        Empyema Necessitatis in a Patient on Peritoneal Dialysis

        Empyema necessitatis is a rare complication of an empyema. Although the incidence is thought to be decreased in the post-antibiotic era, immunocompromised patients such as patients with chronic kidney disease on dialysis are still at a higher risk. A 56-year-old woman on peritoneal dialysis presented with an enlarging mass on the right anterior chest wall. The chest computed tomography scan revealed an empyema necessitatis and the histopathologic findings revealed a granulomatous inflammation with caseation necrosis. The patient was treated with anti-tuberculous medication.

        Background: It is well known that hepatorenal syndrome (HRS) is associated with negative outcomes among patients with spontaneous bacterial peritonitis (SBP). However, it is unclear unclear whether a milder degree of kidney injury as acute kidney injury (AKI), defined as a rise in serum creatinine of ≥ 50% from baseline, or a rise of serum creatinine by 0.3 mg/dL in <48 h has associations with negative outcomes. This study aimed to evaluate the outcomes among SBP patients with and without AKI. Methods: We conducted a retrospective study of cirrhotic patients diagnosed with SBP between 2006 and 2012. Serum creatinine levels at the diagnosis of SBP were compared with baseline levels. Patients` records, as survival or death, were reviewed until March, 2013. Results: Data of 59 cases with SBP were reviewed. Thirty nine cases were male with a mean age of 56.7±9.4 years. There were 25 cases with AKI including 18 HRS and 34 cases without AKI. During follow-up period (mean 209.8±283.1 days), 19 deaths (72%) in the AKI group and 14 (41%) in the non-AKI group were noted. Survival was significantly lower in the AKI group than in non-AKI group (mean 146.9±39.1 vs. 422.3±71.0 days, P<0.001) by log rank test. The results were comparable to those of between patients with and without HRS (mean 138.7±48.7 vs. 376.3±61.9 days, P=0.001). The Cox regression survival ana lys is indi c ated that AKI , Chi ld-Pugh Cl ass C, thrombocytopenia(<150k) as independently negative prognostic indicators of survival. On multivariate analysis, only AKI remained statistically significant (P=0.004, Exp(B)=2.970). Conclusions: Our data indicates that episodes of AKI combined with SBP are associated with negative outcomes among cirrhotic patients. Attentions are necessary to patients who have a rise in serum creatinine elevation with SBP, even though it was a minor degree within normal range.

