        대장내시경검사의 전처치로서 항콜린제 사용에 관한 연구 : 전향적, 이중맹검법으로

        Background: Use of antispasmodic medication prior to colonoscopy is controversial but someone believes antispasmodic may improve visualization of colonic mucosa and ease colonoscope insertion. So, we designed a study to assess the effect of premedication with the antispasmodic, hyoscine-N-butyl bromide(Buscopan') on the performance of colonoscopy. Methods: This study was prospective, double blinded, randomized, controlled study, One hundred three consecutive patients were randomized to receive intravenous buscopan lml(n=52) or placebo(n=51) combined with our standard initial medication(me- peridine 50 mg and midazolam 2 mg). Insertion of colonoscopy was timed, and 100 mm visual analogue scales (VAS) were used for asscssing difficulty of procedure, colonic motility, frequency of positional change, frequency of external compression, difficulty of assistance and degree of discomfort experienced by the patients. Results: There were no significant differences of intubation time between buscopan group(mean time, 7.23 min., range 2~15) and placebo group(7.07 min., range 3-25), (p=0.83) and withdrawal time between buscopan group (6.46 min., range 2-22) and placebo group(6.76 min., range 2 25), (p=0.69). Also, there was no significant differences in intubation time between males and females(buscopan; males 7.00 min., females 7.60 min., p=0.34, placebo; males 7.0~5 min., females 7.08 min., p 0.44). The VAS scores checked by endoscopist(p=0.29), assistant(p=0.32) and patient (p=0.15) were not significantly different in both groups. There were no significant differences in intubation time, VAS scores nf endoscopist, assistant, and patients. Conclusion: Premedication with intravenous bu.opan has no advantage on colonoscopy procedure. Use of antispasmodic medication prior to colonoscopy was not considered as recommendable agent.

        저온 플라즈마 처리한 쌀국수의 품질 특성

        쌀국수의 유통 안전성 확보를 위한 기초자료를 확보하기 위하여 최근 각광받고 있는 저온 플라즈마 처리에 의한 쌀국수의 품질 특성 분석을 하였다. 본 연구에서 이용한 플라즈마는 컨테이너형 유전격벽 플라즈마로 방전 가스는 공기를 활용하여 0, 5, 10 및 20분 처리하였고 미생물 감균효과, 색도, 경도 및 지질산패도 변화를 측정하였다. 쌀국수에 B. cereus 및 E. coli O157:H7을 접종한 후 20분간 플라즈마 처리 시 E. coli O157:H7은 2.75 log CFU/g 감소하였고, B. cereus는 검출되지 않았다. 색도 측정결과 플라즈마 처리에 의해 명도(L<SUP>*</SUP>), 적색도(a<SUP>*</SUP>) 및 황색도(b<SUP>*</SUP>) 모두 증가하였으며, 경도는 처리시간이 증가함에 따라 감소하는 경향을 보였다. 하지만 플라즈마 처리에 의해 지질산화가 일어나는 경향을 보였다. 따라서 공기로 방전된 저온 플라즈마 기술은 쌀국수의 품질안전성을 개선될 것이라고 판단되나 관능적 품질 특성 개선을 위한 적합 플라즈마 모델 선정, 포장방법 개선 등의 후속연구가 필요하다고 판단된다. Cold plasma has been applied to improve quality of food product; however, studies on its effects on microbial and physicochemical qualities of rice noodles are rarely conducted. In this study, changes in the quality characteristics of rice noodles treated by cold plasma were determined. Cold plasma was generated in a square-shaped plastic container (250 W, 15 kHz, ambient air), and dielectric barrier discharge plasma treatments were applied to rice noodle samples for 0, 10, or 20 min. Rice noodles inoculated with either Bacillus cereus or Escherichia coli O157:H7 were subjected to plasma treatment for 20 min, and the approximate bacterial count reduction were 4.10 and 2.75 log CFU/g, respectively. The Hunter color values of the sample were increased after cold plasma treatment. Peroxide values and thiobarbituric acid reactive substance (TBARS) were increased with an increase in cold plasma treatment time. Futhermore, lipid oxidation was enhanced. Although further studies are warranted to evaluate changes in chemical qualities, such as lipid oxidation of rice noodles, induced by cold plasma, the results suggest that cold plasma can improve the microbial and physical qualities of rice noodles.

