      • EGR 쿨러의 전열 특성에 관한 이론 및 수치해석

        EGR system reduces the NOx as the mixed gas included in the recirculated exhaust gas decreases combustion temperature. The temperature of recirculated exhaust gas is controlled with the EGR cooler in the EGR system. In this study, we studied cooling effect of the EGR cooler with different a cross-section shapes of tubes and the number of tubes. The results show that the cross-section shapes of tubes and the number of tubes slightly influence the temperature drop of the recirculated exhaust gas.

        經通草 煎湯液이 家兎의 腎臟機能 및 血漿 Aldosterone 濃度에 미치는 影響

        To evaluate effect of Lignum Aeschynomeni water extract on the renal functions and plasma aldosterone concentration, experiments performed in the rabbit The results obtained were followed : 1. Intravenous administration of Lignum Aeschynomeni water extract at the doses of 0.08㎖/㎏ significantly increased urine volume, glomerular filtration rate and urinary excretion of electrolytes. 2. Intravenous administration of Lignum Aeschynomeni water extract at the doses of 0.8㎖/㎏ remarkably decreased urine volume, free water clearance and glomerular filtration rate. 3. In travenous administratio of Lignum Aeschynomeni water extract at the doses of 0.08㎖/㎏ and 0.8㎖/㎏ significantly decreased plasma aldosterone concentration. These results suggest that Lignum Aeschynomeni water extract at the doses of 0.08㎖/㎏ has a diuretic effect which may be caused by renal hemodynamics and the decrement of aldosterone concentration, and that Anti diuretic effect of large dose may be relateo with Anti diuretic hormone.

        Pleural Effusion in Multiple Myeloma: Characteristics and Practice Patterns

        <P>In many Asian countries battling with the double burden of increasing noninfectious diseases on top of infectious diseases, multiple myeloma (MM) patients presenting with pleural effusion (PE) pose a great diagnostic challenge. Thus, we aimed to analyze the clinical features and practice patterns of such patients. This is a multicenter retrospective study of newly diagnosed MM patients between January 2011 and December 2015. Among 575 MM patients diagnosed during the study period, 80 (13.9%) that were associated with PE were identified and analyzed. The most common cause of PE was parapneumonic (25%), followed by reactive (18.8%). Higher CRP levels and leukocytosis were indicators of parapneumonic PE. There were 7 (8.8%) with myelomatous PE and 2 (2.5%) with tuberculosis. Fifty-six patients underwent additional examinations to determine the exact cause of effusion; 28 patients received computed tomography (CT) of the chest, 5 patients underwent thoracentesis/biopsy, and 23 patients underwent both CT and thoracentesis/biopsy. On the other hand, 24 patients did not undergo additional analyses but were treated empirically. Real-world analyses of practice patterns in MM patients with PE showed the suboptimal use of invasive procedures to determine the exact cause of PE. Since reversible causes and tuberculosis pleurisy are not uncommon, invasive procedures should be actively incorporated as needed.</P>

        Differential effects of blood insulin and HbA1c on cerebral amyloid burden and neurodegeneration in nondiabetic cognitively normal older adults

        <P><B>Abstract</B></P> <P>We tested the hypothesis that lower insulin or higher glycated hemoglobin (HbA1c) levels in blood are associated with increased cerebral beta amyloid (Aβ) deposition and neurodegeneration in nondiabetic cognitively normal (CN) older adults. A total of 205 nondiabetic CN older adults underwent comprehensive clinical assessment, [<SUP>11</SUP>C]Pittsburgh compound B (PiB)-positron emission tomography (PET), [<SUP>18</SUP>F]fluorodeoxyglucose-PET, magnetic resonance imaging, and blood sampling for fasting insulin and HbA1c measurement. Lower blood insulin was significantly associated with increased Aβ positivity rates and decreased cerebral glucose metabolism in the AD-signature region. In contrast, higher HbA1c levels were not associated with Aβ positivity rates but were significantly associated with higher rates of having neurodegeneration in the AD-signature regions. Our results suggest different roles of insulin and HbA1c in AD pathogenesis, in that decreased blood insulin below optimal levels may contribute to increasing cerebral Aβ deposition and neurodegeneration whereas impaired glycemic control may aggravate neurodegeneration through a nonamyloid mechanism in nondiabetic CN older adults.</P>

        Functional and electron-microscopic changes after differential traction injury in the sciatic nerve of a rat

