        Subcategorization of intermediate suspicion thyroid nodules based on suspicious ultrasonographic findings

        Purpose: This study compared the malignancy risk of intermediate suspicion thyroid nodules according to the presence of suspicious ultrasonographic (US) findings. Methods: From January 2014 to December 2014, 299 consecutive intermediate suspicion thyroid nodules in 281 patients (mean age, 50.6±12.5 years) with final diagnoses were included in this study. Two radiologists retrospectively reviewed the US findings and subcategorized the intermediate suspicion category into nodules without suspicious findings and nodules with suspicious findings, including punctate echogenic foci, nonparallel orientation, or irregular margins. The malignancy rates were compared between the two subcategory groups. Results: Of the 299 intermediate suspicion thyroid nodules, 230 (76.9%) were subcategorized as nodules without suspicious findings and 69 (23.1%) as nodules with suspicious findings. The total malignancy rate was 33.4% (100/299) and the malignancy rate of nodules with suspicious findings was significantly higher than that of nodules without suspicious findings (47.8% vs. 29.1%, P=0.004). In nodules with suspicious findings, the most common suspicious finding was punctate echogenic foci (48/82, 58.5%) followed by nonparallel orientation (22/82, 26.8%) and irregular margins (12/82, 14.6%). Thirteen nodules had two suspicious findings simultaneously. A linearly increasing trend in the malignancy rate was observed according to the number of suspicious US findings (P for trend=0.001). Conclusion: Intermediate suspicion thyroid nodules with suspicious findings showed a higher malignancy rate than those without suspicious findings. Further management guidelines for nodules with suspicious findings should differ from guidelines for nodules without suspicious findings, even in the same US category.

        기존의 무선 센서 네트워크에서는 센서 노드는 하나의 전방향성 안테나와 하나의 무선 채널을 공유하여 데이터를 전송하므로 과다한 채널 경쟁과 센서 노드간의 데이터 충돌로 데이터 재전송이 자주 발생하여 불필요한 에너지의 소비가 많고, 소스 노드와 목적지 노드의 데이터 전송시 RTS(Request To Send)와 CTS(Clear To Send) 패킷을 사용하여 전파 전송 범위 전체에 대해 채널 예약을 하므로 공간의 활용도가 낮은 편이다. 본 논문에서는 전방향성 안테나의 문제점 및 단점을 보완하기 위한 방법으로 지향성 안테나를 사용하기 위한 SRDMAC(Spatial Reuse Directional MAC) 프로토콜을 제안한다. 제안된 프로토콜은 센서 노드 간의 데이터 전송시 충돌을 방지하여 데이터 재전송에 소모되는 에너지와 목적지 노드가 위치한 영역 일부분에만 데이터를 전송함으로 에너지 소모량을 줄일 수 있으며, 목적지 노드가 위치한 영역을 제외한 다른 영역의 이웃 노드와의 데이터 전송이 가능하므로 공간 재사용율을 최대한 증가시킬 수 있다.

        최근 센서 네트워크에서 에너지 효율성을 위한 다양한 연구가 진행 중이다. 특히 센서 노드의 저전력을 위해서는 센서 네트워크에서 전송되는 데이터의 횟수나 전송량을 최소한으로 줄이면서 효율적이면서 신뢰성을 가지는 질의에 대한 결과를 얻을 수 있어야 한다. 본 연구에서는 해양 센서 네트워크 상에서 데이터의 전송량을 줄일 수 있는 SDMTree(Sensing Data Management Tree)를 제안한다. 제안된 SDMTree는 질의 최적화를 위해 질의 처리기 구성 요소로 도입 가능하다. 해양 센서 네트워크에서 in-network 각 4레벨에서 하위 노드로부터 받은 데이터를 병합, 관리하기 위한 방법으로 데이터를 속성별로 구분하여 중복된 데이터를 제거하여 트리형태로 구성되기 때문에 질의에 대한 응답에 해당하는 데이터 검색시 정확하고 신속하게 처리할 수 있으며, 트리 구성 또한 중복 데이터 및 중복 영역을 배제하여 구성되므로, 상위노드가 하위 노드로부터 센싱 데이터를 수집하여 저장하기 위한 에너지와 상위 노드에서 하위 노드로 질의를 전송시 질의에 해당하는 특정 영역에만 질의를 전송할 수 있기 때문에 데이터 저장 및 통신에 소모되는 불필요한 에너지를 최대한 줄일 수 있다.

