본 연구의 목적은 GIS 공간분석기법을 활용하여 청주시 행정구역별 소방서비스 공급특성을 분석하고, 소방서비스 공급 취약지역을 도출하는 것이다. 이를 위해 먼저, GIS 공간분석기법의 하나인 Kernel 밀도 분석을 통해, 청주시 내의 불법 주차 및 정차 발생 지점의 밀도를 분석하여 불법 주정차 고밀도 발생 위험 지역을 파악했다, 다음으로 GIS 네트워크 분석을 통해 소방서 출발 기준 5분, 10분 내 소방차 접근 취약 권역을 도출했다. 연구 결과, 청주시에는 적절한 소방 서비스가 공급되지 못하고 있으며, 중심 지역과 외곽 지역의 소방 서비스 취약 지역의 비율에 차이가 있음을 확인하였다. 이러한 서비스 불균형을 극복하기 위해 외곽지역에 적절한 소방 서비스가 배치되어야 하며, 중심 지역에는 불법 주차 단속 등의 조치가 필요하다. 본 연구의 결과는 소방서비스 취약지역을 위해 소방대 배치의 기본 자료로 활용될 수 있다. This study analyzed the characteristics of fire services in Cheongju City and identified vulnerable areas using GIS spatial analysis method. It employed a kernel density analysis to identify the areas with high density of illegal parking and stopping, and the GIS network analysis to calculate fire protection accessibility for fire trucks to reach the destination within both 5 minutes and 10 minutes from the departure at the fire station. This study found that adequate fire service has not been provided for Cheongju City and there is a difference in fire service vulnerability in the outskirt of Cheongju City. To solve this problem, it is recommended that a proper fire-fighting arrangement needs to be provided in the vulnerable areas. The results of this study can be used as the basic data for targeted arrangement of fire-fighting services in Cheongju City.
Primary cutaneous B-cell lymphoblastic lymphoma in an elderly man

Precursor B-cell lymphoblastic lymphoma (B-LBL) is an uncommon high-grade neoplasm of immature B cells. It occurs predominantly in childhood with extranodal involvement such as skin and bone. Therefore, primary cutaneous involvement in elderly adults is a very rare manifestation of B-LBL. Here, we report a 78-year-old man with B-LBL presenting as a single cutaneous lesion which was immunohistochemically positive for leukocyte common antigen (LCA), CD79a, paired box 5 (PAX5), B cell lymphoma-2 (bcl-2), and terminal deoxynucleotidyl transferase (TdT) staining, but was without systemic involvement. The patient was treated using cyclophosphamide, adriamycin, vincristine, and prednisolone (CHOP), and achieved complete response (CR) at the first response assessment conducted after 3 CHOP cycles. After an additional cycle of CHOP treatment, radiotherapy was administered at a total dose of 3,600 cGy over 4 weeks. At the 21-month follow-up, he had maintained CR.

<P><B>Abstract</B></P> <P><B>Background</B></P> <P>Atopic dermatitis (AD) is a highly pruritic, chronic inflammatory skin disease associated with hyperreactivity to environmental triggers. Among those, outdoor air pollutants such as particulate matter (PM) have been reported to aggravate pre-existing AD. However, underlying mechanisms of air pollution-induced aggravation of AD have hardly been studied.</P> <P><B>Objective</B></P> <P>To investigate the molecular mechanisms by which glyoxal, a PM-forming organic compound, exacerbates the symptoms of AD induced by neonatal capsaicin treatment.</P> <P><B>Methods</B></P> <P>Naïve and AD rats had been exposed to either fresh air or vaporized glyoxal for 5 weeks (2 h/day and 5 days/week) since one week of age. Pruritus and dermatitis were measured every week. The skin and blood were collected and immunological traits such as Staphylococcus aureus skin colonization, production of antimicrobial peptides and immunoglobulin, and mRNA expression of inflammatory cytokines were analyzed.</P> <P><B>Results</B></P> <P>Exposure to glyoxal aggravated pruritus and dermatitis in AD rats, but did not induce any symptoms in naïve rats. Staphylococcus aureus skin colonization was increased in the skin of both naïve and AD rats. Expression of antimicrobial peptides such as LL-37 and β-defensin-2 was also increased by exposure to glyoxal in the skin of both naïve and AD rats. The mRNA expression of Th1-related cytokines was elevated on exposure to glyoxal. However, serum immunoglobulin production was not significantly changed by exposure to glyoxal.</P> <P><B>Conclusion</B></P> <P>In AD rats, exposure to glyoxal exacerbated pruritus and cutaneous inflammation, which was associated with increased colonization of <I>S. aureus</I> and subsequent immunological alterations in the skin.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Exposure to glyoxal aggravated the symptoms in AD rats, but did not induce AD in naïve rats. </LI> <LI> <I>S. aureus</I> skin colonization and subsequent expression of antimicrobial peptides were increased after exposure to glyoxal. </LI> <LI> Exposure to glyoxal elevated the production of Th1-related cytokines such as TNF-α and IFN-γ in the AD lesional skin. </LI> </UL> </P>

