Objectives : This open prospective study was performed in order to evaluate the efficacy and extrapyramidal adverse effects of nemonapride in the schizophrenic patients, and was compared wit one of typical antipshchotics. haloperidol and one of atypical antipsychotics, risperidone. Methods : Thirty male and female schizophrenic patients(DSM-Ⅳ) were treated for 12 weeks with haloperidol(n=10), risperidone(n=10) and nemonapride(n=10). The overall efficacy was assessed at baseline, 1st week, 2nd week, 4th week, 8th week, and 12th week by the Positive and Negative Syndrome Scale for Schizophrenia(PANSS). Also the overall safety was assessed in the same time period by the Extrapyramidal Symptom Rating Scale(ESRS). Results : There were no significant differences in PANSS scores(total, positive, negative, and general psychopathology subscale) among haloperidol, risperidone, and nemonapride groups, Treatment responders, defined as at least 20% reduction of baseline total PANSS score, were achieved by 8 patients(80%) in haloperidol group, 10 patients(100%) in risperidone group, and 8 patients(80%) in nemonapride group. And there were no significant differences in ESRS total scores among haloperidol, risperidone, and nemonapride group. Inter-group comparison among haloperidol, risperidone and nemonapride group, as assessed by the ESRS, revealed no significant differences in the shifts to the maximum score, 9.4(±9.4), 6.2(±8.4), and 11.3(±8.1) respectively, and also revealed no significant differences in the mean time reaching the maximum score, 4.5(±4.5) week, 7.8(±4.8) week, and 4.6(±4.4) week, respectivily. Conclusion : There wee no significant differences in the efficacy and extrapyramidal adverse effects among haloperidal, rispecridone and nemonapride groups. These results suggest that nemonapride was as efficacious and safe in he treatment of schizophrenia, as well known and widely used antipsychotics, haloperidol and risperidone.

A search for the genes expressed only in gastric cancer may help to define molecular alterations or aid in diagnosis of gastric cancer. Recently, suppression subtractive hybridization (SSH) has been used as an effective technique for detection of differentially expressed genes. We discovered a noble gene which belongs to the Reg family in stomach cancer tissue using the SSH technique. We identified and named the Reg Ⅳ gene that is highly expressed only in gastric tumor tissue. The Reg family gene has a lot of functions of the growth factor and gene regulation. The cathepsin L-12 gene, which is known to exhibit increased expression in breast cancer and some inflammatory conditions, is also expressed in stomach cancer. Expressions were verified by reverse Northern blot analysis and Northern blot analysis. We expect these genes to be used as an important tool for the diagnosis and/or understanding of the pathogenesis of gastric cancer.
여드름양 발진에서의 말라쎄지아 포자 검출 빈도에 관한 연구

Steroid acne and pityrosporum folliculitis have similar clinical manifestations. So it was not casy to do differential diagnosis of these two diseases. We have tried to find out the different points between these diseases in clinical characteristics, positive rate of the malassezia spores, and their response to antifungal agents. We studied 21 patients with acneiform eruptions which were distinct from acne vulgaris. Patients were divided into two groups by steroid treatment history. Among 21 patients, 6 were administered steroid just before eruption and 15 were free from steroid within 6 months. We have examined malassezia spores with 10% KOH/Parker blue black ink mount from papules, pustules and comedones according to the patients skin lesions. The malassezia spores detected in 5 paticnts(83.3%) in the former and 9 patients(60%) in the latter. In malassezia folliculitis, the frequency of which showed above the (3+) score of malassezia spore under microscopy was higher than in steroid acne. Mean duration of the disease was 70.4 weeks in latter group, which was longer than 5.2 weeks of the former group. And there were some differences between two groups in the distribution of the lesions. Mean treatment duration was same in two groups (4 wks, 3.5 wks). Steroid acne patients would be treated better with antifungal agents than with acne regimens.

