상부요로계감염 환자에서 Gallium-67 Citrate 스캔 및 Tc-99m Dimercaptosuccinic Acid(DMSA)스캔의 임상적 의의

It is often not certain whether the focal defect of renal cortex of Tc-99m Dimercaptosuccinic acid(DMSA) renal scan in patients with acute pyelonephritis means the cortical scar by previous inflammation or recent active inflammatory lesion. Gallium-67 citrate(Ga-67) also has been known to be accumulated at the active inflammatory lesion. In order to evaluate the usefulness of the Ga67 and Tc-99m DMSA renal scan in patients with acute pyelonephritis, we reviewed the results of both study and clinical findings. Nintecn patients with typical clinical findings of acute pyelonephritis and admitted to our institute from Jan. 1998 to Nov. 1998 were included in this study. Tc-99m DMSA and Ga-67 scan were performed mean 3.3 and 7.3 days after admission. Eleven of 19 patients (57.9%) showed abnormally increased focal renal uptake on the Gallium-67 citrate scan and 14 of 19 patients(73.7%) had a cortical defect on the kidneys in Tc-99m DMSA scan. Seven(36.8%) patients had an abnormality in both study. Seven(36.8%) patients had an abnormality only in Tc-99m DMSA scan and 4 (21.1%) patients had an abnormality only in Ga-67 scan. Of 7 patients who had an abnormality on both scan, 5 patients had. an abnormality at same region in both scan and 2 patients had an abnormality at the different sites on the kidneys. With the above results, we speculate that Ga-67 scan and Tc-99m DMSA scan play a complementary role in the patients with upper urinary tract infection.

<P>Tumor suppressor programmed cell death protein 4 (PDCD4) inhibits the translation initiation factor eIF4A, an RNA helicase that catalyzes the unwinding of secondary structure at the 5'-untranslated region of mRNAs and controls the initiation of translation. Here, we determined the crystal structure of the human eIF4A and PDCD4 complex. The structure reveals that one molecule of PDCD4 binds to the two eIF4A molecules through the two different binding modes. While the two MA3 domains of PDCD4 bind to one eIF4A molecule, the C-terminal MA3 domain alone of the same PDCD4 also interacts with another eIF4A molecule. The eIF4A-PDCD4 complex structure suggests that the MA3 domain(s) of PDCD4 binds perpendicular to the interface of the two domains of eIF4A, preventing the domain closure of eIF4A and blocking the binding of RNA to eIF4A, both of which are required events in the function of eIF4A helicase. The structure, together with biochemical analyses, reveals insights into the inhibition mechanism of eIF4A by PDCD4 and provides a framework for designing chemicals that target eIF4A.</P>

N/A We have anlayzed the polar maps of dipyridamole stress/rest Tc-99m-MIBI SPECT and compared the quantitated perfusion defects of dipyridamole stress polar map and the findings of coronary angiograraphy in 56 pateints with coronary artery diseases. We performed the same day dipyridamole stress-rest myocardial SPECT, reconstructed the polar maps according to Cedars-Sinai method and quantitated perfusion defects of total myocardium and the territory of each artery, comparing the polar maps of patients with normal files. Stenosis more than 50 percent was considered significant and myocardial ischemic score was calculated as summed score of percents of main coronary arteries. Positive concordance of myocardial SPECT with coronary angiography were 82.6% with left anterior descending artery(LAD), 85.7% with left circumflex artery(LCx) and 78.6% with right coronary artery(RCA). Perfusion defect of SPECT polar map and the stenosis of coronary artery showed the contingency phi of 0.55(p〈0.0001) with total atreries, 0.38(p=0.016) with LAD, 0.50(p〈0.0001) with LCx and 0.40 (p=0.007) with RCA. Dipyridamole stress percent defect of polar map was correlated with myocardial ischemic score with Spearman's rho of 0.47(p=0.001) in total arteries, 0.48(p=0.001) in LAD, 0.56 (p〈0.001) in LCx and 0.38(p=0.002) in RCA. These findings revealed that defect of the dipyridamole stress myocardial Tc-99m-MIBI SPECT and the percent extent of this defect were related with significant artery stenosis of individual arteries and the degree of stenosis. We thought that we could use the defects in the polar map of dipyridamole stress Tc-99m-MIBI SPECT for the quantification of myocardial perfusion decrease.

N/A Coronary angiography should be performed to verify the patency of bypass graft if coronary artery disease is said to be surgically corrected. Rest/stress myocardial SPECT could be used as a noninvasive alternative to prove patency of bypass graft and functional significance of graft patency by showing improvement of rest perfusion and perfusion reserve after operation. We evaluated the outcome of coronary artery bypass graft surgery by comparing rest/dipyridamole- stress myocardial SPECT before and after operation. Myocardial SPECT were performed 19±23 days before and 108±19 days after operation in 44 patients(M:F=25:19). Segmental perfusion were scored with 0(normal) to 3(defect) independently for each image set of rest and stress. Perfusion scores were compared between pre and postoperative images. Rest/stress images of pre and post- operative SPECT were compared in paired sets. Postoperative change of perfusion was determined as no change, improvement or aggravation for each artery territory. Postoperative outcome of patients were analyzed. Overall, 74%(158 segments) of 215 segments which had perfusion decrease before operation showed improvement. Among 60 artery territories, 37 territories(62%) improved. Among 42 patients with perfusion decrease in preoperative SPECT, 21 patients(47%) improved after operation. Severe persistent defects improved in 43%(6/14). We concluded that bypass surgery improved myocardial perfusion in dipyridamole stress induced or persistent decrease and that rest/stress myocardial SPECT could be used as a guide for which artery should be operated.
암 치료를 위한 188Re ( V ) -DMSA 에 관한 연구 - 방사성동위원소 표지와 생체내 분포

