Background/Aims: The quality of bowel preparation is important for optimal colonoscopy. It is influenced by medical and personal factors. We aimed to evaluate the effect of bowel habit on the quality of bowel preparation and to identify predictors of inadequate bowel preparation among bowel habit factors. Methods: From June 2017 to September 2017, 90 volunteers were enrolled in this study. Each participant answered a questionnaire consisting of multiple questions about personal bowel habits, including stool form, frequency of bowel movements per week, duration, and degree of straining for bowel movement. Then, all volunteers underwent colonoscopic exam. Eleven endoscopists performed colonoscopies and used the Boston Bowel Preparation Scale (BBPS) as the index for bowel preparation. Two expert endoscopists simultaneously reviewed all colonoscopic images to confirm the final BBPS. Univariate and multivariate logistic regression analyses were performed to verify the correlation between bowel preparation adequacy and bowel habit. Results: Among the 90 participants, 20 (22.2%) had inadequate bowel preparation (total BBPS ≤6 or any segmental BBPS ≤1). In univariate analysis, infrequent bowel movement (0–2/week) (odds ratio [OR], 12.60; 95% confidence interval [CI], 1.22 to 129, p=0.03) and moderate straining (more than 1/4 of defecations) (OR, 4.40; 95% CI, 1.44 to 13.39; p=0.01) were significantly associated with inadequate bowel preparation. However, only moderate straining was significantly associated with inadequate bowel preparation in multivariate analysis (OR, 3.99; 95% CI, 1.26 to 12.65; p=0.02). Conclusions: Straining is a significant predictor for inadequate bowel preparation. For patients with straining during bowel movements, an intensified preparation regimen should be considered.
Degradation of CH3NH3PbI3 perovskite materials by localized charges and its polarity dependency

<P>Despite the excellent performance of organic-inorganic hybrid perovskite materials in recent years, the mechanism of their decomposition under actual operating conditions has not yet been elucidated. Herein, we elucidated the breakdown process of CH3NH3PbI3 perovskite crystals and identified the polarity-dependent degradation pathway <I>via</I> localized charges by performing time-evolution measurements of the absorption spectra of perovskite films with different underlying charge transport layers and <I>ab initio</I> molecular dynamics calculations. It was found that the carrier polarity (hole-rich or electron-rich) inside the perovskite films played a critical role in the degradation rate, and the polarity-dependent degradation pathway strongly depended on the combination of the surrounding gaseous molecules. The hole-rich perovskite films degraded more rapidly than the electron-rich ones in the presence of H2O, while this degradation trend was reversed in oxygen-only ambient environments. Interestingly, the hole-rich film is extremely unstable in atmospheric air containing both H2O and O2, whereas the MAPbI3 film with excessive electrons was stable in air. An <I>ab initio</I> molecular dynamics simulation was performed to obtain the detailed degradation pathway of MAPbI3 under atmospheric conditions for different polarities of the localized charge. The simulation results are in good agreement with experimental results, and the production of Pb(OH)I predicted in our simulations is confirmed by X-ray-assisted spectroscopic measurements.</P>
Clinical Significance of Venous Thromboembolism in Patients with Advanced Cholangiocarcinoma

Background/Aims: Patients with active cancer frequently develop venous thromboembolism (VTE). However, there is little data about VTE in patients with advanced cholangiocarcinoma (CCA). Therefore, we investigated the clinical significance of VTE in patients with advanced CCA. Methods: We analyzed the data of a total of 332 unresectable CCA patients diagnosed between 2010 and 2020 in this retrospective study. We investigated the incidence and risk factors for VTE, and its effect on survival in patients with advanced CCA. Results: During a median follow-up of 11.6 months, 118 patients (35.5%) developed VTE. The cumulative incidence of VTE was 22.4% (95% confidence interval [CI], 0.18 to 0.27) at 3 months and 32.8% (95% CI, 0.27 to 0.38) at 12 months. Major vessel invasion was an independent risk factor for VTE (hazard ratio, 2.88; 95% CI, 1.92 to 4.31; p<0.001). Patients who developed VTE during follow-up had shorter overall survival than patients who did not (11.50 months vs 15.83 months, p=0.005). In multivariable analysis, VTE (hazard ratio, 1.58; 95% CI, 1.23 to 2.02; p<0.001) was associated with poor overall survival. Conclusions: Major vessel invasion is related to the occurrence of VTE in advanced CCA. The development of VTE significantly decreases the overall survival and is an important unfavorable prognostic factor for survival.

