      • The most ideal interval between blastocyst biopsy and vitrification applied in preimplantation genetic screening (PGS)

        Study Question: To evaluate the most ideal interval between blastocyst biopsy and vitrification in preimplantation genetic screening (PGS). Study Design, Size, Duration: This is a retrospective study and total 224 patients underwent the PGS from 2012 Dec. to 2015 Mar. All of patients underwent blastocyst vitrification after biopsy and 1~2 euploid blastocyst for transfer after warming. The primary outcome measures were the implantation and pregnancy rates per PGS-frozen embryo transfer cycle. Materials, Setting, Methods: The blastocyst grading including grade 4, 5 and 6 (according to Gardner grading system) on day 5 or day 6 were selected for trophectoderm biopsy. All blastocyst underwent vitrification immediately (interval: 0.5 hour) or 1 to 7 hours after biopsy. At the time of vitrification the grade of blastocyst expansion was also recorded. All patients were divided into two groups according to the grade of expanded (Group1: ≤1/2 expansion (n=41), Group2: ≥3/4 expansion (n=183)). Furthermore, combined two factors including the interval and morphology of blastocyst after biopsy, all patients were further divided into interval 1 (<3 hours and ≤1/2 expansion) and interval 2 (≥3 hours and ≥3/4 expansion). The morphologically best euploid blastocyst(s) (1~2 embryos) was/were selected first for transfer on the next cycle. Main Results: Assessment morphology of blastocyst after biopsy in different interval, at 0.5 hour after biopsy, 100% blastocyst was non-expansion; at 1 hour after biopsy, only 17% blastocyst was 3/4 expansion or all-expansion; at 3 hours after biopsy, 86% blastocyst was 3/4 expansion or all-expansion and after 5.5 hours, 100% blastocyst was all-expansion or hatching. All blastocysts were survival (100%, 359/359) after warming. The mean of embryo transfer number between all groups were no significantly difference. The implantation rate in Group2 (63.4%) was significantly higher than that in Group1 (46.9%, p=0.014). The pregnancy rates in Group4 (73.8%) was sig-nificantly higher than that in Group1 (51.2%, p=0.004). The implantation and pregnancy rates in the group of embryo ≥3/4 expansion combined with ≥3 hours after biopsy (63.6%, 178/280; 73.8%, 127/172) were significantly higher than that in the group of ≤1/2 expansion with <3 hour (45.6, 26/57; 50.0%, 18/36; p=0.0113 and p=0.0056, respectively). Conclusion: The most ideal interval between biopsy and vitrification was least 3 hours and ≥3/4 expansion of blastocyst after biopsy could improve the implantation and pregnancy rates for PGS.

        A Study on the Application of Cultural Semiotics in the Attractive Factor of Flower

        The purpose of this study is to explore the application of cultural semiotics in the attractiveness factor of flower arrangement. In addition, the study also applies the evaluation grid method of Miryoku Engineering to summarize the attractiveness factor of flower arrangement. After conducting an interview with the use of 14 originally-invented flower arrangement cards, we have found that the applications of cultural semiotics in the attractive factors of flower arrangement can be mainly subdivided into the following categories: "floral style", "field space", "food culture", "local feature", "nostalgic memory", "spiritual aspect", and "installation art". From the evaluation grid method of Miryoku Engineering, we know that the first three keynotes to the applications of cultural semiotics in the attractive factors of flower arrangement are "color design", "focus contrast", and "artwork size". On the other hand, creative factors such as "beautified", "serene", "modern", "leisure", and "situated" are also widely applied to the abstract cause. This study is mainly designed to summarize the application of cultural semiotics in the attractive factor of flower arrangement, which provides future flower arrangement with an improvement standard and application criteria to refer to.

        Transaxillary Capsulorrhaphy with Reimplantation to Correct Bottoming-Out Deformity in Breast Mycobacterial Periprosthetic Infection: A Case Report with Literature Review

        Augmentation mammoplasty is one of the most popular cosmetic surgeries, but there is a high reoperation rate (29.7%) commonly due to capsular contracture, implant malpositioning, infection, and unsatisfactory size. Although infection only accounts for 2% of cases, its management is very challenging, especially with nontuberculous mycobacteria (NTM) infection. Breast prosthetic NTM infection is a rare but is a disastrous condition with an incidence of approximately 0.013%. Immediate salvage reimplantation is usually not suggested, and most studies recommend a gap of 3 to 6 months after combination antibiotics therapy before reimplantation. However, delayed reimplantation often leads to great psychological stress and struggle between the doctor and patient. We present the case report of successful reimplantation in treating prosthetic NTM infections in a 28-year-old female. We discuss a novel technique “transaxillary capsulorrhaphy” to correct the bottoming-out deformity. One year after the combination of antibiotics and surgery, the follow-up computed tomography scan showed complete remission of NTM without recurrence. We discuss the surgical technique in detail. The 1-year follow-up assessment (photos and dynamic video) revealed good cosmesis and reliable correction using the new technique. This report is the first formal description and discussion of one-stage reimplantation following NTM infections. Transaxillary capsulorrhaphy allows for a successful salvage operation when an implant is displaced. This approach provides highly favorable result in eastern women undergoing revision augmentation mammoplasty. This study reflects level of evidence V, considering opinions of respected authorities based on clinical experience, descriptive studies, or reports of expert committees.

