        Role of Bedside Ultrasonography in Assessment of Diaphragm Function as a Predictor of Success of Weaning in Mechanically Ventilated Patients

        Background: Weaning failure is common in mechanically ventilated patients, and if ultrasound can predict weaning outcome remains controversial. The purpose of this study was to evaluate the diaphragmatic function (thickness and excursion) measured by ultrasound as a predictor of the extubation outcome. Methods: We included 62 mechanically ventilated patients from the chest intensive care unit in this study. Sixty-two patients who successfully passed the spontaneous breathing trial (SBT) were enrolled. The transthoracic ultrasound of the diaphragm was performed during an SBT to the assess diaphragmatic function (excursion and thickness), and they were classified into the successful extubation group and the failed extubation group. Results: There was a statistically significant increase in the successful extubation group in the diaphragmatic excursion and thickness fraction (p<0.001), a statistically significant negative correlation between the diaphragmatic function and the duration of the mechanical ventilation, and a statistically significant negative correlation between the diaphragmatic excursion and the Acute Physiology and Chronic Health Evaluation II. The diaphragmatic excursion cutoff value predictive of weaning was 1.25 cm, with a specificity of 82.1% and a sensitivity of 97.1% respectively, and the diaphragmatic thickness cut-off value predictive of weaning was 21.5%, with a specificity of 60.7% and a sensitivity of 91.2%, respectively. Conclusion: The diaphragmatic ultrasonography was found to be a promising tool for predicting the extubation outcome for mechanically ventilated patients.

        Urodynamic Detrusor Overactivity in Patients with Overactive Bladder Symptoms

        Purpose: To evaluate the relationship between urodynamic detrusor overactivity (DO) and overactive bladder (OAB) symptoms in men and women. Methods: We reviewed the records of adult males and females who attended a tertiary referral center for urodynamic evaluation of OAB syndrome symptoms with the presence or absence of DO. DO was calculated for symptoms alone or in combination. Results: The overall incidence of DO was 76.1% and 58.7% in male and female OAB patients, respectively. Of men 63% and 61% of women with urgency (OAB dry) had DO, while 93% of men and 69.8% of women with urgency and urgency urinary incontinence (OAB wet) had DO. Of women, 58% who were OAB wet had stress urinary incontinence symptoms with 26.4% having urodynamic stress incontinence. 6% of men and 6.5% of women with OAB symptoms had urodynamic diagnosis of voiding difficulties with postvoid residual greater than 100 mL. Combination of symptoms is more accurate in predicting DO in OAB patients. The multivariate disease model for males included urge urinary incontinence (UUI) and urgency while for females it included UUI and nocturia. Conclusions: There was a better correlation in results between OAB symptoms and the urodynamic diagnosis of DO in men than in women, more so in OAB wet than in OAB dry. Combination of symptoms of the OAB syndrome seems to have a better correlation with objective parameters from the bladder diary, filling cystometry, and with the occurrence of DO.

        Enhanced Magnetic and DC Electrical Properties of Sm-Doped Bi2Fe4O9 Nanoplates Synthesized by a Sol–Gel Method

        In this work, Bi2-xSmxFe4O9 (x = 0.0, 0.02, 0.06, 0.08, 0.1) nanoplates with an average thickness of 62–125 nm were synthesized using a sol–gel method. The samples were characterized by X-ray diffraction, Fourier transform infrared spectroscopy, and scanning electron microscopy. The magnetic measurements show that the nanoplates have weak ferromagnetic ordering. The saturation magnetization of the nanoplates increases as the Sm content increases. The DC electric transport properties were studied by measuring the temperature dependence of the resistivity in the temperature range 300–680 K. The materials show typical semiconductor features, and the conduction mechanisms are governed by electron and small polaron hopping in the low and high temperature measurement ranges, respectively. The Sm doping results in a significant enhancement in the electrical conductivity of the Bi2Fe4O9 nanoplates.

