      • Lymph Node Ratio is More Predictive than Traditional Lymph Node Stratification in Lymph Node Positive Invasive Breast Cancer

        Objective: To evaluate the relationships between lymph node ratio (LNR, the ratio of positive lymph nodes in excised axillary lymph nodes) and disease-free survival (DFS) by comparing with traditional absolute positive lymph node number (pN classification) for prediction of breast cancer (BC) progrnosis. Methods and Patients: We retrospectively reviewed patients who received comprehensive therapy in Department of Breast Surgery, Hubei Cancer Hospital, China from Jan 2002 to Dec 2006 (Group A), and Department of Breast and Thyroid Surgery, Renmin Hospital of Wuhan University, China from Jun 2008 to May 2012 (Group B). Patients were allocated to low-risk (${\leq}0.20$), intermediate-risk (> 0.20 but ${\leq}0.65$), high-risk (>0.65) groups by LNR. The primary endpoint was 5-DFS. Results: A total of 294 patients were included in our study. LNR was verified as a negative prognostic factor for DFS (P=0.002 in Group A, P<0.0001 in Group B). Then we found the effects of pN and LNR delamination on disease-free survival (DFS) had statistical significance (P=0.012 for pN and P=0.031 for LNR stratification in Group A, both of them P<0.001 in Group B). Compared to pN staging, LNR staging displayed superior performance in prognosis, the adjusted hazard ratio of recurrence being 2.07 (95%CI, 1.07 to 4.0) for intermediate risk group (P=0.030) and 2.44 (95%CI, 1.21 to 4.92) for high risk group (P=0.013) in Group A. Conclusions: LNR stratification proved an adverse prognostic factor of DFS in lymph nodes positive invasive BC using cut-off values 0.20 and 0.65, and was more predictive than traditional pN classification for 5-DFS.

        A practical method for simultaneous determination of Poisson’s ratio and Young’s modulus of elasticity of thin films

        In this paper, based on the exact analytical solution of axisymmetric deformation of the circular membrane fixed at its edge under the action of uniformly-distributed loads, we propose a new method to be able to simultaneously determine Poisson's ratio and Young's modulus of elasticity for thin films. We also present a set of exact formulas used for simultaneously determining Poisson's ratio and Young's modulus of elasticity for free-standing thin films or coating thin films without residual stresses or pre-tension. This method has the advantages of simplicity and reliability, and it doesn’t involve the substrate effect.

        Power series solution of circular membrane under uniformly distributed loads: investigation into Hencky transformation

        In this paper, the problem of axisymmetric deformation of the circular membrane fixed at its perimeter under the action of uniformly-distributed loads was resolved by exactly using power series method, and the solution of the problem was presented. An investigation into the so-called Hencky transformation was carried out, based on the solution presented here. The results obtained here indicate that the well-known Hencky solution is, without doubt, correct, but in the published papers the statement about its derivation is incorrect, and the so-called Hencky transformation is invalid and hence may not be extended to use as a general mathematical method.

        Closed-form solution of axisymmetric deformation of prestressed Föppl-Hencky membrane under constrained deflecting

        In this study, the problem of axisymmetric deformation of prestressed Föppl-Hencky membrane under constrained deflecting was analytically solved and its closed-form solution was presented. The small-rotation-angle assumption usually adopted in membrane problems was given up, and the initial stress in membrane was taken into account. Consequently, this closed-form solution has higher calculation accuracy and can be applied for a wider range in comparison with the existing approximate solution. The presented numerical examples demonstrate the validity of the closed-form solution, and show the errors of the contact radius, profile and radial stress of membrane in the existing approximate solution brought by the small-rotation-angle assumption. Moreover, the influence of the initial stress on the contact radius is also discussed based on the numerical examples.

