The Wide Variety of Acute Disseminated Encephalomyelitis in Children: A Clinical Perspective

Acute disseminated encephalomyelitis (ADEM) is an acute demyelinating inflammatory disorder of the central nervous system. It is characterized by encephalopathy, multifocal neurological deficits, and typical magnetic resonance imaging findings of widespread demyelinating lesions, predominantly involving the white matter of the brain and spinal cord. ADEM mainly affects children and is commonly associated with preceding viral and bacterial infections, and, rarely, vaccinations. Despite substantial advances in the understanding of the association of myelin oligodendrocyte glycoprotein antibody with recurrent forms of ADEM or other demyelinating conditions, specific etiologic agents or biological markers have not been identified. Therefore, the diagnosis of ADEM is still based on clinical and radiological findings and the exclusion of other conditions mimicking ADEM. However, a prompt diagnosis and adequate treatment are crucial because diagnostic delays or inappropriate treatment may lead to unwanted neurological sequelae in some children. There is no standardized treatment protocol for ADEM, but the use of corticosteroids, intravenous immunoglobulin, and plasmapheresis has been associated with good clinical outcomes. Adequate treatment has reportedly resulted in favorable outcomes, with full or almost full recovery in most children with ADEM, although some children may develop neurological sequelae, such as cognitive impairment and motor deficits. Further studies are needed to identify biological clues and optimal treatment protocols to minimize the incidence of neurological sequelae.
SNS 관광정보특성의 관광동기, 관광지 선택 영향 연구

This research examines what characteristics SNS tourism information has, and it studies the methodology that provides excellent and reliable SNS tourism information to tourists through the analysis of how the characteristics influence on tourism motivation and tour site selection. Based on the results, it aims to provide implications for improving tourism satisfaction and suggesting sustainable development of tourism industry. In the results of research, the most influential factor in tourism motivation of SNS tourism information characteristics was 'accessibility,' but it showed that 'immediacy' factor did not give any impact on the tourism motivation. Also, it resulted that 'accessibility' and 'interaction' factors of SNS tourism information characteristics significantly affect tour site selection. Meanwhile, the 'immediacy' factor did not influence on tourism motivation. These results suggest that the interested parties of organizations, community, and individuals related to tourism should focus on the convenience of tourism information provided via SNS, rather than immediacy. When providing SNS tourism information, the strategies for offering accessibility to users, such as easy search, easy editing for information delivery, and easy storing method, should be prioritized over other factors.
A Magnetic Resonance-based Seed Localization Method for I-125Prostate Implants

This study was performed to develop and evaluate a semi-automatic seed localization algorithm from magnetic resonance (MR) images for interstitial prostate brachytherapy. The computerized tomography (CT) and MR images (3 mm-slice thickness) of six patients who had received real-time MR imaging-guided interstitial prostate brachytherapy were obtained. An automatic seed localization method was performed on CT images to obtain seed coordinates, and an algorithm for seed localization from MR images of the prostate was developed and tested. The resultant seed distributions from MR images were then compared to CT-derived distribution by matching the same seeds and calculating percent volume receiving 100% of the prescribed dose and the extent of the volume in 3-dimensions. The semiautomatic seed localization method made it possible to extract more than 90% of the seeds with either less than 8% of noises or 3% of missing seeds. The mean volume difference obtained from CT and MR receiving 100% of the prescribed dose was less than 3%. The maximum extent of the volume receiving the prescribed dose were 0.3, 0.6, and 0.2 cm in x, y, and z directions, respectively. These results indicate that the algorithm is very useful in identifying seeds from MR image for post-implant dosimety.
Concept analysis of Negative capability: Hybrid Model

Aim(s): To identify and define the concept of negative capability. Method(s): The hybrid model method was used to perform a conceptual analysis of negative capability. Data were obtained from 32 relevant literature reviews and also from the 3 focus group interviews of twelve nurses. The final attributes, definition, preceding factor and consequences of negative capability were derived by comparing and analyzing the properties confirmed through the field work phase and the theoretical work phase. Result(s): The preceding factor of negative capability was uncertainty. The concept of negative capability was found to have 6 attributes such as submission of self(negation of one’s own thoughts and endures uncertain situations at clinical fields), openness(exposing oneself to new environments, psychological openness based on a relaxed mind and narrative filling in a phenomenon), state of calm detachment(emptying even under pressure to perform the task, stop in detached state, observe objectively and grasp the situation as it is), humanity(empathy, compassion for the patient and fellowship), rationality(compromise with each other, seeking rational evidences and rational choice through flexible methods) and acceptance(diversity recognition, meaning shift based on reflective thinking and accept the status quo). Conclusion(s): The negative capability was defined as ability to submission oneself and pursue problem-solving through rational thinking and acceptance from an objective perspective in situations of uncertainty in the clinical field, including relationships between patients and colleagues and other occupations. Tool for measuring negative capability of nurses and effective programs for enhancing wise clinical judgement, intuitive decision-making, and flexible leadership should be developed in future studies.