        Phytochemical investigation of Ficus pandurataHance (Moraceae) fruits has led to the isolation of two newtriterpenoids, ficupanduratin A [1b-hydroxy-3b-acetoxy-11a-methoxy-urs-12-ene] (11) and ficupanduratin B [21ahydroxy-3b-acetoxy-11a-methoxy-urs-12-ene] (17), alongwith 20 known compounds: a-amyrin acetate (1), a-amyrin(2), 3b-acetoxy-20-taraxasten-22-one (3), 3b-acetoxy-11amethoxy-olean-12-ene (4), 3b-acetoxy-11a-methoxy-12-ursene (5), 11-oxo-a-amyrin acetate (6), 11-oxo-b-amyrinacetate (7), palmitic acid (8), stigmast-4,22-diene-3,6-dione(9), stigmast-4-ene-3,6-dione (10), stigmasterol (12), b-sitosterol(13), stigmast-22-ene-3,6-dione (14), stigmastane-3,6-dione (15), 3b,21b-dihydroxy-11a-methoxy-olean-12-ene (16), 3b-hydroxy-11a-methoxyurs-12-ene (18), 6-hydroxystigmast-4,22-diene-3-one (19), 6-hydroxystigmast-4-ene-3-one (20), 11a,21a-dihydroxy-3b-acetoxy-urs-12-ene(21), and b-sitosterol-3-O-b-D-glucopyranoside (22). Compound21 is reported for the first time from a natural source. The structures of the 20 compounds were elucidated on thebasis of IR, 1D (1H and 13C), 2D (1H–1H COSY, HSQC,HMBC and NOESY) NMR and MS spectroscopic data, inaddition to comparison with literature data. The isolatedcompounds were evaluated for their anti-microbial, antimalarial,anti-leishmanial, and cytotoxic activities. In addition,their radioligand displacement affinity on opioid andcannabinoid receptors was assessed. Compounds 4, 11, and15 exhibited good affinity towards the CB2 receptor, withdisplacement values of 69.7, 62.5 and 86.5 %, respectively. Furthermore, the binding mode of the active compounds inthe active site of the CB2 cannabinoid receptors was investigatedthrough molecular modelling.

        Through the examination of literatures, electronic commerce is a subject which is accepted in enterprises to define e-commerce adoption, trends, and issues that are assisting and obstructing its efficacy. E-commerce offers numerous advantages to consumer satisfaction in any place and helps the company to get a competitive benefit over its competitors. The Internet has expanded the scope of business. Many business information is available by the global network that supports information gathering between organizations, businesses and their clients, while various divisions of a business is increasing at anexponential rate. Meanwhile, there are a few barriers to proper e-commerce usage and adoption, such as reliable internet connections, poor e-commerce supporting infrastructures, logistics systems presenting socio-regulatory and poor transportation barriers and demonstrating the significant improvement of e-commerce reliable and affordable Internet provisions, i.e., Internet cost, intensity, and reasonable level of e-readiness. The operational and strategic significance of information-based virtual value chains for all organizations cannot be emphasized. As a consequence, this study confirms worldwide market elements of e-commerce, such as its issues, benefits, relevance, scope, facilitators and projects prospective obstacles in a developing economy.

        Essential Oil of Thymus capitatus Hoff. et Link. from Matmata, Tunisia: Gas Chromatography-Mass Spectrometry Analysis and Antimicrobial and Antioxidant Activities

        Thymus capitatus Hoff. et Link. essential oil was constituted by thymol (89.06%) as a major component followed by p-cimene (5.04%) and γ-terpinene (3.19%) after analysis by gas chromatography-flame ionization detection and gas chromatography-mass spectrometry. The antioxidant activity assays of the essential oil used in the inhibition of the radical cation 2,2′-azinobis(3-ethylbenzothiazoline-6-sulfonate) and the free radical 2,2-diphenyl-1-picrylhydrazyl showed high 50% inhibitory concentration values of 1.24±0.05mg/L and 0.59±0.02mg/L, respectively. The essential oil of T. capitatus was tested for antimicrobial activity against Gram-positive bacteria (Salmonella analum, Listeria monocytogenes), Gram-negative bacteria (Escherchia coli, Klebsiella pneumoniae), fungi (Mucor ramamnianus, Aspergillus ochraceus), and yeast species (Saccharomyces cerevisiae, Candida albicans) using the agar well diffusion method. It seemed that L. monocytogenes, E. coli, and K. pneumoniae bacteria were inhibited by the essential oil tested. A strong activity was also observed against fungi and yeasts.

