        Ventricular septal defect remains an infrequent but devastating complication of acute MI. Approximately 1∼3% of cases of acute myocardial infarction are complicated by ventricular septal rupture in the prethrombolytic era and the incidence has decreased to 0.2% with reperfusion therapy. This mechanical complication usually occurs within the first 10 to 14 days when necrotic tissue is most abundant and the collateral coronary circulation is not well developed. This lesion is generally associated with complete coronary obstruction rather than severe stenosis. Rupture of the ventricular septum is a severe mechanical complication of acute myocardial infarction, usually resulting in death unless surgical repair is performed. The bad prognosis of this event within the first 2 weeks indicates the need for early surgical rapair. This complication is more frequent after the first acute myocardial infarction in the elderly and secondary to a transmural myocardial infarction. We report two cases of ventricular septal rupture after acute myocardial infarction.

        영·유아교육기관 교사의 직업적 어려움에 관한 연구

        The purpose of this study is to analyze the professional problems perceived by the public & private kindergarten teachers, day care center teachers. The subjects of this study were 188 the public & private kindergarten teachers, day care center teachers. The results of this study were as follows : 1) For the frequency of teacher's perceived the professional problems according to the child education institution type, the nine global areas of factors was ranked the highest in which teachers of the day care center. 2) For the frequency of teacher's perceived the professional problems according to the age background of the child education institution teachers, the nine global areas of factors ranked the highest from twenty age to the middle of thirty age. 3) For the frequency of teacher's perceived the professional problems according to the academic background of the child education institution teachers, the global areas of factors showed a significant statistical difference in which teachers of graduated from a college than a university. 4) For the frequency of teacher's perceived the professional problems according to the career background with child education institution teachers, the global areas of factors showed a significant statistical difference.

        알쯔하이머병 환자의 혈소판 아밀로이드 전구단백질 이상

        Background:Although Alzheimer's disease (AD) is a progressive neurodegenerative disorder without any truly effective pharmacological treatment at present, early accurate diagnosis is important for the delaying of disease progression and proper management It would be desirable to have a peripheral biological marker to identify patients affected by AD Amyloid β-peptide originates from a larger precursor, the amyloid precursor protein (APP). which can be seen in the platelet of its equivalent to those found in brain. To investigate if there is a correlation between level of platelet APP isoform and AD, we evaluate a cohort of subjects including patients affected by sporadic AD, vascular dementia, and controls Subjects & Methods :Thirty-five patients with sporadic AD diagnosed according to the National Institute of Neurological and Communicative Disorders and Stroke-Alzheimer's Disease and Relatec Disorders criteria (NINCDS-ADRDA) and 26 vascular dementia patients diagnosed by the Diagnosis and Statistical Manual of Mental Disorders, Fouth Edition (DSM-Ⅳ) criteria and 12 controls were included Blood sample were drawn from fasting state and collected into 3 mL of 3.8% sodium citrate. Platelets were collected by centrifugation at 500 g for 20 minutes and washed. and the platelet pellet was stored at -80℃ until used APP was assessed by western blot analysis and quantified by densitometry using Image Analyzer Results:In platelets densitometer as well as the result of Western blot was higher compared with those of patients with non-Alzheimer dementia and controls We observed statistically significant reductions in the ratio of 120 kDa to 130 kDa APP to 110 kDa APP (APPr) for patients with probable AD compared with control subjects and patients with vascular dementia APPr levels in AD correlated with the severity of dementia measured by CDR. Accuracy levels measured by Receiver Operating Curve analysis showed that a cut-off level of 0.45 resulted in a sensitivity of 71% and a specificity of 74%, with an area under the curve of 0 793 Conclusion:Platelet APPr allowed to differentiate AD from normal aging and vascular dementia with high sensitivity and specificity These findings suggest that platelet APPr may be a helpful peripheral marker for diagnosis and clinical progression of Alzheimer disease.

