A simple and efficient method has been developed for Knoevenagel condensation between aldehydes or ketones and active methylene group by stirring in methylene chloride at 60 ${^{\circ}C}$ under heterogeneous conditions using silica supported ammonium acetate. The products are obtained in excellent yields and are in a state of high purity. The structures of the products were confirmed by IR, $^1H$ NMR and mass spectral data and comparison with authentic samples available commercially.

An undifferentiated carcinoma (UC) of the gall bladder behaves aggressively and has a grave prognosis. Small cell type undifferentiated carcinoma of the gall bladder is a rare variant. This paper reports a case of UC of gall bladder with PAS-positive diastase- resistant eosinophilic hyaline globules present as liver mass (on imaging) in a male patient. The microscopic findings of the liver and gall bladder after a right tri-segmentectomy showed an un-differentiated malignant neoplasm composed of cells with round to oval nuclei, prominent nucleoli, and scanty neoplasm. No definite cell pattern was identified with these neoplastic cells. A section from the gall bladder revealed a tumor arising from the lining epithelium and infiltrating through the muscularis. This tumor was infiltrating the adherent liver tissue directly and forming a mass of undifferentiated malignant cells. The focal area within the tumor mass showed the presence of PAS-positive, diastase-resistant, eosinophilic hyaline globules within the neoplastic cells. The immunohistochemistry test was diffusely positive for perinuclear anti-neutrophil cytoplasmic antibodies and negative for chromogranin, vimentin, Desmin, alpha-fetoprotein, leukocyte common antigen, CD34, and bcl2. When the clinical and radiological data are inconclusive, careful analysis of the histological and immunophenotypic features is needed to make the final diagnosis of UC of the gall bladder. The biological behavior and prognosis of this tumor remain unclear because of its rarity. Further studies will be needed to understand the characteristics of this deadly tumor and to establish an effective therapy for it.
Reference range of random urinary calcium creatinine ratio in North Indian children and adolescents

Purpose: Urinary calcium creatinine ratio (UCaCrR) is a reliable indicator for monitoring hypercalciuria following vitamin D supplementation. However, the reference range varies from region to region. Previous studies did not take vitamin D and parathyroid hormone status into account while evaluating UCaCrR. Hence, we undertook this study to establish the 95th percentile of UCaCrR as an indicator of hypercalciuria in North Indian children and adolescents. Methods: Four hundred seventy-three participants (boys 62.2%, girls 37.8%) with adequate dietary calcium intake, normal serum levels of 25-hydroxy-vitamin D (>20 ng/mL), and without secondary hyperparathyroidism following supplementation were selected for evaluation of UCaCrR. Results: The mean age and body mass index of subjects were 11.2±2.6 years and 18.0±3.6 kg/m 2 , respectively. The 95th percentile of UCaCrR in the study population was 0.126. The mean, median, and 95th percentile of UCaCrR was significantly higher in prepubertal children (age ≤10 years) (0.0586±0.0374, median=0.0548, 95th percentile=0.136) compared to those >10 years old (0.0503±0.0363, median=0.0407, 95th percentile=0.123, P=0.02). No significant difference in UCaCrR was observed between genders and different weight categories. Conclusion: UCaCrR of 0.13 defines the cutoff value for hypercalciuria in North Indian children and adolescents with adequate dietary intake of calcium and sufficient serum vitamin D levels.

Plants are an abundant source of medicinal compounds, some of which are useful in combating free radical-1002 (flavonoid-poor fraction) of Hippophae rhamnoideswere screened on the basis of their reducing power in the aqueousphase. REC-1001 was selected for further study, since it exhibited 27.38 times higher antioxidant activity than REC-1002.REC-1001 also showed significant (P. .05) membrane protection potential at 50 .g/mL, which was attributed to its abilityto scavenge peroxyl radicals (64.82. 1.25% scavenging within 1,440 min). A significant (P. .05) difference of 67.02% infree radical scavenging activity at 1,000 ng/mL between REC-1001 and vitamin E demonstrated the extract fraction’s worthc-tion. Further, REC-1001 was found to be nontoxic up to 200 mg/kg of body weight. This research suggests that the REC-1001 fraction of H. rhamnoidesextract is a safe and effective antioxidant nutraceutical product.
