        위눈꺼풀이완증 환자에서 피부와 결막의 조직학적 소견

        Purpose: To evaluate histopathologic properties of eyelid skin and conjunctiva and the association between clinical manifestation and histopathologic findings. Methods: A prospective study was conducted on the histologic findings of the upper eyelid skin and conjunctiva performed from November 2009 to February 2010 in 27 patients for upper eyelid blepharoplasty procedures. Histopathologic studies were performed for specimens collected from the upper eyelid skin and inferotemporal bulbar conjunctiva. Preoperative photographs of the upper eyelid and conjunctiva were taken to grade clinical severity of dermatochalasis and conjunctivochalasis. Results: Decrease of collagen density and elastic degeneration in the eyelid skin and conjunctiva were significantly associated with aging (p < 0.001, p = 0.001, p = 0.001, p < 0.001). Clinical severity of dermatochalasis was correlated with a decrease of collagen density and elastic degeneration in the eyelid skin, and clinical severity of conjunctivochalasis was associated with a decrease of collagen density, elastic degeneration and lymphangiectasia in conjunctiva (p < 0.001). Conclusions: Change of collagen fiber and elastic fiber in the eyelid skin and conjunctiva was observed with aging. Relaxation of the eyelid skin and conjunctiva was correlated with histologic findings. J Korean Ophthalmol Soc 2011;52(5):582-588

        성인 코눈물관협착 환자에서 실리콘관 삽입술의 장기적인 결과에 영향을 미치는 요인

        Purpose: To evaluate factors affecting the outcome of silicone tube intubation in patients with nasolacrimal duct obstruction. Methods: A retrospective study of the outcome of silicone tube intubation performed from August 2001 to August 2009 in 98 patients (132 eyes) with nasolacrimal duct obstruction was conducted. Improvement was defined as the disappearance of epiphora after silicone tube intubation and success was defined as maintenance of improvement by the time of the final patient follow-up. Recurrence was also defined as appearance of epiphora after silicone tube extubation. Results: The success rate was 76.5% (101 eyes), failure rate was 8.3% (11 eyes), and recurrence rate was 15.2% (20 eyes). Preoperative canaliculus irrigation test was significantly correlated with symptomatic improvement and recurrence after silicone tube intubation (p = 0.003 and 0.045, respectively). The mean time that patients experienced recurrence was 43.7 months after silicone tube intubation, according to Kaplan-Meier survival analysis. Conclusions: Preoperative canaliculus irrigation test is a simple and effective tool to estimate the outcome of silicone tube intubation. This outcome should be considered in the stage of planning treatment in nasolacrimal duct obstruction patients. J Korean Ophthalmol Soc 2011;52(2):129-135

        고온설비 수명평가를 위한 표면복제 절차의 표준화

        Surface replication is playing an important role in the assessment of creep damage and remaining life of high temperature components. As the replication procedures, however, have not been standardized in domestic industry, its standardization is proposed in this study. For this purpose, the 2.25Cr-IMo steel was heat treated(5 min at 1,300 0C and oil quenched) to produce a simulated HAZ microstructure, and crept in air at 575 0C and under 120 MPa to produce artificial cavities. Then, the effect of surface preparation procedures on the quality of replicas was investigated using this sample. As a result, it was demonstrated that the presence of cavities may be observed readily or missed depending on the surface preparation procedures followed. Therefore it is essential to repeat three polishing/etching cycles at least in order to reveal cavitation damage accurately, even though it may be tedious or time-consuming.