        Emergence of a global techno-economic system is challenging academia and the fashion industry. Consumers, the retail industry and business strategies are strategically contributing to the changing fashion paradigm, which is shifting from the Total (TCE), to Global (GCE) and to Smart Consumer Experience (SCE) concepts. The consumer experience is a pervasive value orientation that is transforming how consumers think, gain information, and make decisions about consumption activities to maximize their total experience value. Moreover, the experiences, products and services that consumers seek are being defined with a global and smart perspective that is made possible by boundary-free access to information and sourcing of products and services. Specifically, the smart consumer experience advocates consumer participation by leveraging the power of communities and networks and enabling consumers to influence retail businesses and co-create future marketspace values. This phenomenon has implications for fashion consumers and fashion retail industry business strategies since fashion is an emulated behavior.

        L1 Cell Adhesion Molecule Promotes Migration and Invasion via JNK Activation in Extrahepatic Cholangiocarcinoma Cells with Activating KRAS Mutation

        Extrahepatic cholangiocarcinoma (ECC), a malignant tumor of biliary origin, has a poor prognosis with limited treatment options. The KRAS oncogene is the most commonly mutated gene in ECC and one of the factors that predicts a poor prognosis and low survival rate. L1 cell adhesion molecule (L1CAM) is expressed in ECC cells and acts as an independent poor prognostic factor in predicting patient survival. In this study we investigate the functional significance of L1CAM in ECC cells with activating KRAS mutation. We selected an ECC cell line, EGI-1, with activating KRAS mutation, and then confirmed its expression of L1CAM by RT-PCR, western blot analysis, and flow cytometry. The suppression of L1CAM expression (using a specific lentivirus-delivered shRNA) significantly decreased the migratory and invasive properties of EGI-1 cells, without altering their proliferation or survival. Analyses of signaling effectors in L1CAM-depleted and control EGI-1 cells indicated that L1CAM suppression decreased the levels of both phosphorylated MKK4 and total MKK4, together with c-Jun N-terminal kinase (JNK) phosphorylation. Further, exposure to a JNK inhibitor (SP600125) decreased migration and invasion of EGI-1 cells. These results suggest that L1CAM promotes cellular migration and invasion via the induction of MKK4 expression, leading to JNK activation. Our study is the first to demonstrate a functional role for L1CAM in ECC carrying the activating KRAS mutation. Given that KRAS is the most commonly mutated oncogene in ECC, L1CAM may serve as an attractive therapeutic target for ECC cells with activating KRAS mutation.

      • Adoption of Eco-Friendly Faux Leather : Examining Consumer Attitude With the Value–Belief–Norm Framework

        <P>Eco-Friendly Faux Leather (EFFL) has recently been developed to minimize harmful environmental impacts; this product also has a low carbon footprint. However, consumers are lagging behind in eco-friendly product adoption due to their value, belief, and attitude gaps. To provide the best available social-psychological account of consumers’ adoption of EFFL products, the consumer attitude model was tested as an extension of the Value-Belief-Norm (VBN) framework. Online survey panel data were collected on 602 respondents from U.S. and U.K. Structural Equation Modeling and Multiple Group Analysis were employed to test the hypothesized attitude model. The findings support the conclusion that there is significant causality among environmental value, pro-environmental belief, and the norm of individual responsibility, but no difference regarding the VBN concurrence was found between the two countries. Practical implications and theoretical suggestions for understanding consumer attitude towards a new eco-friendly product are proposed.</P>

        Digital Breast Tomosynthesis versus MRI as an Adjunct to Full-Field Digital Mammography for Preoperative Evaluation of Breast Cancer according to Mammographic Density

