      • 만성 C형 간염 환자에서 페그인터페론 알파2a와 리바비린 병합 치료중 발생한 벨마비 1예

        페그인터페론과 리바비린 병합요법은 만성 C형 간염의 일차 치료법이다. 저자들은 만성 C형 간염 환자에서 페그인터페론 과 리바비린 병합 요법 중에 발생한 벨마비 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다. 환자는 5년 전부터 만성 C형 간염을 앓아온 48세 남자이며, PEG-IFN α-2a 135μgm 피하주사 주1회와 하루 1200㎎의 리바비린을 투여하였다. 치료시작 후 9개월째 환자는 오른쪽 안면의 근력약화를 호소하였으며 벨마비로 진단되었다. 페그인터페론과 리바비린 병합요법을 지속하면서 관찰하였다. 환자의 벨마비는 페그인터페론 치료를 중단하지 않았음에도 3개월후 증상이 회복되고 이후 벨마비 재발 없이 현재 경과관찰 중이다. 만성 C형 간염에서 페그인터페론과 리바비린 병합 요법시 벨마비의 발생 가능성을 염두에 두어야 하겠다. A Case of Bell's Palsy Associated with Combination Therapy of Pegylated Interferon Alfa-2a (PEG-IFN) and Ribavirin for Chronic Hepatitis C Virus Infection Pegylated interferon alfa(PEG-IFN α) and ribavirin therapy is the first line treatment for chronic hepatitis C. Mild complications of the therapy are common, but more serious complications are rare. We report here a case of Bell's palsy that occurred in a patient with chronic hepatitis C virus infection during combination therapy of PEG-IFN α-2a and ribavirin. The patient was 49-year-old man with chronic hepatitis C (genotype 1b) for 8 years. He had compensated liver cirrhosis with splenomegaly. Therapy with PEG-IFN α- 2a 135mcg/week and ribavirin 1200mg/day was initiated. After 9 months of the therapy, the patient showed a loss of muscular tone on the right side of his face. A diagnosis of Bell's palsy was made. The Bell's palsy resolved over 3 months despite continuation of the combination therapy.

        Inhibition of acute lethal pulmonary inflammation by the IDO–AhR pathway

        <P>The lung is a prototypic organ that was evolved to reduce immunopathology during the immune response to potentially hazardous endogenous and exogenous antigens. In this study, we show that donor CD4(+) T cells transiently induced expression of indoleamine 2,3-dioxygenase (IDO) in lung parenchyma in an IFN-gamma-dependent manner early after allogeneic hematopoietic stem cell transplantation (HSCT). Abrogation of host IDO expression by deletion of the IDO gene or the IFN-gamma gene in donor T cells or by FK506 treatment resulted in acute lethal pulmonary inflammation known as idiopathic pneumonia syndrome (IPS). Interestingly, IL-6 strongly induced IDO expression in an IFN-gamma-independent manner when deacetylation of STAT3 was inhibited. Accordingly, a histone deacetylase inhibitor (HDACi) could reduce IPS in the state where IFN-gamma expression was suppressed by FK506. Finally, L-kynurenine produced by lung epithelial cells and alveolar macrophages during IPS progression suppresses the inflammatory activities of lung epithelial cells and CD4(+) T cells through the aryl hydrocarbon receptor pathway. Taken together, our results reveal that IDO is a critical regulator of acute pulmonary inflammation and that regulation of IDO expression by HDACi may be a therapeutic approach for IPS after HSCT.</P>

      • <i>Pontimonas salivibrio</i> gen. nov., sp. nov., a new member of the family <i>Microbacteriaceae</i> isolated from a seawater reservoir of a solar saltern

        <P>A Gram-staining-positive, non-motile, strictly aerobic, non-spore-forming, vibrio-shaped bacterial strain, CL-TW6<SUP>T</SUP>, was isolated from a reservoir seawater sample from a solar saltern in Korea. Analysis of the 16S rRNA gene sequence of strain CL-TW6<SUP>T</SUP> revealed a clear affiliation with the family <I>Microbacteriaceae</I>. Strain CL-TW6<SUP>T</SUP> showed the closest phylogenetic relationships with the genera <I>Yonghaparkia</I> and <I>Microcella</I>, with 16S rRNA gene sequence similarity of 94.8–95.3 %. The strain grew in the presence of 1–9 % sea salts, at 15–35 °C and at pH 7.0–9.0. The major cellular fatty acids of strain CL-TW6<SUP>T</SUP> were anteiso-C<SUB>15 : 0</SUB> (32.6 %), iso-C<SUB>16 : 0</SUB> (20.4 %), iso-C<SUB>15 : 0</SUB> (13.2 %) and iso-C<SUB>14 : 0</SUB> (11.8 %) and the major menaquinones were MK-9 and MK-10. Cell-wall analysis showed that the peptidoglycan of strain CL-TW6<SUP>T</SUP> contained 2,4-diaminobutyric acid, alanine, glycine and glutamic acid. The major polar lipids were diphosphatidylglycerol and phosphatidylglycerol. The genomic DNA G+C content of strain CL-TW6<SUP>T</SUP> was 60.0 mol%. The combined phenotypic, chemotaxonomic and phylogenetic data showed clearly that strain CL-TW6<SUP>T</SUP> could be distinguished from members of the family <I>Microbacteriaceae</I> with validly published names. Thus, strain CL-TW6<SUP>T</SUP> should be classified as representing a novel genus and species in the family <I>Microbacteriaceae</I>, for which the name <I>Pontimonas salivibrio</I> gen. nov., sp. nov. is proposed. The type strain of <I>Pontimonas salivibrio</I> is CL-TW6<SUP>T</SUP> ( = KCCM 90105<SUP>T</SUP>  = JCM 18206<SUP>T</SUP>).</P>

