Postherpetic neuralgia (PHN) is a chronic and refractory pain disease. It requires longterm treatment and follow-up. Comorbid diseases can change or aggravate the pain condition and responsiveness of patients to PHN treatment. In such cases, the cause of pain should be identified through proper testing, and appropriate treatment should be administered. Herein, we report the case of a 67-year-old man with PHN in the maxillary nerve. As the pain was being controlled with medication and infraorbital nerve block, the patient experienced deterioration of pain caused by a newly worn upper complete denture. The patient’s pain was relieved following correction of the upper complete denture.
A New Blind SLM Scheme With Low Decoding Complexity for OFDM Systems

<P>In this paper, a new blind selected mapping (BSLM) scheme is proposed for orthogonal frequency division multiplexing systems. The proposed BSLM scheme embeds the side information identifying a phase sequence into itself by giving the optimal phase offset to the elements of each phase sequence, which are determined by the biorthogonal vectors for the partitioned subblocks. When <I>U</I> phase sequences and maximum likelihood decoder are used, the proposed BSLM scheme reduces the decoding complexity by (<I>U</I>-2)/<I>U</I> compared with the conventional BSLM scheme. Also, pairwise error probability (PEP) analysis for the proposed BSLM scheme is performed over the fading channel. Based on the PEP analysis, the bit error rate (BER) performance of the proposed BSLM scheme is shown to be determined by the subblock partitioning and phase offset. Finally, it is shown that for QPSK and 16-QAM, the detection failure probability and the BER of the proposed BSLM scheme are almost the same as those of the conventional BSLM scheme through numerical analysis.</P>

배경; 수술 중, 후에 안과적 문제가 발생하는 경우는 매우 드물다. 이 연구는 견관절 수술 후 눈에 이상이 발견된 환자를 확인하고 그의 원인을 후향적으로 조사하였다. 방법; 2018년 1월에서 2019년 3월까지 본원에서 견관절경 수 술을 받은 환자를 대상으로 후향적 연구방법을 통해 환자의 의무기록, 사용 약물들을 확인하였다. 로지스틱회귀분석을 통해 눈의 이상과 관련될 수 있는 여러 인자들을 가지고 연관성을 확인하였다. 결과; 8.5% (18/211)의 환자에서 눈의 이상이 발견되었고, 유착방지제와 안과적 합병증이 통계적 연관성을 보였다(p =0.020). 결론; 수술부위의 유착방지를 위해 사용되는 유착방지제에 젤라틴과 키토산 성분이 포함되어 알레르기 반응을 일으켜 안과적 합병증을 발생시킬 수 있을 것으로 생각된다. Background: Intraoperative and postoperative ocular complications are rare. We retrospectively investigated the abnormal ocular findings in patients who underwent arthroscopic shoulder surgery. Methods: The records were investigated of patients who underwent shoulder arthroscopic surgery at our hospital between January 2018 and March 2019. Ocular complications were confirmed by review of medical records. We performed logistic regression analysis to defined the main factors associated with ocular complications. Results: The overall prevalence of postoperative ocular complications was 8.5% (18/211). A significant association was found between anti-adhesion barrier agent and ocular complications (p =0.020). Conclusions: Anti-adhesion barrier agent used during surgery may be the cause of postoperative ocular complication.

Background/Objectives: Local anesthetic systemic toxicity (LAST) are life-threatening conditions in obstetric anesthesia. Methods/Statistical analysis: The case in which a patient who underwent Cesarean delivery had performed spinal anesthesia due to a change in the position of the epidural catheter. Findings: Patient had presented cardiovascular and neurologic symptoms relevant with LAST, and was treated promptly with intravenous lipid emulsion (ILE). Improvements/Applications: LAST can also occur in spinal anesthesia and can be effectively treated with ILE.

