        우리나라 지역 직업성질환 감시체계의 현황과 전망

        기존의 특수건강진단과 작업환경측정을 통한 직업병 관리가 진폐증, 소음성난청 등의 소수 특정질환에 국한되고 실제 직업병 발생 규모 파악이나 신종 직업병의 발견에 한계를 보인다는 사실은 산업의학전문가들 사이에서도 공감을 이루고 있다. 미국과 영국 등에서의 직업성질환 감시체계에 대한 경험은 우리 나라의 직업성질환 감시체계 구축에도 새로운 자극제가 되면서, 1998년이후 인천, 대전, 여천, 구미, 부울경 지역에서 지역 직업성질환감시체계를 산업보건관리의 중요한 시스템으로 구축하려는 노력이 확산되고 있다. 새로이 구축되어지는 이들 지역 직업성질환 감시체계는 감시하고자 하는 대상질환, 활용 가능한 인적자원 및 자료원, 지역 의료체계의 특수성 등에 따라서 목적과 방법을 달리하면서 독특한 형태로 발전을 하고 있다. 각 지역단위 감시체계들이 그 상황에 맞게 독특한 목적과 전략들을 발전 시키면서도, 향후 발전할 국가적인 차원의 직업성질환 감시체계 구축을 위하여 직업성질환 감시의 기본 전략 등을 공유하는 등의 노력이 필요하다. 환례 정의 및 기본적인 등록 서식의 공유, 직업성질환 감시 자료원의 발굴, 공동의 정보 네트워크 및 직업성질환 감시 데이터베이스 구축 등 직업성질환 감시활동을 지원하기 위한 여러 기초 인프라 구축에 힘을 모아야 할 것이다. 우리 나라에서 직업성질환 감시체계를 성공적으로 구축하기 위해서는 수집된 자료의 질 관리를 위한 직업성질환 감시의 원칙 제정과 감시 전략의 공유 등이 필요하며, 전국적인 직업성질환 감시체계의 하부구조라고 할 수 있는 지역감시체계의 기초 토대 마련과 강화작업이 절실하게 필요하다.

        Estrogen Attenuates Cardiac Ischemia-Reperfusion Injury via Inhibition of Calpain-Mediated Bid Cleavage

        Although several studies have shown that the administration of $17{\beta}$-estradiol (estrogen) is cardioprotective to ischemia-reperfusion (I/R), the molecular mechanisms are largely unknown. Therefore, we investigated the effects of estrogen on myocardial I/R injury in rat that were sham operated (Sham), ovariectomized (OVX), or ovariectomized and then given estrogen supplementation (OE). Langendorff-perfused rat hearts were subjected to I/R stimuli and the effects of estrogen were examined on cardiac performance. Additionally, we examined the mechanism of estrogen-mediated inhibition of apoptosis. Depression in cardiac contractile function and an increment of calpain activity were observed during I/R in the OVX rats. Estrogen replacement recovered cardiac contractile function and attenuated calpain activity, Bid cleavage, and caspases activities. Through in vitro assay using cardiomyocytes, we demonstrated that addition of $H_2O_2\;(100{\mu}M)$ significantly increased calpain activity, which was attenuated by estrogen. Moreover, calpain activity was inhibited by calpain inhibitors such as ALLN or leupeptin, but not by caspase-8 inhibitor peptide. These results suggest that estrogen protects the heart against I/R injury through the decrease of calpain activity, Bid cleavage and caspase-8 activity. These apoptotic mechanisms may playa critical role on I/R-associated cardiac damage.

        Signal Transduction of the Protective Effect of Insulin Like Growth Factor-1 on Adriamycin-Induced Apoptosis in Cardiac Muscle Cells

        To determine whether Insulin-like growth factor (IGF-I) treatment represents a potential means of enhancing the survival of cardiac muscle cells from adriamycin (ADR)-induced cell death, the present study examined the ability of IGF-I to prevent cell death. The study was performed utilising the embryonic, rat, cardiac muscle cell line, H9C2. Incubating cardiac muscle cells in the presence of adriamycin increased cell death, as determined by MTT assay and annexin V-positive cell number. The addition of 100 ng/mL IGF-I, in the presence of adriamycin, decreased apoptosis. The effect of IGF-I on phosphorylation of PI, a substrate of phosphatidylinositol 3-kinase (PI 3-kinase) or protein kinase B (AKT), was also examined in H9C2 cardiac muscle cells. IGF-I increased the phosphorylation of ERK 1 and 2 and $PKC{\;}{\zeta}{\;}kinase$. The use of inhibitors of PI 3-kinase (LY 294002), in the cell death assay, demonstrated partial abrogation of the protective effect of IGF-I. The MEK1 inhibitor-PD098059 and the PKC inhibitor-chelerythrine exhibited no effect on IGF-1-induced cell protection. In the regulatory subunit of PI3K-p85- dominant, negative plasmid-transfected cells, the IGF-1-induced protective effect was reversed. This data demonstrates that IGF-I protects cardiac muscle cells from ADR-induced cell death. Although IGF-I activates several signaling pathways that contribute to its protective effect in other cell types, only activation of PI 3-kinase contributes to this effect in H9C2 cardiac muscle cells.

