      • Disorders of Small and Large Intestine : A Case Of Protein-Losing Enteropathy Associated With Chronic Ulcerative Nongranulomatous Jejunoileitis

        Introduction: Protein losing enteropathy (PLE) is a syndrome associated with excessive loss of serum proteins into the gastrointestinal tract. Hypoproteinemia and edema are the principal clinical manifestations of PLE. The diagnosis of PLE is made by documentation of excessive protein loss into the GI tract, through α 1-antitrypsine clearance or Tc 99m-labeled human albumin scan. The treatment is directed at correction of the underlying disease. We experienced the case of PLE developed by ulcerations of the small bowel without recognizable causes, that was treated by small bowel resection. Case: A 69-year-old man was admitted due to 3-year history of low extremities edema and 10-year history of abdominal pain. Laboratory findings of low albumin and normal liver and renal function led to a presumptive diagnosis of PLE. Albumin scan showed increased radioactivities in distal ileum. A capsule endoscopy was performed for evaluation of small bowel, which identified several ileal ulcerations and strictures. For accurate diagnosis and treatment, small bowel segmental resection was performed. The Gross findings showed several shallow ulcerations and two fibrous stenotic narrowing sites. The microscopic findings showed only nongranulomatous ulcers with focal villous atrophy. He made a steady recovery of albumin level up to 3.0g/dL after operation. At 2 months follow-up, he was well with no further problems and the albumin level was 3.6 g/dL. Discussion: Chronic ulcerative nongranulomatous jejunoileitis is a rare cause of small intestinal ulceration. Patients may present with protein losing enteropathy and abdominal pain. Therapy for this disorder includes initiation or maintenance of a strict gluten-free diet, resection of localized disease, total parenteral nutrition and corticosteroid therapy in refractory cases. As our case, in the patient with localized involvement, surgical resection of the involved bowel offers a good chance for survival.

        Background: Autophagy is an important adaptive mechanism in normal development and in response to changing environmental stimuli in cancer. Previous papers have reported that different types of cancer underwent autophagy to obtain amino acids as energy source of dying cells in nutrient-deprived conditions. However, whether or not autophagy in the process of lung cancer causes death or survival is controversial. Therefore in this study, we investigated whether nutrient deprivation induces autophagy in human H460 lung cancer cells. Methods: H460, lung cancer cells were incubated in RPMI 1640 medium, and the starved media, which are BME and RPMI media without serum, including 2-deoxyl-D-glucose according to time dependence. To evaluate the viability and find out the mechanism of cell death under nutrient-deprived conditions, the MTT assay and flow cytometry were done and analyzed the apoptotic and autophagic related proteins. It is also measured the development of acidic vascular organelles by acridine orange. Results: The nutrient-deprived cancer cell is relatively sensitive to cell death rather than normal nutrition. Massive cytoplasmic vacuolization was seen under nutrient-deprived conditions. Autophagic vacuoles were visible at approximately 12 h and as time ran out, vacuoles became larger and denser with the increasing number of vacuoles. In addition, the proportion of acridine orange stain-positive cells increased according to time dependence. Localization of GFP-LC3 in cytoplasm and expression of LC-3II and Beclin 1 were increased according to time dependence on nutrient-deprived cells. Conclusion: Nutrient deprivation induces cell death through autophagy in H460 lung cancer cells.

        할미꽃(백두옹, Pulsatilla koreana) 첨가량에 따른 오미지의 미생물학적 특성

        This study was performed to analyze the microbiological properties of Oiji soaking solution and Oiji preserved with $7\%$ brine and the others preserved with Pulsatilla koreana additives of various concentration$(0,\;3,\;6,\;9\%)$ during fermentation(24 days) at room temperature$(20\pm2^{\circ}C)$. The results of this experiment are as follows: The total number of microbes showed radical increase up to 6th day of growth period and decreased afterwards. Growth of microbes was impeded in the beginning of fermentation in the case of adding $6\%$ Pulsatilla koreana. The number of lactic acid bacteria tended to increase progressively until 15th day and then decreased, and that in the group of $6\%$ Pulsatilla koreana was relatively lower than those of other groups. Antimicrobial activity against Staphylococcus aereus and Micrococcus luteus was shown weakly from 9th day of fermentation regardless of the concentration of Pulsatilla koreana additives, but inactivated after 21st day of fermentation.
