Several species of Legionella are proven as etiologic agents of human infections. It became clear that, in the serodiagnosis, no single antigen could detect half of the antibody-positive sera and cross-reacting antibodies to other species of Legionella as well as to other genera of bacteria were present. Levels of antibodies to Legionella species other than L. pneumophila 1-4, in normal Korean population, has not been determined. In this study, serum samples for blood chemistry and from blood donors at Yonsei Medical Center, which were collected during 1984~1987, were tested for the antibody to 13 Legionella antigens by immunofluorescent antibody test. Among the chemistry samples, none showed a titer of ≥ 32 to L. jordanis and L. pneumophila 2-5, and a few to L. pneumophila 1 and 6, and quite frequently to other species. The rate was especially high, 14.2% and 16.9% respectively, to L. dumoffii and L. gormanii. Some sera had antibodies to two or three antigens. It is concluded that, as the sensitivity and specificity of the IFA test are low in Korea, too, the cultural method should be more useful for the diagnosis of legionellosis.
Comprehensive Modeling of Corneal Alkali Injury in the Rat Eye

<P>Purpose: To characterize the molecular, clinical, and histopathological profiles in the rat cornea after alkali injury over a 21-day period.Methods: Alkali injury was induced in one eye of male Lewis rats. Corneal opacity and corneal neovascularization were assessed daily. Real-time qRT-PCR analysis and immunohistochemical staining were conducted to examine inflammation, neovascularization, and fibrosis.Results: We found that within 2 hours of chemical exposure, corneal opacification rapidly developed with an acute increase in various cytokine expressions, while several cytokines demonstrated a secondary peak by day 7. Early neutrophil infiltration peaked at day 1 post-injury while macrophage infiltration peaked at day 7. Throughout the time course of the study, corneal opacity persisted and neovascularization, lymphangiogenesis, and fibrosis progressed.Conclusions: This study highlights the molecular, clinical, and histopathological changes throughout the progression of alkali injury in the rat cornea. These profiles will assist in the development of new strategies and therapies for ocular alkali injury.</P>
Simultaneous pancreas-kidney transplantation

Purpose: The purpose of this study is to report the results of simultaneous pancreas-kidney (SPK) transplantations and describe the lessons learned from the early experiences of a single center. Methods: Between January 2002 and June 2013, a total of 8 patients underwent SPK transplantation. Clinical and radiologic data were reviewed retrospectively. Results: Seven patients were diagnosed with type I diabetes mellitus and one patient became insulin-dependent after undergoing a total pancreatectomy because of trauma. Pancreas exocrine drainage was performed by enteric drainage in 4 patients and bladder drainage in 4 patients. Three patients required conversion from initial bladder drainage to enteric drainage due to urinary symptoms and duodenal leakage. Four patients required a relaparotomy due to hemorrhage, ureteral stricture, duodenal leakage, and venous thrombosis. There was no kidney graft loss, and 2 patients had pancreas graft loss because of venous thrombosis and new onset of type II diabetes mellitus. With a median follow-up of 76 months (range, 2?147 months), the death-censored graft survival rates for the pancreas were 85.7% at 1, 3, and 5 years and 42.9% at 10 years. The patient survival rate was 87.5% at 1, 3, 5, and 10 years. Conclusion: The long-term grafts and patient survival in the current series are comparable to previous studies. A successful pancreas transplant program can be established in a single small-volume institute. A meticulous surgical technique and early anticoagulation therapy are required for further improvement in the outcomes.

Purpose: The purpose of this study was to investigate the exercise participation experiences of colorectal cancer survivors. Methods: A phenomenological method was used in this study. Ten adult colorectal cancer survivors were recruited and data were collected through in-depth interviews. Results: The factors involved in the experiences of colorectal cancer survivors’ exercise participation were categorized into 3 parts: (1) mental depression and isolation due to physical change, (2) barriers of exercise, (3) exercise participation for recovery. Though participants were aware of their changed body after cancer treatment and faced difficulties participating in exercise, they want to overcome these challenges through exercise participation. Conclusion: The results of this study show the exercise participation experiences of colorectal cancer survivors. Based on these findings, exercise maintenance can be promoted more effectively, and a higher exercise adherence of colorectal cancer survivors can be achieved.

<P>We examined short- and long-term changes in neurocognitive functions in patients with schizophrenia who were either started or switched to amisulpride in comparison with the normal controls. Fifty-seven patients treated with amisulpride and 60 normal controls completed a comprehensive neurocognitive function test battery at the baseline, the 8-week, and the 1-year follow-up. We conducted and compared the results of both intention-to-treat (ITT) and per-protocol (PP) analyses to account for the follow-up loss. Three general results obtained were as follows: (1) the degree of the improvements in neurocognitive function was comparable to those of other second-generation antipsychotics in both ITT and PP analysis; (2) in light of the relative effect size, the composite effect size and the effect size in most measures in both ITT and PP analyses were smaller for the patient group than those of the control group, signifying that improvement in performance may be largely attributable to practice effects; and (3) nonetheless, there were evidences of both short- and long-term improvements in some cognitive tasks, such as in the Korean-Wechsler Adult Intelligence Scale vocabulary subtest and the Trail Making Test, that may not be accounted by practice effect. These results suggest the need to include a healthy control group to validate the medication effect of cognitive improvements in patients with schizophrenia and to consider practice effect in interpreting the results of repeated administration of neurocognitive function tests.</P>
