The authors experienced 6 cases of Dimecron intoxication, an organophosphate, that required mechanical ventilator treatment. Among 6 cases, 4 cases were intoxication due to suicidal attempt and 2 cases were due to accidental exposure. Among 4 cases of intoxication due to suicidal attempt, only one patient recovered even after aggressive treatments using gastric lavage, mechanical ventilation, atropine and PAM(pralidoxime). Two cases of intoxications due to accidental exposure, 1 case needed mechanical ventilation. Both cases were recovered without any sequele, however. Organophosphate, especially Dimecron, showed high rates of resfiratory muscle paralysis and mechanical ventilator support. The mortality rate of organophosphate agents can be decreased by education, wearing protectors, and aggressive mechanical ventilator treatment when respiratory failure develops.
Tuberculous Tenosynovitis of the Elbow - A Case Report

A 29-year-old man visited our clinic owing to a persistent swelling in the anterior part of the left elbow joint that began one year ago. Through magnetic resonance imaging (MRI), we observed tenosynovitis with multiple rice bodies, and so we performed an excisional biopsy and tenosynovectomy. Through pathology and culture tests, we identified tuberculosis in the tissue biopsy that we harvested intraoperatively. Following the anti-tuberculosis medication relieved the patient’s symptoms without recurrence. Since tuberculosis of the elbow occurs only rarely, and the symptoms mimic those of rheumatoid synovitis or of non-specific chronic synovitis, early diagnosis and appropriate treatment are often delayed. The authors report this rare case of tuberculous tenosynovitis of the elbow with a review of the relevant literature.
Case Report : Cutaneous Malignant Melanoma Associated with Papillary Thyroid Cancer

As the survival from cutaneous malignant melanoma and its clinical concerns have been steadily increasing, the possibility has been raised of an increased risk of second primary cancers in the patients with malignant melanoma. Especially, recent studies have identified an association between cutaneous malignant melanoma and thyroid carcinoma. We here report on a case of cutaneous malignant melanoma that developed in a 61-year-old female patient who had hypothyroidism caused by papillary thyroid carcinoma. We suggest that the individuals who have cutaneous malignant melanoma may be predisposed to other primary cancers and especially thyroid carcinoma. Continuous monitoring of the thyroid function in melanoma patients is required because hypothyroidism can worsen due to malignant melanoma and this is probably associated with thyroid carcinoma. Roxithromycin과 Cisapride 병용시 약물상호작용에 관한 정보 제공 사례

Roxithromycin, which is frequently used for upper and lower respiratory tract infection, is a semi-synthetic, acid-stable macrolide. On using macrolide with cisapride, it decreases metabolism of cisapride and cause cardiac arrhythmia(e.g. QT prolongation) by the inhibitory action of cytochrome P450. It is known that roxithromycin does not represent this action but we report two cases of our hospital suggested to medical staff the possibility of interaction between roxithromycin and cisapride after studying first, second, and third literature. Accoring to one study, roxithromycin was reported that it has low affinity with cytochrome P450, but the other study classified macrolides into three groups by the affinity with cytochrome P450, Roxithromycin and clarithromycin make a complex with cytochrome P450 in the glucocorticoid pretreated rats. Recently, clarithromycin is defined contraindication with cisapride and it is not also desirable to combine roxithromycin with cisapride.
Abnormal Chloroplast Development and Growth Inhibition in Rice Thioredoxin m Knock-Down Plants.

<P>Plant cells contain several thioredoxin isoforms that are characterized by subcellular localization and substrate specificity. Here, we describe the functional characterization of a rice (Oryza sativa) thioredoxin m isoform (Ostrxm) using a reverse genetics technique. Ostrxm showed green tissue-specific and light-responsive mRNA expression. Ostrxm was localized in chloroplasts of rice mesophyll cells, and the recombinant protein showed dithiothreitol-dependent insulin beta-chain reduction activity in vitro. RNA interference (RNAi) of Ostrxm resulted in rice plants with developmental defects, including semidwarfism, pale-green leaves, abnormal chloroplast structure, and reduced carotenoid and chlorophyll content. Ostrxm RNAi plants showed remarkably decreased F(v)/F(m) values under high irradiance conditions (1,000 mumol m(-2) s(-1)) with delayed recovery. Two-dimensional electrophoresis and matrix-assisted laser-desorption/ionization time-of-flight analysis showed that the levels of several chloroplast proteins critical for photosynthesis and biogenesis were significantly decreased in Ostrxm RNAi plants. Furthermore, 2-Cys peroxiredoxin, a known target of thioredoxin, was present in oxidized forms, and hydrogen peroxide levels were increased in Ostrxm RNAi plants. The pleiotropic effects of Ostrxm RNAi suggest that Ostrxm plays an important role in the redox regulation of chloroplast target proteins involved in diverse physiological functions.</P>

We report on three cases of subtrochanteric femoral fractures during trochanteric intramedullary nailing for the treatment of femoral shaft fractures. Trochanteric intramedullary nails, which have a proximal lateral bend, are specifically designed for trochanteric insertion. When combined with the modified insertion technique, trochanteric intramedullary nails reduce iatrogenic fracture comminution and varus malalignment. We herein describe technical aspects of trochanteric intramedullary nailing for femoral shaft fractures to improve its application and prevent implant-derived complications.
