        Hyun Soo Kim, Jong Woon Cheon, Min Su Kim, Chang Kil Jung, Kyung Rok Kim, Jae Won Choi, Dong Woo Kang and Sun Young Kim1 Departments of Internal Medicine and Pathology1, DongKang Medical Center, Ulsan, Korea Actinomycosis is a chronic suppurative disease and caused by Actinomycosis species, principally Actinomyces israelii, which are part of the normal inhabitant on the mucous membrane of the oropharynx, gastrointestinal tract, and urogenital tract. It usually affects cervicofacial, thoracic and abdominal tissue. Cervicofacial type has the highest percentage of occurrence with 50%. Actinomycosis frequently occurs following dental extraction, jaw surgery, chronic infection or poor oral hygiene. It may also be considered as an opportunistic infection in immunocompromised patients such as malignancy, human immunodeficiency virus infection, diabetes mellitus, steroid usage or alcoholism. But, actinomycosis rarely occurs in adults with normal immunity and rare in the esophagus. We report an unusual case of esophageal actinomycosis which was developed in a patient with normal immunity and improved by therapy with intravenous penicillin G followed oral amoxicillin, and we also reviewed the associated literature.

        강변여과에서 파일럿규모 수평집수관의 수두분포 연구

        강변여과용 수평집수관을 모사하는 파일럿 규모의 모래통 실험을 수행하였으며, 다양한 출구유속에 대한 수평집수관에서의 수두분포를 구하였다. Kim24)이 수행한 실험실 규모의 수평집수관에서의 손실수두에 Kim25)이 개발한 이론 즉, 작은 규모의 수평집수관에서의 손실수두로부터 큰 집수관의 손실수두를 계산하는 방법을 적용하여 본 연구의 관측치와 비교함으로써 Kim25)의 이론이 타당함을 알 수 있었다. 또한, 이들의 이론을 적용하여 본 연구에서의 관측치로부터 실규모 집수관에서의 수두분포를 구하는 방안을 예를 들어 설명함으로써 강변여과의 수평집수관 설계방법을 제시하였다. A pilot-scale sand-box experiment simulating a lateral of collector well used in riverbank filtration was performed, and hydraulic head distributions along the lateral were measured according to the various drawdowns at the well. The results of this experiment were compared to the predicted values obtained using a theory previously developed by Kim25) in order for its validation. This theory predicts the head loss in a large-scale lateral given the loss in a small-scale one, and to have actual values for comparison, the results from a lab-scale experiment previously performed by Kim24) were employed. Comparing the experimental values to the extrapolated value indeed confirmed the validity of the theory. A procedure to determine the hydraulic head distribution of a practical-scale lateral was also presented applying the theory to the experimental result of this study in an effort to show the process of lateral design for riverbank filtration.

        Protective effect of Korean Red Ginseng against FK506-induced damage in LLC-PK1 cells

        Background: Compound FK506 is an immunosuppressant agent that is frequently used to prevent rejection of solid organs upon transplant. However, nephrotoxicity due to apoptosis and inflammatory response mediated by FK506 limit its usefulness. In this study, the protective effect of Korean Red Ginseng (KRG) against FK506-induced damage in LLC-PK1 pig kidney epithelial cells was investigated. Methods: LLC-PK1 cells were exposed to FK506 with KRG and cell viability was measured. Western blotting and RT-PCR analyses evaluated protein expression of MAPKs, caspase-3, and KIM-1. TLR-4 gene expression was assessed. Caspase-3 activities were also determined. The number of apoptotic cells was measured using an image-based cytometric assay. Results: The reduction in LLC-PK1 cell viability by 60mM FK506 was recovered by KRG cotreatment in a dose-dependent manner. The phosphorylation of p38, p44/42 MAPKs (ERK), KIM-1, cleaved caspase-3, and TLR-4 mRNA expression was increased markedly in LLC-PK1 cells treated with 60mM FK506. However, with the exception of p-ERK, elevated levels of p-p38, KIM-1, cleaved caspase-3, and TLR-4 mRNA expression were significantly decreased after cotreatment with KRG. Activity level of caspase-3 was also attenuated by KRG cotreatment. Moreover, image-based cytometric assay showed that apoptotic cell death was increased by 60mM FK506 treatment, whereas it was decreased after cotreatment with KRG. Conclusion: Taken together, these results suggest that the molecular mechanism of KRG in the FK506-induced nephrotoxicity may lead to the development of an adjuvant for the inhibition of adverse effect FK506 in the kidney.