      • 국소성 분절성 사구체 경화증의 완전 관해 후 발병한 IgA 신병증 1예

        We report a rare case of primary glomerular disease with IgA nephropathy in a 43-year old man. 7-years ago, he was diagnosed with minimal change nephrotic syndrome. After treatment with steroid for 1-year, azotemia and proteinuria were improved. But, nephrotic range proteinuria was relapsed within 1 week after cessation of steroid. Renal biopsy revealed focal segmental glomerulosclerosis, and achived a complete remission with the use of steroids and cyclophosphamide. Then, he was followed up with normal renal function for 4-years. On admission, the patient showed generalized edema and marked proteinuria. In 3rd renal biopsy, mesangial proliferative glomerulonephritis with diffuse global sclerosis was observed. Electron microscopy showed electron-dense deposit on the mesangium with slight mesangial proliferation, and immunofluoroscence microscopy showed granular deposits of IgA and IgM. These features were consistent with IgA nephropathy, different from results of previous biopsies. The pathogenesis of IgA nephropathy occurred with primary FSGS is not clear, as well as treatment. Now, proteinuria is improving, steroid and cyclophosphamide are probably useful for the disease.

        Sludge Solubilization using Microwave Irradiation in the Presence of Fe Powder

        In this study, microwave irradiation, which is reflected by metals, was used to reduce the amount of sewage sludge, and the results were used to verify solubilization efficiency and determine optimum operation conditions. Biogas production and methane content of the gas under optimized conditions were measured with the biochemical methane potential (BMP) test. The sludge was taken from a thickened sludge tank at J sewage treatment plant (JSTP) in Seoul, Korea. For the experiments, 50 mL of sludge was filled in vessels and the vessels were irradiated with the power of 500, 600, 700, and 800W for 2~5 min. In addition, Fe powder was added by 0.01, 0.05, and 0.1 g to compare the efficiency with and without Fe powder. The results confirmed that solubilization efficiency was higher in the presence of Fe powder. The optimum conditions of 0.01 g addition of Fe powder with 800W irradiation for 5 min, yielded nearly 22.95% higher solubilization efficiency than without Fe powder. The BMP tests were carried out using sludge obtained from the experiments carried out under the optimum conditions. As a result, sludge subjected by 800W with 0.01 g of Fe powder for 5 min displayed the highest level of gas production and methane content. Through this study, it could be confirmed that solubilization efficiency increased by addition of Fe powder.

        A Rapid and Sensitive Two - Site Sandwich Enzyme - Linked Immunosorbent Assay for Detection of α - Fetoprotein in Human Serum

        A rapid and sensitive method has been developed to detect α-fetoprotein (AFP) in human serum by a two-site sandwich enzyme-linked immunosorbent assay (ELISA) with monoclonal antibodies (MAbs) for human AFP within 1 h. To obtain the most sensitive and reliable MAbs, 12 kinds of MAbs (HPJ1 to HPJ12) as a capture antibody and 4 kinds of horseradish peroxidase (HRP) conjugated MAbs as a tracer antibody were investigated. Among these, only HPJ 10-HRP conjugated HPJ 1 (HPJ 10-HPJ 1*) and HPJ 11-HRP conjugated HPJ 10 (HPJ 11-HPJ 10*) were chosen as candidates based on the linearity of the standard curve and the sensitivity of the assay. To further characterize these two pairs, MAbs against human AFP were purified from hybridoma cells, conjugated with HRP, and then characterized to optimize the two-site sandwich ELISA. The HPJ 10-HPJ 1 * pair showed a sensitivity of 1 ng/㎖ and a better reproducibility than the HPJ 11-HPJ 10* pair when the human sera were incubated at 37℃ for 30 min. The results obtained for 480 randomly selected human sera showed 0∼20 ng/㎖ of AFP values for the normal human sera. To test the utility of our kit, AFP concentrations were determined for 951 human sera (including 85 normal sera, 480 random blood sera, 213 HBsAg-positives, 50 anti-HCV antibody positives, and 47 malignant diseases) and compared with other commercially available AFP detecting kits. These results show that the present two-site sandwich ELISA method is a rapid, sensitive, and reliable procedure for detecting AFP in human serum.

