Purpose: We investigated trends in antibiotic resistance for gram-negative bacteria in infants with a urinary tract infection (UTI) over 15 years at a single institution. Methods: A retrospective chart review was conducted for children younger than 24 months who visited the emergency room and were diagnosed with a UTI between January 2000 and December 2014. We selected urine culture data that grew Escherichia coli and Klebsiella pneumoniae. Baseline clinical information and results of antimicrobial susceptibility tests were analyzed by dividing the 15-year study time frame into three periods (A: 2000-2004, B: 2005-2009, and C: 2010-2014). Results: During the study period, 478 applicable children were identified (E. coli, 89.7% and K. pneumoniae, 10.3%). Antibiotic resistance to third-generation cephalosporins was increased from period A to period C (A, 2.1%; B, 8.3%; C, 8.8%; P=0.025). Resistance to quinolones also showed a steady pattern during periods A to C, although it was not statistically significant (A, 7.9%; B, 9.7%; C, 12.4%; P=0.221). The incidence of Extended-spectrum ${\beta}$-lactamase (ESBL)-producing gram-negative bacteria increased from period A to period C (A, 1.4%; B, 7.6%; C, 8.2%; P=0.012). Conclusion: This study revealed that the common uropathogens E. coli and K. pneumoniae experienced increasing resistance rates against third-generation cephalosporins and a constant antibiotic resistance to quinolones in children younger than 24 months. We also showed a recent increased incidence of ESBL-producing gram-negative bacteria in patients with community-acquired UTIs. Therefore, it is necessary to actively surveil resistance in order to properly select empirical antibiotics.

Purpose: We aimed to investigate the disease phenotype of Korean pediatric Crohn's disease (CD) patients at diagnosis according to the Paris classification by comparison with patients from the European multicenter 5-years recruitment of children with newly developed IBD (EUROKIDS registry). Methods: Korean children and adolescents who had been newly diagnosed with CD at the age of <18 years during 2013–2016 were included in this multicenter retrospective study. Disease phenotype at diagnosis was classified according to the Paris classification, and compared with the published data from the EUROKIDS study. Results: A total of 255 patients were included. The median diagnosis age was 14.7 years (range, 0.8–17.9 years). No significant difference was observed in male-to-female ratio with EUROKIDS (1.9:1 vs. 1.45:1, p=0.062). The proportion of children aged <10 years was significantly lower in Koreans (7.1% vs. 19.6%, p<0.001). Colonic disease was less prominent (10.0% vs. 27.3%, p<0.001), while upper GI involvement was more prominent in Korean children (59.3% vs. 46.2%, p<0.001). The proportion with perianal fistulizing disease at diagnosis was significantly higher in Korean patients (44.8% vs. 8.2%, p<0.001). A separate analysis of Korean patients revealed that perianal fistulizing disease at diagnosis was positively associated with male sex and body mass index z-score (odds ratio [OR]=2.12, 95% confidence interval [CI]=1.20–3.76, p=0.010; and OR=1.29, 95% CI=1.05–1.58, p=0.015, respectively). Conclusion: Approximately half of pediatric CD patients in Korea present with perianal fistulas and/or abscesses at diagnosis, which is a distinct feature of CD in Korean children and adolescents compared to their European counterparts. An underlying genetic difference between ethnicities may play a role in this expression of different phenotypes in pediatric CD.

시청자의시야를콘텐츠로가득채워주어강한몰입감을제공하는다면영상제작을위해서는다수의카메라를이용한촬영이 요구된다. 이러한 촬영 시나리오에서는다수의 카메라에 의해 촬영되고 있는 화면들이 하나의 콘텐츠로써 어떻게 보여질지 혹은 실제 상영관에서 어떻게 보여질지 확인하는 모니터링 과정이 매우 중요하다. 기존의 특수 포맷 영상 촬영에 관한 연구들은 모두 스테레오스코픽 입체 영상이나 파노라마 영상 등 각자의 목적에 집중하여 이루어져 왔으며, 최근 확산되고 있는 세 개의 면을이용한 다면 상영관에 최적화된 콘텐츠 제작에 관한 연구는 부재하다. 본 논문에서는 세 대의 카메라를 제어할 수 있는 리그와 다면 영상 포맷에 특화된 모니터링 소프트웨어를 이용한 다면 콘텐츠 현장 촬영 시스템을 제안한다. 제안된 리그는 카메라 간의 각도를 정밀하게 조절하여 세 대의 카메라로 넓은 화각을 촬영할 수 있으며, 원격 통신을 통해 모니터링 소프트웨어와 연동된다. 모니터링 소프트웨어는 실시간으로 들어오는 영상들을자동으로 정렬해주며, 수신 받은 리그의 각도에 따라 영상의 정렬이자동으로 업데이트된다. 또한 촬영 시에 얻어진 정렬 정보를 후반작업에 사용할 수 있게 함으로써, 제작 효율을 크게 향상시킨다. Filming using multiple cameras is required for the production of the multi-screen content. It can fill the viewer’s field of view (FOV) entirely to provide an increased sense of immersion. In such a filming scenario, it is very important to monitor how images captured by multiple cameras are displayed as a single content or how the content will be displayed in an actual theatre. Most recent studies on creating the content of special format have been focused on their own purposes, such as stereoscopic and panoramic images. There is no research on content creation optimized for theatres that use three screens that are spreading recently. In this paper, we propose a novel content production system with a rig that can control three cameras and monitoring software specialized for multi-screen content. The proposed rig can precisely control the angles between the cameras and capture wide angle of view with three cameras. It works with monitoring software via remote communication. The monitoring software automatically aligned the content in real time, and the alignment of the content is updated according to the angle of camera rig. Futher, the producion efficiency is greatly improved by making the alignment information available for post-production.

