        The effect of rituximab dose on infectious complications in ABO-incompatible kidney transplantation

        <P>Background. Rituximab (RIT) improves the outcomes of ABO-incompatible (ABOi) kidney transplantation (KT), but it has been associated with infectious complications. The aim of this study was to investigate infectious complications according to the dose of RIT in ABOi KT. Methods. We analyzed 213 recipients [118 ABO-compatible (ABOc) KT and 95 ABOi KT] who underwent living donor KT between 2010 and 2014. ABOi KT patients were categorized by RIT dose: standard RIT (375 mg/m(2), n = 76) versus reduced RIT (200 mg, n = 19). All patients received basiliximab and maintained on triple immunosuppression consisting of tacrolimus, prednisone and mycophenolate mofetil. Infectious complications and post-transplant outcomes were analyzed for 1 year following KT. Results. The rates of overall infectious complications among the three groups were comparable (22.9% in ABOc KT, 38.2% in standard RIT and 26.3% in reduced RIT, P = 0.069). In the standard RIT group, hepatitis B virus reactivation occurred in three recipients (3.9%) with hepatitis B surface antigen [-]/anti-hepatitis B core antibody[+]. Three cases (3.9%) of Pneumocystis jirovecii pneumonia occurred in the standard RIT group. Serious infections developed in 13 of the ABOc KT (11.0%), 20 from the standard RIT group (26.3%) and 2 from the reduced RIT group (10.5%, P = 0.015). Standard-dose RIT was found to be an independent risk factor for serious infections [hazard ratio: 2.59 (95% confidence interval: 1.33-5.07), P = 0.005]. There were no significant differences in rejection, renal function, graft survival and patient survival between standard and reduced RIT groups. Conclusions. Standard RIT increased the risk of serious infection when compared with reduced-dose RIT. Reduced-dose RIT might be sufficient for ABOi KT without increasing the risk of serious infection.</P>

        Primary Diffuse Leptomeningeal Gliomatosis: Report of a Case Presenting with Chronic Meningitis

        <P>Neoplastic meningitis occurs in approximately 5% of patients with cancer. Primary diffuse leptomeningeal gliomatosis is a rare condition whereby a glioma arises from heterotopic cell nests in the leptomeninges. We report here a case presenting with clinical features similar to those of chronic infectious meningitis without positive cerebrospinal fluid cytology. Neurological signs in our patient deteriorated progressively without responding to antitubercular, antiviral, or antibiotic therapy. Leptomeningeal biopsy sampling revealed the condition to be primary diffuse leptomeningeal gliomatosis.</P>

      • Identification and Characterization of the Rhizobium sp. Strain GIN611 Glycoside Oxidoreductase Resulting in the Deglycosylation of Ginsenosides

        <B>ABSTRACT</B><P>Using enrichment culture,Rhizobiumsp. strain GIN611 was isolated as having activity for deglycosylation of a ginsenoside, compound K (CK). The purified heterodimeric protein complex fromRhizobiumsp. GIN611 consisted of two subunits with molecular masses of 63.5 kDa and 17.5 kDa. In the genome, the coding sequence for the small subunit was located right after the sequence for the large subunit, with one nucleotide overlapping. The large subunit showed CK oxidation activity, and the deglycosylation of compound K was performed via oxidation of ginsenoside glucose by glycoside oxidoreductase. Coexpression of the small subunit helped soluble expression of the large subunit in recombinantEscherichia coli. The purified large subunit also showed oxidation activity against other ginsenoside compounds, such as Rb1, Rb2, Rb3, Rc, F2, CK, Rh2, Re, F1, and the isoflavone daidzin, but at a much lower rate. When oxidized CK was extracted and incubated in phosphate buffer with or without enzyme, (<I>S</I>)-protopanaxadiol [PPD(S)] was detected in both cases, which suggests that deglycosylation of oxidized glucose is spontaneous.</P>

      • Effect of Metal Nitride on Contact Resistivity of Metal-Interlayer-Ge Source/Drain in Sub-10-nm n-Type Ge FinFET

        <P>A metal nitride-interlayer-semiconductor source/ drain (MN-I-S S/D) model is newly proposed to investigate the effect of tantalum nitride (TaN) on the specific contact resistivity (ρ<SUB>c</SUB>) of an MN-I-S S/D with an undoped interlayer (undoped-IL) or a heavily doped IL (n<SUP>+</SUP>-IL) in sub-10-nm n-type Ge FinFETs. In this model, the workfunction variation of TaN was considered following the Rayleigh distribution. Compared with MN-I-S structures with an undoped-IL, structures with an n<SUP>+</SUP>-IL generate much lower ρ<SUB>c</SUB> values (i.e., ~2 × 10<SUP>-9</SUP> Ω · cm<SUP>2</SUP>) and are less prone to variation. In addition, the impact of Pc variation on device performance is investigated using 3-D technology computer aided design simulation for undoped or heavily doped ILs in MN-I-S S/D structures. MN-I-S S/Ds with an n<SUP>+</SUP>-IL can achieve much lower current variation and a higher ON-state drive current.</P>

