      • 위암 환자에서 위절제술후 나타나는 담도확장의 임상적 의의: 추적 CT 소견을 중심으로

        A retrospective review of consecutive 35 patients'pre and post operative abdominal computed tomography was performed to determine frequency and degree of bile duct dilatation following Billroth Ⅱ operation for stomach cancer and it's clinical significance. Degree of intrahepatic biliary dilatation was classified as mild, moderate, marked according to the extension into central, middle and peripheral zone of liver, respectively. Three specialists on the abdominal image participated in analysis of those findings without prior information of the patients. Intrahepatic biliary dilatation was seen in 22 patients(63%) analysed by at least one radiologist, in 13 patients(37%) by at least two radiologists. Bile duct dilatation on CT was common finding and not necessarily meant tumor recurrence. Vagotomy and afferent loop resulted form Billroth Ⅱ were one of possible causes of bile duct dilatation. This study showed in the cases of bile duct dilatation without clinical and other radiological evidence of recurrence, no further study are needed and only enough to follow up study.

        This paper discusses about the implementation of a simulator for the Navigation Control System(NCS) of bimodal tram. To operate a NCS of bimodal tram, values of all sorts of sensors installed in bimodal tram should be transmitted to the NCS, and the NCS calculates the measured sensor values to determine traveling direction, traveling speed, current position etc. The implementation of the simulator consists of a device applying driver’s input transaction function & virtual sensor program and a sub-rack device that controls communication with the NCS to evaluate navigation control function. The virtual sensor program can create routes (map), traveling profiles & seat information et cetera in order to transmit to the NCS, analyzes driver's input values and NCS output values to create virtual sensor values. The sub-rack device takes charge of communication with the NCS using CAN-OPEN, CAN-J1939, MVP protocols. This paper discusses about the implementation of the simulator and afterwards analyzes and evaluates the NCS simulation results.

      • Investigation of Microscopic Crystal Field in Co-Doped Lithium-Iron Phosphate

        <P>We present the study on the structural and the magnetic properties of LiFePO<SUB>4</SUB> and LiFe<SUB>0.8</SUB>Co<SUB>0.2</SUB>PO<SUB>4</SUB>. The crystal structure was determined to be orthorhombic with space group <I>P</I>nma by Rietveld refinement method. We have observed the increase in the lattice constants with Co<SUP>2+</SUP> ion substituted. The Mossbauer spectra of olivine LiFePO<SUB>4</SUB> and LiFe<SUB>0.8</SUB>Co<SUB>0.2</SUB>PO<SUB>4</SUB> have shown distorted line broadening below <I>T</I> <SUB>N</SUB> . In order to explain the observed line broadening, we have analyzed the Mossbauer spectra by considering the full Hamiltonian for the <SUP>57</SUP>Fe nucleus, and both the magnetic dipole and the electric quadrupole interactions. Also, we have observed the anomalous change in the electric quadrupole splitting near the <I>T</I> <SUB>N</SUB>.</P>

        Effect of acidity on the performance of a Ni-based catalyst for hydrogen production through propane steam reforming: K-AlSi<sub> <i>x</i> </sub>O<sub> <i>y</i> </sub> support with different Si/Al ratios

        <P><B>Abstract</B></P> <P>Propane steam reforming (PSR) for the production of H<SUB>2</SUB> was catalyzed by a NiO/K-AlSi<SUB> <I>x</I> </SUB>O<SUB> <I>y</I> </SUB> catalyst synthesized with various Si/Al ratios (Si/Al = 0, 0.3, 0.5, 0.7, and 1.0). The effect of the Si/Al ratio on the acidity of the NiO/K-AlSi<SUB> <I>x</I> </SUB>O<SUB> <I>y</I> </SUB> catalyst for PSR was investigated. NiO/K-AlSi<SUB> <I>x</I> </SUB>O<SUB> <I>y</I> </SUB> gave a higher H<SUB>2</SUB> selectivity and stability during PSR than NiO/K-SiO<SUB>2</SUB> and NiO/K-Al<SUB>2</SUB>O<SUB>3</SUB>. The NH<SUB>3</SUB>-TPD results showed that the acid quantity and strength of NiO/K-AlSi<SUB> <I>x</I> </SUB>O<SUB> <I>y</I> </SUB> changed significantly depending on the Si/Al ratio. With an increased Si/Al ratio, the densities of both weak and strong acid sites increased. The C<SUB>3</SUB>H<SUB>8</SUB>- and CO-TPD results indicated that desorption amounts increased significantly in all NiO/K-AlSi<SUB> <I>x</I> </SUB>O<SUB> <I>y</I> </SUB> catalysts relative to those of NiO/K-SiO<SUB>2</SUB> and NiO/K-Al<SUB>2</SUB>O<SUB>3</SUB>, and the adsorption amount increased with the Si/Al ratio. PSR results showed that the NiO/K-AlSi<SUB> <I>x</I> </SUB>O<SUB> <I>y</I> </SUB> catalyst exhibited much better stability than the NiO/K-SiO<SUB>2</SUB> and NiO/K-Al<SUB>2</SUB>O<SUB>3</SUB> catalysts. This study confirms the following facts: when the acidity is appropriately adjusted for the catalyst, adsorption of the reaction gas increases, which eventually increases the reaction rate and also inhibits strong sintering between the nickel and the Al<SUB>2</SUB>O<SUB>3</SUB> support.       • Usefulness of ROX Index and SpO<sub>2</sub>/FiO<sub>2</sub> Ratio for Predicting High-flow Nasal Cannula Failure in Hypoxemic COVID-19 Patients

