      • Impact of Interferon-Based Treatment on Quality of Life and Work Related Productivity from the Korean Cohort in the MOSAIC Study

        Aims: Chronic Hepatitis C Virus (HCV) infection increases the risk for progressive liver disease, hepatocellular carcinoma and negatively impacts the patient’s quality of life. HCV treatment is evolving with direct acting antivirals but IFN based therapy has been the standard of care for many years and remains available in some countries. The MOSAIC study aims to characterize patients with chronic HCV infection and assess the impact of IFN-containing treatment on health-related quality of life, work related productivity and health care utilization. Methods: MOSAIC is an international prospective multicenter observational study that has been conducted in 20 countries. Consecutive patients with chronic HCV infection were enrolled and those who initiated an IFN based regimen were prospectively followed for 48 weeks. We report results from the Korean cohort Results: 100 patients were enrolled: 86 were treatment naïve and 14 were treatment experienced. 33 patients initiated an IFN based regimen: 6 patients started IFN + RBV, 26 patients started Peg-IFN + RBV, none started Peg-IFN + RBV + DAA and 1 patient received other treatment. Among the treated cohort, demographic and disease characteristics were the following: the mean age was 54.5 years; 14 patients were male. 14 had minimal or no fibrosis, 2 portal fibrosis, 3 bridging fibrosis and 6 patients suffered from cirrhosis. HCV Genotype distribution was as follows: genotype 1: 11; genotype 2: 19 and genotype 3: 3. Table 1 describes the results at baseline and changes over 4, 12 and 48 weeks and end-of-treatment (EOT) for the quality of life and work productivity outcome measures (EQ-5D-5L, HCV-PRO and WPAI). Conclusions: Results from the Korean cohort of the MOSAIC study show a moderate trend for deterioration of health-related quality of life and work productivity associated with IFN based treatment for patients with chronic HCV infection during treatment period.         Epidural analgesia versus intravenous analgesia after minimally invasive repair of pectus excavatum in pediatric patients: a systematic review and meta-analysis

        Background: Postoperative pain control after the minimally invasive repair of pectus excavatum (MIRPE) is essential, but there is a controversy about a better analgesic method between epidural and intravenous (IV) analgesia. This systematic review and meta-analysis aimed to compare the effect of epidural versus IV analgesia following MIRPE. Methods: We searched PubMed, MEDLINE, Embase, Cochrane Central Register, and ClinicalTrials.gov for randomized controlled trials (RCTs) dated up to 31st May 2021. The primary outcome was the area under the curve (AUC) of the weighted mean visual analog scale (VAS) after MIRPE. The secondary outcomes were postoperative nausea, operation time, total operating room time, and postoperative length of hospital stay. Results: Four RCTs involving 243 patients were finally included in this meta-analysis. The AUC of the weighted mean VAS was 343.62 in the epidural group and 375.24 in the IV group. The epidural group showed lower VAS than the IV group at 12 to 48 h after the surgery. Postoperative nausea, operation time and length of hospital stay was not different between two groups. The epidural group had a significantly longer total operating room time due to epidural catheter insertion time. Conclusions: Epidural analgesia after the MIRPE had a better analgesic effect than IV analgesia. However, IV analgesia may also be a viable option, and physicians should wisely choose analgesic modalities after MIRPE.