        Design and Methodology for the Korean Observational and Escitalopram Treatment Studies of Depression in Acute Coronary Syndrome: K-DEPACS and EsDEPACS

        Depression is common after acute coronary syndrome (ACS), adversely affecting cardiac course and prognosis. There have been only a few evidence-based treatment options for depression in ACS. Accordingly, we planned the Korean Depression in ACS (K-DEPACS) study, which investigated depressive disorders in patients with ACS using a naturalistic prospective design, and the Escitalopram for DEPACS (EsDEPACS) trial, which assessed the efficacy and safety of escitalopram for treating major or minor depression in patients with ACS. Participants in the K-DEPACS study were consecutively recruited from patients with ACS who were recently hospitalized at Chonnam National University Hospital, Gwangju, South Korea. Diagnoses were confirmed by coronary angiography from 2005. Data on depressive and cardiovascular characteristics were obtained at 2 weeks, 3 months, 12 months, and every 6 months thereafter following the index ACS admission. The K-DEPACS participants who met the DSM-IV criteria for major or minor depressive disorder were randomly assigned to groups in the 24-week, double-blind, placebo-controlled EsDEPACS trial beginning in 2007. The outcome of treatments for depressive and other psychiatric symptoms, issues related to safety, including general adversity, and cardiovascular factors were assessed. The K-DEPACS study can significantly contribute to research on the complex relationships between depression and ACS. The results of the EsDEPACS trial provide an additional treatment option for clinicians treating these patients.

        Successful kidney transplantation across a positive complement-dependent cytotoxicity crossmatch by using C1q assay-directed, bortezomib-assisted desensitization : A case report

        <P><B>Abstract</B></P><P><B>Rationale:</B></P><P>Human leukocyte antigen (HLA) is the major immunologic barrier in kidney transplantation (KT). Various desensitization protocols to overcome the HLA barrier have increased the opportunity for transplantation in sensitized patients. In addition, technological advances in solid-phase assays have permitted more comprehensive assessment of donor-specific antibodies. Although various desensitization therapies and immunologic techniques have been developed, the final transplantation decision is still based on the classic complement-dependent cytotoxicity (CDC) crossmatch (XM) technique. Some patients who fail to achieve negative XM have lost their transplant opportunities, even after receiving sufficient desensitization therapies.</P><P><B>Patient concerns:</B></P><P>A 57-year-old male with end-stage renal disease secondary to chronic glomerulonephritis was scheduled to have a second transplant from his son, but CDC XM was positive.</P><P><B>Diagnoses:</B></P><P>Initial CDC XM (Initial T-AHG 1:32) and flow-cytometry XM were positive. Anti-HLA-B59 donor specific antibody was detected by Luminex single antigen assay.</P><P><B>Interventions:</B></P><P>Herein, we report a successful case of KT across a positive CDC XM (T-AHG 1:8 at the time of transplantation) by using C1q assay-directed, bortezomib-assisted desensitization. After confirming a negative conversion in the C1q donor-specific antibody, we decided to perform KT accepting a positive AHG-CDC XM of 1:8 at the time of transplantation.</P><P><B>Outcomes:</B></P><P>The posttransplant course was uneventful and a protocol biopsy at 3 months showed no evidence of rejection. The patient had excellent graft function at 12 months posttransplant.</P><P><B>Lessons:</B></P><P>The results of XM test and solid-phase assay should be interpreted in the context of the individual patient.</P>