We design a crisscrossed double-layer birdcage (DLBC) coil by modifying the coil geometry of a standard single-layer BC (SLBC) coil to enhance the homogeneity of transmitting magnetic flux density (B₁+) along the main magnetic field (B0)-direction for small-animal magnetic resonance imaging (MRI) at 300 MHz. The performance assessment of the crisscrossed DLBC coil is conducted by computational analysis with the finitedifference time domain method (FDTD) and compared with SLBC coil in terms of the B₁ and the B₁+ distribution. As per the computational calculation studies, the mean value in the two-dimensional B₁+ map obtained at the mid-axial slice with the proposed DLBC coil is slightly lower than that obtained with the SLBC coil, but the B₁ + value of the DLBC coil in the outermost plane (40 mm away from the central plane) shows improvements of 19.3% and 24.8% over the SLBC coil B₁+ value when simulating a spherical phantom and realistic mouse body modeling. These simulation results indicate that, the B₁ + homogeneity along the z-direction was improved by using DLBC configuration. Our approach enables B₁ + homogeneity improvement along the zdirection, and it can also be applied to ultra-high field (UHF) MRI systems.

This work presents the use of combinations between a wireless radio-frequency surface coil and a wireless 16-leg birdcage coil that are inductively coupled to improve the magnetic field sensitivity and uniformity. A single surface loop coil operating as transmission/reception (Tx/Rx) coil was designed for mouse head imaging at a magnetic field strength of 9.4-T. Numerical analyses using finite-difference time-domain were performed to compute the sensitivity and homogeneity of magnetic and electric flux density fields for each of the coil combinations. Maximum field values and standard deviation were used as statistical parameters to compare the sensitivity and homogeneity of the fields produced by the Tx/Rx surface coil for each case, when the wireless inductively coupled coils were used. The electromagnetic analyses were applied to a cylindrical oil-based phantom and a mouse model. The proposed combinations of the surface coil with the inductively coupled wireless surface and wireless volume coils offer an enhanced magnetic-flux sensitivity and RF excitation field distribution at 9.4-T. The modifications to the surface coil geometry by adding the inductively coupled radio-frequency coil combinations could be applied to the generally used transmit/receive surface coils and extended to parallel radio-frequency transmission array at ultra-high-field magnetic resonance imaging.

Radiofrequency (RF) safety in magnetic resonance imaging (MRI) is mainly verified using the specific absorption rate (SAR) in research involving humans. Universal MRI scanners measure the distribution of the hydrogen nucleus <SUP>1</SUP>H; hence, RF safety previously focused on the single frequency at which <SUP>1</SUP>H resonates. However, MRI technology is gradually transforming into simultaneous multi-frequency (MF) RF excitation to provide significant information related to biochemical changes in addition to morphological changes. These changes necessitate close monitoring of RF safety by considering other SAR aspects. This study proposes three evaluation methods for estimating the SAR value: summation of the SAR values of individual peaks, using the SAR value of the highest individual peak, and using the peak of the combined SAR for simultaneous RF excitation under MF RF excitation. This method, which is environmentally friendly, can be applied to various approaches for evaluating the RF safety in MF MRI applications.

<P>In a previous search for the differentially expressed genes in keratinocyte differentiation, we identified neutrophil gelatinase-associated lipocalin (NGAL) as a calcium-induced gene. In this study, we further verified the expression of NGAL in cultured keratinocytes as well as in several skin diseases. Reverse transcription-polymerase chain reaction (RT-PCR), Western blot, and ELISA clearly showed that NGAL expression was markedly increased in calcium-induced keratinocyte differentiation in vitro. However, in our previous report, NGAL expression was not detected in normal skin tissue except for hair follicle by in situ hybridization and immunohistochemistry, indicating the difference of cell status between in vitro and in vitro conditions. Interestingly, NGAL expression was highly increased in psoriasis-like inflammatory disorders (lichen planus and pityriasis rubura pilaris) and skin cancers (keratoacanthoma and squamous cell carcinoma), implying that NGAL may be related with the epidermal hyperplasia. Collectively, these results reveal the potential importance of NGAL in the maintenance of skin homeostasis.</P>
A Case of Pilomatricoma Showing rapid Enlargement Due to Intradermal and Intratumoral Hemorrhage

Pilomatricoma, known as calcifying epithelioma of Malherbe, is a deep dermal or subcutaneous tumor showing slow growth. There are some reports showing rapid enlargement of the tumors as a result of variable amounts of bleeding into the intratumoral cystic space and into the superficial dermis. We report an uncommon case of a rapidly enlarged pilomatricoma due to simultaneous intratumoral and intradermal hemorrhage.