본 연구는 “인천 고교생의 행복감에 영향을 미치는 요인은 무엇인가”라는 문제의식에서 비롯되었다. 이를 위해 연구자들은 인천 고교생(남고, 여고, 남녀공학) 462명에 대해 기존에 개발된 부모 의 양육태도 검사와 본 연구팀에서 개발한 고등학생 행복요인 척도를 통해 설문조사를 실시하였다. 탐색적 요인검사를 통해 적절한 수준의 신뢰도를 지닌 행복의 측정요인은 학교환경, 교육활동 및 교사와의 관계, 친구와의 관계, 가정생활 만족, 부모의 학업성취 압력, 전반적 만족 및 행복감이 었다. 교육활동, 교사와의 관계, 친구와의 관계에 대한 만족도가 높을수록 그리고 가정생활에 대한 만족도가 높을수록 전반적인 만족 및 행복감도 높은 것으로 나타났다. 문항별 만족도(6점만점) 가 운데 가장 낮은 것은 학업성적(2.53)이었다. 학업성적이 낮은 6~9등급에 속한 학생들은 1-2등급, 3-5등급에 속하는 학생들에 비해 학교활동, 교육활동, 교사와의 관계, 친구와의 관계, 가정생활 만 족에 대해 상대적으로 만족도가 낮았다. 인천 고교생의 행복감을 설명하는 상대적 예측력 수준은 가정생활 만족, 친구와의 관계, 교육활동, 교사와의 관계 순으로 나타났다. 가정생활 만족과 관련하 여 부모의 양육태도 가운데 애정형과 자율성 존중형이 고교생의 행복감에 긍정적 영향을 미치는것으로 확인되었다. 끝으로 고교생의 행복요인에 있어 남녀차 또한 일정 부분 발견되었다. This study started from research problem, “what factors influence the subjective happiness of Incheon high school students?” We conducted survey Incheon high school students(in total 462) through the existing inventory for parenting behavior and subjective happiness factor scale for high school students. By exploratory factor analysis, measurement factors of subjective happiness which obtain favorable reliability run as follows: school surroundings, educational activities & student-teacher relationship, relationship between friends, family life, parental achievement pressure, overall satisfaction and subjective happiness. In proportion as the satisfaction with educational activities & student-teacher relationship, and relationship between friends, the results show that overall satisfaction and subjective happiness is higher. Participants responded the lowest satisfaction(2.53) with academic achievement among each test item(6-point scale). Compared with high school record(level 1-2 & level 3-5), students(level 6-9) showed relatively lower satisfaction with educational activities & student-teacher relationship, relationship between friends, family life. The orders of relative predictability to explain subjective happiness of Incheon high school are as follows: family life, relationship between friends, educational activities, and student-teacher relationship. In terms of parent’s rearing attitudes, affection and respect of autonomy type influence positively subjective happiness of high school students. The factor of gender difference had meaningful influence upon subjective happiness of Incheon high school students.

Mycobacterium fortuitum is a rare pathogen, frequently found in water, soil, animals and plant materials, It can cause infections involving skin, soft tissue and skeletal system after direct inoculation of the pathogen through surgical traumas, Punctures and injections. We report a case of infrapatellar bursitis caused by M. fortuitum in an immunocompetent, 42-year-o1d female, which occurred after bicycle trauma. She experienced marked improvement after surgical excision and debridement of the wound site and antimicrobial therapy.