        S-1 전신 항암 요법 후 발생한 눈물길 폐쇄 2예

        Purpose: To report two cases of lacrimal drainage obstruction associated with the anticancer drug S-1 (tegafur, gimeracil, oteracil ; TS-1Ⓡ, Taiho Pharmaceutical, Japan). Case summary: (Case 1) A 34-year-old male who had previously undergone a total gastrectomy for treatment of gastric cancer visited with epiphora in both eyes that had developed 6 months after S-1 treatment. On examination, a right canalicular obstruction and left punctal and nasolacrimal partial obstruction were noted. Endoscopic dacrycystorhinostomy with lacrimal trephinization was done in the right eye and punctoplasty with punctal membrane removal and silicone tube intubation were performed in the left eye and the epiphora were successfully treated. (Case 2) A 60-year-old male who was treated with S-1 for gastric cancer presented with tearing in both eyes that had developed 2 months after S-1 treatment. Ophthalmic examinations revealed a right nasolacrimal partial obstruction in the right eye and lacrimal sac obstruction in the left. Silicone intubations were performed on both sides and the epiphora were successfully treated. Conclusions: Lacrimal drainage obstructions may develop after S-1 treatment. Therefore, ophthalmologic evaluations should be performed in patients treated with S-1 to detect lacrimal drainage obstruction. J Korean Ophthalmol Soc 2010;51(6):890-894

        개인용 음향기기 사용이 청소년의 청력에 미치는 영향

        Objectives : This study was conducted to evaluate the effects of the personal stereo system on the hearing in adolescents. Methods : A total of 68 adolescents(age: 13-18 years) who visited the ENT Department at a University Hospital in Daegu were personally interviewed. The questionnaires were about general characteristics of the subjects, the time of personal stereo system use (year, hour) and place. Cumulative exposure to the personal stereo system was calculated by the product of the total years and the daily hours of their use. Pure tone audiometry was performed and the hearing threshold was measured at 500, 1000, 2000, 4000 and 8000 Hz. Results : The average time of using a personal stereo system a day was about 3 hours and 75% of the subjects used a personal stereo system for 2-5 years. The elevation of threshold was more prominent in the subjects who used personal stereo systems for 4 years and more compared with those subjects who used them for 3 years and under. The elevation of hearing threshold was also more prominent in the subjects who used personal stereo systems for 4 hours and more a day compared with those subjects who used personal stereo systems for 3 hours and under a day. The elevation of hearing threshold was more prominent in the subjects who used personal stereo systems for 13 hour. years and more compared to the subjects who used them 12 hour. years and under. Conclusions : These results suggest that the elevation of hearing threshold can happen to adolescents who used personal stereo systems for a long time. In order to prevent hearing loss, we need to teach adolescents appropriate usage of the personal stereo system and hearing tests should be included in the periodic school-based physical examination for the adolescents.