        미취학 아동에 있어 신체발달, 손목 골밀도 및 영양섭취와의 상관성

        This study was conducted to examine the correlation among growth development, bone mineral density, and nutrient intakes in preschool children. A total of 62 preschool children measured the anthropometric characteristics and the bone mineral density of carpus using DEXA. The questionnaires categorized to determine health status, life style, dietary behaviors, and dietary intakes were filled out by children's mothers. The average ages of the study subjects were 62.4 months in boys and 62.1 months in girls. The birth height and weight of the subjects were 50.9 ㎝ and 3.4 ㎏ in boys and 50.3 ㎝ and 3.3 ㎏ in girls, respectively. The average height, weight, % body fat, and obesity index were 111.7 ㎝, 19.6 kg, 15.0%, -2.1% in boys and 109.4 ㎝, 18.7 ㎏, 17.5%, 0.2% in girls, respectively. The bone mineral density in carpus was 0.25 g/㎠ in boys and 0.24 g/㎠ in girls. Proportions of children having regular meals were 69.0%, 94.7% and 69.0% for breakfast, lunch, and dinner, respectively. The major reasons for irregular meal were poor appetite and lack of time for breakfast and snacks for dinner. Most of the children(52.6%) had breakfast with the whole family together, and all the children had lunch and dinner with a part of the family. Most of the children answered they had snack more than once a day, and 64.3% had an unbalanced diet. The intakes of energy, calcium, iron, zinc, vitamin A, vitamin B₁, niacin, and vitamin C did not meet the Korean RDAs. The intakes of energy, calcium, iron, zinc, vitamin A, vitamin B₁, niacin, and vitamin C did not meet the Korean RDAs. Average bone mineral density in carpus was positively correlated to the body weight, the total body water, the lean body mass, and obesity index, respectively(p<0.01, p<0.01, p<0.01 and p<0.01). The body weight, the total body water, and the lean body mass showed the significant negative correlation with the animal iron intake(p<0.05, p<0.01 and p<0.01). The bone mineral density the distal carpus was negatively correlated to the animal protein intake and the animal iron intake(p<0.05 and p<0.05). And the bone mineral density in the ultradis carpus and the average bone mineral density in the carpus were positively correlated to the plant calcium intake(p<0.05 and p<0.05). Therefore, more systematic studies to investigate the roles of protein, iron, and calcium in growth development of preschool children were required.

      • <i>Prunus serrulata</i> var. <i>spontanea</i> inhibits mast cell activation and mast cell-mediated anaphylaxis

        <P><B>Ethnopharmacological relevance</B></P> <P>A promising approach to treat a variety of diseases are considered as complementary and alternative herbal medicines. <I>Prunus serrulata</I> var. <I>spontanea</I> L. (Rosaceae) is used as herbal medicine to treat allergic diseases according to the Donguibogam, a tradition medical book of the Joseon Dynasty in Korea.</P> <P><B>Aim of the study</B></P> <P>We prepared the aqueous extract of the bark of <I>P. serrulata</I> (AEBPS) and aimed to investigate the effects in mouse anaphylaxis models and various types of mast cells, including RBL-2H3, primary cultured peritoneal and bone marrow-derived mast cells.</P> <P><B>Materials and methods</B></P> <P>We used ovalbumin (OVA)-induced active systemic anaphylaxis (ASA) and immunoglobulin (Ig) E-mediated passive cutaneous anaphylaxis (PCA) models, <I>in vivo</I>. The control drug dexamethasone (10 mg/kg) was used to compare the effectiveness of AEBPS (1–100 mg/kg). <I>In vitro</I>, IgE-stimulated mast cells were used to confirm the role of AEBPS (1–100 μg/mL). For statistical analyses, <I>p</I> values less than 0.05 were considered to be significant.</P> <P><B>Results</B></P> <P>In ASA model, oral administration of AEBPS suppressed the hypothermia and increased level of serum histamine in a dose-dependent manner. AEBPS attenuated the serum IgE, OVA-specific IgE, and interleukin (IL)-4. Oral administration of AEBPS also blocked mast cell-dependent PCA. AEBPS suppressed degranulation of mast cells by reducing intracellular calcium level in mast cells. AEBPS inhibited tumor necrosis factor-α and IL-4 expression and secretion in a concentration-dependent manner through the reduction of nuclear factor-κB.</P> <P><B>Conclusions</B></P> <P>On the basis of these findings, AEBPS could serve as a potential therapeutic target for the management of mast cell-mediated allergic inflammation and as a regulator of mast cell activation.</P> <P><B>Graphical abstract</B></P> <P>[DISPLAY OMISSION]</P>

        Effects of Panax ginseng on Morphine-induced Immune Suppression

        To investigate the possibility of Panax ginseng as a therapeutic agent for the immune suppression, ginseng total saponin (GTS) extracted from korean red ginseng was tested on immune functions from morphine-induced immune suppressed mice. To study how immune functions are affected by morphine and also to test whether GTS can be an useful therapeutic agent for morphine toxicity, several parameters were employed, body weight, immune organ weight, B cell functions, and T cell function. Morphine impaired the development of body weight and immune organ weight such as spleen and thymus. Morphine also depressed a B-cell function, antibody production. T-cell functions studied by type IV hypersensitivity test were most markedly affected by morphine treatment. GTS restored most of morphine-induced immune suppression. GTS restored the morphine-induced decrease in spleen weight to body weight ratio in a dose dependent manner, but not the body weight decrease. Also all of the morphine-induced impairments of B cell functions and cellmediated immunity were fully recovered by GTS. These results suggest that ginseng product could be very helpful for the treatment of immune suppression occurring in morphine abusers.

