      • Multidisciplinary approaches to recreate a beautiful smile of a patient with bulimia nervosa: a case report

        본 증례는 26세 여성환자로 폭식증과 이로 인한 구토 및 위산역류로 상악 전치부에 광범위한 치질 손상과 더불어 산부식으로 인해 상아질 노출로 기능뿐만 아니라 심미적인 문제를 주소로 내원했다. 환자는 단시간 내 심미성 회복을 원하는 상태로 교정 등의 치료는 원치 않았다. 이를 감안해 치료계획은 이미 손상된 치질을 최대한 보존하면서 심미성 및 기능 회복을 목표로 하였다. 상악 전치부의 치관 비율 및 치은 형태 개선을 위해 치은절제술 및 치관연장술을 시행했으며, 전부도재관을 이용해 수복하였다. 회전 이동되어있는 우측 상악 측절치는 치질 삭제 없이 veneer 형태로 제작했다. 산 부식 및 마모에 강한 지르코니아에 도재로 순측을 veneering해 기능 회복뿐만 아니라 심미적인 면에서도 만족할만한 결과를 얻을 수 있었다. 추후 미백치료를 고려해 최종보철물의 색상을 결정했으며, 환자는 색상과 형태에 만족했으며 자신감있는 미소를 회복할 수 있었다. The patient was a 26-year old female who was not satisfied with the aesthetic approach of her maxillary incisors. The treatment goal throughout the process was to manage the risk and recreate the original and youthful appearance of the smile. The biomechanical risk was managed by providing coverage of the exposed dentin and rebuilding the eroded structures with minimal tooth reduction throughout the treatment. Functional risk and prognosis for this patient were both improved by achieving acceptable function. The patient was satisfied with the full-ceramic restorations and the symmetry, harmony gingival architecture.

        Polymorphism, Expression of Natural Resistance-associated Macrophage Protein 1 Encoding Gene (NRAMP1) and Its Association with Immune Traits in Pigs

        Natural resistance-associated macrophage protein 1 encoding gene (NRAMP1) plays an important role in immune response against intracellular pathogens. To evaluate the effects of NRAMP1 gene on immune capacity in pigs, tissue expression of NRAMP1 mRNA was observed by real time quantitative polymerase chain reaction (PCR), and the results revealed NRAMP1 expressed widely in nine tissues. One single nucleotide polymorphism (SNP) (ENSSSCG00000025058: g.130 C>T) in exon1 and one SNP (ENSSSCG00000025058: g.657 A>G) in intron1 region of porcine NRAMP1 gene were demonstrated by DNA sequencing and PCR-RFLP analysis. A further analysis of SNP genotypes associated with immune traits including contain of white blood cell (WBC), granulocyte, lymphocyte, monocyte (MO), rate of cytotoxin in monocyte (MC) and $CD4^-CD8^+$ T lymphocyte subpopulations in blood was carried out in four pig populations including Large White and three Chinese indigenous breeds (Wannan Black, Huai pig and Wei pig). The results showed that the SNP (ENSSSCG00000025058: g.130 C>T) was significantly associated with level of WBC % (p = 0.031), MO% (p = 0.024), MC% (p = 0.013) and $CD4^-CD8^+$ T lymphocyte (p = 0.023). The other SNP (ENSSSCG00000025058: g.657 A>G) was significantly associated with the level of MO% (p = 0.012), MC% (p = 0.019) and $CD4^-CD8^+$ T lymphocyte (p = 0.037). These results indicate that the NRAMP1 gene can be regarded as a molecular marker for genetic selection of disease susceptibility in pig breeding.

        The effect of a desensitizer and $CO_2$ laser irradiation on bond performance between eroded dentin and resin composite

        PURPOSE. This study was aimed to evaluate effect of the desensitizing pretreatments on the micro-tensile bond strengths (${\mu}TBS$) to eroded dentin and sound dentin. MATERIALS AND METHODS. Forty-two extracted molars were prepared to form a flat dentin surface, and then they were divided into two groups. Group I was stored in distilled water while group II was subjected to a pH cycling. Each group was then subdivided into three subgroups according to desensitizing pretreatment used: a) pretreatment with desensitizer (Gluma); b) pretreatment with $CO_2$ Laser (Ultra Dream Pluse); c) without any pretreatment. All prepared surfaces were bonded with Single Bond 2 and built up with resin composite (Filtek Z250). The micro-tensile bond test was performed. Fracture modes were evaluated by stereomicroscopy. Pretreated surfaces and bonded interfaces were characterized by scanning electron microscope (SEM). The data obtained was analyzed by two-way ANOVA (${\alpha}$=0.05). RESULTS. For both sound and eroded dentin, samples treated with desensitizer showed the greatest ${\mu}TBS$, followed by samples without any treatment. And samples treated with $CO_2$ laser showed the lowest ${\mu}TBS$. SEM study indicated that teeth with eroded dentin appeared prone to debonding, as demonstrated by existence of large gaps between adhesive layers and dentin. CONCLUSION. Pretreatment with Gluma increased the ${\mu}TBS$ of Single Bond 2 for eroded and sound teeth. $CO_2$ laser irradiation weakened bond performance for sound teeth but had no effect on eroded teeth.

