Background/Aims: While surgical resection remains the standard of care in the treatment of upper urinary tract malignancies, nephrectomy is a risk factor for the development of chronic kidney disease (CKD). The aim of this study was to determine whether histologic evaluation of non-neoplastic kidney could enable early identification of unrecognized kidney disease and could be of prognostic value in predicting postoperative renal outcomes. Methods: We retrospectively analyzed 51 patients with upper urinary tract malignancies who received uninephrectomy or uninephroureterectomy. A thorough pathologic evaluation of non-neoplastic kidney including special stains, immunofluorescence, and electron microscopic studies was performed. The degree of parenchymal changes was graded from 0 to 15. Results: Of 51 patients, only 13 showed normal kidney pathology. Fifteen patients showed glomerular abnormalities, 14 showed diabetic nephropathy, and 11 showed vascular nephropathy. There was one case each of reflux nephropathy and chronic pyelonephritis. The median histologic score was 5 points. Only 25.4% of patients had ≤ 3 points. Score more than 5 was observed in 47.1% of patients. Postoperative estimated glomerular filtration rate (eGFR) at 3 to 36 months were obtained from 90.2% of patients, and of those, 34.8% had de novo CKD. Since no one had CKD in partial nephrectomized patients, we determined risk factors for CKD in radical nephrectomized patients. Cox regression analysis revealed that postoperative AKI, preoperative eGFR, and histologic score of non-neoplastic kidney were the independent predictors for CKD. Conclusions: We conclude that routine pathologic evaluation of non-neoplastic kidney provides valuable diagnostic and prognostic information.
Nerve cuff electrode using embedded magnets and its application to hypoglossal nerve stimulation

<P> <I>Objective</I>. A novel nerve cuff electrode with embedded magnets was fabricated and developed. In this study, a pair of magnets was fully embedded and encapsulated in a liquid crystal polymer (LCP) substrate to utilize magnetic force in order to replace the conventional installing techniques of cuff electrodes. <I>In vitro</I> and <I>in vivo</I> experiments were conducted to evaluate the feasibility of the magnet-embedded nerve cuff electrode (MENCE). Lastly, several issues pertaining to the MENCE such as the cuff-to-nerve diameter ratio, the force of the magnets, and possible concerns were discussed in the discussion section. <I>Approach</I>. Electrochemical impedance spectrum and cyclic voltammetry assessments were conducted to measure the impedance and charge storage capacity of the cathodal phase (CSC<SUB>c</SUB>). The MENCE was installed onto the hypoglossal nerve (HN) of a rabbit and the movement of the genioglossus was recorded through C-arm fluoroscopy while the HN was stimulated by a pulsed current. <I>Main results</I>. The measured impedance was 0.638?∠?−67.8° kΩ at 1 kHz and 5.27?∠?−82.1° kΩ at 100 Hz. The average values of access resistance and cut-off frequency were 0.145 kΩ and 3.98 kHz, respectively. The CSC<SUB>c</SUB> of the electrode was measured as 1.69 mC cm<SUP>−2</SUP> at the scan rate of 1 mV s<SUP>−1</SUP>. The movement of the genioglossus contraction was observed under a pulsed current with an amplitude level of 0.106 mA, a rate of 0.635 kHz, and a duration of 0.375 ms applied through the MENCE. <I>Significance.</I> A few methods to close and secure cuff electrodes have been researched, but they are associated with several drawbacks. To overcome these, we used magnetic force as a closing method of the cuff electrode. The MENCE can be precisely installed on a target nerve without any surgical techniques such as suturing or molding. Furthermore, it is convenient to remove the installed MENCE because it requires little force to detach one magnet from the other, enabling repeatable installation and removal. We anticipate that the MENCE will become a very useful tool given its unique properties as a cuff electrode for neural engineering.</P>

Fashion store patronage behavior can be observed when consumer's desires and expectations are fulfilled. Thus, in order to achieve successful business in sales, a retailer must continually analyze why target consumers choose particular stores and patronize them. This study identified the relevant factors of the patronage behavior of female college students residing in Seoul. Thus, this study can achieve Its goal of enabling retailers to predict shopping orientations and emphasis on store attributes, so that help retailer to establish new positions of their shops to differentiate them from others and, also, to reposition existing stores. The aims of this study were as follows; First, identify the classification of stores that female college students in Seoul patronize. Second, classify the female students' shopping orientations, emphasizing store attributes, and identify relationships between them and fashion store patronage behavior. Relationships between the factors of shopping orientations and store attributes, and patronage behavior were identified by use of Tukey's test, Duncan test, and ANOVA. As a result, it showed significant differences among patronage groups according to nearly every factor of shopping orientations and store attributes. As based on the results, this study is expected to assist that retailers can establish effective marketing strategies in order to make continuously consumers' partronage behavior of a store.

