        An Observational, Multicenter, Cohort Study Evaluating the Antiviral Efficacy and Safety in Korean Patients With Chronic Hepatitis B Receiving Pegylated Interferon-alpha 2a (Pegasys) : TRACES Study

        <P><B>Abstract</B></P><P>Currently, limited data are available regarding the efficacy and safety of pegylated interferon alpha-2a (PEG-IFN α-2a) in Korean patients with chronic hepatitis B (CHB), in whom hepatitis B virus (HBV) genotype C is the most common type.</P><P>We collected data from 439 patients (HBeAg positive, n = 349; HBeAg negative, n = 90) with CHB who were treated with PEG-IFN α-2a as a first-line therapy from 18 institutions. Treatment responses at the end of treatment (ET) and at 6 months posttreatment (PT6) were compared between the patients who were treated for 24 weeks versus 48 weeks, and adverse events (AEs) were evaluated.</P><P>In HBeAg-positive patients, those who received PEG-IFN α-2a for 48 weeks showed significantly higher HBV DNA suppression (HBV DNA < 2000 IU/mL) than those who were treated for 24 weeks (48 weeks vs 24 weeks; at ET, 44.4% vs 36.7%, <I>P</I> = 0.035; at PT6, 35.9% vs 13.3%, <I>P</I> = 0.035). The HBeAg seroconversion rate at ET was 18.1% in 48-week treatment group, which is significantly higher than the 2.2% (<I>P</I> < 0.001) that was seen in 24-week treatment group. This finding also continued at PT6 (29.0% vs 10.0%, <I>P</I> < 0.001). Following 48 weeks of treatment in HBeAg-negative patients, HBV DNA suppression at ET was higher than in HBeAg-positive patients (87.8% vs 44.4%). AEs were typical of those associated with PEG-IFN α-2a.</P><P>In naïve Korean HBeAg-positive CHB patients treated with PEG-IFN α-2a, higher rates of HBV DNA suppression and HBeAg seroconversion were achieved in the 48-week treatment group than in the 24-week treatment group without additional risk of AEs.</P>

        Currently, the number of elderly people aged 65 or older in Korea is increasing steadily, reaching 14.3 %. As the number of elderly people increases, the number of elderly welfare facilities such as nursing homes is also increasing, and the number of elderly welfare facilities has increased to 75,708 in 2016 from 50,517 in 2003. In November 2010, a fire occurred at an elderly care center in Pohang city, and There were 17 deaths and 10 injured. Since then National Fire Safety Code have been significantly strengthened. However, accidents of evacuees such as medical welfare facilities for the elderly are continuously occurring. Now, Institutional research on evacuation planning is underway in many areas. we calculated the evacuation time (ASET : Available Safe Egress Time) according to presence or absence of fire facilities and the evacuation time (RSET : Required safe egress time) associated with the type of building in the elderly medical welfare facilities. In this study, fire facilities, which are essential for elderly medical welfare facilities, are operated for efficient evacuation by using Pyrosim and Pathfinder for sofa in living facilities (elderly medical welfare facilities) And whether they can evacuate. The conclusion was that normal operation of fire facilities and additional building considerations (such as ramps and exit widths) could be identified for evacuation of non-durable patients.

