        인천 인현동 호프집 화재 피해자 분석

        Purpose : This study was conducted to develop field triage, transportation, distribution, and prehospital care at a fire disaster by analyzing the victims of the fore that broke out at a bar in Incheon. Method : We analyzed the cases of the victims of a fire in Incheon in Oct. 1999. We determined the primary care hospital, the arrival time, the burn size, the outcome, and the injury type from the medical records, the concerned organ records, and interviews with concerned persons. Result : The total number of victims was 137: 56 prehospital deaths, 1 hospital death, and 80 survivals. The Pearson correlation coefficient between the burn size and the severity was -0.175. There were 121(89.6%) cases of inhalation injury, 59 (43.7%) cases of flame burns, 66 (48.9%) cases of hypoxic brain damage, and 16 (11.9%) cases involving other types of injury. Conclusion : The causes of death of the fire victims were inhalation injury and hypoxic brain damage due to CO poisoning and other toxic inhalants. We propose the use of a simple triage and rapid treatment(START) system and a reassessment the delayed category in fire disasters.

        What is the most important in articles of food is hygienic safety. Because food is the most common thing in our everyday life, however, the importance of its hygienic safety and other many problems caused by food may be easily neglected. Since PL Law came into effect in July of the last year(2002) in Korea, many domestic firms have taken great interests in PL. And the purpose of PL Law is to provide a means for those injured by defective products to receive redress. Additionally, enterprises get to place more importance on the safety of the products, which led to improvement in competitiveness and enhancement in quality. Consequently customers can get better products. So, it aims not only to compensate the injured by defective products but also to act an incentive to makers/suppliers of food to make their products more safe. Therefore, this study was aiming to analyze the expected risks to articles of food after the enforcement of PL Law, and to establish a defending plan.

        Mucinous carcinoma is a predictive factor for the risk of open conversion from laparoscopic colectomy in colorectal cancer

        Purpose: Although laparoscopic surgery is widely accepted in the treatment of colorectal cancer, conversion to open surgery is associated with the rate of unfavorable outcomes. The aim of this study was to determine the factors associated with open conversion from laparoscopic surgery for colorectal cancer. Methods: A total of 3,002 patients who underwent laparoscopic colectomy as an initial plan for the treatment of colorectal cancer located from the sigmoid colon to the rectum were retrospectively evaluated between January 2009 and December 2018 at Samsung Medical Center in Korea. Risk factors significantly associated with open conversion were determined using univariate and multivariate regression models. Results: Among the 3,002 patients, open conversion was performed in 120 patients (4%). Age >60 years (adjusted odds ratio [AOR], 2.370), preoperative bowel obstruction (AOR, 2.348), clinical T4 stage (AOR, 2.201), and serum carcinoembryonic antigen level >5 ng/mL (AOR, 2.289) were significantly associated with open conversion. Moreover, mucinous carcinoma was a significantly more frequent histopathologic type than adenocarcinoma (10.0% vs. 3.2%, P<0.001) in the open conversion group with an AOR of 2.549 (confidence interval, 1.259–5.159; P=0.009). Conclusion: The present study presented a novel finding, i.e. mucinous carcinoma as the histopathologic type could be an independent predictive factor for conversion from laparoscopic colectomy to open surgery. Identifying patients with mucinous carcinoma will help stratify the risk of open conversion preoperatively.

