage current, high external quantum effi ciency, and thereby, high detectivity. Reliable and high-performance PM-OPD are obtained using multi-interfacial EBLs composed of metals such as Ni, Al, Agand Au, and their oxides, through a controlled annealing. The interfacial layer between the front electrode and absorptionlayer is metal–metal oxide layer with wide bandgap, for improved hole mobility and stable electron blocking under the drivingvoltages of the PM-OPD. Through the optimization of the structural stability of the PM-OPD, a high specifi c detectivityof 2.59 × 10 13 Jones is obtained under a low reverse voltage. The resulting photodetectors exhibit improved stability underrelatively low reverse voltages because of the Schottky barrier induced between the donor and acceptor, electrode components,and passivation eff ect of the free-electrons driven by the introduced multi-interfacial layers.

      • Wireless charing pillow for a fully implantable hearing aid: Design of a circular array coil based on finite element analysis for reducing magnetic weak zones.

        <P>Many types of fully implantable hearing aids have been developed. Most of these devices are implanted behind the ear. To maintain the implanted device for a long period of time, a rechargeable battery and wireless power transmission are used. Because inductive coupling is the most renowned method for wireless power transmission, many types of fully implantable hearing aids are transcutaneously powered using inductively coupled coils. Some patients with an implantable hearing aid require a method for conveniently charging their hearing aid while they are resting or sleeping. To address this need, a wireless charging pillow has been developed that employs a circular array coil as one of its primary parts. In this device, all primary coils are simultaneously driven to maintain an effective charging area regardless of head motion. In this case, however, there may be a magnetic weak zone that cannot be charged at the specific secondary coil's location on the array coil. In this study, assuming that a maximum charging distance is 4 cm, a circular array coil-serving as a primary part of the charging pillow-was designed using finite element analysis. Based on experimental results, the proposed device can charge an implantable hearing aid without a magnetic weak zone within 4 cm of the perpendicular distance between the primary and secondary coils.</P>

        20-O-β-D-glucopyranosyl-20(S)-protopanaxadiol, a metabolite of ginsenoside Rb1, enhances the production of hyaluronic acid through the activation of ERK and Akt mediated by Src tyrosin kinase in human keratinocytes.

        <P>The aim of the present study was to determine the mechanisms through which 20-O-beta-D-glucopyranosy120(S)-protopanaxadiol (20GPPD) promotes the production of hyaluronic acid (HA) in human keratinocytes. 20GPPD is the primary bioactive metabolite of Rb1, a major ginsenoside found in ginseng (Panax ginseng). We sought to elucidate the underlying mechanisms behind the 20GPPD-induced production of HA. We found that 20GPPD induced an increase in HA production by elevating hyaluronan synthase 2 (HAS2) expression in human keratinocytes. The phosphorylation of extracellular signal-regulated kinase (ERK) and Akt was also enhanced by 20GPPD in a dose-dependent manner. The pharmacological inhibition of ERK (using U0126) or Akt (using LY294002) suppressed the 20GPPD-induced expression of HAS2, whereas treatment with an epidermal growth factor receptor (EGFR) inhibitor (AG1478) or an intracellular Ca2+ chelator (BAPTA/AM) did not exert any observable effects. The increased Src phosphorylation was also confirmed following treatment with 20GPPD in the human keratinocytes. Following pre-treatment with the Src inhibitor, PP2, both HA production and HAS2 expression were attenuated. Furthermore, the 20GPPD-enhanced ERK and Akt signaling decreased following treatment with PP2. Taken together, our results suggest that Src kinase plays a critical role in the 20GPPD-induced production of HA by acting as an upstream modulator of ERK and Akt activity in human keratinocytes.</P>

      • Rice heavy metal induced gene (OsHMI1) harboring RING-H2 domain: Its expression pattern, E3 ligase activity, subcellular localization, and the interaction partners.

        Plants are known to have homeostatic cellular mechanisms to control the concentration of heavy metal inside the cell. We tried to retrieve rice RING finger protein genes, which are believed to regulate substrates via ubiqitinations, related to metal ions detoxification mechanisms. A total of 48 rice RING finger proteins were randomly selected and then examined for their expression patterns as exposed to cadmium and arsenic treatments. We discovered a RING finger protein gene that was significant up-regulated against both treatments and then named Oryza sativa heavy metal induced 1 (OsHMI1). We tested subsequently OsHMI1 expression patterns against to salinity, dehydration, cold, heat stress and phytohormones treatments. In addition, we evaluated its subcellular localization and determined E3 ligase activity. The interaction partner proteins were screened via yeast-two hybridization. These results might shed further light on the understanding of homeostatic cellular mechanisms to control heavy metal detoxification via protein degradation in plants.