        Using Ultrasonography Measurements to Determine the Depth of the GB 21 Acupoint to Prevent Pneumothorax

        Introductions: Pneumothorax is the most frequent severe acupuncture-related adverse event occurring at the GB 21 acupoint. This study used ultrasonography (US) to measure the depth of the GB 21 acupoint in adults and assessed its applicability for ensuring safe acupuncture practices. Materials and methods: A US probe was used to measure the vertical depth from the skin to the pleural line of the apex of the lungs at the right and left GB 21 acupoints. The sex, age, body weight, height, and body mass index of each person were recorded. Student t test and generalized estimating equations were used for statistical analysis. Results: Sixty women and 41 men with a median age of 29 years were included in this study. The depth of GB 21 increased with body weight, height, and body mass index (p < 0.001). The mean depth of GB 21 in men (17.4 mm) was greater than that in women (14.6 mm;p < 0.001). The depth difference between the right and left shoulders was significant in men (p < 0.001) but not in women. Conclusions: US can be used to measure GB 21 acupoint depth. Further control studies on its use to ensure safe clinical practices, including preventing acupuncture-related pneumothorax, are warranted.

        Analytical and Numerical Analysis on a New Type of Bolted Connection for Modular Steel Construction

        Modular steel construction (MSC) has exhibited widespread applications in civil engineering. The inter-module connection is a key issue for the seismic performance of MSC. A detailed finite element model (FEM) of a new type of bolted connection was firstly developed. Its accuracy was validated by comparing numerical results with previous experimental results in terms of hysteretic curve and failure mode. A theoretical mechanical model was developed and its capabilities on predictions of initial rotational stiffness and ultimate moment resistance of the connection were checked through validations against test results. Further, a parametric study on several specimens with different axial load ratios, boundary conditions and section of the beams was conducted. The prediction capability of the theoretical model on seismic behavior of the connection with different beam sections was checked by comparing the skeleton curves of FEA results with theoretical ones. Subsequently, a simplified FEM was established and validated to accelerate the numerical simulation of the seismic performance of the connection.

        An analytical solution of bending thin plates with different moduli in tension and compression

        Materials which exhibit different elastic moduli in tension and compression are known as bimodular materials. The bimodular materials model, which is founded on the criterion of positivenegative signs of principal stress, is important for the structural analysis and design. However, due to the inherent complexity of the constitutive relation, it is difficult to obtain an analytical solution of a bimodular bending components except in particular simple problems. Based on the existent simplified model, this paper solves analytically bending thin plates with different moduli in tension and compression. By using the continuity conditions of stress components in unknown neutral layer, we determine the location of the neutral layer, and derive the governing differential equation for deflection, the flexural rigidity, and the internal forces in the thin plate. We also use a circular thin plate with bimodulus to illustrate the application of this solution derived in this paper. The results show that the introduction of different moduli has influences on the flexural stiffness of the bending thin plate.

      • Virtual Non-Contrast Computer Tomography (CT) with Spectral CT as an Alternative to Conventional Unenhanced CT in the Assessment of Gastric Cancer

        Objective: The purpose of this study was to evaluate computed tomography (CT) virtual non-contrast (VNC) spectral imaging for gastric carcinoma. Materials and Methods: Fifty-two patients with histologically proven gastric carcinomas underwent gemstone spectral imaging (GSI) including non-contrast and contrast-enhanced hepatic arterial, portal venous, and equilibrium phase acquisitions prior to surgery. VNC arterial phase (VNCa), VNC venous phase (VNCv), and VNC equilibrium phase (VNCe) images were obtained by subtracting iodine from iodine/water images. Images were analyzed with respect to image quality, gastric carcinoma-intragastric water contrast-to-noise ratio (CNR), gastric carcinoma-perigastric fat CNR, serosal invasion, and enlarged lymph nodes around the lesions. Results: Carcinoma-water CNR values were significantly higher in VNCa, VNCv, and VNCe images than in normal CT images (2.72, 2.60, 2.61, respectively, vs 2.35, $p{\leq}0.008$). Carcinoma-perigastric fat CNR values were significantly lower in VNCa, VNCv, and VNCe images than in normal CT images (7.63, 7.49, 7.32, respectively, vs 8.48, p< 0.001). There were no significant differences of carcinoma-water CNR and carcinoma-perigastric fat CNR among VNCa, VNCv, and VNCe images. There was no difference in the determination of invasion or enlarged lymph nodes between normal CT and VNCa images. Conclusions: VNC arterial phase images may be a surrogate for conventional non-contrast CT images in gastric carcinoma evaluation.