        PC(Precast Concrete)생산품질 확보를 통한 안전관리 개선에 관한 연구

        2021년 11월16일 경기도 화성시 소재의 물류창고 신축공사현장 옥상층에서 PC(Precast Concrete) 슬래브 토핑콘크리트 타설 작업 중 중앙부 하부 인장강선에 비해 상부 압축콘크리트의 약한 강도가 상부 토핑콘크리트 타설의 높은 응력을 버티지 못하고 콘크리트의 휨파괴사고로 발생한 PC슬래브가 약 10M 아래층 슬래브로 탈락 및 낙하사고가 발생하였다. 원인 조사를 위해 PC슬래브 토핑콘크리트 타설 작업 중 탈락한 PC슬래브의 설계 시 압축강도를 코어채취법을 활용하여 확인해 본 결과 해당 부재 단부에서 채취한 콘크리트 공시체 3개의 파괴검사를 실시해 파악한 압축강도는 21.7MPa (NO.1-20.6MPa, NO.2-18.1MPa, NO.2-2-26.5MPa)로 설계압축강도인 40MPa의 약 55% 정도 수준의 강도 발현만 되고 있는 것으로 나타났다. 압축강도가 설계압축강도인 40MPa의 약 55% 정도 수준으로 발현된 주요 원인은 PC생산 과정 중 진행되는 스팀양생의 기준을 지키지 못한 것으로 스팀양생의 기준은 양생온도 60±10℃(최고 80℃이내), 온도상승 속도는 20℃/h 이하로 하고, 온도곡선(Concrete Maturity)으로 양생온도관리를 하도록 되어있으나 PC생산 공장 확인 결과 노천에서 저온의 스팀양생으로 몰드 탈형 전 기준강도에 도달하지 못한 것으로 보고되었다. 코로나-19 팬더믹 이후 물류창고의 필요성이 지속되고 있으며 특히 물류창고 공사에 강점이 있는 PC를 이용한 골조공사의 수요가 폭증하고 있어 PC제작사의 공장가동률이 생산한계에 도달하자 압축강도 확보 등 시공품질 및 안전에 문제가 되고 있는 영세한 사업장에서 PC를 무분별하고 생산하고 있으나 압축강도 등 생산품질 미확보에 대한 관련법규가 규정되지 않아 물류창고 신축공사 현장 등에서는 낙하 및 추락사고의 위험에 노출되어 있다. 본 연구는 PC공장 인증제도가 도입되어야 한다는 연구목적으로 PC낙하사고 및 국가별 인증제도의 국내외 운영 사례를 배경으로 연구방법을 선정 하였으며 제품 상세는 다루지 못한 한계점이 있어 계속되는 연구가 이루어져야 할 것이다. On 16th of November 2021, the PC slab had been fallen out and dropped to approximately 10M downstairs slab during the PC slab concrete pouring work on the rooftop floor of new logistics warehouse construction in Hwaseoung, Gyeonggi-do, and this accident was mainly caused by the bending fracture of concrete since the weaker strength of the upper compressed concrete compared to the tensile strength of central lower parts failed to withstand the high stress of upper topping concrete pouring. For the investigation of root-cause for above mentioned accident, the design compressive strength had been tested by utilizing the core sampling(extracting) method for the PC slab which was fallen out while pouring topping concrete, and as a result, the average compressive strength of three(3) concrete specimens collected from the relevant member through the destructive test was observed as 21.7MPa (No.1 – 20.6MPa, No.2 – 18.1MPa, No.3 – 26.5 MPa) which is only 55% level of the design compressive strength of 40MPa. The main cause that the average compressive strength was observed at about 55% level of the design compressive strength of 40MPa is the failure to comply with the required standard/specification for steam curing during the PC production processing. The required conditions to be complied for the steam curing are 60±10℃(Max. 80℃ or less) of curing temperature, 20℃/h or less of temperature rise rate and shall be managed by the concrete maturity, however, it was reported as a result of investigating the PC manufacturer that the strength had not reached to the standard strength before the mold deformation due to the steam curing with low temperature condition at opened space. The logistics warehouses have been continuously requested to be expanded after COVID-19 Pandemic, and the demand of PC which has merit in logistics warehouse construction work has been rapidly increasing. As a result, the operation rate of PC manufacturers currently reaches the maximum production limit, instead, PCs have been indiscriminately produced at small workplaces which have not able to satisfy the design compressive strength, construction quality and safety requirements. In the current situation, the construction site and workers are exposed to the potential risk of falling accidents since any relevant law or regulation on non-securing quality requirements such as the compressive strength is not stipulated yet. For this reason, the PC products with secured quality requirements shall delivered and construction safety management shall be improved by implementation of PC Factory Certification System at the earliest. For the purpose of this study, the PC factory certification system should be introduced, the research method was selected based on PC drop accidents and domestic and international operational cases of each country's certification system.

        복강 내 발생한 Immunoglobulin G4 연관 염증성 가성종양

        The term inflammatory pseudotumor (IPT) has been used to describe inflammatory and fibrosing tumoral processes of an undetermined cause that may involve a variety of organ system. IgG4-related disease is a newly recognized fibroinflammatory condition characterized by IgG4-producing plasma cell expansion in affected organs and, often but not always, elevated serum IgG4 concentrations. IgG4-related IPTs, a subtype of IPT, are characterized by dense infiltration of IgG4-positive plasma cells and stromal fibrosis. The association between inflammatory pseudotumor and IgG4 was first reported with a regard to sclerosing pancreatitis. Despite there are many reports on intraperitoneal IPTs including both cellular and lymphoplasmacytic type, only a few cases have been confirmed to be IgG4-related. We experienced a case of intraperitoneal IgG4-related inflammatory pseudotumor in an 83-year-old woman presenting with epigastric pain and malaise. Surgical specimens revealed an IgG4-related inflammatory pseudotumor.

        A 59-year-old woman was admitted to our hospital with polydipsia and general weakness. She had a 30-year history of bipolar disorder and was being treated with risperidone (4 mg/day) and lithium carbonate (1,200 mg/day). During her time in hospital, her urine output and serum osmolality increased, and her urine osmolality decreased. She was found to have myoglobulinuria, an elevated creatine kinase level, and abnormal renal function. Based on these findings, the patient was diagnosed with diabetes insipidus and rhabdomyolysis secondary to lithium therapy. After fluid therapy and the withdrawal of lithium, her clinical symptoms improved significantly. Her urine volume decreased gradually after treatment with amiloride. The effects of lithium on the muscle system are unknown. Hyperosmolarity caused by lithium-induced diabetes insipidus is considered a contributing factor in rhabdomyolysis.         A numerical methodology, which can analyze the two-dimensional phase change process of a metal with natural convection effect in the liquid, is developed and applied to the solidification process of pure aluminum in a confined rectangular mold. Stable numerical solutions are obtained for the solidification processes of a pure aluminum in a confined rectangular mold, which proves the applicability of the present numerical analysis for the phase change processes of a metal.