Background: Acinetobacter baumannii infections cause high morbidity and mortality in intensive care unit (ICU) patients. However, there are limited data on the changes of longterm epidemiology of imipenem resistance in A. baumannii bacteremia among pediatric ICU (PICU) patients. Methods: A retrospective review was performed on patients with A. baumannii bacteremia in PICU of a tertiary teaching hospital from 2000 to 2016. Antimicrobial susceptibility tests, multilocus sequence typing (MLST), and polymerase chain reaction for antimicrobial resistance genes were performed for available isolates. Results: A. baumannii bacteremia occurred in 27 patients; imipenem-sensitive A. baumannii (ISAB, n = 10, 37%) and imipenem-resistant A. baumannii (IRAB, n = 17, 63%). There was a clear shift in the antibiogram of A. baumannii during the study period. From 2000 to 2003, all isolates were ISAB (n = 6). From 2005 to 2008, both IRAB (n = 5) and ISAB (n = 4) were isolated. However, from 2009, all isolates were IRAB (n = 12). Ten isolates were available for additional test and confirmed as IRAB. MLST analysis showed that among 10 isolates, sequence type 138 was predominant (n = 7). All 10 isolates were positive for OXA-23-like and OXA-51-like carbapenemase. Of 27 bacteremia patients, 11 were male (41%), the median age at bacteremia onset was 5.2 years (range, 0–18.6 years). In 33% (9/27) of patients, A. baumannii was isolated from tracheal aspirate prior to development of bacteremia (median, 8 days; range, 5–124 days). The overall case-fatality rate was 63% (17/27) within 28 days. There was no statistical difference in the case fatality rate between ISAB and IRAB groups (50% vs. 71%; P = 0.422). Conclusion: IRAB bacteremia causes serious threat in patients in PICU. Proactive infection control measures and antimicrobial stewardship are crucial for managing IRAB infection in PICU.

<P>RNA interference (RNAi) has been widely adopted to repress specific gene expression and is easily achieved by designing small interfering RNAs (siRNAs) with perfect sequence complementarity to the intended target mRNAs. Although siRNAs direct Argonaute (Ago), a core component of the RNA-induced silencing complex (RISC), to recognize and silence target mRNAs, they also inevitably function as microRNAs (miRNAs) and suppress hundreds of off-targets. Such miRNA-like off-target repression is potentially detrimental, resulting in unwanted toxicity and phenotypes. Despite early recognition of the severity of miRNA-like off-target repression, this effect has often been overlooked because of difficulties in recognizing and avoiding off-targets. However, recent advances in genome-wide methods and knowledge of Ago-miRNA target interactions have set the stage for properly evaluating and controlling miRNA-like off-target repression. Here, we describe the intrinsic problems of miRNA-like off-target effects caused by canonical and noncanonical interactions. We particularly focus on various genome-wide approaches and chemical modifications for the evaluation and prevention of off-target repression to facilitate the use of RNAi with secured specificity.</P>
Epigenetic Activation of Tensin 4 Promotes Gastric Cancer Progression

Gastric cancer (GC) is a complex disease influenced by multiple genetic and epigenetic factors. Chronic inflammation caused by Helicobacter pylori infection and dietary risk factors can result in the accumulation of aberrant DNA methylation in gastric mucosa, which promotes GC development. Tensin 4 (TNS4), a member of the Tensin family of proteins, is localized to focal adhesion sites, which connect the extracellular matrix and cytoskeletal network. We identified upregulation of TNS4 in GC using quantitative reverse transcription PCR with 174 paired samples of GC tumors and adjacent normal tissues. Transcriptional activation of TNS4 occurred even during the early stage of tumor development. TNS4 depletion in GC cell lines that expressed high to moderate levels of TNS4, i.e., SNU-601, KATO III, and MKN74, reduced cell proliferation and migration, whereas ectopic expression of TNS4 in those lines that expressed lower levels of TNS4, i.e., SNU-638, MKN1, and MKN45 increased colony formation and cell migration. The promoter region of TNS4 was hypomethylated in GC cell lines that showed upregulation of TNS4. We also found a significant negative correlation between TNS4 expression and CpG methylation in 250 GC tumors based on The Cancer Genome Atlas (TCGA) data. This study elucidates the epigenetic mechanism of TNS4 activation and functional roles of TNS4 in GC development and progression and suggests a possible approach for future GC treatments.

Mounting a twin motor is one way to reduce the weight and size of a driving motor of an electric vehicle. When twin motors are applied, parking systems must also be applied to each motor, so mechanical considerations are more complicated. Double parking systems require additional performance review items from existing parking systems. For double parking systems, scenarios that were not considered in the single type, such as left/right asymmetry and the force required to operate both systems, should be considered. The simulation was studied in terms of safety functional aspects, which are the original purpose of the parking system, and emotional quality aspects closely related to safety.

A conventional parking system of automobile is built in an automatic transmission(AT). This system is responsible for the safety performance and sends the drivers intention to the vehicle. In checking these functions, a design verification has been performed by using three indicators through the simulations of multibody dynamics. This analysis technique was applied to the development of parking systems suitable for the number for vehicles, the error from the test was satisfied within 10% on average. For the cost down purpose, a new parking system has been developed that can perform the original function with a simpler structure than the conventional type. The Grubler formula was used to determine the structural complexity of the parking system. The new system consists of a rotating cam drive type, not a rod assembly type. When new system was applied to the vehicle, it was verified by using the simulation that had the same performance characteristics as the conventional system. As a result, it was confirmed that it could be used.