Methods: We collected data from 18 institutes of 451 Korean patients with CHB who were treated with PEG-IFNa-2a as a first line therapy (370 patients with HBeAg positive and 81 with HBeAg negative). Treatment responses at the end of treatment (ET) and at 6 months post-treatment (PT6) were compared between patients treated for 24 weeks vs. 48 weeks, and adverse events were evaluated. Results: In HBeAg-positive patients, the patients who had received PEG-IFNa-2a for 48 weeks than 24 weeks achieved significantly higher virological response (HBV DNA < 2,000 IU/mL) (48 vs. 24 weeks at ET, 44.4% vs. 27.2%, p = 0.005; at PT6, 46.7% vs. 17.2%, p=0.001). Complete virological response (HBV DNA < 60 IU/mL) at ET was also achieved in more patients with a longer treatment (48 vs. 24 weeks 22.0% vs. 13.0%, p=0.005). HBeAg seroconversion rate at ET was 18.1% with 48 weeks treatment, which is significantly higher than 10.1% (p=0.012) with 24 weeks treatment. This finding was continued to PT6 (23.7% vs. 17.2%, p = 0.028). The rate of ALT normalization was increased from 61.4% at ET to 76.8% at PT6 on 48 weeks-treatment. HBsAg seroconversion was not common (48 vs. 24 weeks at ET, 0.4% vs. 0%; at PT6, 0.8% vs. 0%). In HBeAg-negative patients, virologic response at ET was higher than that in HBeAg-positive patients following 48 weeks treatment (serum HBV DNA< 60 and < 2,000 IU/mL; 60.8% and 87.8%, respectively). HBsAg seroconversion was 1.4% at ET and it was maintained at PT6. Adverse events were typical of those associated with PEG-IFNa-2a. Conclusion: In Korean CHB patients, PEG-IFNa-2a showed substantial treatment response and good tolerability. In patients with HBeAg-positive CHB, the longer treatment of PEG-IFNa-2a (48 weeks rather than 24 weeks) was more efficacious with similar safety profiles.

        무극 함금석영맥광상 화강암질모암에서의 원소들의 분산형태와 분산범위

        Geochemical studies on gold-bearing quartz veins and granitic wallrocks from the Mugeug mine were carried out in order to investigate primary dispersion patterns of elements and to quantify primary dispersion widths of elements with distance from the gold-bearing quartz veins. The best fitting model of dispersion pattern in altered wallrock around the gold-bearing quartz veins is an exponential function for Au, As, Sb, $Na_2O$ and Sr, and a quadratic function for CaO, $K_2O$, MnO, Ba, Rb and Cs. The primary dispersion widths are 15~343 cm in the hanging wall, and 33~173 cm in the footwall. The width of primary dispersion in the hanging wall is twice as thick as that in the footwall mainly due to the thermal effect and volatile components. The primary dispersion width is increased as the increase of gold-bearing quartz vein width and contents of As and Sb in gold-bearing quartz veins, but is not related to Au content in gold-bearing quartz veins.

        무극 함금석영맥광상 주변모암에서의 모암변질과 원소들의 일차분산

        Mineralogical and geochemical studies on gold-bearing quartz veins and wallrock from the Mugeug mine were carried out in order to investigate the variation of mineralogical composition and the geochemical behavior of elements with distance from the gold-bearing quartz veins. Gold-bearing quartz veins occur in early Cretaceous medium- to coarse-grained biotite granite. The unaltered wallrock is composed mainly of quartz, plagioclase, orthoclase, microcline, biotite and hornblende with accessory minerals of sphene and apatite. Mineralogical changes in altered wallrock around the gold-bearing quartz veins were observed as follows; 1) biotite and hornblende altered into chlorite, and next to sericite, 2) plagioclase, orthoclase and microcline altered into sericite, and 3) calcite and quartz introduced into wallrock. Contents of $K_2O$, Rb, Cs, Au, As and Sb in altered wallrock increase, whereas those of $Na_2O$, CaO, Ba, and Sr decrease with proximity to the gold-bearing quartz veins. The loss on ignition also increases with the increase of alteration mineral. The width of primary dispersion increases in order $Au=SiO_2<As=Cs=Rb<K_2O=Sb$ and $MnO<Na_2O=CaO=Ba<Sr$. The sericitization index, $K_2O/(K_2O+Na_2O)$, is an important indicator to interpret the degree of alteration at the Mugeug mine, which is more than 0.8 in strongly and moderately altered granite, 0.5~0.8 in wea altered granite, and less than 0.5 in unaltered granite. Alteration indices for major and trace elements, and the ratio of Rb/Sr are also useful to discriminate alteration zones.