      • Neurotrophic Artemin Promotes Motility and Invasiveness of MIA PaCa-2 Pancreatic Cancer Cells

        Objective: To analyze the capacity of neurotrophic artemin to promote the motility and invasiveness of MIA PaCa-2 pancreatic cancer cells. Methods: MIA PaCa-2 was cultured in vitro and studied using transwell chambers for motility and invasiveness on treatment with different concentrations of aArtemin or its receptor $GFR{\alpha}3$ were also determined. Expression of matrix metalloproteinase-2 (MMP-2) and epithelial cadherin (E-cadherin) was quantified using RT-PCR and Western blotting. Results: MIA PaCa-2 pancreatic cancer cell motility and invasiveness was significantly increased with artemin and its receptor $GFR{\alpha}3$ with dose dependence (P<0.01). MMP-2 production was also significantly increased (t = 6.35, t = 7.32), while E-cadherin was significantly lowered (t = 4.27, t = 5.61) (P <0.01). Conclusion: Artemin and its receptor $GFR{\alpha}3$ can promote pancreatic cancer cell motility and invasiveness and contribute to aggressive behavior. The mechanism may be related to increased expression of MMP-2 molecule and down-regulation of E-cadherin expression.

        Effect of Monensin and Live Yeast Supplementation on Growth Performance, Nutrient Digestibility, Carcass Characteristics and Ruminal Fermentation Parameters in Lambs Fed Steam-flaked Corn-based Diets

        In performance, digestibility and slaughter trials, a total of forty five male weaned lambs were used to examine the effects of monensin and live yeast supplementations on growth performance, nutrient digestibility, carcass characteristics and ruminal fermentation parameters when the lambs were fed steam-flaked corn-based diets. Animals were allotted to one of three treatment diets in a completely randomized design. The three treatment diets were: (1) basal diet (CON) with steam-flaked corn as a sole grain source, (2) basal diet supplemented with monensin (MO), and (3) basal diet supplemented with live yeast (LY). Total average daily intake (ADI) was unaffected by MO and LY supplementations. LY supplementation increased (p<0.05) average daily gain (ADG) by 13.1% compared with the CON diet. Both MO and LY supplementations resulted in a significant improvement (p<0.05) of feed efficiency over the CON diet (4.47, 4.68 vs. 5.05). Hemicellulose digestibility was higher (p<0.05) for lambs in the LY supplementation group (62.4%) as compared with the CON group (55.7%), but no differences were observed in digestibilities of dry matter (DM), organic matter (OM), crude protein (CP), neutral detergent fiber (NDF) and acid detergent fiber (ADF). All carcass traits were not influenced by dietary supplementations. Ruminal pH in lambs fed the LY supplemental diet was more stable than that with the CON diet (6.57 vs. 6.17). Neither MO nor LY supplementation influenced the concentration of ruminal ammonia-N and total volatile fatty acid (VFA), and molar percentages of individual VFA. Plasma urea-N concentration was decreased (p<0.05) by MO and LY supplementations, while plasma ${\beta}$-hydroxybutyrate (BHBA), glucose and other blood parameters were unaffected. In conclusion, while both MO and LY supplementations had a positive impact on feed efficiency and LY supplementation stabilized ruminal pH and improved fiber utilization, none of the supplements had the capacity to significantly enhance the carcass characteristics.

        Background: The mitochondrial enzyme 3-mercaptopyruvate sulfurtransferase (MPST) is a source of endogenous hydrogen sul.de (H2S), a gaseous signaling molecule implicated in a wide range of physiological processes. The mechanisms of pathogenesis underlying nonalcoholic fatty liver disease (NAFLD) remain unclear. The possible role of MPST in the development of NAFLD has never been investigated. Methods: The NAFLD cell model was established by treating L02 cells with free fatty acid (FFA) overload. A variety of cellular and molecular approaches were used to study the effects of MPST on hepatic steatosis, oxidative stress and inflammation involved in NAFLD. Results: After culturing L02 cells by FFA for 24h, we detected the increased protein level of MPST. MPST knockdown in L02 cells resulted in a marked decrease of lipid accumulation and downregulation of SREBP-1 pathway and melioration of oxdiative stress, embodied in decreased level of H2O2, MDA and IL-6, meanwhile, increased levels of ATP and MMP. Unexpectedly, we observed a significantly increased level of H2S after knockdown of MPST. And the expression of CSE was enhanced when the MPST was decreased. When the level of H2S was decreased, the TG and H2O2 content within FFA-induced hepatocytes were greatly raised. Meanwhile, we demonstrated the reversed expression of SREBP-1/ ACC pathway with the reduced level of H2S. Conclusions: We showed that MPST knockdown could stimulate the compensatory process of CSE, causing the increasing of H2S which is recently considered as a novel antioxidant gas. The increased endogenous H2S could improve hepatocyte steatosis and partly improve the process of oxidative stress and in. ammatory state of steatosis hepatocytes. MPST is implicated in NAFLD via its important H2S metabolism. It provided new insight into the pathogenic mechanisms of NAFLD, pointing to potential target for therapeutic strategy.