        Background ROX index after high-flow nasal cannula (HFNC) initiation can be helpful to identify low-risk patients of HFNC failure in patients with acute hypoxemic respiratory failure. However, it remains unknown which variables are useful for predicting HFNC failure in coronavirus disease 2019 (COVID-19) receiving HFNC. This study aims to determine the discrimination ability of the ROX index and SpO2/FiO2 ratio in COVID-19 patients after HFNC initiation. Methods This multicenter study was conducted at seven university-affiliated hospitals in Korea. We retrospectively reviewed the hospitalized patients diagnosed with COVID-19 between February 10, 2020 and February 28, 2021. The ROX index and SpO2/ FiO2 ratio were calculated at 1 hour and 4 hours after HFNC initiation. The primary outcome was HFNC failure, defined as the subsequent use of mechanical ventilation despite using HFNC. Discrimination of prediction for HFNC failure was evaluated by receiver operating characteristic (ROC) curve analysis. Results During the study period, 1,565 patients with COVID-19 were hospitalized and eligible 133 patients who received HFNC were analyzed. Among them, 63 (47.4%) were successfully weaned from HFNC, and 70 (52.6%) were intubated. SpO2/FiO2 ratio at 1 hour after HFNC initiation was a more accurate predictor of HFNC failure compared to ROX index (AUC 0.762; 95% CI: 0.679- 0.846 vs. 0.733; 95% CI: 0.640-0.826) (Figure 1). In the multivariable analysis, patients older than 70 years old had 3.4 times of HFNC failure compared to younger patients (AUC 3.367 [95% CI: 1.358-8.349], p=0.009), and SpO2/FiO2 at 1 hour (AUC 0.983 [95% CI: 0.972-0.994], p=0.003) were associated with HFNC failure. Conclusions SpO2/FiO2 ratio at 1 hour after HFNC initiation demonstrated a useful predictive ability for HFNC failure. SpO2/FiO2 could be a bedside tool to identify the need for mechanical ventilation in COVID-19 patients receiving HFNC.

        Downsizing gas sensors based on semiconducting metal oxide: Effects of electrodes on gas sensing properties

        <P><B>Abstract</B></P> <P>We report the highly sensitive and selective downsized gas sensors for the IoT application. Sensing areas of Pt-interdigitated electrodes (IDEs) were varied to investigate the relation between sensing materials and electrodes. In<SUB>2</SUB>O<SUB>3</SUB> nanocolumns were deposited on the pre-patterned Pt IDEs using glancing angle deposition (GLAD). The effect of the interface resistance between electrodes and sensing materials, and the intergrain resistance between nanocolumns is analyzed by linear regression at different sensing area and incident angle of GLAD. In<SUB>2</SUB>O<SUB>3</SUB> (angle: 85°, sensing area: 0.3mm×0.3mm) nanocolumns with double Schottky barriers show the highest response and selectivity with fast response time of 10s to VOCs among the samples fabricated in this study. The analysis reveals that the intrinsic response of In<SUB>2</SUB>O<SUB>3</SUB> (angle: 85°, sensing area: 0.3mm×0.3mm) nanocolumns are dominantly affected by intergrain resistance, resulting in a high response. Our demonstration for the fundamental aspect of downsizing gas sensor makes an important contribution to the chemical sensor field with broad interest.</P> <P><B>Highlights</B></P> <P> <UL> <LI> The sensing areas are varied to investigate the relation between sensing materials and electrodes. </LI> <LI> The effect of the interface resistance and the intergrain resistance are analyzed by linear regression. </LI> <LI> The performance of the downsizing gas sensor is greatly influenced by the interface resistance. </LI> </UL> </P>