Background: Endoscopic procedures of the esophagus are more complicated than those of other regions of the gastrointestinal tract. They have a relatively long procedure time and high risk of complications, such as perforation and bleeding. Perforations that occur during the procedure can accompany pneumoperitoneum and pneumomediastinum through leakage of insufflation air and cause severe ventilatory impairment.Case: A 58-year-old male patient underwent enucleation of leiomyoma in the esophagus using endoscopy under general anesthesia. Ventilatory impairment occurred 15 min after commencement of the procedure. Subsequently, subcutaneous emphysema and severe abdominal distension were observed. We suggested the possibility of microperforation during the procedure to the endoscopist, and he performed endoscopic clipping around the excision site of leiomyoma. Conclusions: Providing anesthetic care by anesthesiologists during endoscopic procedures is considered necessary for patient safety. Complications of endoscopic procedures can be detected and managed early without sequelae during anesthetic care.

The objective of this study was to investigate the microbial count of duck meat and duck meat products commercially available in Korea. High hydrostatic pressure (HHP) treatment was applied at 0.1, 300, 400, and 500 MPa for 5 min to enhance the microbiological safety of duck meats. The levels of total aerobic bacteria were in the ranges of 3.53-6.19 and 3.62-6.85 Log CFU/g in raw and smoked duck products, respectively. By DNA sequence analysis, we identified microorganisms responsible for spoilage, with the most common species in the raw and smoked duck products being Aeromonas spp. or Pseudomonas spp. and Leuconostoc mesenteroides, respectively. HHP treatment significantly reduced the levels of total aerobic bacteria in raw and smoked duck products. This study demonstrates that HHP treatment may be used to effectively improve the safety of raw and smoked duck meat products.
Evaluation of General Toxicity and Genotoxicity of the Silkworm Extract Powder

The silkworm extract powder contain 1-deoxynojirimycin (DNJ), a potent α-glycosidase inhibitor, has therapeutic potency against diabetes mellitus. Therefore, natural products containing DNJ from mulberry leaves and silkworm are consumed as health functional food. The present study was performed to evaluate the safety of the silkworm extract powder, a health food which containing the DNJ. The repeated toxicity studies and gentic toxicity studies of the silkworm extract powder were performed to obtain the data for new functional food approval in MFDS. The safety was evaluated by a single-dose oral toxicity study and a 90 day repeated-dose oral toxicity study in Sprague-Dawley rats. The silkworm extract powder was also evaluated for its mutagenic potential in a battery of genetic toxicity test: in vitro bacterial reverse mutation assay, in vitro chromosomal aberration test, and in vivo mouse bone marrow micronucleus assay. The results of the genetic toxicology assays were negative in all of the assays. The approximate lethal dose in single oral dose toxicity study was considered to be higher than 5000 mg/kg in rats. In the 90 day study, the dose levels were wet at 0, 500, 1000, 2000 mg/kg/day, and 10 animals/sex/dose were treated with oral gavage. The parameters that were monitored were clinical signs, body weights, food and water consumptions, ophthalmic examination, urinalysis, hematology, serum biochemistry, necropsy findings, organ weights, and histopathological examination. No adverse effects were observed after the 90 day administration of the silkworm extract powder. The No-Observed-Adverse-Effect-Level (NOAEL) of silkworm extract powder in the 90 day study was 2000 mg/kg/day in both sexes, and no target organ was identified.

In- and Ga-doped ZnO (IGZO) films were deposited on 5 nm thick Cu film buffered poly-carbonate substrates with RF magnetron sputtering and the effects of the Cu buffer layer on the optical and electrical properties of the films were investigated. The IGZO single layer films exhibited an electrical resistivity of 1.2×10-1 Ω cm while the IGZO/Cu bi-layered films exhibited a lower resistivity of 1.6×10-3 Ω cm. With respect to optical properties, the optical band gap of the IGZO films appeared to decrease as a result of an increasing carrier concentration due to the Cu buffer layer. In addition, the RMS roughness (8.2 nm) of the IGZO films also decreased to 6.8 nm by a Cu buffer layer in AFM observation. Although the optical transmittance in the range of visible wavelengths was deteriorated by the Cu buffer layer, the IGZO films with a 5 nm thick Cu buffer layer exhibited a higher figure of merit of 2.6×10-4 Ω-1 compared with the IGZO single layer films due to enhanced optoelectrical performance.
