        Modic Degenerative Marrow Changes in the Thoracic Spine : A Single Center Experience

        Objective : The purposes of this study were to evaluate the prevalence, types, and locations of Modic changes (MCs) in the thoracic spine in a large number of subjects, and to investigate the relation between the distributions of MCs and disc herniations (DHs) in the thoracic spine. Methods : Two experienced musculoskeletal radiologists assessed the presence of MCs and DHs by consensus in the thoracic MRIs of 144 patients with non-specific back pain. Patient ages ranged from 22 to 88 years (mean=$53.3{\pm}14.66$ years), and 72 were female (50%). The prevalence, distribution, relation of MCs and DHs was recorded. Results : MC was observed in 8 of the 144 patients (5.6%) and 10 of 1728 segments (0.58%). The most common MC was type II. Of the 8 patients exhibiting MC, 6 had type II (75.0%), and 2 had mixed MCs (type I/II or type II/III). MCs were distributed mainly at the mid-thoracic level (from T5/6 to T9/10). DH was detected in 18 patients (12.5%), 36 of 1728 segments (2.1%). Of the 10 segments exhibiting MC, 5 had DHs at the same level (50.0%). Accordingly, DH was strongly associated with MC (p=0.000). Conclusion : A low prevalence of MC was observed in the thoracic spine, and type II MC predominated. The low prevalence of MC in the thoracic spine suggests that it was caused by a relative lack of mobility as compared with the cervical and lumbar spines. And DHs were found to be strongly associated with MCs even in the thoracic spine.

        Objective: The aim of this study is to investigate the relationship between motion graphics and sensibilities in the touch display environment, and suggests the design direction of sensibility-based motion graphics in the infotainment environment. Background: With the development of autonomous vehicle technology, various attempts have been made to deliver new sensibilities to users through infotainment system. However, due to the limitations of autonomous vehicle technology, the interface between the infotainment system and the user is limited to the touch display. Method: Motion graphics have been implemented for graphic buttons. For the starting location and pattern of movement, which are design elements that do not exist in the existing design guideline, the relationship between design elements and sensibilities was examined. The experiment was conducted on the sensibility of Fluid, Energetic and Simple that can be generated in the in-vehicle infotainment system. Results: In order to generate the Energetic, the movement should be designed in a radial pattern at the center, and in order to generate the Simple, the movement should be designed without a pattern in the entire area of the button. On the other hand, in the case of the Fluid, it was confirmed that the differentiated Fluid could not be generated through the starting location and pattern of the movement. Conclusion: Sensibility-based motion graphics design guidelines were derived through the starting location and pattern of movement in consideration of the usage context of button motion graphics. Application: The design guidelines might be a useful material for designers who want to design button motion graphics in consideration of sensibility.

        Optogenetics: Solving the Enigma of Sleep

        Sleep is a crucial and evolutionarily conserved phenomenon, but the mechanisms that control sleep-wake behavior and underlie sleep disorders are not yet fully understood. One major challenge for sleep research was the lack of technology that allows for cell-type- and circuit-specific investigation of neurons and neural circuitry. A decade ago, a novel methodology known as optogenetics was developed, which uses light to control specific cell types of neurons, either to activate or inhibit neuronal firings. The strength of optogenetics in neuroscience is the precise control of neuronal activities in millisecond scale and the ability to dissect the neural circuits and cell types to understand their functions. There have been substantial advancements made in the field of sleep research through the implementation of optogenetics. This review provides a brief introduction on the optogenetics and a consolidated summary of recent findings published in sleep research using optogenetics.

        Personal experience with microvascular decompression and partial sensory rhizotomy for trigeminal neuralgia

        Background: Trigeminal neuralgia (TN) is a severe, paroxysmal pain in the distribution of the fifth cranial nerve. Microvascular decompression (MVD) is the most widely used surgical treatment for TN. We undertook this study to analyze the effects of and complications of MVD and to refine the surgical procedure for treating TN. Methods: A total of 88 patients underwent for TN underwent surgery at our hospital. Among them, 77 patients underwent MVD alone, and 11 underwent partial sensory rhizotomy (PSR) with or without MVD. The medical records of these patients were retrospectively analyzed for patient characteristics, clinical results, offending vessels, and complications if any. Results: The mean follow-up duration was 43.2 months (range, 3-216 months). The most common site of pain was V2+V3 territory (n=27), followed by V2 (n=25) and V3 (n=23). The most common offending vessels were the superior cerebellar artery and anterior inferior cerebellar artery in that order. The overall rate of postoperative complications was 46.1%; however, most complications were transient. There were two cases of permanent partial hearing disturbance. In the MVD alone group, the cure rate was 67.5%, and the improvement rate was 26.0%. Among 11 patients who underwent PSR with or without MVD, the cure rate was 50.0%, and the improvement rate was 30.0%. Conclusion: The clinical results of MVD were satisfactory. Although the outcomes of PSR were not as favorable as those of pure MVD in this study, PSR can be considered in cases where there is no significant vascular compressive lesion or uncertainty of the causative vessel at the surgery.

