Celiac disease (CD) is an immune-mediated enteropathy and is a rare disease in Asia, including in Korea. However, the ingestion of wheat products, which can act as a precipitating factor of CD, has increased rapidly. CD is a common cause of malabsorption, but many patients can present with various atypical manifestations as first presented symptoms, including anemia, osteopenia, infertility, and neurological symptoms. Thus, making a diagnosis is challenging. We report a case of CD that mimicked amyotrophic lateral sclerosis (ALS). The patient was a sexagenary man with a history of progressive motor weakness for 2 years. He was highly suspected as having ALS. During evaluation of his neurological symptoms, esophagogastroduodenoscopy (EGD) was performed because he had experienced loose stools and weight loss for the previous 7 months. On EGD, the duodenal mucosa appeared smooth. A biopsy revealed severe lymphoplasma cell infiltration with flattened villi. His serum endomysial antibody (immunoglobulin A) titer was 1:160 (reference, <1:40). Finally, he was diagnosed as having CD, and a gluten-free diet was immediately begun. At a 4-month follow-up, his weight and the quality of his stool had improved gradually, and the neurological manifestations had not progressed. (Intest Res 2017;15:540-542)

Quantum chemical analysis is presented for S NAr cold fluorination of diaryliodonium salts by 19F? observed by Wirth et al. [Eur. J. Org. Chem. 2015, 625]. We examine the origin of experimentally obtained low yields (0?5%) of fluorination by calculating and comparing the relative Gibbs free energies of pre-reaction complexes and the Gibbs free energies of activation for fluorination, bromination, and intramolecular cyclization. Focusing on the counter-anion in diaryliodonium salt, we propose that S NAr bromination would be the predominant process rather than fluorination, resulting in very small amount of fluorinated product. Discussions are made for possible means of improving the yields in cold fluorination.
Impact of Fatty Liver on Acute Pancreatitis Severity

<P><I>Aim</I>. Acute pancreatitis is typically a mild disease, but some patients develop severe courses. Fatty liver changes are seen in patients with acute pancreatitis, but its clinical significance has not been well-studied. We aimed to investigate the relationship between fatty liver and the severity of acute pancreatitis. <I>Methods</I>. Unenhanced CT images of patients with acute pancreatitis were retrospectively reviewed by a radiologist, and mean hepatic and splenic attenuation was measured in Hounsfield units (HU). Fatty liver was defined as mean hepatic/splenic HU < 1. <I>Results</I>. Among 200 patients, fatty liver was found in 67 (33.5%) and nonfatty liver in 133 (66.5%). Compared with patients without fatty liver, the severity of pancreatitis and levels of serum C-reactive protein were higher in fatty liver patients. The prevalence of local complications, persistent organ failure, and mortality were also higher in patients with fatty liver. Even after adjusting for age, sex, body mass index, and cause of pancreatitis, fatty liver was significantly associated with moderately severe or severe acute pancreatitis. <I>Conclusions.</I> Fatty liver may play a prognostic role in acute pancreatitis. Fatty liver could be incorporated into future predictive scoring models.</P>

A quartz tuning fork is an electromechanical resonator with self-actuating and self-sensing capabilities and is widely used as a force sensor in atomic force microscopy and spectroscopy. While the electrical response of a tuning fork is affected by the two prongs’ mechanical motion and stray capacitive current, a purely mechanical motion signal of the tuning fork is required for a quantitative analysis. Here, we demonstrate the extraction of a mechanical motion signal from the electrical signal of an electrically driven quartz tuning fork in various environments, including vacuum, air, and liquid. We show that the extraction formalism is well implemented in vacuum and air, but it does not work in liquid due to the largely enhanced damping and ions present in liquids. Furthermore, using the mechanical signal extracted from the electrical signal, we determine the interaction force exerted on the tip of the tuning fork in ambient air. The present extraction method enables versatile use of electrically driven tuning forks for force, mass, and environmental sensing, in which true mechanical motion signals should be used for accurate and quantitative analysis.

<P>The Coanda effect is a natural phenomenon, which is that flowing fluid on a certain surface is pulled to the surface due to the viscosity of the fluid. This phenomenon can be utilized to produce the lift force of a flying object. This paper presents a new type of unmanned aerial vehicle (UAV) lifted by the Coanda effect, called SCoanda. We introduce the empirical study of its design and implementations. The principle of the Coanda effect is briefly explained in the first section. The design optimization and control of the UAV, and implementation issues are explained. The structural design of SCoanda is devised to use the maximum lift force produced by the Coanda effect. Since there are a plenty of cases about the shape of SCoanda, it has been limited to oval. It is investigated through computational fluid dynamics. As the result, an optimized design is determined among several candidates and appropriate structural materials are selected via tests. Also, dynamic modeling is suggested under several conditions of SCoanda before simulations are conducted to build up a controller and determine gains according to the dynamic modeling. Finally, the UAV is manufactured and its performances are successfully demonstrated in the indoor and the outdoor environments.</P>