        Background: During maxillofacial trauma or oral cancer surgery, peripheral nerve might be damaged by tractioninjury. The purpose of this study was to evaluate functional and histomorphometric changes after traction nerveinjury in the sciatic nerve of a rat model. Methods: A total of 24 Sprague-Dawley rats were equally divided into three groups: unstretched (sham/control,group A), stretched with 0.7N (group B) and 1.5N (group C). Traction injury was performed for 10 min in B and Cgroups. Functional recovery of the sciatic nerve was evaluated by walking track analysis, toe spread test, andpinprick test 2 weeks after injury. The weight of gastrocnemius muscles of both sides was measured to evaluateweight ratio (ipsilateral/contralateral). Total number of axons, axon fiber size, myelin thickness, G-ratio, axonnumber/mm2, diameter of fiber, changes of longitudinal width, and formation of the edema and hematoma wereevaluated by transmission electron microscopy. Results: The sciatic function indexes were −11.48±4.0, −15.11±14.84, and −49.12±35.42 for groups A, B, and C,respectively. Pinprick test showed 3.0, 2.86±0.38, and 1.38±0.52 for A, B, and group C. Muscle weight ratios were0.98±0.13 for group A, 0.70±0.10 for group B, and 0.54±0.05 for group C. There were significant differences in toespread test, pinprick test, and muscle weight ratio between control group and experimental group (p<0.001). In theexperimental group, fiber number, fiber size, G-ratio, fiber number/mm2, myelin thickness, diameter of fiber, andlongitudinal width were decreased with statistical significance. Conclusion: The present study demonstrated that the nerve traction injury in the rat sciatic nerve damaged themotor and sensory function and axonal integrity. The amount of functional nerve damage was proportional to theamount of traction power and dependent on the initial tensile strengths (0.7N and 1.5N).

      • Association of Cerebral Amyloidosis, Blood Pressure, and Neuronal Injury with Late-Life Onset Depression

        <P>Previous literature suggests that Alzheimer's disease (AD) process may contribute to late-life onset depression (LLOD). Therefore, we investigated the association of LLOD with cerebral amyloidosis and neuronal injury, the two key brain changes in AD, along with vascular risks. Twenty nine non-demented individuals who first experienced major depressive disorder (MDD) after age of 60 years were included as LLOD subjects, and 27 non-demented elderly individuals without lifetime experience of MDD were included as normal controls (NC). Comorbid mild cognitive impairment (MCI) was diagnosed in 48% of LLOD subjects and in 0% of NC. LLOD, irrespective of comorbid MCI diagnosis, was associated with prominent prefrontal cortical atrophy. Compared to NC, LLOD subjects with comorbid MCI (LLOD<SUB>MCI</SUB>) showed increased cerebral <SUP>11</SUP>C-Pittsburg compound B (PiB) retention and plasma beta-amyloid 1–40 and 1–42 peptides, as measures of cerebral amyloidosis; and, such relationship was not observed in overall LLOD or LLOD without MCI (LLOD<SUB>woMCI</SUB>). LLOD subjects, particularly the LLOD<SUB>woMCI</SUB>, had higher systolic blood pressure (SBP) than NC. When analyzed in the same multiple logistic regression model that included prefrontal gray matter (GM) density, cerebral amyloidosis, and SBP as independent variables, only prefrontal GM density showed a significant independent association with LLOD regardless of MCI comorbidity status. Our findings suggest AD process might be related to LLOD via prefrontal neuronal injury in the MCI stage, whereas vascular processes—SBP elevation, in particular—are associated with LLOD via prefrontal neuronal injury even in cognitively intact or less impaired individuals.</P>

        Advantage of achieving deep response following frontline daratumumab-VTd compared to VRd in transplant-eligible multiple myeloma: multicenter study

        Background The goal of induction therapy for multiple myeloma (MM) is to achieve adequate disease control. Current guidelines favor triplet (bortezomib-lenalidomide-dexamethasone; VRd) or quadruplet regimens (daratumumab, bortezomib-thalidomide-dexamethasone; D-VTd). In the absence of a direct comparison between two treatment regimens, we conducted this study to compare the outcomes and safety of VRd and D-VTd. Methods Newly diagnosed MM patients aged >18 years who underwent induction therapy followed by autologous stem cell transplantation (ASCT) between November 2020 and December 2021 were identified. Finally, patients with VRd (N=37) and those with D-VTd (N=43) were enrolled. Results After induction, 10.8% of the VRd group showed stringent complete remission (sCR), 21.6% showed complete response (CR), 35.1% showed very good partial response (VGPR), and 32.4% showed partial response (PR). Of the D-VTd group, 9.3% showed sCR, 34.9% CR, 48.8% VGPR, and 4.2% PR (VGPR or better: 67.6% in VRd vs. 93% in D-VTd, P=0.004). After ASCT, 68.6% of the VRd group showed CR or sCR, while 90.5% of the D-VTd group showed CR or sCR (P=0.016). VRd was associated with an increased incidence of skin rash (P=0.044). Other than rashes, there were no significant differences in terms of adverse events between the two groups. Conclusion Our study supports the use of a front-line quadruplet induction regimen containing a CD38 monoclonal antibody for transplant-eligible patients with newly diagnosed MM.

        Analysis of two-dimensional unsteady flows with a free surface in a rectangular container subject to a linear reciprocating force is performed by numerical and experimental methods. FVM is used for the numerical computation of the two-dimensional flows. We consider the surface tension as well as the viscous/elastic properties of the free surface. One-dimensional analysis as well as experiment is used in establishing the free surface properties. The steady recirculatory flow is visualized by a laser sheet. It is shown that the one-dimensional analysis provides useful informations associated with the free surface properties.
