        마취중 과한기가 동맥혈과 뇌척수액의 산-염기 변화에 미치는 영향

        The use of controlled gyperventilation during neurosurgical procedures prevents the deleterious effects of hypercarbia on the cerbral blood flow and intracranial pressure. Hyperventilation with hypocarbia produces cerbral vasoconstriction, reduced cercbbral blood flow and a reduction in brain size in the majority of patients with increased intracranial pressure. But, since excessive cerebral vasoconstriction might induce cerebral ischemia, there has been much discussion concernin the optimal level of hypocarbia. Several studies have shown biochemical evidence of a change in cerebral glucose utilization to anaerobic metabolism during hupocarbia. In our investigation, the effect of huperventilation of 10 neurosurgical patients was evaluated by blood gas analysis and the estimation of lactate and pyruvate in arterial blood and the cerebrospinal fluid. The results were as follows: 1) PaCO_2 decreased from a prearesthetic value of 38±2.2 mmHg to 22±2.1 mmHg 1 hour postinduction and 24±2.2 mmHg at 2 hours due to hyperventilation. pH was 7.58±0.047 1 haur postionduction and 7.56±0.018 at 2 hours. PaO_2 was 251±33.0 mmHg 1 hour postinduction 1 hour and 215±20.9 mmHg at 2 hours under a 50% inspired oxygen concentration(FiO2=0.5). 2) The arterial blood lactate value increased statistically significantly from a preanesthetic value of 9.3±15 mmg% to 11.8±1.47 mg% 1 hour postinduction(p<0.01) to 12.5±1.53 mg% at 2 hours(p<0.005). However all vales were wirthin the normal range(4.7±15.1 mg%), and the lacte/pyruvate rato did not change. 3) In the cerebrospinal fluid, pH was 7.45±0.057, PCO_2 was 34±3.5 mmHg and PO_2 was 91±6.7 mmHg following hyperventilation for 1 hour. The lactate value of the cerebrospinsifluid was 19.2±3.14mg%(normal range: 11.0∼27.0mg%) and the lactate/pyruvate ratio was 14.5±2.39. 4) No evidence of an excessive increase in CSF lactate was seen in any case. The above findings suggest that maintenance of an adequate oxygen concentration and a carbon dioxide value over 20 mmHg would prevent cerebral ischemia following hypocarbia due to hyperventilation.

      • Ginsenoside Rg1 enhances CD4^(+) T-cell activities and modulates Th1/Th2 differentiation

        Panax ginseng is commonly used as a tonic medicine in Asian countries such as Korea, China, and Japan. It has been reported that ginsenoside Rg1 in P. ginseng increases the proportion of T helper(Th) cells among the total number of T cells and promotes IL-2 gene expression in murine splenocytes. This implies that ginsenoside Rg1 increases the immune activity of CD4 T cells, however, the exact mechanism remains unknown. The present study elucidated the direct effect of Rg1 on helper T-cell activities and on Th1/Th2 lineage development. The results demonstrated that ginsenoside Rg1 had no mitogenic effects on unstimulated CD4^(+) T cells, but augmented CD4^(-) T-cell proliferation upon activation with anti-CD3/anti-CD28 autibodies in a dose-dependent manner. Rg1 also enhanced the expression of cell surface protein CD69 on CD4^(-) T cells. In Th0 condition, ginsenoside Rg1 increases the expression of IL-2 mRNA, and enhances the expression of IL-4 mRNA on CD4^(+) T cells, suggesting that Rg1 prefers to induce Th2 lineage development. In addition, ginsenoside Rg1 increases IL-4 secretion in CD4^(+) T cells under Th2 skewed condition, while decreasing IFN-ysecretion of cells in Th1 polarizing condition. Thus, Rg1 enhances Th2 lineage development from the naive CD4^(+) T cell both by increasing Th2 specific cytokine secretion and by repressing Th1 specific cytokine production. Therefore, these results suggest that ginsenoside Rg1 is a desirable agent for enhancing CD4^(-) T-cell activity, as well as the correction of Th1-dominant pathological disorders.