        해산식품과 채소에서 Vibrio parahaemolyticus 검출을 위한 배지배양법과 real-time PCR의 비교검증

        V. parahaemolyticus는 국내에서 여름과 가을철에 걸쳐 발생되며 생선이나 패류 등의 해산식품을 날것으로 섭취하거나 불완전하게 익혀 먹을 경우 식중독을 일으키는 병원균이다. 본 연구의 목적은 표준 검출기법인 배지배양법을 이용한 V. parahaemolyticus의 검출과 real-time PCR을 이용한 V. parahaemolyticus의 검출에 있어서 그 유효성과 효율성을 검증하는 것이다. V. parahaemolyticus의 발생기록과 발생가능성이 있는 여러 해산식품과 무순에 적절한 균량을 접종하고 APW로 증균배양하였다. 증균배양이 끝난 후 TCBS선택배지에 배양액을 획선도말하고, 동시에 증균배양액에서 1 ㎖을 채취하여 real-time PCR을 실시하였다. TCBS에서 초록색이 나온 집락을 1-3개 선별하여 TSI 배지에 접종하여 screening test를 거친 후 API 20NE strip을 사용하여 확인동정하였다. 또한 정상세균총이 목적균의 성장과 검출에 어느 정도의 영향을 미치는지 평가하기 위하여 25 g의 식품 내 정상세균총의 수준을 함께 측정하였다. 실험결과, 자체 제작한 real-time PCR서열은 V. parahaemolyticus를 특이적으로 검출할 수 있었고 검출한계는 PBS에서 10³ CFU/㎖ 였다. 또한, 식품 내 정상세균총이 높을 경우 증균배양 시 목적균의 성장에 영향을 미칠 수 있다는 것을 확인하였다. Real-time PCR은 180개의 전체 샘플 중 76개의 양성 결과를 보여 66개의 양성 결과를 낸 배지배양법에 비해 더 많은 양성 검출율을 보였으나 통계학적인 유의차는 발견되지 않았다. Real-time PCR은 표준검출법인 배지배양법과 비교해 볼때 동등하거나 우수한 검출력을 지닌 것으로 보이며 이러한 realtime PCR법은 24시간 이내에 확정동정까지 가능하여 시간과 노동력의 소모가 많은 배지배양법에 앞서 선별검사로 사용할 경우 시간, 비용, 노동력 절감에 매우 유효할 것으로 판단된다. Vibrio parahaemolyticus (V. parahaemolyticus), which is commonly found in raw seafood, causes gastroenteritis in humans. Rapid and effective methods have been developed as culture methods require up to 5-7 days. In this study, real-time PCR was compared with the standard culture method for detecting V. parahaemolyticus in seafood and radish sprout samples. Five hundred grams of the samples were artificially contaminated with V. parahaemolyticus then divided into 20 samples. The samples were incubated in alkaline peptone water and then streaked onto thiosulfate-citrate-bile saltssucrose agar. Biochemical tests for suspicious colonies were performed using the API 20NE strip. In parallel, real-time PCR was performed targeting the toxR gene using the enrichment broth. The real-time PCR was sensitive in discriminating V. parahaemolyticus from other foodborne pathogens. The detection limit of the real-time PCR was 10³ CFU/㎖ in phosphate-buffered saline. Although the real-time PCR detected more positive samples (76 out of 180, 42%) than the culture method (66 out of 180, 37%), there was no significant statistical difference (p>0.05) between the two methods. In conclusion, real-time PCR assays could be an alternative to the standard culture method for detecting V. parahaemolyticus in seafood and radish sprouts, which has many advantages in terms of detection time, labor, and sensitivity.

        Wheat is one of the most important food crops in the world and provides 20% of nutritional requirement to the consumers in developing countries. The consumption per capita of wheat is persistent but the self-sufficiency has decreased owing to the less competitive world market and double cropping system with rice in Korea. This study was conducted to obtain baseline data on the selection lines with early maturity and good agronomic traits. Geumgangmil, a leading Korean variety, was crossed with CIMMYT’s line, and a DH (double haploid) line was released using pollens of Maize in Mexico. The 71 inbred lines were grown in fields at NICS, Miryang. The culm length of inbred lines were normally distributed from 37cm to 106cm, though that of the P1 (Geumgangmil) was 71 and P2 (CIMMYT’s Line) was 77cm. Especially, some lines were very short and measured below 50cm. The spike lengths of most of the lines were longer than that of P1 (7.5cm), but were normally distributed. The number of spikelet was less than that in P2 (19ea.), but was more than P1(10ea.). The heading day of most of the lines was between P1 (11th April) and P2 (25th April) and the maturity day of the lines was similar to P1 (1st June) but 5 lines showed 2~6 days shorter maturity period than that of P1. However, 20 lines were found to have 2~10 days longer maturity period than that of P2.