      • 6세기 중반~7세기 高句麗·新羅의 境界와 그 변천

        독자적인 천하관을 가졌던 고구려는 565년에 北齊로부터 신라왕이 東夷校尉를 수여 받음으로서 북제가 신라를 동이 세계를 대표한다는 인식에 동의하기 어려웠다. 고구려는 중국 왕조로부터 동이 세계의 대표로 인정받기 위해서는 한강유역을 회복하는 것이라고 인식하였다. 온달이 이와 같은 고구려의 과제를 달성하기 위해, 故土의 回復을 위해 출정하였지만, 성공을 거두지 못하였다. 온달이 한강유역을 회복하기 위하여 출정한 시기는 590∼594년으로 추정되며, 온달이 전사한 장소인 아단성은 서울 아차산성으로 비정된다. 온달이 6세기 중반에 신라에게 빼앗긴 한강유역을 회복하기 위하여 출정하였으나 실패하였다. 신라 진흥왕은 568년에 함흥평야 일대까지 진출하여 고구려를 강하게 압박했다. 그런데 『三國史記』 신라본기에 568년 10월에 북한산주와 비열 홀주를 폐지하고, 남천주와 달홀주를 설치하였다고 전하는 기록이 있으나, 그러한 배경을 정확하게 알기는 어렵다. 다만 570년대에 신라는 고구려와의 사이가 벌어지면서 더 이상의 영토 확장을 포기하는 대신, 이때까지 확보한 영토를 공고하게 守護하기 위하여 정군단의 주둔지를 후방으로 移置 시켰다고 볼 수 있다. 7세기 신라와 고구려가 전투를 벌인 곳은 北漢山城(서울 아차산성), 七重城(파주시 적성면), 牛鳴山城(강원도 춘천), 述川城(여주시 흥천면 일대) 등 임진강과 한강유역 사이의 지역이다. 이 가운데 우명산성과 칠중성이 7세기 양국의 국경에 위치한 것으로 보인다. 특히 우명산성의 위치에 대해 여러 가지 견해가 제기되어 논란이 분분한데, 본고에서는 우명산성을 춘천지역으로 비정한다. 신라가 551년에 북한강 상류지역을 차지하였고, 608년에 고구려가 춘천지역에 위치한 우명산성을 공격하여 차지하였으며, 629년 이후에 다시 신라가 춘천지역을 회복하고 637년에 우수주를 설치하였다. 그리고 6세기 중반 이후 신라가 비열성을 계속 차지하였고, 643년 무렵에 고구려가 동해안 방면에서 비열성(비열홀)을 공격하여 차지하고 하슬라 근처까지 진출하였다. 한편 신라는 568년(진흥왕 29)에 함경도 이원과 함흥방면까지 진출하였는데, 이때 진흥왕은 서울∼원산을 잇는 추가령구조곡을 거쳐 북한산(한강 이북의 서울)에 이르러, 오늘날 북한산 비봉에 북한산비를 건립하였다고 알려졌다. 이에 따른다면, 6세기 중반에 신라가 원산에서 서울에 이르는 중간 경유지에 해당하는 철원지역까지 진출하였다고 볼 수 있을 것이다. 아마도 이후 어느 시기에 고구려가 다시 남진하여 낭비성을 차지하였다고 추정할 수 있다. 629년에 신라가 고구려 낭비성을 공격하여 함락하였는데, 낭비성은 현재 포천에 위치한 반월산성으로 비정된다. 결국 포천 반월산성이 고구려의 세력 하에 있었다면 한강유역의 북한산성은 직접적인 위협을 받았을 것이며 북한강유역의 양평, 춘천까지 그 영향이 미치기 때문에, 신라는 백제의 공격 속에서 위협을 무릅쓰고 629년 선제공격을 하였다. 이와 같은 절박한 상황 속에서 신라는 결국 낭비성을 함락시키고 한탄강을 넘어 철원까지 진출하였다. In the 26th year of King Jinheung's reign(565), Silla was awarded the title of 'King of Silla, King of Nakranggun, Gyowi of Sajijeoldongi(使持節東夷校尉樂浪郡公新羅王)' from Bukje. Through this, we can see that Bukje recognized the rapidly growing Silla, which occupied the Han River basin, as a kingdom representing the world of Dongi instead of Goguryeo. Goguryeo had its independent worldview self-centred. Goguryeo tried to retake the Han River basin and restore its previous status. Accordingly, Ondal(溫達) attempted to recapture the former territory between 590 and 594 but failed. In the 7th century, Silla and Goguryeo fought in the area between the Imjingang River and the Hangang River, including the Bukhansanseong Fortress(the Achasanseong Fortress in Seoul), the Chiljungseong Castle(七重城), and the Umyeongsanseong Fortress (Chuncheon in Gangwon-do). Goguryeo failed to raid the Bookhansan Fortress(北 漢山城) in 603. Then in 608, it made a conquest Umyeongsanseong Fortress(牛鳴山城) in Chuncheon. Silla attacked the Nangbiseong Castle of Goguryeo and occupied it in 629. It retook the Chuncheon area and then established Udo county in 637. Since the loss of the Nangbiseong Castle, Goguryeo became blocked from the route from Pocheon to the Hangang River basin. Therefore, Goguryeo attacked the Chiljungseung Castle in Jeokseong in Paju. I understood the reason for this attack on the Chiljungseong Castle from two importance of it. Firstly, it was a core gate to pass through in order to advance to the Bukhansan Fortress. Secondly, in the aspect of Goguryeo, as it could no longer find an entry point in the southern direction, attempted to advance towards the Chiljungseong Castle on the main transportation route.
