저자들은 신부전이 동반된 당뇨병 환자에서 발생된 부비동형 모균증에 대하여 수술과 함께 liposomal amphotericin B 및 GM-CSF의 복합치료를 하여 성공적으로 치료한 1예를 경험하였기에 문헌 고찰과 함께 보고하는 바이다. Mucormycosis (zygomycosis) primarily affects diabetic or immunocompromised patients and typically progresses rapidly, necessitating surgical excision and antifungal therapy with amphotericin B. Large doses of amphotericin B are needed for cure, but it has the risk of causing significant renal toxicity. The recent development of liposomal amphotericin B allows antifungal therapy to be administered with potentially improved efficacy and reduced nephrotoxicity. We have experienced a case of paranasal mucormycosis successfully treated with surgery, liposomal amphotericin B and GM-CSF. A 59-year-old male suffering from diabetes mellitus for 6 years was admitted with pain at left maxillary area. He was diagnosed as mucormycosis after cytologic exam on the necrotic nasal mucosa, which showed typical hyphae. He have had diabetic nephropathy with macroproteinuria and had rapidly rising serum creatinine levels with the amphotericin B treatment: creatinine levels reverted to basal level with the use of liposomal amphotericin B. Despite surgical excision and continued antifungal therapy, his infection was not effectively controlled. Therefore, GM-CSF was administered additionally to improve phagocytic activity of leukocytes. He was finally cured after receiving a combination of aggressive surgery, liposomal amphotericin B and GM-CSF. To our knowledge, this is the first detailed clinical description of the treatment of mucormycosis with liposomal amphotericin B in Korea.

This work was carried out to investigate the behavior of sugars contained in raw soybean during cooking and meju preparation processes, and the effects of sugar addition to the raw kanjang digestion liquor made from G8 and SB meju on the lactic acid and alcoholic fermentation of kanjang. Sharp reduction in sugars content in soybean during cooking and meju preparation process was observed. Rapid lactic acid and alcoholic fermentation in the G8 and SB kanjang with 5% added glucose was observed but not in the corresponding kanjang without sugar addition after inoculation of Pediococcus halophilus, Zygosaccharomyces rouxii and Candida versatilis starter culture to the low-salted raw kanjang digestion liquor made respectively from G8 and SB meju. 0.46% and 0.88% of lactic acid and 1% and 2% of alcohol content in the G8 and SB kanjang respectively was observed 160 hours after the inoculation of the lactic acid bacteria starter culture.

A few case of deep granulomatous cutaneous infection have been reported in the Korean dermatologic literature, but Trichophyton(T.) mentagrophytes granuloma is rare. We report a case of Trichophytic granuloma caused by T. mentagrophytes in a 10-year-old male, who showed three, mild pruritic, coin to walnut-sized, erythematous plaques with indurations and crusts on the face for 1 month. Fungal culture of the biopsy specimen and scales of the lesion revealed T. mentagrophytes. Histopathologic examination of the skin lesion showed chronic granulomatous inflammation in the dermis and fungal hyphae in the hair shaft. After four weeks of terbinafine administration, lesions were cleared with mild erythematous macules. [Kor J Med Mycol 7(2): 92-96]

A linear correlation, by the stepwise multiple regression analysis, was found between the sensory test of Sigumjang aroma and the gas chromatographic data which were transformed with logarithm. GC data is the most objective method to evaluate Sigumjang aroma. A multiple correlation coefficient and a determination coefficient of more than 0.9 were obtained at the 9th and 13th steps, respectively. At step 31, the coefficient of determination level of 0.95 was attained. The accuracy of its estimation became higher as the number of the variables entered into the regression model increased. Over 90% of the Sigumjang aroma was explained by 13 compounds identified on GC. The contributing proportion of the peak 26 was the highest followed by peaks 57 (9.27%), 29 (7.51%), 54 (6.01%), 8 (5.99%), 49 (4.97%), and 13 (4.11%).
전계효과에서 정전위 형성을 이한 Insulator 제작과 두께 변화에 따른 저항특성

현재 X-ray 검출 시스템에서 Integration 방식의 성능 개선 연구방향은 국한되어지며, 이로 인해 개선치의 한계를 나타내고 있다. 또한 물질 특성의 한계로 인해 더욱 미세한 선량을 검출하기 어려운 실정이다. 이와 달리 전계효과를 나타내 는 검출 기술은 디락-점이라는 높은 저항 peak를 활용함으로써 미세 선량 검출이 가능하다. 이를 활용하기 위한 전계 형 성은 절연물질을 통한 정전위 형성층이 필수적으로 요구되며, 절연물질의 두께에 따라 달라 질 수 있는 절연 특성에 대한 연구가 필요한 실정이다. 이에 본 연구에서는 PIB 증착 방식을 이용한 정전위 형성층 제작과 그에 따른 절연 효과 및 특성에 대한 분석을 통하여 활용 가능성을 검증하였다. 절연물질로 Si based oxide를 활용하여 다양한 절연두께를 제작하였으며, 인가전압에 따라 측정되는 drift current를 저항으로 산출하여 절연 두께에 따른 저항의 특성을 분석하였다. 그 결과, 2 V와 3 V의 인가전압을 걸어주었을 때, 최대치의 저항 값을 나타내었으며, 이러한 최대 점은 절연물질의 두께가 증가할수록 3 V에서 2 V로 최대치를 나타내는 peak가 shift되는 현상이 나타났다. 이러한 결과를 토대로 PIB방식을 이용한 절연 물질층 제작은 전계효과 검출시스템의 차세대 검출기 연구에 대한 활용 가능성을 입증하였다. For the current X-ray detection system, researches into an integration system for improving performance is limited, indicating a limit to improvement. Furthermore, the limit of material properties makes it difficult to detect finer dose. Alternatively, the detection technology indicating filed effect uses resistance peak referred to as Dirac point, enabling it to detect minute-dose. To form an electric field to utilize this, it is critical to form an electric potential layer through insulation materials and there is the need of conducting studies on the dielectric properties which may vary depending on the thickness of insulation materials. Thus, in this study, the author prepared constant-potential formation layer using PIB deposition and analyzed the applicability of it through analyzing its insulating effect and properties. Constant-potential formation layers of various thicknesses were prepared using Si-based oxide as insulation material and the drift current measured in accordance with the applied voltage was calculated into resistance so as to analyze the properties of resistance in accordance with insulation thicknesses. As a result, the applied voltage of 2V and 3V showed resistance peak and this resistance peak shifted from 3V to 2V with an increase in the thickness of insulation material. Through the preparation of insulation material layer using PIB method based on the above results, the author has proved the applicability of the findings of this study to the researches into next-generation field effect detection system.