      • Characterization and spatiotemporal expression of gonadotropin-releasing hormone in the Pacific abalone, <i>Haliotis discus hannai</i>

        <P><B>Abstract</B></P> <P>Gonadotropin-releasing hormone (GnRH) is a key neuropeptide regulating reproduction in humans and other vertebrates. Recently, GnRH-like cDNAs and peptides were reported in marine mollusks, implying that GnRH-mediated reproduction is an ancient neuroendocrine system that arose prior to the divergence of protostomes and deuterostomes. Here, we evaluated the reproductive control system mediated by GnRH in the Pacific abalone <I>Haliotis discus hannai</I>. We cloned a prepro-GnRH cDNA (<I>Hdh-GnRH</I>) from the pleural-pedal ganglion (PPG) in <I>H. discus hannai</I>, and analyzed its spatiotemporal gene expression pattern. The open reading frame of <I>Hdh-GnRH</I> encodes a protein of 101 amino acids, consisting of a signal peptide, a GnRH dodecapeptide, a cleavage site, and a GnRH-associated peptide. This structure and sequence are highly similar to GnRH-like peptides reported for mollusks and other invertebrates. Quantitative polymerase chain reaction demonstrated that <I>Hdh-GnRH</I> mRNA was more strongly expressed in the ganglions (PPG and cerebral ganglion [CG]) than in other tissues (gonads, gills, intestine, hemocytes, muscle, and mantle) in both sexes. In females, the expression levels of <I>Hdh-GnRH</I> mRNA in the PPG and branchial ganglion (BG) were significantly higher at the ripe and partial spent stages than at the early and late active stages. In males, <I>Hdh-GnRH</I> mRNA levels in the BG showed a significant increase in the partial spent stage. Unexpectedly, <I>Hdh-GnRH</I> levels in the CG were not significantly different among the examined stages in both sexes. These results suggest that <I>Hdh-GnRH</I> mRNA expression profiles in the BG and possibly the PPG are tightly correlated with abalone reproductive activities.</P>

        Enhancement of the Immune Responses of Mice to Bacillus anthracis Protective Antigen by CIA07 Combined with Alum

        Anthrax is an acute zoonotic disease caused by infection with Bacillus anthracis. B. anthracis spores are highly resistant to environmental degradation and are used as a biological weapon. In this study, we investigated the adjuvant activity of CIA07 to anthrax protective antigen (PA). A/J mice were immunized intraperitoneally once, or twice with a 4-week interval, with recombinant PA alone or combined with alum, CpG1826, or CIA07 as adjuvant, and serum anti-PA IgG antibody responses were measured 4 weeks after each immunization. All three adjuvants significantly enhanced anti-PA IgG antibody titer 4 weeks after the priming and boosting immunizations, and alum gave the highest titer. In order to evaluate the adjuvant activity of CIA07 in the presence of alum, Balb/c mice were immunized 3 times at 1-week intervals with PA in combination with alum, CIA07 or alum plus CIA07, and the immune responses were assessed 2 weeks after the third immunization. The serum anti-PA IgG antibody titer of the CIA07-treated group was l4-fold higher than the group given PA alone, and the coadministration of CIA07 with alum further increased the titer 3.5-fold (P < 0.05). The toxin neutralizing activity of the sera from the mice given the combination of CIA07 and alum was 109-times higher than the animals given PA alone. The mice given CIA07 plus alum also showed a marked increase in the number of IFN-$\gamma$-, IL-2-, and IL-4-producing $CD4^+$ T cells among their splenocytes. These data suggest the potential of CIA07 in combination with alum as an adjuvant for the development of a potent anthrax vaccine.

        Generation and Characterization of Cell-Permeable Green Fluorescent Protein Mediated by the Basic Domain of Human Immunodeficiency Virus Type 1 Tat

        The human immunodeficiency virus type 1 (HIV-1) Tat is one of the viral gene products essential for HIV replication. The exogenous Tat protein is transduced through the plasma membrane and then accumulated in a cell. The basic domain of the Tat protein, which is rich in arginine and lysine residues and called the protein transduction domain (PTD), has been identified to be responsible for this transduction activity. To better understand the nature of the transduction mediated by this highly basic domain of HIV-1 Tat, the Green Fluorescent Protein (GFP) was expressed and purified as a fusion protein with a peptide derived from the HIV-1 Tat basic domain in Escherichia coli. The transduction of Tat-GFP into mammalian cells was then determined by a Western blot analysis and fluorescence microscopy. The cells treated with Tat-GFP exhibited dose-and time-dependent increases in their intracellular level of the protein. The effective transduction of denatured Tat-GFP into both the nucleus and the cytoplasm of mammalian cells was also demonstrated, thereby indicating that the unfolding of the transduced protein is required for efficient transduction. Accordingly, the availability of recombinant Tat-GFP can facilitate the simple and specific identification of the protein transduction mediated by the HIV-1 Tat basic domain in living cells either by fluorescence microscopy or by a fluorescence-activated cell sorter analysis.