      • Nitratireductor aquimarinus sp. nov., isolated from a culture of the diatom Skeletonema costatum, and emended description of the genus Nitratireductor.

        <P>Two Gram-negative-staining, aerobic bacterial strains, designated CL-SC21(T) and CL-SC22, were isolated from a culture of the diatom Skeletonema costatum (Korean Marine Microalgae Culture Center, KMMCC strain B-396) established from the East Sea, Korea. The two novel strains shared 99.9 % 16S rRNA gene sequence similarity. Analysis of the 16S rRNA gene sequences showed an affiliation with the genus Nitratireductor, with the strains sharing 96.5-97.5 % similarity with the type strains of recognized species of the genus Nitratireductor and being most closely related to Nitratireductor aquibiodomus NL21(T). Phylogenetic analyses of the 16S rRNA gene sequences showed that strain CL-SC21(T) together with strain CL-SC22 belonged to the genus Nitratireductor and formed a robust clade among closely related Nitratireductor species. The polar lipids comprised phosphatidylcholine, phosphatidylglycerol, diphosphatidylglycerol, phosphatidylethanolamine, phosphatidylmonomethylethanolamine, two unidentified aminophospholipids, an unidentified aminolipid, an unidentified phospholipid and five unidentified lipids. Ubiquinone 10 was the major quinone. The major cellular fatty acids of strains CL-SC21(T) and CL-SC22 were C(18 : 1)ω7c (70.6-72.3 %) and C(19 : 0)ω8c cyclo (10.9-11.8 %). The genomic DNA G+C contents of strains CL-SC21(T) and CL-SC22 were 56.7 and 57.1 mol%, respectively. The level of DNA-DNA relatedness between strains CL-SC21(T) and CL-SC22 was 86 % (reciprocal 91 %), indicating that the two isolates represented a single species. However, levels of DNA-DNA relatedness between N. aquibiodomus NL21(T) and strains CL-SC21(T) and CL-SC22 were 28 % (reciprocal 45 %) and 25 % (reciprocal 50 %), respectively. Phylogenetic analysis and the results of biochemical tests showed that strains CL-SC21(T) and CL-SC22 were different from all recognized species of the genus Nitratireductor. Thus, strains CL-SC21(T) and CL-SC22 represent a novel species of the genus Nitratireductor, for which the name Nitratireductor aquimarinus sp. nov. is proposed. The type strain is CL-SC21(T) ( = KCCM 90090(T) = JCM 17288(T)).</P>

        자연산 대하(Fenneropenaeus chinensis)에서 검출된 노랑머리 바이러스 Genotype 8의 계통분류학적 특성

        Yellow head virus (YHV) is a rod-shaped positive-sense single-stranded RNA virus, classified into the genus Okavirus, family Roniviridae, and order Nidovirales. In this study, 200 fleshy prawns (Fenneropenaeus chinensis) collected from the vicinity of Narodo in Goheung-gun, Korea, were screened for the presence of yellow head complex viruses and related genotype such as YHV genotype 8. The detection rate of YHV genotype 8 among the 200 fleshy prawns, determined using nested RT-PCR (reverse transcription polymerase chain reation), was 39.0%. Phylogenetic analysis of the ORF1b gene of YHV showed that eight distinct genetic lineages were detected. The four strains of YHV genotype 8 obtained in this study formed a robust clade with the YHV genotype 8 group that was first isolated from fleshy prawns in China suspected to have acute hepatopancreatic necrosis disease (AHPND).