        한국인의 아침 결식 빈도와 영양 보충제 섭취의 연관성

        Objectives: Although many studies indicate health effect of breakfast, little is known whether taking breakfast regularly has effect on intake of nutritional supplements. Therefore, this study was performed to examine the relationship between breakfast absence frequency and intake of nutritional supplement in Koreans. Methods: The analysis used the data from The Korea National Health and Nutrition Survey (2018). Main independent variable is breakfast frequency and other valid variables are sex, residence, age, education level, economic status, current smoking status, drinking frequency, body mass index, numbers of time to eat out, exercising habits, underlying conditions, subjective health status and job status. Dependent variable is intake of nutritional supplements. Breakfast frequency variable is divided in to four groups, which are none, 1-2 days, 3-4 days, and 5-7 days per week. Intake of nutritional supplements variable is divided in to two group, which are none taking and taking the supplements. Data were analyzed using PASW Statistics 18 and chi-square and multiple logistic regression analysis. Results: Regular breakfast eating group (5-7 days per week) took nutritional supplements 1.563 times more than non eating group (p=0.002). This study interestingly suggests that breakfast frequency and intake of nutritional supplements had positive correlation. Conclusions: Having breakfast regularly leads to taking nutritional supplements, and will eventually lead to health promotion. Thus, educating proper dietary habit is needed for encouraging to have breakfast regularly.

      • 초등학교 글쓰기 수업의 문제점과 개선 방안

        이 글은 초등학교 글쓰기 수업의 양상을 살펴 문제를 찾고 그 해결 방법을 탐색하는 데 목적을 두었다. 초등학교에서 글쓰기 교육은 ‘교과 수업의 쓰기’ 시간에 이루어지는 경우와 ‘비교과 수업의 쓰기’ 시간에 이루어지는 경우로 구분되는데 이 두 상황에서 글쓰기를 지도하는 방법은 확연하게 구분된다. 쓰기 교과 수업 시간의 글쓰기 지도는 ‘일반 수업, 계획된 연구 수업’이 있는데, 주로 쓰기 교과서를 중심으로 과정 중심 모형에 따라 이뤄진다. 과정 중심의 쓰기 교육은 ‘내용 생성 → 내용 조직 → 글쓰기 → 다듬기’라는 일련의 과정으로 글쓰기를 가르치는데 두 가지 논의점이 나타난다. 첫째, 과정 적용의 문제로, 쓰기 과정을 적용할 때 한 시간에 전 과정을 고정하여 놓고 학년, 장르, 수업 목표에 관계없이 연속적으로 똑 같이 수업을 하는 점이다. 둘째는 전략 적용의 문제로, 쓰기의 어느 과정에 최적의 전략 고정하여 교육하려고 하며 그 전략도 사고 과정에 편중해 있다는 점이다. 비교과 수업의 쓰기는 ‘재량 활동 수업, 특기 적성 방과 후 학교 수업, 특별 활동 수업, 홈페이지를 통한 글쓰기 지도’ 등이 있는데 주로 교사개인이나 교과교육연구회에서 개발한 프로그램을 활용한다. 여기에는 ‘생활 소재를 강조하는 활동형 프로그램, 기능이나 전략을 강조하는 전략형 프로그램, 주제 통합을 강조하는 범교과형 프로그램, 다양한 글쓰기를 강조하는 문종형 프로그램’ 등이 있다. 이 프로그램들을 활용할 때 다음 세 가지의 논의점이 나타난다. 첫째는 이해력과 사고력 신장에 대한 내용이 부족하다는 점이고, 둘째는 글을 쓰는 목적과 상황에 대한 정보를 제공하지 않는다는 점이며, 셋째는 주제 통합에서 배경 지식을 확대할 방법이 구체적이지 못하다는 점이다. This study is designed to take a look at the aspects of wring class at elementary schools, to find out its problems and to grope for reform measures. Writing education at elementary schools are divided into the sessions of subject-based writing and those of other writing;one is cleary different from the other in terms of the teacher`s instructions. The writing instructions of subject sessions involve `general class` and ``planned research class``, which are based on process-based models with their focus on writing textbooks. The process-based models are made up of the following procedures:content creation → content organization → writing → polishing. They have problems with the fixation of the process, the sequence of process application, and the maximum suitability and lop-sidedness of strategics. On the other hand, the non-subject composition activities include discretionary-activity class, after-school sessions aimed at improving special ability and aptitude, special-activity class and the instructions of how to write on homepages, etc. The researcher make the most use of programs emphasizing daily-routine material, function or strategics, subject integration, and genre, etc. developed by individual teachers or subject-education research committees. The problems are characterized by the insufficiency of contents stressing the betterment of understanding and contemplative faculty, failures to present information on writing purposes and situation and the obscurity of means to expand background knowledge in case of subject-matter integration.