        보호종인 히어리의 자생지내외 보전과 지역사회 협력 모델 개발 - III. 몇 개의 히어리 자연집단과 벌채집단 간 개체군 비교

        히어리의 자연집단과 벌채집단간의 비교는 전라남도 순천시 승주읍 접치재, 월등면, 황전면, 경상남도 진주시 명석면, 경기도 포천시 백운산 5곳의 벌채지와 주변 자연림에 분포하는 히어리군락을 대상으로 비교하였다. $100m^2$당 맹아수와 개체당 평균 맹아수는 벌채 집단에서 자연집단의 것보다 더 높게 나타났다. 반면 맹아수별 수고와 직경은 자연집단에서 더 높게 나타났다. 종조성과 식생상관성은 없으며 벌채집단과 자연집단 모두에서 큰차이 없이 생육하였다. 개괄지인 벌채집단에서 근맹아 뿐만 아니라 실생묘로도 번식하였다. 이처럼 히어리는 환경적응력이 커서 인간이 훼손시킨 벌채집단에서 근맹아와 실생묘로 번식함으로써 종 자체의 소멸은 없고, 오히려 일시적인 교란이후에 개체군 확장을 보였다. The comparison of Corylopsis coreana populations between natural groups and deforestation groups is carried in five regions such as Seungju, Woldeung, Hwangjeon in Suncheon, Myeongseok in Jinju and Mt. Baegun in Pocheon. The number of sprouting per both $100m^2$ and each individual stump were appeared more in deforestation than in natural groups. In contrast, the height and the diameter of individuals by the number of sprouting are higher and larger in the natural groups. The relationship between the floristic composition and vegetation in the two groups of C. coreana populations was not appeared. Corylopsis coreana are propagated sprouting as well as seedling in deforestation groups located an opening area. Because of the powerful environmental adaptability through growth of the sprouting and the seedling in deforestation groups, the population of C. coreana are not extinction of species oneself, and on the contrary are showed expansion of C. coreana population after temporary disturbance.

        Manganese ore deposits of the Samhan Changgun Properties are located at the valley of west-lope-side of Changgun-bong (1132m) occupied over the Myon border between Sochon-myon and Jaesan-myon Pongwha-gun, Kyongsang-Pukdo. Geology of the more property and it’s vicinity consists of Wonnan formation and Yulri formation of pre-Cambrain and Changgun limestone formation, Mica-schist formation, quartizite formation and Jaesan formation (containing coal bearing zone the unknown age. And granites and dykes were intruded into the above formation later. 1. Management deposits is embedded the formation of Janggun limestone especially Contact zone in the contact zone to of Chunyang Granite limestone enclosed by Granite, and Maginal zone of fault line in the limestone. Therefore, Chunyang Granite is closely related to ore deposit. Pegmatite which is near by ore deposit was intruded before mineralization and it seems to be a channelway of ore solution. The most important ore deposits of property grouped into south deposit, east deposit, east-Gachon deposit, South-Gachon deposit, Durimgok deposit and West deposit, out-crops at several place. Besides these deposits there also are several prospects on outcrop scathered. Hydrothermal alteration take place strongly in the well rock and it’s sequence are Characterized as following; 1) Dolomitization 2) Carbonization 3) Mamgamotozation 4) Pyritization 5) Silicification 6) Oxidation 2. The grade of manganese dioxide is up to Mn 45% in Maximum, but generally, averaging Mn 30~35% of high grade ore and averaging Mn 30~32% of manganese carbonates are mined in his property.