        앤디 워홀 (Andy Warhol) 의 작품세계와 패션계에 미친 영향 (I)

        Andy Warhol is one of the great artists in the modern history of art who compares favorably with Picasso. His activities had his object on pervasion of a pop art and profit of the art industry. Many previous studies on his activities have attracted and held the attention of his works of art after 1960s. However, in spite of his working as publisher of a fashion magazine, fashion illustrator, fashion model, displayer, and etc. in 1950s, there are few studies on his activities of 1950s. Hence, this study investigates his works of art in 1950s and intensively analyzes his effect on modern fashion. In 1950s, Andy Warhol had already been distinguished in works of art from other commercial designers through the illustration for advertising design, window display of stores, and usage of Hollywood stars as an essential element in his works for whom he had sighed when he was young. His works were also characterized by a rejection of the originality of commercial arts. He had an great effect on youth`s fashion in 1950s and 1960s, Their fashion style and images, at that time, were progressive and opposite to a mainstream of pop cultures, which caused industry manufacturers and consumers to understand their subculture. As a result, it is concluded that Andy Warhol took the lead in the fashion in 1950s and his activities in 1950s had influence on the diffusion of the public punk fashion in 1970s and the street fashion in 1980s.

        Successful Motor Evoked Potential Monitoring in Cervical Myelopathy : Related Factors and the Effect of Increased Stimulation Intensity

        Objective : Intraoperative neurophysiological monitoring (IONM) has been widely used during spine surgery to reduce or prevent neurologic deficits, however, its application to the surgical management for cervical myelopathy remains controversial. This study aimed to assess the success rate of IONM in patients with cervical myelopathy and to investigate the factors associated with successful baseline monitoring and the effect of increasing the stimulation intensity by focusing on motor evoked potentials (MEPs). Methods : The data of 88 patients who underwent surgery for cervical myelopathy with IONM between January 2016 and June 2018 were retrospectively reviewed. The success rate of baseline MEP monitoring at the initial stimulation of 400 V was investigated. In unmonitorable cases, the stimulation intensity was increased to 999 V, and the success rate final MEP monitoring was reinvestigated. In addition, factors related to the success rate of baseline MEP monitoring were investigated using independent t-test, Wilcoxon rank-sum test, chi-squared test, and Fisher's exact probability test for statistical analysis. The factors included age, sex, body mass index, diabetes mellitus, smoking history, symptom duration, Torg-Pavlov ratio, space available for the cord (SAC), cord compression ratio (CCR), intramedullary increased signal intensity (SI) on magnetic resonance imaging, SI length, SI ratio, the Medical Research Council (MRC) grade, the preoperative modified Nurick grade and Japanese Orthopedic Association (JOA) score. Results : The overall success rate for reliable MEP response was 52.3% after increasing the stimulation intensity. No complications were observed to be associated with increased intensity. The factors related to the success rate of final MEP monitoring were found to be SAC (p<0.001), CCR (p<0.001), MRC grade (p<0.001), preoperative modified Nurick grade (p<0.001), and JOA score (p<0.001). The cut-off score for successful MEP monitoring was 5.67 mm for SAC, 47.33% for the CCR, 3 points for MRC grade, 2 points for the modified Nurick grade, and 12 points for the JOA score. Conclusion : Increasing the stimulation intensity could significantly improve the success rate of baseline MEP monitoring for unmonitorable cases at the initial stimulation in cervical myelopathy. In particular, the SAC, CCR, MRC grade, preoperative Nurick grade and JOA score may be considered as the more important related factors associated with the success rate of MEP monitoring. Therefore, the degree of preoperative neurological functional deficits and the presence of spinal cord compression on imaging could be used as new detailed criteria for the application of IONM in patients with cervical myelopathy.

      • Immune-enhancing Oral supplement after Gastric Cancer Surgery

        Objectives: The aim of this study was to evaluate the effect of immune-enhancing oral supplement including glutamine, arginine, and leucine on the immunological change after gastric cancer surgery. Methods: Patients undergoing subtotal gastrectomy were randomized to control and study group. Study group received immune-enhancing supplement including glutamine, arginine, and leucine (Curact®) and control group received placebo supplement including same calories dextrose. Postoperative supplements were started at the postoperative day (POD) 7th and continued for 3 weeks after discharge. Immunological data were collected included count of White blood cell (WBC), C-reactive protein (CRP), immunoglobulin (Ig), CD4, and CD8 at preoperative, POD 5th and POD 28th, respectively. Nutritional data were collected included albumin, prealbumin, cholesterol, and patient-generated subjective global assessment (PG-SGA). Results: 60 patients were randomized, of whom 15 patients were dropped and 45 patients were analyzed (control 23 patients, study 22 patients). Characteristics, nutrition and immune related factors at preoperative and POD 5th were not different between two groups. At POD 28th, mean CRP and count of WBC were significantly lower in study group, and mean level of Ig M was significantly higher in study group than the control group (137.9 vs. 108.7, p=0.021). Ig G, CD4 and CD8 tended to be preserved in the study group, however, there was no statistical difference between two groups. PG-SGA became significantly worse after surgery in both group, however there was no difference between two groups. Conclusions: Immune-enhancing oral supplement might have positive effect on immune preservation and decreasing inflammation after gastric cancer surgery.
