        1980년대 초 후천성면역결핍증의 원인 병원체가 인체면역결핍바이러스로 밝혀진 이후 그 동안 많은 치료제가 개발되어 사용되고 있으며 궁극적으로 질병의 예방을 위한 백신 개발을 위해 많은 과학자들이 연구를 진행하고 있다. 그러나 아직까지 상용화된 백신은 개발되지 않고 있는 실정이다. 그동안 30개가 넘는 후보 물질에 80여개가 넘는 임상 연구가 시행되었으나 만족할 만한 결과를 얻지 못했으며 이렇게 백신 개발에 어려움을 겪고 있는 이유는 여러 가지가 있다. 우선 백신 개발에 적합한 실험 동물 모델이 매우 제한되어 있어 사람을 대상으로 연구를 해야 하는 어려움이 있다. 또한 바이러스의 유전적 다양성으로 인하여 이들에게 모두 효과적인 백신을 만들기는 대단히 어렵다. 그럼에도 불구하고 많은 연구들이 진행되고 있으며 최근에는 과거와 달리 새로운 접근 방법들도 시도되고 있다.

        A Comparative Study of Tensile Strength of Three Operative Fixation Techniques for Metacarpal Shaft Fractures in Adults: A Cadaver Study

        Background: We sought to estimate the ultimate tensile strength after metacarpal shaft fracture repair in adults using three operative fixation methods: plate fixation, Kirschner wire (K-wire) fixation, and intramedullary headless compression screw fixation. We also compared the advantages and disadvantages of each operative technique. Methods: We acquired 30 metacarpal bones from four Korean adult cadavers without trauma, operative history, or deformities. The 30 metacarpal bones were divided into ten groups consisting of three metacarpal bones each with matching sizes and lengths. They were fractured, reduced, and fixed with plate and screws, K-wires, or headless compression screws. We performed force testing, collected ultimate tensile strength data, and created a stress-strain graph. Results: The ultimate tensile strength of ten groups according to the fixation method was as follows: late and screw fixation, 246.1 N (range, 175.3 to 452.4 N); K-wire fixation, 134.6 N (62.7 to 175.0 N); and intramedullary headless compression screw fixation, 181.2 N (119.2 to 211.7 N). The median tensile strength of each fixation method was significantly different. In addition, the post-hoc test showed significant difference between the plate and screw fixation and K-wire fixation, between the headless compression screw fixation and K-wire fixation, and between the headless compression screw fixation and plate and screw fixation. Conclusions: The tensile strength median values decreased in the following order showing significant differences among the fixation methods: plate and screw fixation, headless compression screw fixation, K-wire fixation. Significant differences were also observed between the plate and screw fixation and K-wire fixation, between the headless compression screw fixation and K-wire fixation, and between the headless compression screw fixation and plate and screw fixation.

      • 톡소플라즈마 림프절염 1예

        톡소플라즈마 림프절염은 외국의 경우 비교적 흔하여 경부 림프절 종대와 양성 비후의 소견을 톡소플라즈마에 의한 경우가 많다. 국내에서는 항체양성율이 외국에 비해 낮아 실제 증례도 그리 많지 않을 것으로 추정되며 보고된 경우도 드물다. 그러나 외국인과의 접촉이 빈번해진 최근에는 예전에 비해 드물지 않을 것으로 추정되며, 따라서 경부에 림프절이 양성 비후와 특징적인 병리 소견을 보인다면 톡소플라즈마 림프절염의 가능성을 생각해 보는 것이 좋겠다. 이에 저자 등은 면역기능이 정상인 사람에서 톡소플라즈마 림프절염을 경험하였기에 문헌 고찰과 함께 보고하는 바이다. Toxoplasma lymphadenitis is the most common clinical manifestation of toxoplasmosis in an immunocpmpetent host. In foreign countries, it is a relatively common etiology of reactive hyperplasia of lymph nodes, but it is still rare in Korea. The cervical lymph nodes are most commonly involved, whereas systemic manifestations are rare. We are reporting a case of toxoplasma lymphadenitis in an immunocompetent host. A 31-year-old man was admitted to the hospital because of submandibular lymph node enlargement for 3 months. Toxoplasma lymphadenitis was diagnosed by positive IgG and IgM toxoplasma antibody titer and specific pathologic findings of submandibular lymph node, such as reactive follicular hyperplasia, epitheioid histiocytes in the interfollicular area, and infiltrations of subcapsular monocytoid B lymphocytes. In Korea, travels to and from other countries and contacts with foreigners are increasing, possibly increasing the incidence of toxoplasma lymphadenitis, which should be considered in the differential diagnosis of lymphadenitis.