        Objective: To compare digital breast tomosynthesis (DBT) and MRI as an adjunct to full-field digital mammography (FFDM) for the preoperative evaluation of women with breast cancer based on mammographic density. Materials and Methods: This retrospective study enrolled 280 patients with breast cancer who had undergone FFDM, DBT, and MRI for preoperative local tumor staging. Three radiologists independently sought the index cancer and additional ipsilateral and contralateral breast cancers using either FFDM alone, DBT plus FFDM, or MRI plus FFDM. Diagnostic performances across the three radiologists were compared among the reading modes in all patients and subgroups with dense (n = 186) and non-dense breasts (n = 94) according to mammographic density. Results: Of 280 patients, 46 (16.4%) had 48 additional (39 ipsilateral and nine contralateral) cancers in addition to the index cancer. For index cancers, both DBT plus FFDM and MRI plus FFDM showed sensitivities of 100% in the non-dense group. In the dense group, DBT plus FFDM showed lower sensitivity than that of MRI plus FFDM (94.6% vs. 99.6%, p < 0.001). For additional ipsilateral cancers, DBT plus FFDM showed specificity and positive predictive value (PPV) of 100% in the non-dense group, but sensitivity and negative predictive value (NPV) were not statistically different from those of MRI plus FFDM (p > 0.05). In the dense group, DBT plus FFDM showed higher specificity (98.2% vs. 94.1%, p = 0.005) and PPV (83.1% vs. 65.4%; p = 0.036) than those of MRI plus FFDM, but lower sensitivity (59.9% vs. 75.3%; p = 0.049). For contralateral cancers, DBT plus FFDM showed higher specificity than that of MRI plus FFDM (99.0% vs. 96.7%, p = 0.014), however, the other values did not differ (all p > 0.05) in the dense group. Conclusion: DBT plus FFDM showed an overall higher specificity than that of MRI plus FFDM regardless of breast density, perhaps without substantial loss in sensitivity and NPV in the diagnosis of additional cancers. Thus, DBT may have the potential to be used as a preoperative breast cancer staging tool.

        Delayed Cancer Diagnosis in Thyroid Nodules Initially Treated as Benign With Radiofrequency Ablation: Ultrasound Characteristics and Predictors for Cancer

        Objective: Regrowth after radiofrequency ablation (RFA) of symptomatic large thyroid nodules, initially treated as benign, sometimes turns out to be malignancies. This study aimed to assess the ultrasound (US) characteristics of thyroid nodules initially treated as benign with RFA and later diagnosed as cancers, predictive factors for cancers masquerading as benign, and methods to avoid RFA in these cancers. Materials and Methods: We reviewed the medical records of 134 consecutive patients with 148 nodules who underwent RFA between February 2008 and November 2016 for the debulking of symptomatic thyroid nodules diagnosed as benign using US-guided biopsy. We investigated the pre-RFA characteristics of the thyroid nodules, changes at follow-up after RFA, and the final surgical pathology. Results: Nodule regrowth after RFA was observed in 36 (24.3%) of the 148 benign nodules. Twenty-two of the 36 nodules were surgically removed, and malignancies were confirmed in seven (19.4% of 36). Of the 22 nodules removed surgically, pre-RFA median volume (range) was significantly larger for malignant nodules than for benign nodules: 22.4 (13.9–84.5) vs. 13.4 (7.3–16.8) mL (P = 0.04). There was no significant difference in the regrowth interval between benign and malignant nodules (P = 0.49). The median volume reduction rate (range) at 12 months was significantly lower for malignant nodules than for benign nodules (51.4% [0–57.8] vs. 83.8% [47.9–89.6]) (P = 0.01). The pre-RFA benignity of all seven malignant nodules was confirmed using two US-guided fine-needle aspirations (FNAs), except for one nodule, which was confirmed using US-guided core-needle biopsy (CNB). Regrown malignant nodules were diagnosed as suspicious follicular neoplasms by CNB. Histological examination of the malignant nodules revealed follicular thyroid carcinomas, except for one follicular variant, a papillary thyroid carcinoma. Conclusion: Symptomatic large benign thyroid nodules showing regrowth or suboptimal reduction after RFA may have malignant potential. The confirmation of these nodules is better with CNB than with FNA.

        Profiling Customer Engagement with "Snuggie" Experience in Social Media

        In order to understand meaningful customer experience in social media, this study profiles customer engagement by exposing the essential brand experience rooms in hyper-reality contexts. This study selects Snuggie as a target brand as it uses multiple contact points, including social media, to provide meaningful experience to customers. With their unique marketing strategy, Snuggie became a popular brand among the U. S. customers beyond just a wearable blanket. Upon analyzing a total of 364 customer reviews about Snuggie in Amazon.com, five experience rooms were exposed; "Physical artifacts" and "customer involvement" are influential experience rooms which signify interactions between products and customers, while "intangible artifacts", "technology" and "customer placement" reflect a lower degree of experiential engagement. This approach suggests a theoretical foundation in understanding the customer engagement concepts by the means of brand experience dimensions in social media. The ability to create compelling engagement in social media depends on the successful facilitation of relationships and information, which lead to a creative, communicative and interactive experience.
