Patterns of failure for hypopharynx cancer patients treated with limited high-dose radiotherapy treatment volumes

Purpose: Optimal radiotherapy treatment volumes for patients with locally advanced hypopharynx squamous cell carcinoma should ensure maximal tumor coverage with minimal inclusion of normal surrounding structures. Here we evaluated the effectiveness of a direct 3-mm high-dose gross tumor volume to planning target volume expansion on clinical outcomes for hypopharynx cancers. Materials and Methods: We performed a retrospective analysis of patients with hypopharynx carcinoma treated between 2004 and 2018 with primary radiotherapy using a direct high-dose gross tumor volume to planning target volume expansion and with or without concurrent systemic therapy. Diagnostic imaging of recurrences was co-registered with the planning CT. Spatial and volumetric analyses of contoured recurrences were compared with planned isodose lines. Failures were initially defined as in field, marginal, elective nodal, and out of field. Each failure was further classified as central high-dose, peripheral high-dose, central intermediate/low-dose, peripheral intermediate/low-dose, and extraneous. Clinical outcomes were analyzed by Kaplan-Meier estimation. Results: Thirty-six patients were identified. At a median follow-up at 52.4 months, estimated 5-year overall survival was 59.3% (95% confidence interval [CI], 36.3%–74.1%), 5-year local and nodal control was 71.7% (95% CI, 47.1%–86.3%) and 69.9% (95% CI, 57.0%–82.6%), respectively. The most common failure was in the high-dose primary target volume. The gastrostomy tube retention rate at 1 year among patients without recurrence was 13.0% (95% CI, 3.2%–29.7%). Conclusion: Minimal high-dose target volume expansions for hypopharynx cancers were associated with favorable locoregional control. This approach may enable therapy intensification to improve clinical outcomes.

Cariprazine — an Alternative Treatment for Clozapine-resistant Schizophrenia?

Treatment-resistant schizophrenia (TRS) poses a significant therapeutic challenge in psychiatric practice. Clozapine is recognized as a treatment of choice in TRS but is not always effective in alleviating patients’ symptoms. Additionally, clozapine therapy is associated with multiple side effects and monitoring requirements that often limit its use and negatively affect patients’ compliance with the treatment. Although clozapine augmentation options are available, there is currently no alternative monotherapy proven to be effective in TRS. We present a case of a young man with TRS who failed to respond to appropriate trials of risperidone, aripiprazole and also clozapine, and who experienced impairing adverse effects of clozapine that made further clozapine treatment not only futile but also detrimental to his health. He was successfully treated with cariprazine monotherapy, which culminated in the remission of his both positive and negative symptoms of psychosis as well as in the marked improvement in social functioning. Cariprazine, a newer atypical antipsychotic endowed with a D3-preferring mode of action, may offer a better tolerated and more acceptable treatment option for patients with difficult-to-treat psychotic symptoms.

Between Plasticity and Performance

전자음악은 예술 장르 가운데 매우 특이한 위치를 차지하고 있다. 한편으로는 회화나 조각과 같은 조형예술과 그 궤를 함께하고 있는데, 예를 들어 작곡가들은 스튜디오 내에서 행하는 주조, 조형, 조각과 같은 행위들이 가지고 있는 촉각, 운동감각, 그리고 자기수용적 측면에서 작곡 행위와 동일한 특성을 종종 발견한다. 다른 한편으로 드라마, 무용, 기악음악과 같은 공연예술적인 측면에서의 전자음악은, 실시간 조작이 가능한 다채널 사운드 시스템의 보급과 더불어 기존 형식의 공연예술과 성공적으로 결합할 수 있었다. 본 연구에서는 이 표면적 역설을 적극적으로 설명하고자 하는데, 조형예술과 공연예술 사이에서의 존재론적 구분과 더불어 전자음악이 조형미술, 또는 공연미술과 얼마나 존재론적 측면에서 닮아 있는가를 살펴보고 다음과 같은 결론을 맺고자 한다. 전자음악은 조형예술적 측면에서 또는 공연예술적 접근에 의해서도 특징지어질 수 있다. 그러나 많은 경우에는 이 두 지점 어딘가에 존재하는 것으로 설명된다. 이러한 고찰은 왜 때로는 철학자, 미학자, 음악학자들이 전자음악을 확고하고 흥미로운 예술 장르로서 그 가치를 분명히 하는 데 어려움을 겪는지 설명하는 데 도움이 될 것이다. Electroacoustic music occupies a curious position within the arts. On the one hand, it appears to be aligned with the plastic arts, such as painting and sculpture; composers often refer to the haptic, kinaesthetic and even proprioceptive nature of their compositional acts in terms of crafting, moulding and sculpting sounds in the studio. On the other hand, electroacoustic music appears be aligned with the performing arts, such as drama, dance and most forms of instrumental music; the proliferation of diffusion systems and the increasing sophistication of tools for rea-time spatialisation lend credence to associations with traditional notions of performance practice. This paper seeks to demystify this ostensible paradox. It starts considering an ontological distinction that holds between the plastic arts and the performing arts, goes on to consider whether electroacoustic music is ontologically similar to one or the other, and concludes with the following point: electroacoustic music may be characterised by either plasticity or performance but, in many cases, it falls between these polarities. This observation may help to explain why some philosophers, aestheticians and musicologists have struggled to accept electroacoustic music whilst enabling one to identify where the unique value of this exciting and uncompromising art-form resides.