        TSLP Polymorphisms in Atopic Dermatitis and Atopic March in Koreans

        Background: Atopic march (AM) is the progression from atopic dermatitis (AD) to allergic rhinitis and asthma. The development of AD is as high as 20% in children worldwide and continues to increase. AD seems to be caused by both genetic and environmental factors. Recently, polymorphisms of the thymic stromal lymphopoietin (TSLP) gene associated with allergic disorders were reported in ethnic groups from various countries. Objective: Identification of TSLP polymorphisms in Koreans with AD or AM. Methods: Wholeexome sequencing was performed in 20 AD and 20 AM patients. Results: Nine single nucleotide polymorphisms (SNPs) of TSLP were detected (rs191607411, rs3806933, rs2289276, rs2289277, rs2289278, rs139817258, rs11466749, rs11466750, rs10073816). These SNPs have been correlated with susceptibility to allergic diseases in ethnic groups from China, Japan, Turkey, and Costa Rica in previous studies. Remarkably, one of 20 patients in the AD group lacked all SNPs, compared to six of 20 patients in the AM group. Odds ratios showed that Korean patients without the nine TSLP variants had an 8.14 times higher risk of moving from AD to AM. Two haplotype blocks were validated in 60 AD and 59 AM patients using Sanger sequencing. The haplotype blocks (rs3806933 and rs2289276) and (rs11466749 and rs11466750) were in high linkage disequilibrium, re-spectively (D’=0.97, D’=1). Conclusion: The increase of major allele frequency of respective nine TSLP variants may enhance the risk of AM. These data will contribute to improved genetic surveillance system in the early diagnosis technology of allergic disease. (Ann Dermatol 30(5) 529∼ 535, 2018)

        Influence of Transmucosal Designs of Dental Implant on Tissue Regeneration in Beagle Dogs

        This study was conducted to evaluate the influence of concave machined and concave microgrooved profiles of the transmucosal portion of implants on early peri-implant tissue regeneration. We assessed implants with 3 different transmucosal profiles: the bevel group that had only a bevel in the transmucosal part, bioseal group and bioseal/ groove group that had a micro-groove in the S-shaped portion of the implant. A total of 36 implants (12 of each type) were placed in the edentulous mandibular ridges of 6 beagle dogs. We used radiographs to investigate the mesio-distal change of the marginal bone. Bucco-lingual bone resorption and soft tissue reactions were evaluated histologically. Radiographic and histological analysis did not show any difference in mesio-distal or bucco-lingual marginal bone resorption among any of the 3 groups (p > 0.05). The bioseal and bioseal/groove groups had more rigid connective tissue attachment than that in the bevel group. However, the bevel group had significantly long junctional epithelium attachment and the bioseal and bioseal/groove groups had long connective tissue attachment (p < 0.05). Implants with S-shaped concavity of the transmucosal portion resulted in a firmer connective tissue barrier, and thus, a better soft tissue regeneration, than implants with straight bevel.

        Association of DOCK8, IL17RA, and KLK12 Polymorphisms with Atopic Dermatitis in Koreans

        Background: Early-onset and severe atopic dermatitis (AD) in patients increase the probability of the development of allergic rhinitis or asthma. Treatment and prevention strategies in infants and young children with AD are targeted toward treating the symptoms, restoring skin barrier functions, and reducing the absorption of environmental allergens in an attempt to attenuate or block the onset of asthma and food allergy. Objective: Given that the initiating events in AD remain poorly understood, identifying those at risk and implementing strategies to prevent AD is necessary. Methods: Whole-exome sequencing (WES) was performed in a 43 control group and a disease group with 20 AD patients without atopic march (AM) and 20 with AM. Sanger sequencing was carried out to validate found variants in cohorts. Results: DOCK8, IL17RA, and KLK12 single-nucleotide polymorphisms were identified by WES as missense mutations: c.1289C>A, p.P97T (rs529208); c.1685C>A, p.P562G (rs12484684); and c.457+27>C, rs3745540, respectively. A case-control study show that total immunoglobulin E (IgE) level was significantly increased in the AA genotype of DOCK8 compared to the CA genotype in allergic patients. The rs12484684 of IL17RA increased risk of adult-onset AD (odds ratio: 1.63) compared to the control for (A) allele frequency. AD and AM Patients with the IL17RA CA genotype also had elevated IgE levels. rs3745540 of KLK12 was associated with AD in dominant model (odds ratio: 2.86). Conclusion: DOCK8 (rs529208), IL17RA (rs12484684), and KLK12 (rs3745540), were identified using a new WES filtering method. the result suggests that polymorphism of DOCK8 and IL17RA might be related to increase the total IgE level.         Birt-Hogg-Dubé Syndrome, a Rare Case in Korea Confirmed by Genetic Analysis

