Endometriosis is an enigmatic disease characterized by development of endometrial tissue outside of the uterus, causing pain and infertility. This guideline will provide evidence based information concerning diagnosis and treatment of endometriosis. Constructive dialogue should allow patients to be able to trust the advice given by their practitioner as they will be confident that they have and will be able to use this guideline to inform this decision-making process. This guideline has been developed with the aim of providing guidance on endometriosis. The effectiveness of the various treatments as well as their risks and benefi ts are discussed in relation to their use in the treatment of endometriosis. We wish the information contained in this guideline will help clinicians reach a reasonable and beneficial decision with the up-to-date information.
Inactivation of <i>Vibrio vulnificus</i> Hemolysin by Oligomerization but Not Proteolysis

<P><I>Vibrio vulnificus</I> extracellular protease (VvpE) is believed to destroy its hemolysin (VvhA) in the late growth phase, without obvious experimental evidence. So, we attempted to elucidate the mechanism. The hemolytic activity steeply increased with the expression of the VvhA in the early growth phase, and then abruptly declined with the expression of VvpE in the late growth phase. However, the VvhA activity also abruptly declined in a VvpE-deficient mutant. In Western blot, the degradation of VvhA was not observed; instead, the oligomerization of VvhA increased with the concomitant loss of hemolytic activity. These results evidently indicate that the inactivation of VvhA is due to the novel oligomerization of VvhA by unknown mechanism, but not to the destruction of VvhA by VvpE, so that the routine functional assay measuring hemolytic activity cannot reflect the actual production of VvhA.</P>

A New Cymbidium variety "Yellow Evening" was developed from a cross between "Oji" and "Valley Flower Cherry Life" followed by seedling and line selections at the National Institute of Horticultural and Herbal Science (NIHHS), Rural Development Administration (RDA) in 2004. After investigation of the characteristics and selection for 9 years (1996-2004), it has been selected specifically pot orchid flower. "Yellow Evening" has medium plant height and semi-erect peduncle. It has medium flowering habit and brownish yellow flowers (RHS, Y11B: petal and sepal) with grayed purple GP183A) spot on lip, when fully opened. Each peduncle has 15.3 flowers with diameter of about 7.4 cm. It has adequate peduncle height, with some curved and spreading petal and sepals. Under optical culture conditions, it started to bloom early-winter (November).

A new hybrid, Phalaenopsis'Pink Marble' was made by the National Institute of Horticultural and Herbal Science, Rural Development Administration, in 2005. This hybrid was selected from self-crossed progenies of P.'21-1' (collected number) in 1999. In 2001, one line was selected based on the aspects such as flower color, leaf shape, flower stalk, and vigorous growth. Trials were conducted from 2003 to 2005 for evaluation and selection of this cultivar.'Pink Marble' had a medium flowering habit and a dark pink spot (RHS, RPN74B) on white petal and sepal when fully opened. The number of flowers on each peduncle was 7.5, and flower diameter was 52.3 cm. The general impression of petals and sepals is a plate shape. The thick sepal could extend the long flowering time. The average length of leaf and peduncle were 16.5 cm and 6.8 cm, respectively. It had a half-erect leaf form, and was a fast-growing cultivar. This hybrid is relatively easy to clone.

The objective of this study was to examine how circulating leptin concentrations and resting energy expenditures (REE) are related to body composition in obese adults, and to examine differences in these parameters according to gender. Twenty-three subjects, 6 males and 17 females, were recruited from patients with a body mass index (BMI) of greater than 27 at the Obesity Clinic of the K University Hospital. Anthropometric assessments and biochemical analyses were performed, and REEs were measured. In spite of having similar BMI values the plasma leptin levels of females (20.0$\pm$6.5 ng/ml) were significantly higher (p<.05) than those of males (14.2$\pm$6.1) ng/ml). In females, plasma leptin concentrations were found to be positively related to body weight. BMI, waist-hip ratio (WHR), fat mass (FM), body fat, and to the circumferences of forearm, waist and hip (p<.0001). However, in males, plasma leptin concentrations were positively related only to suprailiac thickness (p<.05). The higher plasma leptin levels in females compared to males may, at least partially. be explained by the females' higher subcutaneous fat mass. Plasma leptin concentrations appeared to reflect not only total fat mass but also regional fat distribution, especially in females. REE values of males (2254.3$\pm$256.2 kcal/day) were significantly higher (p<.01) than those of females (1799.1$\pm$454.7 kcal/day). REE values for females were positively related to body weight, BMI, lean body mass (LBM), FM, body fat, and to the circumferences of waist and hip (p<.05); however, REE values for males were (positively) related only to LBM (p<.05). REE values were not related to plasma leptin concentrations for either males or females, indicating that the plasma level of leptin might not be a predictor for REE value.

<P>Cervicogenic dizziness is dizziness triggered by movement or positioning of the cervical spine and is often accompanied by neck pain or stiffness. This is a prospective, pragmatic, assessor-blind, randomized controlled trial aimed at testing the efficacy and safety of adjuvant Chuna Manual Therapy (CMT) in patients with cervicogenic dizziness under usual care treatments. Fifty patients with cervicogenic dizziness will be randomly allocated to CMT or usual care (UC) groups in a 1 : 1 ratio. Extensive screening procedures, including examinations for central nervous system problems and nystagmus, will be applied to exclude other dizziness-inducing disorders. The eligible participants will receive 12 sessions of CMT plus UC or only UC over 6 weeks. CMT includes mandatory and discretionary techniques, whereas UC includes electrotherapy, thermotherapy, and patient education. The efficacy will be evaluated primarily as Dizziness Handicap Inventory score. The severity and frequency of dizziness, the level of neck pain or stiffness, and the cervical range of motion will also be evaluated. Safety will be assessed by adverse events. The data will be statistically analyzed at <I>p</I> < 0.05.<I> Trial Registration</I>. A survey of patients' perspectives of steroid injection (ppyeojusa) in Korea

Background: Steroid injections are commonly used in pain clinics to relieve pain and treat inflammation. In Korea, these steroid injections are well known as 'ppyeojusa', which means to inject into the bone in Korean. Some patients often have a negative perception of this treatment method due to inaccurate information about the treatment and side effects of steroids. The purpose of this study is to investigate patients' perception and knowledge of ppyeojusa. Methods: A questionnaire about ppyeojusa was completed by patients who visited one of the pain clinics in nine university hospitals, from August 1 to September 10, 2017. Results: Three-hundred seventy-four patients completed the survey. Eighty-five percent of patients had had ppyeojusa, and 74% of the respondents had heard of ppyeojusa from the mass media, friends or relatives. Only 39% of the patients answered that this injection was safe without side effects if properly spaced. Of the patients surveyed, 21% responded that ppyeojusa are "injections into the bone"; while 15% responded that ppyeojusa are "terrible injections that melted 'the bone if used a lot'". Half of the patients did not know what the active constituent is in ppyeojusa. If steroid injections are advised by the pain specialists, 89% of the patients would consent. Conclusions: Most pain clinic patients have heard of ppyeojusa. Most patients obtained information about ppyeojusa from mass media, rather than their physicians. Therefore, it is likely that most patients have inaccurate knowledge.