      • 3D Asthenopia in Horizontal Deviation

        <P><I>Purpose:</I> This study was conducted to investigate the asthenopic symptoms in patients with exotropia and esotropia while watching stereoscopic 3D (S3D) television (TV).</P><P><I>Methods:</I> A total 77 subjects who more than 9 years of age were enrolled in this study. We divided them into three groups; Thirty-four patients with exodeviation (Exo group), 11 patients with esodeviation (Eso group) and 32 volunteers with normal binocular vision (control group). The S3D images were shown to all patients with S3D high-definition TV for a period of 20 min. Best corrected visual acuity, refractive errors, angle of strabismus, stereopsis test and history of strabismus surgery, were evaluated. After watching S3D TV for 20 min, a survey of subjective symptoms was conducted with a questionnaire to evaluate the degree of S3D perception and asthenopic symptoms such as headache, dizziness and ocular fatigue while watching 3D TV.</P><P><I>Results:</I> The mean amounts of deviation in the Exo group and Eso group were 11.2 PD and 7.73PD, respectively. Mean stereoacuity was 102.7 arc sec in the the Exo group and 1389.1 arc sec in the Eso group. In the control group, it was 41.9 arc sec. Twenty-nine patients in the Exo group showed excellent stereopsis (≤60 arc sec at near), but all 11 subjects of the Eso group showed 140 arc sec or worse and showed more decreased 3D perception than the Exo and the control group (<I>p</I> < 0.001, Kruskal-Wallis test). The Exo group reported more eye fatigue (<I>p</I> < 0.001, Kruskal-Wallis test) than the Eso and the control group. However, the scores of ocular fatigue in the patients who had undergone corrective surgery were less than in the patients who had not in the Exo group (<I>p</I> < 0.001, Kruskal-Wallis test) and the amount of exodeviation was not correlated with the asthenopic symptoms (dizziness, <I>r</I> = 0.034, <I>p</I> = 0.33; headache, <I>r</I> = 0.320, <I>p</I> = 0.119; eye fatigue, <I>r</I> = 0.135, <I>p</I> = 0.519, Spearman rank correlation test, respectively).</P><P><I>Conclusion:</I> Symptoms of 3D asthenopia were related to the presence of exodeviation but not to esodeviation. This may indicate that S3D symptoms are closely related to the convergence demand.</P>

        Studies on Lytic, Tailed Bacillus cereus-specific Phage for Use in a Ferromagnetoelastic Biosensor as a Novel Recognition Element

        This study investigated the feasibility of the lytic, tailed Bacillus cereus-specific phage for use in a ferromagnetoelastic (FME) biosensor as a novel recognition element. The phage was immobilized at various concentrations through either direct adsorption or a combination of 11-mercapto-1-undecanoic acid (11-MUA) and [N-(3-dimethylaminopropyl)-N’-carbodiimide hydrochloride and N-hydroxysuccinimide (EDC/NHS)]. The effects of time and temperature on its lytic properties were investigated through the exposure of B. cereus (4 and 8 logCFU/ml) to the phage (8 logPFU/ml) for various incubation periods at 22°C and at various temperatures for 30 and 60 min. As the phage concentration increased, both immobilization methods also significantly increased the phage density (p < 0.05). SEM images confirmed that the phage density on the FME platform corresponded to the increased phage concentration. As the combination of 11-MUA and EDC/NHS enhanced the phage density and orientation by up to 4.3-fold, it was selected for use. When various incubation was conducted, no significant differences were observed in the survival rate of B. cereus within 30 min, which was in contrast to the significant decreases observed at 45 and 60 min (p < 0.05). In addition, temperature exerted no significant effects on the survival rate across the entire temperature range. This study demonstrated the feasibility of the lytic, tailed B. cereus-specific phage as a novel recognition element for use in an FME biosensor. Thus, the phage could be placed on the surface of foods for at least 30 min without any significant loss of B. cereus, as a result of the inherent lytic activity of the B. cereus-specific phage as a novel recognition element.