        Simple benign tumors can present as part of a syndrome with substantial mortality. Fibrofolliculomas are benign skin tumors most often associated with the Birt-Hogg-Dubésyndrome (BHDS). The most life-threatening complication of this syndrome is renal cancer and other major features include multiple lung cysts and spontaneous pneumothorax. We present the case of a 54 year-old man with multiple flesh-colored papules on his face confirmed histologically as fibrofolliculomas. He had a history of recurrent pneumothorax and chest computed tomography showed multiple lung cysts. To confirm the diagnosis of BHDS, we conducted gene analysis that revealed a single nucleotide duplication in the folliculin (FLCN) gene (Exon 11, C.1285dupC). BHDS confirmed by the FLCN gene mutation is rarely reported in Korea. Appropriate investigation is recommended whenever a patient with benign skin tumors is encountered.         Efficient Production of Porcine Circovirus Type 2 Capsid Protein using Baculovirus

        Porcine circovirus type 2 (PCV2) is a single-stranded circular DNA virus associated with Postweaning multisystemic wasting syndrome (PMWS), which is considered to be an important infectious swine viral disease. PCV2 capsid protein encoded by ORF2 is a structural protein and expected as the high immunogenicity protein. In this study, we generated recombinant baculovirus containing ORF2 of PCV2 and analyzed the optimal conditions for the production of capsid protein in insect cell. Production and status of recombinant capsid protein in insect cell were confirmed by SDSPAGE and Western blot analysis using His tag antibody and anti-PCV2 serum. The yield of recombinant capsid protein was high like as shown visible on SDS-PAGE. Optimal multiplicity of infection (MOI) and infection time of recombinant virus were determined as 5 MOI and 4 days, respectively. ORF2 is known to have Nlinked glycosylation site, but we couldn`t detect the glycosylation of recombinant protein in insect cells.

      • Effects of exercise training on mitochondrial function in atorvastatin-treated rat heart

        Statins (3-hydroxy-3-methylglutaryl coenzyme A reductase inhibitors), which are cholesterol-lowering drugs widely used in the treatment of cardiovascular disease, cause adverse side effects in skeletal muscle ranging from fatigue to fatal rhabdomyolysis, leading to mitochondrial dysfunction. However, whether statins affect heart mitochondrial function is not clear. In addition, the effects of aerobic exercise training on mitochondrial function in statin-treated heart have not been clearly elucidated. Therefore, the purpose of this study was to determine the effects of in vivo atorvastatin treatment and aerobic exercise training on mitochondrial function (e.g., mitochondrial O2 respiration, H2O2 emission, Ca2+ retention capacity) in the rat heart. Male Wistar rats were randomly assigned to control (CON), 5 mg atorvastatin-treated group (STAT), and 5 mg atorvastatin treatment plus aerobic exercise group (STAT + EX) (N=12 rats/group). Animals were administered via oral gavage with a vehicle, 5 mg/kg/day atorvastatin dissolved in 0.25 % w/v hydroxypropyl methylcellulose for 12 weeks. The left ventricle (LV) was permeabilized by saponin for determination of mitochondrial respiratory capacity, mitochondrial H2O2 emission, and Ca2+ retention capacity. LV mitochondrial O2 respiration and Ca2+ retention capacity were not affected by 12 weeks treatment of atorvastatin. However, aerobic exercise training improved mitochondrial O2 respiration and Ca2+ retention capacity in LV. In contrast, mitochondrial H2O2 emission and Ca2+ retention capacity were reduced by exercise training. These data demonstrated that although LV mitochondrial function was not affected by atorvastatin treatment for 12 weeks, aerobic exercise training improved the mitochondrial function in the rat LV, suggesting that aerobic exercise training plays a pivotal role in improving mitochondrial function in the heart.