This paper presents the fluctuation of internal pressure and the deformation characteristics of cam ring in a balanced type vane pump. Cam-ring is operated in the condition of high pressure and low pressure repeatedly. Pressure overshoot occur due to the accelerated fluid, so result in pressure ripple, flow ripple, noise, and internal part broken. As a result of measuring and analysis, we have found oscillation of pressure and compression chamber pressure depend on the rotational speeds. The Results indicate that the surge pressures of the chambers at the instant moment of discharge and closure are affected by the rotating speed.
무선 ECG 센서와 안드로이드 앱을 이용한 부정맥 진단 및 치료 시스템 구현에 관한 연구

부정맥은 현대인에게 빈번히 발생하는 질환 중 하나이다. 부정맥의 종류는 일시적으로 발생하였다가 자연스럽게 치유되는 경미한 부정맥 증상으로부터 발생하면 신체에 심각한 손상이나 돌연사로 이어질 수 있는 위협적인 부정맥 질환까지 매우 다양하다. 특히 심장질환과 관련하여 지속적인 모니터링과 치료가 요구되는 부정맥 증상으로는 심실빈맥(ventricular tachycardia, VT), 심방세동 (atrial fibrillation, AF), 심실세동(ventricular fibrillation, VF)등이 있으며, 이러한 증상은 심장 기능의 이상과 직접적인 관련이 있는 것으로 보고되어있고, 방치 시 인체에 치명적일 수 있어 조기 진단과 효과적인 치료가 요구된다. 따라서 본 논문에서는 이러한 생명에 영향을 줄 수 있는 심각한 부정맥 질환에 대한 지속적인 모니터링과 효과적인 치료를 손쉽게 수행할 수 있는 부정맥 진단 및 치료 시스템 구현에 관한 연구 내용을 보고한다. 본 연구에서는 부정맥의 지속적인 모니터링과 조기 진단을 위해서 무선 ECG (electrocardiogram) 센서를 사용하였고, ECG 신호에 대한 신호처리 (signal processing) 기술을 이용한 알고리즘 개발과 이를 연동하여 경고 신호를 의사에게 전송할 수 있는 안드로이드 앱을 개발하였다. 이러한 시스템 구축은 부정맥을 장시간 모니터링 하는 것이 가능하며, 의사에게 부정맥 탐지에 대한 보조적 가이드라인을 제공함으로써 증상에 대한 조기 진단과 효과적인 치료를 가능하게 하도록 하는 것을 목표로 한다. 무선 센서를 통하여 획득되는 심전도 신호는 R-R Peak, QRS Wave의 주파수 특성과 P wave, T wave 탐지 요소를 연산하여 심실빈맥, 심실세동, 심방세동을 효과적으로 탐지하도록 구축하였으며, 안드로이드 앱으로의 최적화 전송을 위하여 지속적으로 발전시키고 있다. 향후 연구 계획으로는 이러한 시스템이 의사의 안드로이드 앱을 통한 원격처방 시스템과 연동하여 원격 조정 (remote control)을 통한 약물 치료도 가능한 진단 및 치료 시스템을 구축할 계획이다.

Since 2015, Korea has been operating regional sports science centers in 10 cities across the country, and is pursuing a plan to systematically link athletes’ physical fitness through physical fitness measurements according to the athlete’s life cycle. For 243 elite athletes whose physical strength was measured by the Seoul Sports Science Center, the development direction of each sport type of the elite athletes in Seoul is analyzed through classification and grade. As a result of the study, in terms of basic physical strength, the difference in physical strength by sport type was significant in all items of muscle strength, muscular endurance, muscle power, cardiopulmonary endurance, agility, balance, and coordination except flexibility. As a result of analyzing the difference in the level of the physical strength factor according to the application of the fitness grade criteria for each sport type, the A type showed significantly higher differences in grip strength and back strength than the B and D types. It was significantly higher in sit-up than type A. Type B showed significantly higher physical endurance, muscle power, cardiopulmonary endurance, agility, coordination, and equilibrium compared to the A and D types, and D type showed higher physical strength level than the A type in cardiopulmonary endurance, agility, and balance. Based on the results of this study, it is considered necessary to compare the performance and physical strength of male high school students in Seoul and the characteristics of athletes by sport type to review the training method and improvement of the performance.
levEffectiveness of Electroconvulsive Therapy Augmentation on Clozapine-Resistant Schizophrenia

Objective-This retrospective case series study of the effectiveness of electroconvulsive therapy (ECT) augmentation on clozapine-resistant schizophrenia was conducted by EMR review. Methods-Clozapine-resistance was defined as persistent psychotic symptoms despite at least 12 weeks of clozapine administration with blood levels over 350 ng/mL in order to rule out pseudo-resistance. Seven in-patients who were taking clozapine and treated with ECT were selected. We analyzed the psychopathology and subscales changed by ECT. Results-The average number of ECT sessions was 13.4 (±4.6). Total Positive and Negative Syndrome Scale (PANSS) score was significantly reduced by 17.9 (±12.8) points (p=0.0384) on average, which represented a reduction of 25.5% (±14.3). 71.4% (5/7) of patients were identified as clinical remission, with at least a 20% reduction in PANSS score. PANSS reduction was associated with number of ECT sessions, stimulus level in the final session, and blood clozapine levels before ECT. However, the negative subscale on the PANSS were not reduced by ECT in any patient. We did not observe any persistent adverse cognitive effects. Conclusion-This study supports that ECT augmentation on clozapine-resistant schizophrenia reveals clinically effective and safe. Further research should be done involving a larger number of patients to investigate the effectiveness of clozapine/ECT combination therapy.