        온풍 조건에서 수분 탈락 정도에 따른 피부 분류 및 개선 방안에 대한 연구

        피부를 탄력있고 부드럽게 하는 역할은 각질층에 존재하는 수분량에 의해 좌우된다. 피부 수분량은 냉온풍, 건조환경 등 다양한 환경 변화에 의해 영향을 받음이 알려져 있으나, 개인 피부 차이에 따른 피부수분량 변화와 회복 정도에 대해서는 많은 연구가 이루어 지지 않은 실정이다. 본 연구에서는 온풍 조건하에서 피시험자들의 피부 수분 탈락 및 회복 정도를 비교 평가하여 새로운 피부 타입을 제시하고, 온풍 조건에서 저하되는 피부 수분량을 개선 시켜주는 효능 물질을 개발하고자 하였다. 온풍 환경 조성을 위해, 건강한 피험자(남: 10 명, 여: 39 명, 25 세 - 63 세)의 전완부에 온풍(30 cm, 40 ℃, 6 m/s)을 30 min 간 피부에 노출시켜, 피부 수분량의 변화를 평가하였다. 26명(남: 4 명, 여: 22 명, 평균 연령: 42.7 ± 9.4)이 온풍 노출전에 비하여 온풍 노출 후 수분량이 유의하게 감소하며, 노출 후 30 min이 지나도 회복이 되지 않았다. 온풍노출 후 수분량이 떨어지는 피험자(여: 10 명)를 대상으로 보수력이 높은 크림을 3 주간 전완부에 적용한 이후 동일 온풍 조건하에서 피부 수분량을 측정한 결과, 노출 30 min 후 피부 수분량이 온풍 노출 전 수준으로 회복됨을 확인하였다. 본 연구를 통하여 피부는 건조 조건에서 쉽게 수분을 잃어버리는 피부(탈수형 피부)가 존재하는 것을 확인하였다. 이는 앞으로 화장품 개발을 보습 기능뿐만 아니라, 이러한 환경변화에 따른 피부수분이 쉽게 빠져나가는 피부(탈수형 피부)의 특성에 맞는 제품의 효능 개발이 필요함을 보여준다. Elasticity and softness of the skin depend on the level of moisture present in the stratum corneum, which is known to be affected by various environmental changes, such as cold and hot winds and dry environments. However, not many studies have been conducted on changes in skin moisture and the degree of recovery due to individual skin differences. In the present study, we aimed to investigate the effect of warm air heating on skin hydration levels and develop moisturizing formulas to improve lowered skin hydration levels. In order to deliver warm air heating condition, heating dryer (40 ℃, 6 m/s, 30 cm apart from forearm) was applied into inner forearm of healthy subjects (male: 10, female: 39, age: 25 - 63) Among 49 subjects, 26 subjects showed significantly lowered skin hydration levels until 30 min after warm air heating exposure (lowered group). In addition, moisturizing cream with high water holding capacity was applied to forearm of 10 subjects in lowered group for 3 weeks and skin hydration levels after warm air heating were significantly improved at the levels of before application of warm air heating. From this study, we found out that there is a skin type that skin hydration levels are significantly decreased under warm air heating condition (dehydrated skin) and this dehydrated skin can be improved by moisturizing formulas with high water holding capacity.