      • The rice RING E3 ligase OsHIR1 participates in positive regulation of arsenic and cadmium uptake

        The metalloid arsenic (As) and the hevy metal cadmium (Cd) are ubiquitously found at low concentrations in the earth, while high concentrations of the both elements in soil and crop are severe dangerous to human health. We have tried to retrieve RING E3 ligase gene, which is believed to regulate substrate proteins in As or Cd uptake via ubiquitin 26S proteasome pathway, related to inhibit metal ion transport system. A total of 48 rice RING E3 ligases were randomly selected and then conducted semi-quantitative RT-PCR for their expression patterns as exposed to As and Cd treatments. We discovered one gene, Oryza sativa heavy metal induced RING E3 ligase 1 (OsHIR1) that was significantly up-regulated against both treatments. A total of 31 positive interaction clones with OsHIR1 were screened depending on their strong α-galactosidase activity via yeast-two hybrid screen. Bimolecular fluorescence complementation analysis evidenced that the OsHIR1 protein was clearly interacted with each of six partner protein including aquaporin tonoplast intrinsic protein 4;1 (OsTIP4;1) in the plasma membrane. Protein degradation assay showed that OsHIR1 strongly degraded the protein level of OsTIP4;1 via ubiquitin 26S proteasome system. Heterogeneous overexpression of OsHIR1 in Arabidopsis showed As- and Cd-insensitive phenotype. In addition, the transgenic plant showed low levels of As and Cd accumulation than the control plant in leaf and root. Here, we report the novel finding that OsHIR1 E3 ligase positively regulates OsTIP4;1 related to As and Cd uptake.

        카드뮴중독 환자의 임상상 - 특히 신기능을 중심으로

        카드뮴은 도금, 플라스틱, 유리, 합금, 전기소재 등의 제조에 널리 사용되고 있으며 이로 인한중독은 요세관 장애, 폐기종, 폐암, 골연화증, 요로결석 등을 일으킨다. 제련업, 배터리 제조업 등카드뮴 관련 산업이 많은 국내에서 카드뮴 중독증에 대한 체계적인 연구는 아직 없는 실정이다. 이에 저자들은 한국산업안전공단 산업보건연구원과 공동으로 카드뮴 중독이 진단된 5예에 대한요검사, 혈액검사, 신장조직 검사, 골밀도검사, 폐기능 검사 등을 실시하여 결과를 보고한다. 요중 카드뮴 농도가 5예 모두 1회 이상 5 μg/g creatinine 이상으로 측정[1차 38.51±42.68(7.27-101.07) μg/g creatinine, 2차 10.70±7.16(3.88-21.05) μg/g creatinine]되어 카드뮴 중독으로 진단하였다. 요중 N-acetyl-β-D-glucosaminidase(NAG)는 9.19±2.21(7.25-12.79)U/g creatinine[기준치 4.5U/g creatinine]로 증가되어 있었다. 요중 β2-microglobulin, Metallothionein(MT), 24시간요 단백량, 24시간요 칼슘량 모두 정상이었다. 요중 카드뮴 농도와 요중 NAG 배설량과는 양의 상관관계(r=0.912, p=0.031)를 보였다. 사구체 여과율은 109.8±8.9(96.5-118) mL/min로 정상이었다. 혈액 검사, 간기능검사, 신기능검사, 전해질 검사는 모두 정상이었다. 요검사에서 1예의 단백뇨 및 혈뇨(조직검사에서 IgA 신증으로 진단)를 제외하곤 모두정상 이었다. 병리조직검사에서는 전예 모두 간질의 경한 섬유화를 보였으나 염증세포의 침윤은 없었다. IgA 신증이 1예에서 동반되었다. 1예에서 골다공증 및 폐쇄성 폐질환이 동반되었다. Cadmium-containing compounds are widely used in the manufacturing of pigments, plastics, glass, metal alloys, and electrical equipment. Chronic low exposure of cadmium causes renal tubular dysfunction, emphysema, bronchial cancer, osteomalasia, multiple fracture or urethral stone. Occupations exposed to cadmium are increasing in industsralized country such as Korea, but there is a few systemic study on cadmium intoxication until nowadays. We studied the effects of cadmium on renal function, pulmonary function and bone mineral density in 5 workers exposed to cadmium. Cadmium in urine were increased [38.51±42.68 (7.27-101.07) μg/g creatinine(1 st exam), 0.70±7.16 (3.88-21.05) μg/g creatinine(2 nd exam)], respectively. Urine N-acetyl-β-D-glucosaminidase(NAG) excretion rates[9.19±2.21(7.25-12.79) U/g creatinine] were increased, but urinary β2-microglobulin(β2- MG), urinary methalothionein(MT) excretion, 24 hour urinary protein, calcium excretions were not increased. Cadmium in urine was correlated with urine NAG(r=0.912, p=0.031) but no relationship with others. Glomerular filtration rate were all within normal limits. Complete blood counts, liver function, renal function, electrolytes were all within normal limits. In urinalysis and sediment examination, 4 cases showed no abnormalities, but 1 case showed proteinuria(30 mg/dL) and hematuria with dysmorphic RBC dominance(This case was diagnosed accompanying IgA nephropathy). In pathologic examination, all biopsy specimens showed mild interstitial fibrosis without inflammatory cell infiltration. Osteoporosis and obstructive lung defect were diagnosed in one case. We found urinary NAG excretion were increased and mild interstitial fibrosis at biopsy specimen were seen in all cases. It is necessary to follow- up for detection of proteinuria and decrement of GFR in each patients.