        N/A Objectives : Oxidized LDL is thought to play a key role in atherogenesis. Among their wide variety of biological properties, oxidized LDL injures the endothelium as an early event of atherogenesis. However, the mechanisms by which oxidized LDL injures endothelial cells are not definitely known. In order to evaluate the involvement of GTP-binding protein on the mechanism of apoptosis, we studied the effects of pertussis and cholera toxin on oxidized LDL-induced apoptosis in bovine aortic endothelial cells(BAECs). Methods : Oxidized LDL elicited apoptosis in cultured BAECs as shown by characteristic morphological and biochemical changes. Chromatin condensation and nucleus fragmentation were visualized by using fluorescence microscopy of intact cells staining by acridine orange/ ethidium bromide. DNA fragmentation was quantified by an ELISA with specific antibody for bromodeoxyuridine- labelled DNA fragments and confirmed with DNA ladder formation. Results : Studies using a combination of bacterial toxins which change the function of GTP-binding protein suggest that oxidized LDL-induced apoptosis was regulated by GTP-binding protein. Oxidized LDL-induced apoptosis was not changed by pretreatment of BAECs with pertussis toxin. In contrast, pretreatment with cholera toxin completely prevented the oxidized LDL- induced apoptosis. Conclusion : These results show that oxidized LDL induces apoptosis of BAECs and suggest that cholera toxin-sensitive G-proteins are involved in signal transduction of oxidized LDL-induced apoptosis of BAEC.

      • 생분해성 플라스틱이 포함된 유기성 생활 쓰레기의 퇴비화

        준 생산용 퇴비화 장치를 이용하여 생분해성 플라스틱을 포함하는 유기성 도시생활쓰레기기의 퇴비화를 연구하였다. 유기성 도시생활쓰레기는 실제 도시생활쓰레기 구성비에서 무기물을 제외한 유기성 물질만의 비로 만들어 사용하였다. 유기성 도시생활쓰레기를 구성하는 각각의 물질에 대한 생분해도를 ISO법으로 측정하였고, 퇴비의 화학적 물리적 특성도 조사하였다. 구성 물질에 대한 생분해도 결과에서, 생분해성 고분자를 포함한 각 물질들은 퇴비화에 충분한 생분해도를 나타내는 것으로 밝혀졌다. 또한, 유기성 도시 생활쓰레기로부터 비독성의 퇴비를 얻을 수 있음을 알았다. The composting study of organic municipal solid waste containing biodegradable plastic was performed by using pilot scale composting apparatus. The organic municipal solid waste(OMSW) was artificially prepared for composting. The composition of artificial OMSW was the same as that of real municipal solid waste excluding inorganic waste. The biodegradability of individual component of OMSW was studied according to the ISO method. The chemical and physical characteristics were also observed. Biodegradability study revealed that every component of OMSW including biodegradable plastic had sufficient degradability for real composting. And it was found that non-toxic compost could be produced from OMSW containing biodegradable plastic.

      • 동물원에서 사육중인 Japanese Macaque (Macaca fuscata)에서 Zoletil 마취에 관한 연구

        To evaluate the effects of Zoletil (tiletamine-zolazepam) that have been widely used for the chemical restraint and anesthesia of primates, on physiologic alteration, blood gas analysis and anesthetic duration in the Japanese macaque(Macaca fuscata), this study was carried out. Zoleril was administered by intramuscular injection, Evaluation of temperature, heart rate, respiration rate and blood gas analysis were performed before administration ancl at 1,10, 20, 30, 40, 50 and 60 min after administration, and induction and maintenance time was recorded. There was no significant difference in heart rate, PCO₂PO₂after Zolcril administration and temperature, respration rate, pH were significant difference compared with those of before administration, The induction time was 2.5±l.Omin and maintenance time was 86±23.2min. It was considered that Zoletil could he usefully used for the sedation and immobilization of Japanese macaque reared in zoological garden.

