To evaluate electrophysiologic changes of the epicardium and to determine effects of nifedipine during acute myocardial infarction and subsequent reperfusion, the left anterior descending coronary artery (LAD) was ligated for 30 minutes and reperfused for following 120 minutes in twenty anesthetized open chest dogs which were grouped into control (n=9) and nifedipine (n=11) group. In nifedipine group, intravenous nifedipine 0.2 mg was given over 5 minutes at 5 minute after ligation. ECG, cardiac output, systemic arterial pressure, and pulmonary arterial pressure were measured and epicardial mapping was performed 7 times before ligation for control, 15 minute and 30 minute after ligation, and 30 minute, 60 minute, 90 minute, and 120 minute after reperfusion. The changes of the local activation wave form duration and the conduction delay were measured by the computer analyzer. The results were as follows: 1. In control group, the ventricular fibrillation occurred in 1 dog during ligation and in 2 dogs during reperfusion. In nifedipine group, the ventricular fibrillation also occurred in 1 dog during ligation and in 2 dogs during reperfusion. 2. In control group, the cardiac output revealed significant decrease at 15 minute and 30 minute after ligation and more decrease at 30 minute after ligation and more decrease at 30 minute after ligation, and 30 minute after reperfusion. 3. In both control and nifedipine groups, the local activation wave form duration of the nischemic area was abruptily prolonged at 15 minute and 30 minute after ligation, and recovered to the near control level at 30 minute after reperfusion, which lasted for 120 minutes after reperfusion without change. 4. In both control and nifedipine groups, the conduction delay was significantly prolonged at 15 minute and 30 minute after ligation, and recovered to the near control level at 30 minute after reperfusion, which lasted for 120 minutes after reperfusion without change. These data suggest that electrophysiologic derangement induced by LAD ligation can be considerably improved by subsequent reperfusion but nifedipine had no significant effects on the changes of local activation wave form duration and conduction delay. These data also suggest that nifedipine had beneficial effect on left ventricular dysfunction during LAD ligation and reperfusion.

Two new compounds lanast-5,8-dien-3β-ol-27-oic acid-3β-D-glucopyranosyl (4'-1'')-10'',11''-dimethoxy anthracene (1), 2-methoxy-6-(n-nonacontan-5'',6''-dionyl)-11-hydroxy-13-methyl-11β-D-rhamnopyranoside anthracene (2) have been isolated from the hairy root cultures of Catharanthus roseus. Their structures have been elucidated with the help of 500 MHz NMR using one- and two-dimensional NMR in combination with IR, EI/MS, FAB/MS and HRFABMS spectroscopy.

A common form of relapse in adenocarcinoma of the stomach is intraperitioneal dissemination, in fact, among gastric adenocarcinoma patients who have undergone surgery intended to cure, approximately 50% of the patients develope initial recurrence in the peritoneal cavity regardless of the anatomic site of the primary tumor within the stomach. The efficacy of systemic postoperative chemotherapy to prevent peritoneal recurrence of gastrric adenocarcinoma is not satisfactory. There is still a great need for improved therapeutic strategies on the disseminated microscopic disease and small miliary nodules remaining on the peritoneal surface or lymphatics after operation. The authers have made fluorouracil-polyglycolic acid composite disks(Fu-PGA disks) with fluorouracil and biodegradable polymer: polyglycolic acid for more effective intraperitoneal chemotherapy. We inserted the Fu-PGA disk(s) in the peritoneal cavity of rat and pharmacokinetic study was performed to measure fluorouracil concentration in the peritoneal fluid, plasma, liver, kidney and heart tissue at 24 hour, 72 hour and 168 hour after insertion of Fu-PGA disk(s). Myelosuppressive action of this composite also was determined following its administration. The data of this study suggested that Fu-PGA composite will be a new device releasing drugs in a controlled manner and having targetability to peritoneum, and this device will be improving the efficacy of intraperitoneal chemotherapy for gastric adenocarcinoma.

Purpose: Although progenitor cells may contribute to intimal hyperplasia (IH) after arterial injury, positive contribution of IH is variable with type of injury or cells. This study was designed to examine whether differentiated muscle derived stem cells (MDSC) attenuate IH in rat. Methods: MDSCs were retrieved using preplate techniques from rat calf muscle and MDSCs (preplate 6th culture fraction, pp6) were exposed to VEGF (50 ng/㎖) for endothelial differentiation prior to injection. Male rats were divided into two groups (cell treated vs. control) and underwent carotid balloon injury with 2-Fr catheter. The virus containing Green fluorescent protein (GFP) gene was transfected into cells for monitoring. Cells (5×10?) were indwelled into carotid artery for 30 minutes after injury and then blood flow was restored. Arteries were harvested at various intervals (1, 2 and 4 weeks) after injury. The intima to media thickness ratio (IMTR) was calculated with morphometric analysis. Results: Endothelial surface markers such as VE-CADHERIN were strongly expressed on differentiated MDSCs. At 4 weeks after injury, IH was predominantly observed in control group compared to cell treated group. The intensity of GFP was strongly observed at 1 week and declined at 4 weeks in carotid artery wall at MDSC group. CD31(+) endothelial cells were observed at MDSC group compared to control. The mean IMTR in cell treated groups were significantly lower than control at 2 weeks (P=0.005) and 4 weeks (P≤0.001). Conclusion: Our study demonstrates that MDSCs therapy promotes re-endothelialization and leads to attenuation of IH after balloon injury in rat.

저자들은 흉통과 경도의 호흡곤란으로 내원하여 기흉진단된 환자에서 치료적 흉관 삽입 시술 후, 갑작스런 음압으로 허탈된 폐가 펴지면서 발생한 재팽창성 폐부종의 증례를 이달의 X-선에 보고하는 바이다. Reexpansion pulmonary edema is not a common phenomenon after chest tube insertion but some reports from 0% to 14%. There are various resulting complications, including acute respiratory distress syndrome. We report a case of focal reexpansion pulmonary edema after chest tube insertion. A 49-year-old male came to the hospital due to ongoing dyspnea and left chest pain for 3 days. On chest X-ray, the patient had a left pneumothrax. We planned to insert a chest tube for symptom relief. To determine whether or not the chest had expanded as a result of the chest tube insertion, the patient underwent repeated chest X-rays the following day. The patient experienced brief respiratory symptoms upon initial suction; a chest PA showed patchy consolidated infiltration at the inserted site. After 5 days of conservative management, the recovered completely.