        Computational, kinetic, and electrochemical studies of polyaniline functionalized ZnO and ZnO-SiO2 nanoparticles as corrosion protection films on carbon steel in acidic sodium chloride solutions

        Two novel nanocomposites based on polyaniline functionalized ZnO and ZnO-SiO2 nanoparticles(ZnO@PANi and ZnSiO@PANi) were synthesized, characterized, and evaluated as protective films for steelpipelines in the pickling process at 25–55 C. The prepared nanocomposites were described using differentspectroscopic characterization methods including UV–vis, FTIR, DLS, XPS methods, and other physicochemicaltechniques including XRD, FESEM, and HR-TEM. The novelty of these films is in the fact thatthe ZnO and ZnO-SiO2 nanoparticles are functionalized by polyaniline which is electrochemically stablein acidic solution and has high conductivity. Electrochemical systems such as open circuit potentials(OCP), electrochemical impedance spectroscopy (EIS), and potentiodynamic polarization (PDP) were utilizedto attain the kinetics and mechanistic findings of the corrosion protection route. The outcomes indicatethat the two fabricated ZnO@PANi and ZnSiO@PANi are efficient acidic corrosion inhibitors. Theprotection performance of ZnSiO@PANi (98.6%) was more pronounced at a dose (150 mg/L) thanZnO@PANi (92.3%) and individual PANi (83.4%) as achieved from PDP findings. The effect of temperatureand flowing conditions reinforces further the performance of both nanocomposites. Surface characterizationusing FESEM/EDS delivered more indication for the steel surface protection with the ZnO@PANi andZnSiO@PANi nanocomposites. Molecular modeling using DFT calculations and MC simulations supportedthe experimental findings. The binding energies of the compounds and Fe interface follow the order ofZnO@PANi > ZnSiO@PANi, which is in good agreement with the empirical results. An important suggestionof these results is the probability of substituting traditional toxic small molecules with cost-effectiveand non-toxic polymeric nanocomposites as protective films for use in the oil and gas industry.

        Pinocembrin Reduces Arthritic Symptoms in Mouse Model via Targeting Sox4 Signaling Molecules

        Rheumatoid arthritis (RA) is a chronic autoimmune, multifactorial, inflammatory disorder characterized by hyperplasia and infiltration of inflammatory cells at the synovial lining leading to destruction of cartilage and bone tissues. Pinocembrin (PCB) is a natural flavonoid extracted as a pure molecule from honey, propolis, and some plants. In this study, we evaluated the antiarthritic effect of PCB in adjuvant induced arthritis (AIA) mice. Treating the AIA mouse model with PCB reduced the arthritis symptoms/score, including edema size, extent of hind paw redness, abnormal movement, and holding inability. At the pathological level, PCB significantly decreased the joint erosion and percentages of infiltrated inflammatory cells. Biochemically, PCB interacts with the transcription factor, SRY-related HMG-box 4 (Sox4), and then modulates its dysregulated expression and the expression of Sox4/Stat3 signaling molecules in AIA mice. These molecules include tumor necrosis factor-α, nuclear transcription factor kappaB, and cyclooxygenase-2, besides the microRNAs; miR-132, miR-202-5p, and miR-7235, which are dysregulated in adjuvant-induced arthritis model relative to the control mice. The possible PCB interaction with Sox4 transcriptional protein was confirmed through molecular docking where three hydrogen bonds were formed at ARG and LYS residues at a stable binding energy of −4.72. Taken together, our data demonstrate that PCB could serve as a therapeutic drug in treatment of RA.