        Multiple non-contiguous spinal fracture is a special type of multi-level spinal injury, which is rare but most frequently occur in motor vehicle accident or a falling from a height. We report five patients of multiple non-contiguous spinal fractures. All patients underwent segmental pedicle screws fixation without fusion for preserving facet joints and minimizing blood loss and operation time. We performed necessary operation for any concomitant injuries at the same day.

        천골 부전골절에서 풍선 성형술의 유용성 -증례 보고-

        최근 고령화로 발생이 증가하는 골다공증성 천추 부전골절에 대해 척추 성형술의 한 방법으로 풍선 성형술을 이용하여 치료가 시도되고 있다. 85세 여자환자로 특별한 외상소견 없이 발생한 천추 1번 골다공증성 부전골절에 대해 보존적 치료에 호전이 없어 천추풍선성형술을 시행하여 좋은 결과을 보여 치료의 한 방법으로서 유용하기에 문헌 고찰과 함께 보고하는 바이다. Kyphoplasty has recently attended as a potential treatment for sacral insufficiency fracture. We report a 85-years-old female patient with osteoporotic S1 insufficiency fracture with absence of trauma history treated with kyphoplasty which has no symptom improve with conservative treatment. Kyphoplasty is an effective and useful procedure in the treatment of the sacral insufficiency fracture, additionally reviewed of the literatures.

        영강의 어류상과 군집구조

        2013년 3월부터 10월까지 낙동강수계의 1차 지류인 영강의 37개 조사지점에 대하여 어류상을 조사한 결과 총 13과 31속 39종 2형의 어류가 확인되었다. 채집된 어류 중 잉어과 어류가 23종 2형(61.0%)으로 가장 많았으며 다음은 미꾸리과 어류가 4종(9.87%)이었다. 한반도 고유종은 17종으로 채집어종수의 41.5%를 차지하였다. 멸종위기종은 얼룩 새코미꾸리 (Koreocobitis naktongensis)와 모래주사 (Microphysogobio koreensis)의 두 종이었으며, 외래종은 무지개송어(Oncorhynchus mykiss)와 배스(Micropterus salmoides)의 두 종이었다. 국내의 다른 하천에서 옮겨진 이입종은 끄리(Opsariichthys uncirostris amurensis), 눈동자개 (Pseudobagrus koreanus), 빙어 (Hypomesus nipponensis)의 3종이었다. 개체수를 기준으로 한 우점종은 참갈겨니 NS형(Z. koreanus NS, 23.24%), 아우점종은 참갈겨니 NE형 (Z. koreanus NE, 12.72%)와 피라미 R형(Z. platypus R, 12.35%)이었다. 어류 군집분석을 실시한 결과 영강 전체는 다양도 1.134, 균등도 0.703, 우점도 0.110, 종풍부도 4.348로 나타나 다양하고 안정된 어류군집을 이루고 있었다. 조사지점 사이의 유사도지수를 이용하여 수지도를 작성한 결과 상류집단, 중상류집단, 중류집단, 하류집단의 4집단으로 구분할 수 있었다. Freshwater fish fauna and community structure were investigated at thirty seven stations in the Yeong river, Nakdong river system, Korea from March to October 2013. During the surveyed period thirty nine species and two types belonging to thirteen families were collected. In this collection fishes of Cyprinidae were most numerous possessing 61.0% of all. There were seventeen Korean endemic species including Acheilognathus koreensis, two endangered species such as Koreocobitis naktongensis and Microphysogobio koreensis and two exotic species such as Oncorhynchus mykiss and Micropterus salmoides. Fish species transferred from other native rivers were three species such as Opsariichthys uncirostris amurensis, Pseudobagrus koreanus and Hypomesus nipponensis. Dominant species based on the number of individuals was Zacco koreanus NS (23.24%) and sub-dominant species were Z. koreanus NE (12.72%) and Z. platypus R (12.35%). The community analysis revealed that the structure of fish community in the Yeong river was very stable and diverse in having diversity 1.134, evenness 0.703, dominance 0.110 and species richness 4.348. Fish community was divided into four groups based on similarity indices among surveyed stations such as upper reach, mid-upper reach, middle reach and lower reach group.