        Health Care Utilization and Expenditures of Patients with Diabetes Comorbid with Depression Disorder: A National Population-Based Cohort Study

        Objective: The study investigated to compare health care utilization and expenditures between diabetic patients with and without depression in Taiwan. Methods: Health care utilization and expenditure among diabetic patients with and without depression disorder during 2000 and 2004 were examined using Taiwan’s population-based National Health Insurance claims database. Health care utilization included outpatient visits and the use of inpatient services, and health expenditures were outpatient, inpatient, and total medical expenditures. Moreover, general estimation equation models were used for analyzing the factors associated with outpatient visits and expenditures. Multiple logistic regression analysis was applied for identifying the factors associated with hospitalization. Results: The average annual outpatient visits and annual total medical expenditures in the study period were 44.23–52.20; NT$87,496–133,077 and 30.75–32.92; NT$64,411–80,955 for diabetic patients with and without depression. After adjustment for covariates, our results revealed that gender and complication were associated with out-patient visits. Moreover, the time factor was associated with the total medical expenditure, and residential urbanization and complication factors were associated with hospitalization. Conclusion: Health care utilization and expenditures for diabetic patients with depression were significantly higher than those without depression. Sex, complications, time, and urbanization are the factors associated with health care utilization and expenditures.

        Galangin Prevents Acute Hepatorenal Toxicity in Novel Propacetamol-Induced Acetaminophen-Overdosed Mice

        Acetaminophen (APAP) overdose causes severe liver and kidney damage. APAP-induced liver injury (AILI) represents the most frequent cause of drug-induced liver failure. APAP is relatively insoluble and can only be taken orally; however, its prodrug, propacetamol, is water soluble and usually injected directly. In this study, we examined the timedependent effects of AILI after propacetamol injection in mice. After analyses of alanine aminotransferase and aspartate aminotransferase activities and liver histopathology, we demonstrated that a novel AILI mouse model can be established by single propacetamol injection. Furthermore, we compared the protective and therapeutic effects of galangin with a known liver protective extract, silymarin, and the only clinical agent for treating APAP toxicity, N-acetylcysteine (NAC), at the same dose in the model mice. We observed that galangin and silymarin were more effective than NAC for protecting against AILI. However, only NAC greatly improved both the survival time and rate consequent to a lethal dose of propacetamol. To decipher the hepatic protective mechanism(s) of galangin, galangin pretreatment significantly decreased the hepatic oxidative stress, increased hepatic glutathione level, and decreased hepatic microsomal CYP2E1 levels induced by propacetamol injection. In addition, propacetamol injection also reproduced the probability of APAP-induced kidney injury (AIKI), appearing similar to a clinical APAP overdose. Only galangin pretreatment showed the protective effect of AIKI. Thus, we have established a novel mouse model for AILI and AIKI using a single propacetamol injection. We also demonstrated that galangin provides significant protection against AILI and AIKI in this mouse model.

        Application of Kano Model to Explore the Charm Factors of Meticulous Ink Painting about Flower in Vases

        This study takes the topic of Flowers in Vases of the exhibition ‘Pure Offerings of a Myriad Plants: Paintings on Flower Vases and Potted Scenes’ which hold by National Palace Museum, Taiwan, 2018 as main axis. The thought process and construction of the aesthetics of Meticulous Ink Painting about Flowers in Vases and its charm factors were analyzed. By understanding these charm factors, the appropriate countermeasures for the application can be taken. In this study, we applied the evaluation structure method of charm engineering, interviewed with nine high-art-involved people(five males and four females). The questionnaire survey had been created to find out the satisfactory quality. Applying Kano Model and Refined Kano Model to classify quality and the results were analyzed, we highlight the importance of each quality and suggestions for improvement. The attributes of the charm factors that affect the audience’s appreciation which also have been explored and summarized. In this study, we established the foundation of the charm factors of Meticulous Ink Painting about Flowers in Vases which can be used as a reference for painting or product design.

        A Research on the Impact of Taiwan’s Cultural Colors on Cultural Identity

        Taiwan is a country composed of diverse ethnic groups, and the people in Taiwan have not completely identified with a particular culture in the midst of changing history. According to a recent research, Taiwan has entered the phase of “national identity.” It is the hope of this research that the Taiwanese people will be able to identify themselves with the uniqueness of the Taiwanese culture, and will have greater self-confidence in the process of cultural identity. This research studied the colors expressed in the Taiwanese culture because these colors are infused in everyday living and accumulated in culture. We see these colors every day and get accustomed to them, and often do not know they contain special meanings. So we do not understand the cultural meanings embedded in these colors. Countries closest to Taiwan like Korea, Japan, Thailand, India have organizations that are almost, if not already, at the national level committed to researching and preserving cultural colors. Only Taiwan lacks such an organization. So most people do not understand cultural colors, and miss an observational perspective on Taiwan’s cultural identify. This research explored the influence of Taiwan’s cultural colors on cultural identity, and included a curriculum design and implementation with a theme on “Cultural Colors in Life” in a university’s general education course for the first and second-year students. This curriculum used Walker’s “Big Idea” to design and plan three themed courses on etiquette and customs, nature and ecosystems, land and beliefs in Taiwan; and henceforth introducing the three cultural colors of “Taiwan Form(Red),” “Taiwan Chic(Green)” and “Taiwan Gene (Gold)” into the course content. Each theme included teaching activities like the explanation of the cultural color, the viewing and analysis of a short film “Colorful Taiwan” by Public Television Service(PTS), and so on. A total of four consecutive weeks of teaching activities related to the theme were planned and implemented. The analysis of student questionnaire results shows that after taking the “Cultural Colors in Life” course, students understand more clearly about the cultural colors represented by “Taiwan Red,” “Taiwan Green” and “Taiwan Gold”, and raise their comprehension and identification with the Taiwanese culture.