        ANTIMICROBIAL ACTIVITY OFARTEMISIA SPECIES AGAINST CLINICALLY ISOLATED STREPTOCOCCUS MUTANS

        Streptococcus mutans는 Mutans streptococci 그룹에 속하는 치아우식 유발균으로 알려져 있다. 국내 자생쑥 종의 추출물이 이러한 치아우식을 유발하는 Streptococcus mutans균에 대해 활성 억제 효과가 있다는 것이 표준균주를 이용한 실험에서 밝혀져 있으며, 본 연구에서는 환자에서 분리된 S. mutans를 중심으로 쑥의 항균 효과를 알아보았다. 네 가지 임상 분리균주; Ch, L1, L2, P와 네 가지 표준균주: KCTC (Korean Culture for Type Collection) 3298, KCTC 3605 (ATCC 25175), KCTC 3306, Streptococcus mutans GS5를 이용하여 세 가지 종류의 쑥; 참쑥(Artemisia lavandulaefolia DC), 비쑥(Artemisia scoparia), 사철쑥(Artemisia capillaries)의 효능을 검증하였다. 항균활성은 세 가지 종류의 쑥 추출물의 Streptococcus mutans에 대한 최소 억제농도(MIC)와 최소살균농도(MBC), time-kill studies를 통해 알아보았다. 최소억제농도는 표준균주에 대해 각각 참쑥이 0.8 ug/mL, 비쑥이 0.8-1.6 ug/mL, 사철쑥이 0.2-25.6 ug/mL이 었으며, 분리된 균주에 대해서는 참쑥이 0.4-0.8 ug/mL, 비쑥이 0.4-0.8 ug/mL, 사철쑥 1.6-6.4 ug/mL이었다. 최소살균농도는 표준균주에 대해서 참쑥이 1.6-6.4 ug/mL, 비쑥이 6.4-12.8 ug/mL이였다. 분리균주에 대해서는 참쑥이 0.8-3.2 ug/mL, 비쑥이 0.8-3.2 ug/mL, 사철쑥은 6.4-12.8 ug/mL이었다. Time-kill assay에서 KCTC 3298이 4 hrs에 100% 살균되었으며, GS5도 거의 100% 살균되었고 L1과 L2 P는 0.5 hrs에 99.90% 살균되었다. 0.5 hrs에 KCTC 3298와 GS5는 99.99% 이상 살균되었고, 모든 균주들이 2.5 hrs에 99.99% 살균되었다. 전체적인 실험 결과 세 가지 종류의 쑥은 표준 균주 및 네 가지 분리 균주 모두에서 항균 효과가 있음을 확인하였다. Streptococcus mutans plays a major role in the formation of dental plaque and it is considered one of the important pathogens in the development of dental caries. Established dental plaque can be more resistant to antimicrobial agents and offers nutrient rich and relatively stable cariogenic conditions for bacteria. Thus bacteria growing in dental plaque have strong resistance to antimicrobial agents and they are not removed easily by the flow of saliva. Many researchers have been performed using natural materials, especially herbal extracts to prevent dental plaque. However, the strains of mutans streptococci used in the researches were not from Koreans. Therefore, it would be necessary to evaluate the antimicrobial activity of herbal extracts against clinical isolates of Streptococcus mutans isolated from saliva of Koreans living in Jeollabuk-do. For this study four clinical isolates were isolated from saliva samples of seventeen Korean people to investigate whether essential oils of Artemisia spp. have antimicrobial activity against them including four reference strains of Streptococcus mutans. Minimum inhibitory concentrations, Minimum bactericidal concentrations and time kill studies were performed and the results showed that Artemisia lavandulaefolia DC, Artemisia scoparia, and Artemisia capillaries have antimicrobial activity against the Streptococcus mutans clinical isolates and reference strains.

        Temporal Transcriptome Analysis of SARS-CoV-2-Infected Lung and Spleen in Human ACE2-Transgenic Mice

        Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is a highly transmissible and potentially fatal virus. So far, most comprehensive analyses encompassing clinical and transcriptional manifestation have concentrated on the lungs. Here, we confirmed evident signs of viral infection in the lungs and spleen of SARS-CoV-2-infected K18-hACE2 mice, which replicate the phenotype and infection symptoms in hospitalized humans. Seven days post viral detection in organs, infected mice showed decreased vital signs, leading to death. Bronchopneumonia due to infiltration of leukocytes in the lungs and reduction in the spleen lymphocyte region were observed. Transcriptome profiling implicated the meticulous regulation of distress and recovery from cytokine-mediated immunity by distinct immune cell types in a time-dependent manner. In lungs, the chemokine-driven response to viral invasion was highly elevated at 2 days post infection (dpi). In late infection, diseased lungs, post the innate immune process, showed recovery signs. The spleen established an even more immediate line of defense than the lungs, and the cytokine expression profile dropped at 7 dpi. At 5 dpi, spleen samples diverged into two distinct groups with different transcriptome profile and pathophysiology. Inhibition of consecutive host cell viral entry and massive immunoglobulin production and proteolysis inhibition seemed that one group endeavored to survive, while the other group struggled with developmental regeneration against consistent viral intrusion through the replication cycle. Our results may contribute to improved understanding of the longitudinal response to viral infection and development of potential therapeutics for hospitalized patients affected by SARS-CoV-2.

        Pre-validation study of spectrophotometric direct peptide reactivity assay (Spectro-DPRA) as a modified in chemico skin sensitization test method

        Skin sensitization is induced when certain chemicals bind to skin proteins. Direct peptide reactivity assay (DPRA) has been adopted by the OECD as an alternative method to evaluate skin sensitization by assessing a substance's reaction to two model peptides. A modified spectrophotometric method, Spectro-DPRA, can evaluate skin sensitization, in a high throughput fashion, to obviate some limitations of DPRA. Pre-validation studies for Spectro-DPRA were conducted to determine transferability and proficiency, within- and between-laboratory reproducibility, and predictive ability based on GLP principles at three laboratories (AP, KTR, and KCL). All laboratories confirmed high (> 90%) concordance for evaluating the sensitivity induced by ten chemical substances. The concordance among the three tests performed by each laboratory was 90% for AP, 100% for KTR, and 100% for KCL. The mean accuracy of the laboratories was 93.3% [compared to the standard operating procedure (SOP)]. The reproducibility among the three laboratories was as high as 86.7%; the accuracy was 86.7% for AP, 100% for KTR, and 86.7% for KCL (compared to the SOP). An additional 54 substances were assessed in 3 separate labs to verify the prediction rate. Based on the result, 29 out of 33 substances were classified as sensitizers, and 19 out of 21 identified as non-sensitizers; the corresponding sensitivity, specificity, and accuracy values were 87.9%, 90.5%, and 88.9%, respectively. These findings indicate that the Spectro-DPRA can address the molecular initiating event with improved predictability and reproducibility, while saving time and cost compared to DPRA or ADRA.

      • Prediction rate evaluation of spectro-DPRA as a modified in chemico skin sensitization test method

        Some chemicals can bind to skin proteins and induce sensitization. Direct peptide reactivity assay (DPRA) listed in the OECD guideline is an alternative method to evaluate the skin sensitization, by assessing a substance"s reaction to peptides. Newly modified spectrophotometric method for skin sensitization evaluation (spectro-DPRA) in Korea can improve some defect of DPRA. Pre-validation studies for spectro-DPRA were conducted at three institutions (AP, KTR, and KCL) to exam the transferability and proficiency, within and between laboratory reproducibility, and predictive ability based on GLP principle. All organizations confirmed more than 90% of agreements for the proficiency evaluation using ten chemical substances. Within the laboratory reproducibility evaluation, the agreement rate between the three tests performed by each institution was confirmed as 90% for AP, 100% for KTR, and 100% for KCL, and 90% agreement was approved when comparing with LLNA data. In evaluating reproducibility between the three laboratories, the agreement was 86.7%, showing a high value, and the degree of agreement with the LLNA data result was confirmed as 86.7% for AP, 100% for KTR, and 86.7% for KCL. Additional 54 substances were used to verify the prediction rate. As a result, 29 out of 33 sensitizers were identified as sensitizers, and 19 out of 21 non-sensitizers were identified as non-sensitizers, confirming a sensitivity of 87.8%, a specificity of 90.4%, and an accuracy of 88.8%. According to the results, the spectro-DPRA skin sensitization test presented adequate predictive capacity compared with the existing DPRA or ADRA while representing a method to save time and cost.