      • A Disposable Photovoltaic Patch Controlling Cellular Microenvironment for Wound Healing

        <P>Electrical stimulation (ES) is known to affect the wound healing process by modulating skin cell behaviors. However, the conventional clinical devices that can generate ES for promoting wound healing require patient hospitalization due to large-scale of the extracorporeal devices. Herein, we introduce a disposable photovoltaic patch that can be applied to skin wound sites to control cellular microenvironment for promoting wound healing by generating ES. In vitro experiment results show that exogenous ES could enhance cell migration, proliferation, expression of extracellular matrix proteins, and myoblast differentiation of fibroblasts which are critical for wound healing. Our disposable photovoltaic patches were attached to the back of skin wound induced mice. Our patch successfully provided ES, generated by photovoltaic energy harvested from the organic solar cell under visible light illumination. In vivo experiment results show that the patch promoted cutaneous wound healing via enhanced host-inductive cell proliferation, cytokine secretion, and protein synthesis which is critical for wound healing process. Unlike the current treatments for wound healing that engage passive healing processes and often are unsuccessful, our wearable photovoltaic patch can stimulate regenerative activities of endogenous cells and actively contribute to the wound healing processes.</P>

        본 논문에서는 40kRPM 4/4극 고속 단상 하이브리드 스위치드 릴럭턴스 전동기(Single-Phase Hybrid Switched Reluctance Motor, SHSRM) 을 제안한다. 제안 모델은 기존 18kRPM SHSRM의 직렬 권선을 2-병렬 권선으로 변경하여 정토크 영역을 40kRPM까지 확장시켰다. 전자계 특성 해석은 FEA 3D을 통해 수행하였고 영구자석의 와전류손을 줄이기 위해 영구자석을 3분할하였다.

        최근 SOA와 클라우드 컴퓨팅 기반의 IT 서비스가 증가함에 따라 정보처리 분야에서 SLA(Service Level Agreement)가 주목을 받고 있다. 통상의 SLA는 종이에 자연언어로 표현되는 것이어서 SLA 체결, SLA 관리나 SLA 기반 정보처리의 자동화가 제한된다. 오늘날과 같은 SOA, 온 디맨드 서비스, 유틸리티 컴퓨팅, 클라우드 컴퓨팅 환경에서는 서비스를 통합 자동관리할 수 있는 기술이 필요하다. 본 논문에서는 능동문서 기반의 ASLM(Active Service Level Managenment)의 계약체결 사용자 인터페이스(UI: User Interface)를 구현하여 일반 문서의 경우와 비교하여 보았다. 그 결과 ASLM의 UI가 SLA 체결, SLA 유지보수 자동화, 그리고 사용자의 편의성에서 SLA 기반의 계약업무 처리에 보다 효과적임을 알 수 있었다.

        Efficacy and Safety of Fexuprazan in Patients with Acute or Chronic Gastritis

        Background/Aims: Fexuprazan is a novel potassium-competitive acid blocker that could be of benefit to patients with gastric mucosal injury. The aim of this study was to assess the 2-week efficacy and safety of fexuprazan in patients with acute or chronic gastritis. Methods: In this study, 327 patients with acute or chronic gastritis who had one or more gastric erosions on endoscopy and subjective symptoms were randomized into three groups receiving fexuprazan 20 mg once a day (q.d.), fexuprazan 10 mg twice a day (b.i.d.), or placebo for 2 weeks. The posttreatment assessments were the primary endpoint (erosion improvement rate), secondary endpoints (cure rates of erosion and edema and improvement rates of redness, hemorrhage, and subjective symptoms), and drug-related adverse events. Results: Among the patients, 57.8% (59/102), 65.7% (67/102), and 40.6% (39/96) showed erosion improvement 2 weeks after receiving fexuprazan 20 mg q.d., fexuprazan 10 mg b.i.d., and placebo, respectively. Both fexuprazan 20 mg q.d. and 10 mg b.i.d. showed superior efficacy to the placebo (p=0.017 and p<0.001, respectively). Likewise, both fexuprazan 20 mg q.d. and 10 mg b.i.d. also showed higher erosion healing rates than the placebo (p=0.033 and p=0.010, respectively). No difference was noted in the edema healing rate and the improvement rates for redness, hemorrhage, and subjective symptoms between the fexuprazan and placebo groups. No significant difference was noted in the incidence of adverse drug reactions. Conclusions: Fexuprazan 20 mg q.d. and 10 mg b.i.d. for 2 weeks showed therapeutic efficacy superior to that of placebo in patients with acute or chronic gastritis

        The ion exchange filter applied to the hydrogen electric vehicle cooling system requires periodic exchange. Frequent and unpredictable exchange of items causes maintenance costs and inconvenience to consumers, and weakens marketability to manufacturers. By developing the technology for analyzing removed parts of the ion exchange filter, the main cause of exhausted is identified and reduction measures are established. And the principle of an unusual acceleration phenomenon in vehicle environment is identified and design concepts are proposed to improve. A method to alleviate the acceleration phenomenon was verified through experiments. In addition, to statistical cross-validation the driving data of customers collected in servers were classified into two groups. The characteristics of group 1 (rapid replacement) and group 2 (normal replacement) were compared with the experimental results performed.