Clinical features of snoring patients during sedative endoscopy

Background/Aims: Snoring is the sound of turbulence and vibration of the upper respiratory tissues and has been identified as a risk factor of obstructive sleep apnea (OSA) and cardiovascular disease. The aim of this study was to identify associated clinical factors in snoring patients undergoing sedative endoscopy. Methods: A total of 49 patients who snored during standard sedative endoscopy and 127 controls were prospectively enrolled from June 2015 to June 2016. The Korean version of the Berlin Questionnaire was used to identify risk factors of OSA. Clinical information, including comorbidities, was collected from electronic medical records. Results: The snoring group showed a higher risk of OSA (42.9% vs. 26.8%, p = 0.039), and a higher prevalence of coronary artery disease (10.2% vs. 0.8%, p = 0.007) and advanced gastric cancer (12.2% vs. 2.4%, p = 0.015) compared with the control group. Multivariate analysis showed that coronary artery disease (odds ratio [OR], 13.93; 95% confidence interval [CI], 1.24 to 155.90; p = 0.033) and advanced gastric cancer (OR, 5.21; 95% CI, 1.01 to 26.98; p = 0.049) were significantly associated with snoring. However, a history of gastrectomy showed only a marginally significant association with snoring (OR, 2.16; 95% CI, 0.91 to 5.11; p = 0.079). Conclusions: Patients who snore during sedative endoscopy may need to be evaluated for possible coronary artery disease.

Purpose: The normal values for lung resistance and lung capacity of children, as determined by impulse oscillometry (IOS), are different for children of different ethnicities. However, reference values there is no available reference value for Korean preschool children have yet to be determined. The aim of the present study was to determine the normal ranges of IOS parameters in Korean preschool children. Methods: A total of 133 healthy Korean preschool children were selected from 639 children (aged 3 to 6 years) who attended kindergarten in Seongnam, Gyeonggi province, Korea. Healthy children were defined according to the European Respiratory Society (ERS) criteria. All subjects underwent lung function tests using IOS. The relationships between IOS value (respiratory resistance (Rrs) and reactance (Xrs) at 5 and 10 Hz and resonance frequency (RF)) and age, height, and weight were analyzed by simple linear and multiple linear regression analyses. Results: The IOS success rate was 89.5%, yielding data on 119 children. Linear regression identified height as the best predictor of Rrs and Xrs. Using stepwise multiple linear regressions based on age, height, and weight, we determined regression equations and coefficients of determination ($R^2$) for boys ($Rrs_5=1.934-0.009{\times}Height$, $R^2$=12.1%; $Xrs_5=0.774+0.006{\times}Height-0.002{\times}Age$, $R^2$=20.2% and for girls $(Rrs_5=2.201-0.012{\times}Height$, $R^2$=18.2%; $Xrs_5=-0.674+0.004{\times}Height$, $R^2$=10.5%). Conclusion: This study provides reference values for IOS measurements of normal Korean preschool children. These provide a basis for the diagnosis and monitoring of preschool children with a variety of respiratory diseases.

<P>The Beijing family of <I>Mycobacterium tuberculosis</I> has been emerging in the world. However, there are few nationwide data of genotypic distribution in Korea. This study aimed to identify the genotypic diversity of clinical isolates of <I>M. tuberculosis</I> and to demonstrate the population of Beijing family in Korea. We collected 96 clinical <I>M. tuberculosis</I> isolates from 11 university hospitals nationwide in Korea from 2008 to 2009. We observed 24 clusters in IS<I>6110</I>-RFLP analysis and 19 patterns in spoligotyping. Seventy-five isolates were confirmed to be Beijing family. Two isolates of the K strain and 12 isolates of the K family strain were also found. We found that drug resistance phenotypes were more strongly associated with Beijing family than non-Beijing family (<I>P</I>=0.003). This study gives an overview of the distribution of genotypes of <I>M. tuberculosis</I> in Korea. These findings indicate that we have to pay more attention to control of <I>M. tuberculosis</I> strains associated with the Beijing family.</P>