        Interaction of Ruthenium(II)[(1,10-phenanthroline)<sub>2</sub>benzodipyrido[3,2-a:2',3'-c]-phenazine]<sup>2+</sup> with Single Stranded Poly(dA) and Poly(dT): Turning off the Light Switch

        The spectral properties, namely the circular dichroism, electric absorption and luminescence properties, of Λ- and Δ-[Ru(II)(1,10-phenanthroline)2benzodipyrido[b:3,2-h:2',3'-j]phenazine]2+ ([Ru(phen)2BDPPZ]2+) in the presence and absence of single stranded poly(dA) and poly(dT) were compared in this work. In the presence of single stranded DNAs, hypochromism in the absorption spectrum and significant changes in the circular dichroism spectrum in the ligand absorption band were apparent, indicating the strong interaction of the [Ru(phen)2BDPPZ]2+ complex with the single stranded DNAs. The luminescence intensity of the Ru(II) complex decreased stoichiometrically with increasing concentrations of the single stranded DNAs. All of these spectral changes were independent of the configuration of the Ru(II) complex and the nature of the DNA bases. Therefore, it is conceivable that both enantiomers of the [Ru(phen)2BDPPZ]2+ complex interact electrostatically with the negatively charged phosphate groups of DNA. However, the spectral properties of [Ru(II)(1,10-phenanthroline)3]2+ were not altered even in the presence of single stranded DNAs. Therefore, the size of the ligand involved in the interaction of the metal complex with the phosphate group of DNA may play an important role, even when the nature of the interaction is electrostatic.

        Brain involvement by leprosy presenting as a frontal cystic lesion

        <P>Leprosy has a predilection for peripheral nerves and is not considered to involve the CNS. The idea that the CNS is exempt from <I>Mycobacterium leprae</I> bacilli has been suspected from a clinical perspective or CSF study in leprosy patients. However, there has been no direct evidence for CNS involvement by leprosy in a living patient. To the best of the authors' knowledge, the present case is the first report providing histopathological and molecular evidence for CNS involvement by leprosy in a living patient. Brain MRI revealed a 2-cm cystic lesion in the right frontal lobe of the patient. The medical history revealed that the patient had been receiving multidrug therapy for borderline lepromatous leprosy. Neuronavigation-guided craniotomy and lesion removal were performed due to a presumptive diagnosis of low-grade glioma. The brain specimen demonstrated variably thickened blood vessels and densely scattered foamy macrophages in the perivascular spaces and parenchymal stroma. Fite acid-fast stain displayed red granular inclusions that were suggestive for fragmented <I>M. leprae. M. leprae</I>-specific nested polymerase chain reaction amplification showed positive bands, and DNA sequencing also demonstrated homology with the <I>M. leprae</I> genome. This case supports the notion that <I>M. leprae</I> can involve the cerebral cortex regardless of cranial nerve engagement.</P>

        기능성 소화불량증 환자에서 자율신경 기능 이상과 위배출시간의 연관성

        Background/Aims: Autonomic nerve dysfunction may be responsible for the pathophysiologic mechanisms of various functional gastrointestinal disorders including functional dyspepsia. We evaluated whether the autonomic nerve dysfunction is related with delayed gastric emptying time. Methods: Forty patients (M:F=11:29, mean age=42.9 years) with functional dyspepsia and 14 asymptomatic controls (M: F=6:8, mean age=41.9 years) were enrolled in this study. Autonomic nerve function was measured by a standardized battery of five cardiovascular reflex tests. Gastric emptying time of solid meal was measured by scintigraphic method and expressed as the retention rate at 120 minutes (RR120). Results: RR120 was greater in the patients group than in the controls (48.1±19.7% vs 28.3±14.1%). Definite, severe, and atypical abnormal autonomic nerve functions were observed in 45% (18/40) of the patients group but not in the controls. The controls showed normal or early abnormality (5/14). Autonomic nerve dysfunction was not reted to delayed gastric emptying (p=0.42) and there was no significant correlation between the autonomic score and RR120 (p=0.55). Conclusions: Patients with functional dyspepsia had significant autonomic nerve dysfunction, as compared to asymptomatic controls. However, autonomic nerve dysfunction was not related with delayed gastric emptying time.