NORTH KOREAN HIP HOP? REFLECTIONS ON MUSICAL DIPLOMACY AND THE DPRK

This article analyzes the effectiveness of classical music diplomacy toward the DPRK as seen in the New York Philharmonic visit of 2008. After contextualizing the role of the symphony orchestra with reference to North Korea and engaging with some questions of theory and orchestral program, the article goes on to suggest that popular music, specifically rap music, is likely to have a much more lasting impact in the DPRK. As such, the article enters into debates about North Korean cultural identity, U.S. foreign policy (particularly “soft power” directed at the DPRK), and musical practices on the Korean peninsula.

Race and Legitimacy in Acts 17:26: An Approach from Political Theology

더글라스(Frederick Douglas)는 자신의 유명한 편지에서 사도행전 17장 26절에 기대어 “노예들에게 독립기념일이란 무엇인가?”라고 질문한다. 더글라스는 “당신들은 ‘인류의 모든 족속을 한 혈통으로 만드사 온 땅에 살게 하심’을 믿는다고 고백했다”라고 지적함으로써, 자신의 반대 세력들의 믿음을 인용하여 자신의 주장에 대한 정당성을 확보하고자 한 것이었다. 본 논문은 사도행전 17장 26절을 노예폐지론자, 다문화주의자, 그리고 인종분리주의자들이 어떻게 수용하였는지 살펴본다. 이 과정에서 본 논문은, 코츠코(Adam Kotsko)의 신자유주의 이론 모델처럼, 인종문제를 정치신학적 관점에서 접근하고자 한다. 따라서 필자는 사도행전 17:26에서 인종 담론은 차이점들을 지우고 모두가 동일하게 보이게 만든다는 점을 지적하고자 한다. Frederick Douglass appeals to Acts 17:26 in his famous letter, “What to the slave is the Fourth of July?” Douglass intends to gain legitimacy to his argument by conjuring the beliefs of his opponents, when he says, “You profess to believe ‘that, of one blood, God made all nations of men to dwell on the face of all the earth.’” While Acts certainly relies on political theology to legitimize “men” in the face of “all nations,” this paper will concern itself with recent invocations of this form of legitimization of the human, one which is made up of different races. In particular, I want to look at the series of essays from The Gospel Coalition"s Tim Keller and his treatment of race. This paper will analyze uses of Acts 17:26 in so-called “multiculturalist” discourses and Tim Keller’s work on The Gospel Coalition. Through this analysis, this paper will consider race, like Kotsko"s neoliberalism, as its own legitimizing political theology. Race legitimates all into a sameness that erases difference.

<P>The azimuthal correlations of D mesons with charged particles were measured with the ALICE apparatus in pp collisions at [FORMULA OMISSION] and p–Pb collisions at [FORMULA OMISSION] at the Large Hadron Collider. [FORMULA OMISSION], [FORMULA OMISSION], and [FORMULA OMISSION] mesons and their charge conjugates with transverse momentum [FORMULA OMISSION] and rapidity in the nucleon-nucleon centre-of-mass system [FORMULA OMISSION] (pp collisions) and [FORMULA OMISSION] (p–Pb collisions) were correlated to charged particles with [FORMULA OMISSION]. The yield of charged particles in the correlation peak induced by the jet containing the D meson and the peak width are compatible within uncertainties in the two collision systems. The data are described within uncertainties by Monte-Carlo simulations based on PYTHIA, POWHEG, and EPOS 3 event generators.</P>