        낙동강 상류의 어류상과 군집 구조

        2014년 3월부터 10월까지 낙동강 상류에서 44개 조사 지점에 대하여 어류상을 조사한 결과, 총 14과 34속 42종 4형의 어류가 확인되었다. 채집된 어류 중 잉어과 어류가 17종 4 형 (43.2%)으로 가장 많았으며, 다음은 미꾸리과 어류가 6종(13.6%)이었다. 한반도 고유종은 Kichulchoia multifasciata 등의 17종이었다. 멸종위기종은 Koreocobitis naktongensis, Lethenteron reissneri 및 Brachymystax lenok tsinlingensis의 3 종이었으며, 외래종은 Lepomis macrochirus와 Micropterus salmoides의 2종이었다. 국내의 다른 하천에서 옮겨진 이입종은 Hemibarbus mylodon, Coreoleuciscus slpendidus Han river type, Opsariichthys uncirostris amurensis, Iksookimia koreensis, Liobagrus andersoni 등의 9종이었다. 개체수를 기준으로 한 우점종은 Rhynchocypris oxycephalus (21.44%), 아우점종은 Z. koreanus NE형 (18.55%)이었다. 어류 군집 분석을실시한 결과 영강 전체는 다양도 1.101, 균등도 0.670, 우점도0.400, 종풍부도 4.454로 나타나 다양하고 안정된 어류 군집을이루고 있었다. 조사 지점 사이의 유사도지수를 이용하여 수지도를 작성한 결과 최상류, 상류1, 상류2, 중상류 및 중류 집단의 5집단으로 구분할 수 있었다.       • Traditional oriental herbal medicine, Jukyeoondam-tang, occludes aconitine-induced ventricular arrhythmia in hearts

        We showed the effects of the traditional herbal medicine, Jukyeoondam-tang (JO-T, Zhu-ru-Wen-Dan-Tang in Chinese), on ventricular arrhythmia induced by aconitine. Electrophysiological experiments with conventional microelectrode techniques revealed that JO-T potently suppressed the aconitine-induced arrhythmias in ventricular strips of the rat. In the aconitine-induced arrhythmia model of the rat, pretreatment with JO-T $(100\;{\mu}g/ml)$ completely occluded the appearance of ventricular tachyarrhythmia (VT) or ventricular fibrillation (VF) induced by aconitine. Furthermore, the aconitine-induced ventricular arrhythmia was occluded by $Na^+$ channel blocker quinidine but was not occluded by $K^+$ channel blocker glibenclamide $(3\;{\mu}mol/L)\;and\;Ca^{2+}$ channel blocker nifedipine $(10\;{\mu}mol/L)$. We also confirmed the effect of JO-T in the ischemia-reperfusion (I/R)-induced arrhythmia model of the rat. JO-T did not affect the I/R-induced arrhythmias in rats. JO-T may alleviate the risk of ventricular arrhythmias following aconitine. These results suggest that JO-T is a potent antiarrhythmic drug having a$Na^+$ channel-blocking action.

        Test of Larvicidal Effect of Some Commercial Natural Products on Lepidoptran Plutella xylostella and Spodoptera Litura Larvae

        A diverse kind of environment-friendly agricultural materials (EFAM) for the control of insect pests is on the market. These EFAMs are a part of essential sources for the accomplishment of successful, sustainable, and environment-friendly agriculture. Thus, accurate information of these EFAMs is one that required for the success of environment-friendly agriculture, but, in reality, still appropriate information is absolutely in shortage. In this study, we, therefore, tested the efficacy of commercial EFAMs against two lepidopteran insect larvae, the diamondback moth Plutella xylostella (Lepidoptera: plutellidae) and the tobacco cutworm Spodoptera litura (Lepidoptera: Noctuidae). After the two insect pests were successfully stabilized in indoor environment the larvicidal activity was tested at 24±1°C, relative humidity (RH) of 60±5%, and a photoperiod of 16L : 8D, and mortality was determined 48 hrs after EFAMs are treated. The EFAMs that showed more than 90% of larvicidal activity were each six among 16 against both R xylostella and S. litura and only three of them showed consistent larvicidal activity against both species, signifying species specificity of EFAMs and importance of selection of proper EFAMs depending on target insect pest.
