Background: This study investigated the impact of premorbid functional dependency on post-stroke mortality in patients with anterior circulation stroke (ACS) and posterior circulation stroke (PCS).Methods: This study enrolled 9,698 patients who experienced ischemic stroke between January 2011 and December 2022. The patients were classified into the ACS and PCS groups. Premorbid functional dependency was defined as modified Rankin Scale of ≥3. The risks of premorbid functional dependency and mortality at 3 months and 1-year post-stroke were assessed. A subgroup analysis was further performed to evaluate the risk of premorbid functional dependency in patients who underwent intravenous thrombolysis and endovascular treatment (EVT).Results: Among 6,358 patients with ACS and 3,340 with PCS, those with premorbid dependency were older, predominantly female, and had a higher proportion of vascular risk factors and stroke severity. Premorbid functional dependency was associated with increased mortality at both 3 months and 1 year in the PCS (odds ratio [OR], 1.79; 95% confidence interval [CI], 1.00–3.13; P=0.04 and OR, 2.87; 95% CI, 1.86–4.38; P<0.001, respectively), but not in the ACS (OR, 1.08; 95% CI, 0.77–1.51; P=0.639 and OR, 1.22; 95% CI, 0.93–1.59; P=0.140, respectively) group. Among patients who underwent EVT, premorbid functional dependency increased the risk of mortality at 1 year in the ACS group (OR, 1.80; 95% CI, 1.04–3.08; P=0.034), but was not associated with the risk in the PCS group (OR 2.56; 95% CI 0.64–10.15; P=0.176).Conclusion: Premorbid functional dependency increases the risk of mortality in patients with PCS.

<P>The texture development and microstructural evolution have been investigated for the AZ31 alloys rolled by cross-roll rolling at 823 K. After the AZ31 magnesium sheets were rolled by cross-roll rolling at 823 K, the total reduction in thickness of 65% were achieved after two pass rolling. The resultant microstructures are homogeneous and refined for the surface and center layers. The (0002) basal texture is significantly weakened after annealing at 623 K for 1 h for the cross-roll rolled sheets.</P>
The wave energy scattering by interaction with the refracted breakwater and varying trench depth

<P> The purpose of this study is to estimate the wave height located at the front face of the offshore refracted breakwater when the trench is dredged in the exterior region of the breakwater at various depths. The problems that arise with regard to the different obliquely incident wave field involve having two-dimensional planes and having the configuration of the trench region designated by a single horizontal rectangular system. The numerical approach involves applying the Green function based on the boundary integral approach. The results of the present numerical works are illustrated. The ratio of the wave height reduction at the front face of the refracted breakwater is shown to be more than 25% due to the trench’s effect on the seabed. The corner of the refracted breakwater is where the wave energy is concentrated, and through utilizing the trench, the wave energy can be subdued to reduce the damage on the breakwater. The trench brings about a reduction in wave amplitude and wave energy in, around, and directly downstream of the trench. </P>
Atf3 induction is a therapeutic target for obesity and metabolic diseases

<P><B>Abstract</B></P> <P>Activating transcription factor 3 (<I>Atf3</I>) has been previously demonstrated to impact obesity and metabolism. However, a metabolic role of Atf3 in mice remains debatable. We investigated the role of Atf3 in mice and further investigated Atf3 expression as a therapeutic target for obesity and metabolic diseases. Atf3 knockout (KO) mice fed with a high fat diet (HFD) aggravated weight gain and impaired glucose metabolism compared to littermate control wild type (WT) mice. Atf3 KO aged mice fed with a chow diet (CD) for longer than 10 months also displayed increased body weight and fat mass compared to WT aged mice. We also assessed requirements of Atf3 in a phytochemical mediated anti-obese effect. Effect of sulfuretin, a previously known phytochemical Atf3 inducer, in counteracting weight gain and improving glucose tolerance was almost completely abolished in the absence of Atf3, indicating that Atf3 induction can be a molecular target for preventing obesity and metabolic diseases. We further identified other Atf3 small molecule inducers that exhibit inhibitory effects on lipid accumulation in adipocytes. These data highlight the role of Atf3 in obesity and further suggest the use of chemical Atf3 inducers for prevention of obesity and metabolic diseases.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Atf3 knockout mice increases body weight and impaired glucose tolerance in high fat diet fed mice. </LI> <LI> Atf3 knockout mice increases body weight in chow diet fed aged mice. </LI> <LI> Sulfuretin, a phytochemical Atf3 iducer, requires Atf3 expression to prevent obesity. </LI> </UL> </P>

Purpose. The purpose of this study was to compare the type and frequency of prosthetic complications associated with attachment types for implant overdenture. Material and methods. In this retrospective study, 38 patients (mean age, 63.5 years) have been treated with implant overdentures from 2007 to 2014. Ten patients received a bar-clip attachment. Eleven patients had received a milled bar with Locator attachment. Seventeen patients had received a Locator attachment. The mean follow-up period was 36.9 months (range, 15-83 months). The type and frequency of prosthetic complications was recorded. The frequency was analyzed to determine the statistical difference among the 3 different attachments by using one-way ANOVA (α = .05) and Bonferroni post hoc method at a 5% level of significance. Results. The total number of prosthetic complications was higher in the bar-clip attachment (55 events) than in the milled bar with Locator attachment (39 events) and the Locator attachment (34 events). There were no statistically significant differences, and the most common prosthetic complication was the loss of retention. In the bar-clip attachment group, the average frequency of prosthetic complications was 3.0 events per prosthesis during the first year. In the milled bar with Locator attachment and Locator attachment groups, the average frequencies were 1.45 events and 2.35 events, respectively. Statistically significant differences were observed in the frequency of the complication. (p = .043) Conclusions. Compared to the bar-clip attachment, implant overdentures that use milled bars with the Locator attachment have a significantly lower incidence of prosthetic complications in the first year of follow-up after placement.