N/A The purpose of this study was to examine the radiolabeling and biodistribution of 188Re(V)-DMSA as a therapeutic cancer radiopharmaceutical. We made a DMSA kit(NaHC03 1.5 mg, meso-2,3-dimercaptosuccinic acid 1.0 mg L(+)-ascorbic acid 0.7mg, SnCl2·2H2O 0.34 mg, pH 2.9) for labeling with 188Re. In this kit, 188ReO4- 5 mCi/2 ml added and boiled at 100℃ for 3 hr in water bath. The final pH adjusted to 7.5 with 7% NaHCO3 solution. We checked the labelling efficacy with TLC-SG(n-butanol : acetic acid:H2O = 3 : 2 : 3) and examined the stability both in room temperature and in serum at 37℃. Biodistribution(l, 3, 13, 24, 48 hr) of 188Re(V)-DMSA compound was evaluated in Sarcoma 180 tumor-bearing mice. Each labeling efficiency and stability at room temperature for 48 hours was over 98% and 95%, respectely. The stability in serum were 82%(6 hr) and 85%(48 hr). Tumor uptake of 188Re(V)-DMSA in Sarcoma 180- bearing mice were 0.66±0.15%(1 hr), 0.51±0.10%(3 hr), 0.19±0..05%(24 hr) and 0.13±0.02%(48 hr). These result are consistent with those of 99mTc(V)-DMSA which were reported previously In conclusion, 188Re(V)-DMSA may be a useful therapeutic radiopharmaceutical for treating some cancers and metastatic bone lesion.

For measurement of ventricular performance, ejection fraction (EF) has gained wide acceptance. But EF is influenced not only by changes in muscle function but also by changes in cardiac loading conditions. In case of valvular geart disease which is variable in loading conditions, EF cannot be reliable as an index of myocardial contractility. The end systolic pressure (ESP)-end systolic volume (ESV) relation, howver, is known to represent myocardial contractility, independent of changes in loading conditions. Similar results can be obtained by using peak-systolic presure (PSP) instead of ESP. To evaluate the utility of the peak systolic pressure-end systolic volume index (PSP-ESVI) relation as an index of myocardial function, we measured PSP & ESVI in 19 partents with coronary artery disease before (PSP& ESVI) and after (PSP & ESVI) sublingeal administration of nitroglycerin, PSP was measured with standard mercury sphygmomanometer during gated blood pool scintigraphic study. ESVI was measured by count derived method after attenuation correction. PSP & ESVI measurement was started when the fall of PSP was greater than 5 mmHg after 7-14 minutes post-administration of nitroglycerin. Mean values (±S.D.) of PSP & ESVI was 124.9(±20.7) mmHg 59.4(±39.9) ml/M². Mean values (±S.D) of PSP & ESVI was 113.2(±19.9)mmHg & 37.5(±26.1)ml/M². There was a significant difference between mean values of PSP & PSP (p$lt;0.01), and mean values of ESVI & ESVI (p$lt;0.01)PSP-PSP/ESVI-ESVI, PSP/ESVI and EF were in the range of 0.14-5.19 mmHg/ml/M², 0.67-7.68 mmHg/ml/M² and 10.8%-74.5% respectively. PSP-PSP/ESVI-ESVI and EF showed exponetial correlation (r=0.85, P$lt;0.01). The correlation coefficient between PSP/ESVI and EF was 0.73(p$lt;0.01). With the above results, we suggest that PSP₁-PSP₂/ESVI₁-ESVI₂ and PSP₁/ESV1 can be usde as an index of myocardial function.

Obtaining a sufficient amount of healthy keratinocytes from a small tissue is difficult. However, ORS cells can be a good source of epithelium since they are easily obtainable and patients do not have to suffer from scar formation at donor sites. Accordingly, the current study modified the conventional primary culture technique to overcome the low propagation and easy aging of epithelial cells during culturing. In a conventional primary culture, the average yield of human ORS tells is 2.↑ $\times$ 10$^3$cells/follicle based on direct incubation in a trypsin (0.1%)/EDTA(0.02%) solution for 15 min at 37$^{\circ}C$, however, our modified method was able to obtain about 6.9 $\times$ 10$^3$cel1s/follicle using a two-step enzyme digestion method involving dispase (1.2 U/mL) and a trypsin (0.1%)/EDTA (0.02%) solution. Thus, the yield of primary cultured ORS cells could be increasd three times higher. Furthermore, a total of 2.0 $\times$ 10$^{7}$ cells was obtained in a serum-free medium. while a modified E-medium with mitomycin C-treated feeder tells produced a total of 6.3 $\times$ 10$^{7}$ Cel1s over 17 days When Starting With 7.5 $\times$ 10$^4$cells. Finally, We Confirmed the effectiveness of our ORS tell isolation method by presenting their ability for reconstructing the bioartificial skin epithelium in vitro