        Islet isolation from adult designated pathogen‐free pigs: use of the newer bovine nervous tissue–free enzymes and a revised donor selection strategy would improve the islet graft function

Xenotransplantation 2011; 18: 369–379. © 2011 John Wiley & Sons A/S.</P><P><B>Abstract: </B><B> Background: </B> In clinical trials using adult porcine islet products, islets should be isolated from the designated pathogen‐free (DPF) pigs under the current good manufacturing practice (GMP) regulations. Our previous studies suggested that male DPF pigs are better donors than retired breeder pigs and histomorphometrical parameters of donor pancreas predict the porcine islet quality. We aimed to investigate whether the use of the newer bovine nervous tissue–free enzymes and a revised donor selection strategy could improve the islet graft function in the context of islet isolation with DPF pigs.</P><P><B>Methods: </B> Using 30 DPF pigs within a closed herd, we compared the islet yield of porcine islets isolated with Liberase PI (n =<I> </I>11, as a historical control group), Liberase MTF C/T, which is a GMP‐grade enzyme (n<I> </I>=<I> </I>12), and CIzyme collagenase MA/BP protease (n<I> </I>=<I> </I>7). We analyzed the relationship between the diabetes reversal rate of recipient NOD/SCID mice (n<I> </I>=<I> </I>75) and histomorphometric parameters of each donor pancreas as well as donor characteristics.</P><P><B>Results: </B> Proportion of islets larger than 200 μm from the biopsied donor pancreas (P<I> </I>=<I> </I>0.006) better predicted islet yield than age (P<I> </I>=<I> </I>0.760) or body weight (P<I> </I>=<I> </I>0.371) of donor. The proportion of islets larger than 200 μm from the biopsied donor pancreas was not related to the sex of the donor miniature pig (P<I> </I>=<I> </I>0.358). The islet yield obtained with the three enzymes did not differ, even after stratification of the donor with the histomorphometric parameters of the biopsied donor pancreas and the sex of donor. The use of the newer bovine nervous tissue–free enzymes (P<I> </I><<I> </I>0.001), a higher proportion of large islets in donor pancreas (P<I> </I>=<I> </I>0.006), and a male sex of the donor (P<I> = </I>0.025) were independent predictors of earlier diabetes reversal.</P><P><B>Conclusions: </B> Use of the newer bovine nervous tissue–free enzymes including a GMP‐grade enzyme resulted in better islet quality than that of islet isolated using Liberase PI. To obtain high‐quality islet from DPF pigs, the donor should be male pig and histomorphometrical parameters from donor pancreas should be considered.</P>

        Overview of Various Measurement Tools for Shoulder Kinematics

        Shoulder kinematics is important, as it is associated with shoulder arthropathy and pain mechanisms. Various static and dynamic analysis methods are prevalent for shoulder kinematics. These include 2-dimensional plane x-ray, computed tomography, and magnetic resonance imaging, cadaver study, electromagnetic motion analysis, transcortical bone pins technique, and in vivo 3-dimensional motion analysis. Although these methods provide the value of the shoulder kinematics angle, they are unable to explain why such changes occur. Since each method has its pros and cons, it is important to understand all factors accurately, and to choose a method that best meets the purpose of the researcher.