      • 전해산성수를 이용한 새로운 내시경 소독 시스템(Cleantop WM-1??)의 소독효율

        Background : Electrolyzed acid water (EAW) is a new liquid chemical germicide containing a mixture of oxidizing species and prepared by mixing a small amount of salt with tap water in an electrolyzer. We attempted an in-use evaluation of the high-level disinfection capability of Cleantop WM-1□, a new endoscope reprocessing system using EAW. Methods : Forty-seven flexible endoscopes were randomly collected just after upper endoscopic examinations and were disinfected using Cleantop WM-1□. Parts of the endoscopes without contact to EAW were disinfected with 75% ethyl alcohol. Immediately after the disinfection procedure, samples were taken from the biopsy channel (S-1), tip of the insertion tube (S-2), the umbilical cord (S-3), and the angulation knob (S-4). Results : In EAW-disinfected parts of the endoscopes, the culture-positive rates were 4.3% (2/47) in S-1 samples and 12.8% (6/47) in S-2 samples. In ethyl alcohol-disinfected area, the culture-positive rates were 2.1% (1/47) in S-3 samples and 25.5% (12/47) in S-4 samples. The colony counts of culture-positive samples ranged from 1 to 144. Pseudomonas aerugi-nasa was recovered from the angulation knob of an endoscope, but other contaminating organisms were mostly : normal flora or opportunistic pathogens. Conclusion : Although part of the endoscopes disinfected with 75% ethyl alcohol were contaminated with more organisms than EAW-disinfected areas, Cleantop WM-1□ showed a relatively good disinfection efficacy in reprocessing patient-used endoscopes.       • 영상 복원을 위한 Wiener 필터의 설계

        The goal of restoration techniques is to improve a given image in some sense. We consider restoration to be a process that attempts to recover an image that has been degraded by using some priori knowledge of the degradation phenomenon. The restoration of images distorted by systems with noisy point spread function and additive detection noise is considered. The criterion considered for the restoration is based on the wiener filter. The overall computation of the Wiener filter can be carried out in the frequency domain using the FFT. The Wiener filer gives better restoration result.

        극저준위 감마선 분광시스템의 백그라운드 저감화를 위한 중성자 차폐체 설계

        극저준위 방사능측정시스템의 백그라운드에 영향을 주는 중성자를 차폐하기 위한 차폐체를 설계하였다. 중성자 차폐방법은 고 밀도 폴리에틸렌을 이용하여 고속중성자를 감속한 후 B4C를 이용하여 감속된 열중성자를 흡수하는 방법을 이용하였다. 몬테카를로 모사방법인 MCNP4B 코드를 이용하여 계산한 결과 고 밀도 폴리에틸렌의 두께가 10 cm 일 때 열중성자속이 최대가 되는 것으로 나타났으며 감속된 중성자의 흡수는 용제에 자연상태의 B4C 분말을 30 w% 섞을 경우 2 mm의 두께에서 94%의 중성자 흡수가 일어나는 것으로 나타났다. 또한 몬테카를로 모사를 통한 계산결과의 타당성 여부를 조사하기 위하여 중성자 차폐실험 장치를 제작하여 실험 결과와 비교하였으며, 비교 결과 실험값과 일치하는 것으로 나타났다 In order to shield the neutrons affecting the background of Low Level Gamma Ray Spectrometer, a neutron shielder was designed. The method used in this study for neutron shielding was the deceleration of fast neutrons by high density polyethylene(HDPE) and the absorption of those slowing-down neutrons by B4C. The calculation results of neutron interaction in HDPE using Monte Carlo simulation code MCNP4B showed that the thermal-neutron flux was maximum at 10 cm thickness of HDPE. The results also showed that 95% of the thermal neutrons were absorbed by 2 mm thickness of B4C absorber consisted of 30 w% B4C and 70 w% polymer. The results of the Monte Carlo calculation were in good agreement with the experimental value obtained by a neutron shielding apparatus designd for this purpose.

      • 2,4,6-Collidinium Dichromate에 의한 알코올의 산화 특성 연구

        2, 4, 6-Collidinium dichromate(CDC) is more, stable. nonhygroscopic, and milder than other chromium(VI)oxidants. The new oxidant was characterized via rate of oxidation. CDC with pyridinium P-toluenesulfonate(PPTS) in dichloromethane was effective in oxidation of allylic, benzylic, and secondary alcohols to the corresponding carbonyl compounds at room temperature. while CDC alone did not oxidize above alcohols. Oxidation of ally lie and benzylic alcohols progressed more repididly than that of secondary alcohols. When molecular sieve(MS)was added to the CDC-PPTS system, rate of oxidation and yields increased. Saturated aliphatic primary alcohols were decomposed in two system, CDC-PPTS and CDC-PPTS system added MS. The oxidation selectivity of CDC-PPTS system was increased about 1.75 times than that of PDC-PPTS system.

      • 약물전달체를 위한 폴리에틸렌글리콜과 폴리카프로락톤을 이용한 나노입자

        The self-aggregation behavior and microscopic characteristics of the PEG-b-PCL copolymer self-aggregates, prepared by solvent evaporation method, were investigated by `H NMR, dynamic light scattering (DLS), and fluorescence spectroscopy. The PEG-PCL block copolymer formed nano-sized self-aggregates in an aqueous environment by intraand/or intermolecular association between hydrophobic PCL chains. Then, drug loading and its release behavior of clonazepam loaded PEG-b-PCL nanoparticles were investigated. The result revealed that hydrophobic-hydrophilic balances played a crucial role for drug loading and its release behaviors.
