<P>Flicker noise is the most crucial issue in an instrumentation amplifier (IA) for neural recordings because low-frequency neural signals overlap with the frequency of the amplifier's flicker noise. A Chopping technique, often used to reduce the flicker noise, is not desirable for high-impedance input sources due to the charge injection and clock feedthrough from the MOSFETs of the input chopper to the signal source, resulting in a significant increase in the total input-referred noise. Whereas MOSFETs have less flicker noise at the moment of turning on, and the intrinsic flicker noise can be then reduced by turning on and off the MOSFETs in the IA. This brief proposes a bulk switching IA, which can avoid the input current noise. A prototype IA is implemented in a 65 nm CMOS occupying 0.053 mm<SUP>2</SUP>, and it achieves the input-referred noise of 0.74 μ<SUB>Vrms</SUB> (local field potential) for 100 k Ω source impedance, a 3.3 times reduction compared with that of the chopper IAs, while consuming only 3.96 μW from a 1.2 V supply.</P>

<P>Superconducting magnetic energy storage (SMES) systems with High Temperature Superconducting (HTS) wires have been actively developed world-wide. A 600 kJ class SMES with Bi-2223 HTS wire has been in development as a national project since 2004 and is currently approaching the final testing stage of the first of three phases. In the second phase of the project, several MJ class HTS SMES will be developed. In this paper, designs of magnets for 5 MJ class SMES with DI-BSSCO and YBCO coated conductor are presented and compared.</P>
Homotypic Interaction of Stabilin-2 Plays a Critical Role in Lymph Node Metastasis of Tongue Cancer

<P>Background/Aim: Lymph node (LN) metastasis of solid types of tumors has important clinical significance and it is therefore critical to identify molecular biomarkers that would enable the selection of patients with LN metastases. Patients and Methods: We evaluated the expression of stabilin-2 in primary oral tongue tumors and metastatic LNs using immunohistochemical staining. The correlation between risk factors and nodal metastasis was assessed and disease-free survival was analyzed. Results: Stabilin-2 expression remained a significant predictor of LN metastasis and the factor affecting recurrence in tongue cancer. Most importantly, all metastatic tumors of tongue, lung, stomach and colon cancers stained positive for stabilin-2 and stabilin-2 was expressed strongly in the sinusoidal endothelial cell of metastatic LNs. Conclusion: Stabilin-2 can play a critical role in the first entrapping step of LN metastasis through homotypic interaction with the lymphatic endothelium and appears to be a tumor biomarker predicting for LN metastasis in patients with solid tumors.</P>

Purpose:?The aims of this study were to assess frequency of stress induced cardiomyopathy (SCMP) and predictors for incidence of SCMP in patients who admitted to medical ICU. We also explored the predictors of in-hospital mortality in patients with SCMP.?Method:?It is a retrospective cohort study. A total of 338 patients admitted to medical ICU and underwent transthoracic echocardiography (TTE) between January 2015 and December 2015 were enrolled. Definition of SCMP was defines as follows: 1) transient hypokinesis, akinesis of the left ventricular mid segments with or without apical involvement or the regional wall motion abnormalities extend beyond a single epicardial vascular distribution; 2) absence of obstructive coronary disease; 3) new EKG abnormalities or modest elevation in cardiac troponin; 4) absence of pheochromocytoma, myocarditis. Logistic regression model was used to analysis predictors for incidence of SCMP and predictors of in-hospital mortality in SCMP respectively. Cox regression model was used to analysis the influence of SCMP to mortality.?Results:?Of the 338 patients, SCMP was observed in 42 patients (12.4%). Compared to 296 patients without SCMP, patients with SCMP had lower frequency of DM (19% vs 29.2%, p=0.01), COPD (4.8% vs 16.9%, p=0.04) and higher prevalence of using mechanical ventilator (MV, 90.5% vs 71.9%, p=0.03), and lower ejection fraction (EF, 38.0±15.6 vs 61.7±11.8, p<0.001). In multivariate analysis, lower BMI (odd ratio, 0.88; 95% CI, 0.78 to 0.99; p=0.04) and lower EF (odd ratio, 0.90; 95% CI, 0.87 to 0.92; p<0.01) were associated with development of SCMP. In a Cox regression model adjusted with several confounders (age, DM, malignancy, liver cirrhosis, shock, acute kidney injury, use of MV) SCMP showed trend to increase of 60-day mortality (p=0.052). Among 42 patients with SCMP, acute kidney injury(AKI) was the only risk factor for predicting in-hospital mortality (odd ratio, 8.19; 95% CI, 1.35 to 47.75; p=0.02).?Conclusions:?Our study showed that prevalence of SCMP was 12.4% and trend to increase of 60-day mortality of critically ill patients. In patients with SCMP, AKI was the only risk factor for predicting in-hospital mortality.