      • The whole-exome sequencing in three families with atopic dermatitis

        Background: The prevalence of atopic dermatitis (AD) has increased over a recent 10-year. AD tends to run in families that passed down from generation to generation and commonly starts in childhood. It is as high as 20% in children less than 5 years old. Thus, early discovery and therapy of AD are important. It has emerged the necessity of biomarker for early detection of AD. Objectives: The goal of this study is to find novel gene markers for diagnosis of early-onset atopic dermatitis in Koreans. Methods: Peripheral blood was obtained from three families (6 early-onset AD patients and 6 controls) with autosomal dominant condition. Whole Exome Sequencing (WES) was performed using SureSelect Human All Exon V4+UTR 71 Mb. Variants of atopic dermatitis were filtered step by step to gain the important candidate genes. Results: We have confirmed overlapping genes in common variants of filter 5 in three family. 15 genes were overlapped in filter 5 and two of them reached to filter 7 can be called “rare variant”. Uniquely, COL6A6 gene appeared in all of three family. We also found variants of filaggrin, filaggrin2 and IL4R that is well-known makers of atopic dermatitis. Conclusion: We were able to detect COL6A6 gene by finding overlapping genes in rare and common variants of three family. It may become a novel gene as early-onset AD marker.

      • 각막형태검사법을 이용한 각막양상과 눈의 굴절력과의 관계

        We evaluated the topography of 200 normal cornea of Korean. The mean age was 37 years and mean refractive astigmatism was 0.82±0.69D. The corneal patterns were classified by Dr. Bogan such as round, oval, symmetric bow tie, asymmetric bow tie, and irregular pattern. Its incidence was 14.0%, 11.0%, 16.0%, 30.0% and 29.0%, respectively. There was no statistically significant difference in the distribution of topographic pattern in male and female and among age groups. The symmetric bow tie type had the greatest spherical equivalent and astigmatism and there was no statistically significant difference in those two values between round and oval group. Two bow-tie groups were not also statistically significantly different from each other in spherical equivalent and astigmatism. However, two bow tie groups were significantly different from round and oval group. So, it was possible to divide corneal topographic pattern into three groups such as round and oval, two bow tie, and irregualr group. We could understand normal corneal status related to corneal pattern and refractive power through this qualitative system for classifying corneal topography.

        A 5 year old, 13kg, Jindo dogs was presented with clinical signs of anorexia, weakness and exercise intolerance, and diagnosed as dirofilariasis by hematology, blood chemistry and ultrasonography. In hematology and blood chemistry, the presence of microfilariae in the blood. anemia, leukocytosis, hemoglobinuria and jaundice were revealed. In ultrasonography, heartworms were found in the right ventricle artery and pulmonary artery as structures of hyperechoic dots or streaks. Because of the dog's poor physical status, the owner requested euthanasia. In autopsy, fifty odd of heartworms were ascertained in the right ventricle and pulmonary artery. Tricuspid valve in the orifice of right ventricle and right atrium, and chordae tendineae were deliquesced.

      • 급성 인두 및 편도염에서 4-nitro-2-phenoxymethanesulfonanilide(Nimesulid)의 임상효과

        Nimesulid, 4-nitro-2-phenoxymethanesulfonanilide, a non-steroidal antiinflammatory drug endowed with potent anti inflammatory, analgesic and antipyretic activities was tested at a daily dose of 200mg b.i.d. against 40 cases of acute pharynogotonsillitis. The therapeutic effect was appeared in 36 cases(90%): 13 cases(32.5%) in Group I, 12 cases(30%) in Group Ⅱ, 11 cases(27.5%) in Group Ⅲ. 2 cases complained gastrointestinal discomfort.