        관절면을 침범한 경골 외측 고평부 골절에 대한 관절경을 이용한 정복 및 내고정술

        목적: 경골 외측 고평부 골절에서 추가적인 피질골 창을 내지 않고 관절경을 이용한 정복술 및 골이식술 없이 내고정한 후 이에 따른 결과를 평가해 보았다. 대상 및 방법: 2009년 3월부터 2017년 3월까지 3차원 컴퓨터 단층촬영(computed tomography, CT)에서 관절면의 함몰 및 전위가 5 mm 이상이면서 Schatzker 분류상 II형에 해당하는 경골 외측 고평부 골절에 대해 관절경을 이용하여 수술적 치료를 하고 최소 18개월 이상 추시가 가능한 27예를 대상으로 하였다. 경골 피질골에 추가적인 창을 내지 않고 전외측 연골판하방 삽입구(inframeniscal portal)를 통해 골막거상기(freer elevator)를 이용하여 감입되어 있는 골편을 정복하여 관절면의 정복을 얻은 후 골이식 없이 5.0 mm 혹은 6.5 mm 유관나사를 이용하여 내고정술을 시행하였다. 결과 판정은 임상적 결과와 방사선적 결과로 나누어 Rasmussen의 방법을 이용하여 분석하였고 유관나사 제거술을 받은 13예의 환자에 한해서 관절경을 이용하여 2차 추시를 시행하였다. 결과: 최종 추시에서 모든 예에서 골유합을 얻었고, 평균 골유합 기간은 8.7주(8-12주)였다. Rasmussen 기준에 따라 방사선적 결과는 25예(92.6%)에서 우수 또는 양호의 만족스러운 결과를 얻었고 임상적 결과는 24예(88.9%)에서 우수 또는 양호의 결과를 얻었다. 2차 추시를 시행한 환자 13예에서 섬유연골로 잘 치유됨을 확인하였고 술 후 외래 추시 중 CT 촬영한 8예에서 골결손 부위 골이식 없이 유합된 소견을 확인하였다. 결론: 경골 외측 고평부 골절에서 추가적인 피질골 창을 내지 않고 관절경을 이용한 정복 및 골이식 없이 시행한 내고정술은 관절면을 정확히 정복할 수 있는 유용한 방법이며 임상적으로 양호한 결과를 얻을 수 있는 방법으로 생각된다. Purpose: To evaluate the results of tibial lateral plateau fractures using arthroscopic-assisted reduction and internal fixation without cortical window or bone grafts. Materials and Methods: From March 2009 to March 2017, 27 patients with Schatzker type II tibial plateau fractures with articular depression and displacement over 5 mm on a computed tomography (CT) scan, who were treated with arthroscopic reduction and internal fixation and followed-up for at least 18 months, were enrolled in this study. Under arthroscopic guidance, the depressed fracture fragment was reduced using a freer and fixed with 5.0 or 6.5 mm cannulated screws through the inframeniscal portal without a cortical window or bone graft. The clinical and radiological results were evaluated using a Rasmussen system. Second look arthroscopy was performed in thirteen patients during the implant removal operation. Results: All fractures healed completely with a mean union time of 8.7 weeks (range from 8 to 12 weeks). Twenty four patients had good to excellent clinical results and 25 patients had good to excellent radiological results according to the Rasmussen classification. A well-healed articular surface with fibrocartilage was also found in 13 cases with second look arthroscopy. The 8 cases on CT scan at outpatient department follow-up showed bone union without bone grafting. Conclusion: Arthroscopic-assisted fixation of tibial lateral plateau fractures is a useful method without a cortical window or bone graft that produces good clinical results.

        열분해 폴리에틸렌 왁스의 아크릴산 그래프트 반응 특성

        열분해 반응을 통해 제조된 비극성 폴리에틸렌 왁스에 대해 극성 아크릴산 단량체를 그래프트 반응시켜 기능성 첨가제를 제조하였고, 반응인자가 그래프트 효율에 미치는 영향을 조사하였다. 폴리에틸렌 왁스의 분자량이 작을수록 그래프트율이 증가하였다. 아크릴산 함량이 증가할수록 그래프트율이 증가하였으나 과량의 아크릴산 함량에서는 비극성 반응물과의 혼화성이 불량하여 그래프트율이 감소하였다. 반응시간이 증가하거나 반응온도가 증가함에 따라 폴리에틸렌 사슬에 그래프트 반응활성점이 증가하여 그래프트율이 향상되었다. 개시제로는 반감기가 더 긴 라디칼 개시제가 동일 반응조건에서 더 높은 그래프트율을 나타내었다. 그래프트율이 증가할수록 그래프트 폴리에틸렌 왁스의 유리전이온도는 감소하였다. Functional additives were prepared by grafting polar acrylic acid on nonpolar polyethylene wax which was produced by pyrolysis. The effect of reaction factors on grafting degree was investigated. The grafting degree increased as the molecular weight of polyethylene wax decreased. Although the grafting degree increased as acrylic acid content increased, the grafting degree became lower in case of excess acrylic acid content due to the incompatibility with nonpolar reaction medium. The grafting degree increased as both reaction temperature and reaction time increased because of the increased formation of reaction active sites on polyethylene chains. The radical initiator of longer half life gave the higher grafting degree at the same reaction conditions. Glass transition temperature of grafted polyethylene wax decreased as the grafting degree increased.