<P>The environment of a pregnant mother has a life-long impact on later life of offspring. Maternal stress is known to cause low birth weight and programs several physiological dysfunctions in offspring. However, the direct effects of maternal stress on the developing fetus remain largely unknown. The present study focused on the effect of chronic maternal stress on the developmental program and its molecular mechanisms. Pregnant mice were given 6-hour immobilization stress every day from 8.5 days post coitum. Fetal body weight was significantly decreased by maternal stress throughout development. Importantly, developmental events were retarded in the stressed fetuses. Around embryonic day 13.5 (E13.5), the developmental increment of somite numbers was delayed, although this difference recovered by E15.5. Limb bud formation and regression of interdigital webbing were also retarded by approximately 0.5 days. Subsequently, transcriptomes of developing limbs were analyzed by cDNA microarrays. Approximately, one-tenth of detected transcripts were significantly influenced by maternal stress. Q-PCR AQ analyses further demonstrated that the expression of a subset of limb development-associated genes, including Igf1, Aldh1a2, and Acta1, was changed in the stressed fetus. In conclusion, our findings suggest that maternal stress can retard limb and somite development in mice, with profound impacts on the developmental genetic program of limb.</P>

Background: The objective structured clinical examination (OSCE) is used in the colleges of Korean Medicine, but few studies have validated the OSCE evaluation criteria or post-education questionnaires. Diagnostic ultrasound is used in Korean medicine treatment including acupuncture, acupotomy, and pharmacopuncture to increase the safety and efficacy of treatment. We aimed to develop and validate a OSCE checklist and questionnaire for diagnostic musculoskeletal ultrasound training.Methods: A OSCE checklist and rubric for diagnostic ultrasound training, and questionnaire was developed using literature research. Eight expert panelists verified each draft item in a single-round survey. Items with a content validity ratio (CVR) < 0.75 were excluded or modified to reflect the experts’ opinions.Results: The OSCE checklist and rubric for diagnostic ultrasound training focusing on volar wrist and carpal tunnel syndrome included: 15 items revised according to CVR and expert opinions, the pre-examination procedure, structures to be identified by ultrasound, scans with 2 diagnostic criteria for carpal tunnel syndrome, an explanation of the exam results, and the post exam procedure. The questionnaire consisted of 15 items, including the overall evaluation of training, the effect of the OSCE, and the perception of the ultrasound. All 6 self-evaluation items were not revised, as they had a CVR of ≥ 0.75.Conclusion: An ultrasound OSCE for scanning the volar wrist and diagnosing carpal tunnel syndrome was developed using 15 validated tasks, 15 survey questions about ultrasound training, and 6 questions for selfevaluation. These results may be used in the future for education in diagnostic ultrasound.

디지털 헬스 케어에 대한 관심이 증가하고 있으며, 우울증 치료 모바일 앱이 우울증 완화에 도움이 되었다는 연구 결과가 있다. 하지만 우울증 정도에 따른 앱 로그 활동 패턴에 대한 연구 결과는 부족하다. 본 논문에서는 국내에 증가하고 있는 우울증 환자들의 증상을 완화시켜줄 수 있는 마음건강” 모바일 앱의 로그 데이터를 바탕으로 우울증 정도에 따른 앱 사용패턴을 알아보았다. 총 351 명의 우울증을 겪고 있는 20 대에서 30 대 사이의 서울시 거주 사용자를 대상으로 분석하여 우울증 그룹별(심각, 보통, 가벼움)로 사용 패턴에 대한 유의미한 특징을 파악할 수 있었고 앱을 통한 우울증 완화를 지원하기 위한 디자인 방향을 제시하였다. 본 연구를 바탕으로 우울증 그룹별로 사용 로그를 기반으로 하는 콘텐츠 추천까지 이어질 수 있을 것이라 기대한다.

We present an interpolating, univariate subdivision scheme which preserves the discrete curvature and tangent direction at each step of subdivision. Since the polygon have a geometric information of some original(in some sense) curve as a discrete curvature, we can expect that the limit curve has the same curvature at each vertex as the control polygon. We estimate the curvature bound of odd vertices and give an error estimate for restoring a curve from sampled vertices on curves.

<P>In the development of large scale superconducting magnetic energy storage (SMES) systems, the problem of mechanical stresses induced in the windings by Lorentz force becomes more critical as dimensions of system and magnetic field increase. In this paper, an optimal design process of a 600 kJ SMES magnet combined with mechanical stress analysis is presented. A stress analysis method based on electromagnetic finite element analysis (FEA) is explained in detail. The results of the analysis led to the development of an optimum design, electro-magnetically and mechanically, of a single-pole double pancake coil (DPC) type 600 kJ SMES magnet. The stress in each DPC are described along with recommendations for winding tension in the manufacturing process to minimize radial and hoop stress in each DPC.</P>
