<P>This study assessed the activity and toxicity of primary carboplatin-based chemoradiotherapy (CarboRT) and compared CarboRT with cisplatin-based chemoradiotherapy (CisRT) in patients with locally advanced cervical cancer and poor general condition. CarboRT was better tolerated than CisRT without compromising tumor response and survival in these patients.</P>

Plasmablastic lymphoma (PBL) is a very aggressive variant of diffuse large B-cell lymphoma initially described in the oral cavity of human immunodeficiency virus (HIV)-infected individuals. PBL has a poor prognosis with a high rate of relapse and death. It is important to note that there is no established standard of care for PBL as it is unknown if the outcome of patients with PBL has improved in the highly active antiretroviral therapy (HAART) era. In a recent study of patients with HIV-positive PBL treated with chemotherapy, the use of HAART was associated with a statistical trend toward improved survival. We report a case of PBL in a stomach of a 44-year-old man with HIV infection. The patient was diagnosed with HIV infection 1 month previously and received gastroscopy with epigastric pain. On gastroscopy a friable ulcerofungating mass was observed and biopsy showed a large cell lymphoma with plasmacytic differentiation with diffusely positive for CD79a, CD138, and CD10 and partially positive for CD38. Based on these findings, a diagnosis of stage IE PBL was made. He was treated with highly intensive chemotherapy (ESHAP: etoposide, methyprednisolone, cytarabine, and cisplatin) and highly active HAART concurrently. After 2 courses of chemotherapy a complete remission (CR) was achieved and 2 courses of consolidation treatment was administered consecutively. Therapy-related toxicity was tolerable, however G-CSF was used to recovery hematologic toxicity, especially grade 3 to 4 neutropenia. The patient has remained in CR for 16 months after high intensity chemotherapy. An important aspect of the initial treatment of PBL is the use of chemotherapy. In this case we used more intensive regimen compared to conventional CHOP/CHOP-like regimen with HAART. Considering poor survival of CHOP chemotherapy, HAART with highly intensive chemotherapy are recommended.

<P>A challenge in using plasmonic nanostructure–drug conjugates for thermo–chemo combination cancer therapy lies in the huge size discrepancy; the size difference can critically differentiate their biodistributions and hamper the synergistic effect. Properly tuning the plasmonic wavelength for photothermal therapy typically results in the nanostructure size reaching ∼100 nm. We report a new combination cancer therapy platform that consists of relatively small 10 nm pH-responsive spherical gold nanoparticles and conjugated doxorubicins. They are designed to form aggregates in mild acidic environment such as in a tumor. The aggregates serve as a photothermal agent that can selectively exploit external light by their collective plasmon modes. Simultaneously, the conjugated doxorubicins are released. The spatiotemporal concertion is confirmed at the subcellular, cellular, and organ levels. Both agents colocalize in the cell nuclei. The conjugates accumulate in cancer cells by the rapid phagocytic actions and effective blockage of exocytosis by the increased aggregate size. They also effectively accumulate in tumors up to 17 times over the control because of the enhanced permeation and retention. The conjugates exhibit a synergistic effect enhanced by nearly an order of magnitude in cellular level. The synergistic effect is demonstrated by the remarkable reductions in both the therapeutically effective drug dosage and the photothermal laser threshold. Using an animal model, effective tumor growth suppression is demonstrated. The conjugates induce apoptosis to tumors without any noticeable damage to other organs. The nutrient components of 3 differently preparedwhite jade silkworm(also knownas Baek Ok Jam) larval powders (WJSLPs) were examined. A proximate analysis of 3 WJSLPs revealed that steamed and freeze-dried mature silkworm larval powder (SMSP) had the highest protein contents among the 3WJSLPs. In addition, the ASP, SER, GLY, ALA, TYR, and THR contents in SMSP and freeze-driedmature silkworm larval powder (FMSP) significantly increased compared to those in freeze-dried 3rd day of 5th instar silkworm larval powder (FDSP). Even though SMSP and FMSP had reducedmineral contents compared to those of FDSP, the essential minerals, such as Ca,Mg, K, Na, P, S, Zn, Fe,Mn, and Cu, were highly enriched in SMSP and FMSP. The secondmost enriched nutrient in the 3WJSLPswas crude lipids. The 3 WJSLPs showed high contents ofΩ-3 (n-3) fatty acids,which are one of themost popular health improvement supplements.Werecently showed that SMSP could extend the lifespan and prevent the onset and progression of Parkinson's disease in Drosophila models (Nguyen et al., 2016). Thus, further studies in the functionalities of the 3 WJSLPs will reveal their unknown health improvement effects.