      • Expression Characteristics of Proteins of the Insulin-like Growth Factor Axis in Non-small Cell Lung Cancer Patients with Preexisting Type 2 Diabetes Mellitus

        Background: Preexisting type 2 diabetes mellitus (T2DM) affects the prognosis and mortality of patients with some cancers. Insulin like growth factor (IGF) and insulin receptor (IR) signaling axes play important roles in both cancer and diabetes development. We aimed to explore the expression characteristics of proteins in IGF/IR axis in non-small cell lung cancer (NSCLC) cases with preexisting T2DM. Methods: Fifty-five NSCLC patients with preexisting T2DM were retrospectively included and matched by 55 NSCLC without diabetes at a 1:1 ratio. The expression of proteins in IGF/IR axis was detected by immunohistochemical staining. Clinicopathological data were collected to analyze their relationship with the protein expression. Results: Both IGF 1 receptor (IGF-1R) and insulin receptor substrate 2 (IRS-2) showed higher expression in the NSCLC with T2DM group, compared with those without T2DM. The high expression of IGF-1R and IRS-2 were found to be negatively associated with lymph node metastases and T staging in the T2DM group, respectively, and IRS-2 expression was also found more in the subgroup whose T2DM duration was more than 4 years. No difference was detected in the expression of IRS-1, IGF-1, IGF-2, IGFBP3, IR and mTOR between groups with or without T2DM. Conclusion: Our study found higher expression of IGF-1R and IRS-2 proteins in NSCLC patients with preexisting T2DM, and that there was an association with early stage NSCLC, which suggested that IGF signaling may play an important early event in development of NSCLC associated with diabetes.

      • Adopting Text Mining on Rehabilitation Therapy Repositioning for Stroke

        <P>Stroke is a common disabling disease that severely affects the daily life of patients. Accumulating evidence indicates that rehabilitation therapy can improve movement function. However, no clear guidelines have specific and effective rehabilitation therapy schemes, and the development of new rehabilitation techniques has been relatively slow. This study used a text mining approach, the ABC model, to identify an existing rehabilitation candidate therapy method that is most likely to be repositioned for stroke. In the model, we built the internal links of stroke (A), assessment scales (B), and rehabilitation therapies (C) in PubMed and the links were related to upper limb function measurements for patients with stroke. In the first step, using E-utility, we retrieved both stroke-related assessment scales and rehabilitation therapy records and then compiled two datasets, which were called Stroke_Scales and Stroke_Therapies, respectively. In the next step, we crawled all rehabilitation therapies co-occurring with the Stroke_Therapies and then named them as All_Therapies. Therapies that were already included in Stroke_Therapies were deleted from All_Therapies; therefore, the remaining therapies were the potential rehabilitation therapies, which could be repositioned for stroke after subsequent filtration by a manual check. We identified the top-ranked repositioning rehabilitation therapy and subsequently examined its clinical validation. Hand-arm bimanual intensive training (HABIT) was ranked the first in our repositioning rehabilitation therapies and had the most interaction links with Stroke_Scales. HABIT significantly improved clinical scores on assessment scales [Fugl-Meyer Assessment (FMA) and action research arm test (ARAT)] in the clinical validation study for acute stroke patients with upper limb dysfunction. Therefore, based on the ABC model and clinical validation, HABIT is a promising repositioned rehabilitation therapy for stroke, and the ABC model is an effective text mining approach for rehabilitation therapy repositioning. The findings in this study would be helpful in clinical knowledge discovery.</P>

        Clinical Application Value of Lactobacillus Plantarum PS128 in Patients with Anxiety Disorders

        Objective: PS128 is a novel psycho biotic strain, it has been reported to play an important role in neuropsychiatric disorders. This study investigated the clinical effect of PS128 supplementation on patients with anxiety. Methods: A total of 200 patients with anxiety were recruited, and divided into two groups (n = 100/group). The control group received oral treatment with citalopram, and the PS128 group received PS128 capsules based on citalopram treatment. Hamilton Anxiety Scale (HAMA) and Self-Rating Anxiety Scale (SAS) were used to evaluate the anxiety levels. After 2 months of continuous administration, clinical efficacy was evaluated according to HAMA score. Results: There was no significant difference in HAMA and SAS scores between the two groups before treatment. With the treatment prolonged, the HAMA and SAS score decreased gradually in both control and PS128 groups, and the decrease rate of PS128 group was significantly greater than that of the control group. The clinical effective rates of PS128 group were higher than those in the control group, high levels of clinical cure rate were also detected in the PS128 group. Compared with the control group (22%), the incidence of adverse reactions was significantly reduced for patients in the PS128 group (4%). Conclusion: The treatment effect of citalopram combined with PS128 against anxiety is satisfactory clinically. It can greatly improve the anxiety symptoms of patients, increase the cure rate, reduce adverse reactions.
