C. fetus 심내막염은 드물게 발생하는 질환으로 주로 면역능이 저하된 환자에서 발병하고 대동맥판을 침범하는 경우가 많으며 적절한 치료가 되지 않을 경우 치사율이 높다. 저자들은 기저질환이 없던 69세 남자에서 C. fetus 심내막염을 경험하였기에 문헌고찰과 함께 보고하는 바이다. 환자는 발열로 내원하여 시행한 혈액배양 검사상 C. fetus가 동정되었고 경식도 초음파 검사상 삼첨판에 증식증이 발견되어 심내막염 진단하에 ceftriaxone에 이어 meropenem으로 총 4주간 항생제를 투여받고 호전되었다가 한달 후 다시 재발하였고 판막부전 소견 진행하여 삼첨판 치환술을 시행받고 전신상태 호전되어 퇴원하였다. Campylobacter fetus is a rare cause of endocarditis. To date, only about 30 cases of C. fetus endocarditis have been reported mainly in the elderly with chronic underlying illnesses. Most cases showed tendency to involve aortic valve. This report describes a case of native tricuspid valve infective endocarditis in a 69-year-old man without any underlying disease. Despite adequate antibiotic therapy for 4-weeks, endocarditis recurred and leading to surgical valve replacement.
흉요추부 방출성 골절의 척수강 복원에 대한 방사선학적 고찰

We analyzed 44 cases of burst fractures on the thoracolumbar junction which were operated with posterior instrumentation technique at the Department of Chungnam University Hospital from 1988 to July 1995. Reduction of the spinal fragment through kyphosis correction and distraction were evaluated using conventional radiographs and CT imaging between postop., POD 6 months and POD 12months in 23 cases of burst tractures. The purpose of this study was to evaluate the spinal canal remodeling using radiographic means. The result were as follows : 1. The average kyphotic correction angle was 7.4˚ and there was no correlation between the reduction of retropulsed fragments and the amount of kyphotic angle correction. 2. There was significantly correlation between the amount of reduction of posterior vertebral body and amount of retropulsed fragment. 3. There was no correlation between the degree of canal compromise and the Denis type of thoracolumbar burst fractures. 4. There was significantly correlation between the shape of the patent portion of the spinal canal and neulologic deficits.

Clusterin is a secretory glycoprotein, which is highly up-regulated in a variety of normal and injury tissues undergoing apoptosis including infarct region of the myocardium. Here, we report that clusterin protects H9c2 cardiomyocytes from $H_2O_2$-induced apoptosis by triggering the activation of Akt and GSK-$3{\beta}$. Treatment with $H_2O_2$ induces apoptosis of H9c2 cells by promoting caspase cleavage and cytochrome c release from mitochondria. However, co-treatment with clusterin reverses the induction of apoptotic signaling by $H_2O_2$, thereby recovers cell viability. The protective effect of clusterin on $H_2O_2$-induced apoptosis is impaired by PI3K inhibitor LY294002, which effectively suppresses clusterin-induced activation of Akt and GSK-$3{\beta}$. In addition, the protective effect of clusterin is independednt on its receptor megalin, because inhibition of megalin has no effect on clusturin-mediated Akt/GSK-$3{\beta}$ phosphoylation and H9c2 cell viability. Collectively, these results suggest that clusterin has a role protecting cardiomyocytes from oxidative stress and the Akt/GSK-$3{\beta}$ signaling mediates anti-apoptotic effect of clusterin.

Purpose: The overall incidence of central precocious puberty (CPP) has increased in recent decades, and brain magnetic resonance imaging (MRI) evaluations are recommended in cases of suspected brain lesions. This study aimed to investigate the prevalence of MRI abnormalities and to evaluate the need for routine brain MRI in patients with newly diagnosed CPP. Methods: This retrospective study reviewed the data of patients newly diagnosed with CPP who underwent routine pituitary MRI at Korea University Anam Hospital from March 2020 to September 2021. A total of 199 girls and 24 boys was enrolled in this study. Positive MRI findings were categorized as abnormal pituitary, nonpituitary incidental, and pathological. In addition, we investigated the incidence of MRI abnormalities and evaluated their associations with clinical and biochemical factors. Results: Positive brain MRI findings were observed in 84 patients (37.7%). Pituitary abnormalities were found in 54 patients (24.2%), with Rathke cleft cysts being the most common (16.1%). Incidental nonpituitary findings were observed in 29 patients (13.0%), while a pathological brain lesion (diagnosed as hypothalamic hamartoma) was observed in only 1 female patient (0.4%). No significant differences in sex or age were found in incidence of pituitary abnormalities or nonpituitary incidental findings. Compared with headache controls, significant associations were observed between abnormal pituitary findings on MRI and CPP (unadjusted odds ratio, 3.979; 95% confidence interval, 1.726–9.173). Conclusion: True pathological findings were rare, even though the prevalence of abnormalities on pituitary MRI in patients with CPP was relatively high. Considering its cost-effectiveness, MRI screenings should be carefully considered in patients with CPP.

산업 제어 시스템은 사회 기반 시설이나 산업체의 설비를 감시하고 제어하는 시스템을 의미한다. 제어시스템의 효율적 관리를 위해 상용 기술과의 유사성이 증가함에 따라 기존 정보시스템들이 가지는 보안 취약점 또한 수반하게 되었다. 따라서 제어시스템에 대한 침해사고 또한 증가하게 되었으며, 우리나라의 경우 대표적으로 한수원 해킹 사건이 있다. 이렇듯 기반시설에 대한 침해 사고가 늘어나 산업제어시스템 보안에 대한 중요성이 강조되면서 국내외로 제어시스템 보안 표준이 제정 · 개정되고 있다. 특히, NERC CIP, ISA/IEC 62443, NIST 800 series 와 같은 표준은 매년 또는 격년마다 개정이 되는 등 매우 활발히 변화하는 사이버 위협에 적극적으로 대응하고 있다. 본 논문에서는 2006년부터 현재까지 발생한 주요 침해사고에 대해 알아보고, 동 기간 내 국제 표준 동향 및 국내 정보보호 관리체계를 조사한다. 이를 통해 국내 정보보호 관리체계의 구체적인 문제점을 파악하여 보다 나은 산업제어시스템의 안전성을 확보하고자 한다.
