인슐린 비의존성 당뇨병에 의한 말기 신부전증 환자에서 iPTH치 및 골밀도에 관한 연구

The patients with diabetic ESRD have been reported to be suffered from more serious morbidity and mortality than other ESRD patients. In order to define if there are certain differences in the clinical parematers including bone metabolism, such as bone mineral density (BMD), serum iPTH level and Tc-99m MDP whole body bone scan between diabetic and nondiabetic ESRD, we evaluated 52 patients with ESRD (20 diabetic ESRD and 32 non-diabetic ESRD) beginning first hemodialysis or peritoneal dialysis at the Department of Internal Medicine in Chungnam National University Hospital, from January to December, 1998. The results were as follows : 1. Twenty patients with diabetic ESRD have been suffered from non-insulin dependent diabetes mellius and diabetic nephropathy. The causes of ESRD of 32 non-diabetic ESRD patients were hypertension (56%), chronic glomerular nephritis (26%), chronic interstitial nephritis (3%), and others (15%), respectively. 2. Serum creatinine, albumin, and phosphorus levels of diabetic ESRD patients were significantly lower than those of non-diabetic ESRD patients(all p<0.05), and creatinine clearance was also significantly higher in diabetic ERSD patients than non-diabetic ESRD patients (p<0.05) at the beginning of renal replacement therapy. 3. There was no significant difference in BMD, serum calcium, iPTH, alkaline phosphatase, 24 hours urine protein excretion, hemoglobin, and Tc-99m MDP whole body bone scan score between diabetic and non-diabetic ESRD patients (all p>0.05). 4. On the multivariable linear regression analysis, there was no statistically significant influence of patient's age, presence or absence of diabetes, iPTH, calcium, phosphorus, ALP, albumin, creatinine and ferritin levels on t-score of bone densitometry. In conclusion, we speculate that diabetic ESRD patients begin renal replacement therapy earlier than other ESRD patients although there was no significant difference in bone metabolism parameters, such as iPTH and bone mineral density.
국내 토목산업의 설계-시공 균형발전 방안 도출에 관한 연구

최근 해외건설시장에서는 산업설비 분야를 중심으로 기획, 설계, 구매조달, 시공을 통합하는 EPC(Engineering, Procurement, & Construction) 형태의 발주물량이 증가하고 있으며, 토목산업 분야에서도 점차 Design-Build 발주사례가 증가하고 있다. 이러한 발주환경의 변화에 따라 프로젝트종합관리, 기본설계 등 건설엔지니어링의 중요성이 대두되고 있으나, 아직까지 국내 건설산업은 시공 위주의 산업구조에서 벗어나지 못하고 있다. 따라서 본 연구는 국내 토목산업의 엔지니어링 역량을 제고하고, 장기적으로는 설계-시공 균형발전을 위한 산업 차원의 발전방안을 제시하는데 목적을 두고 진행되었다. 이를 위해 우선 국내외 건설산업 현황을 분석하였으며, 국내 건설업체들의 인식수준 및 니즈 파악을 위한 설문조사와 발전방안의 실효성 검토를 위한 전문가 심층면담을 순차적으로 진행하였다. 본 연구에서 도출한 국내 토목산업의 설계-시공 균형발전 방안은 1) 토목설계업 관리체계 효율화, 2) 토목설계업 관련 제도 선진화, 3) 고부가가치 우수인력 육성, 4) 엔지니어링조합 및 컨소시엄 구성, 5) 해외건설 협력기구 조직 및 운영으로 구분하여 제시되었다. In the recent global construction environment, EPC contracts that integrate planning, design, procurement, and construction have been widely used for industrial facilities. Similarly, as the importance of the engineering capability such as project management consulting(PMC) and front end engineering design(FEED) has been increased, design-build contracts also have been gradually adopted for infrastructure projects. Whereas, the Korean infrastructure industry has fragmentary features which mainly emphasize on the construction sector. Therefore, this study aims at developing strategies for enhancing the engineering capability and for balancing the competitiveness between engineering and construction sectors in the Korean infrastructure industry. To this end, the authors first carried out market analysis for both global and domestic market. Then, a structured survey and expert in-depth interviews were sequentially performed in order to reflect practitioners' perspectives and needs for institutional strategies. As a result, five strategies drawn from this study are as follows: 1) rearranging the government's roles for supporting the engineering sector; 2) establishing institutional bases being compatible with global standards; 3) fostering value-added workforce for undertaking complex tasks; 4) constructing an consortium between an engineering association and general contractors; and 5) building a nationwide collaborative networks for overseas market expansion.
방사성 옥도 ( 131Ⅰ ) 가 Guinea Pig 의 난소 , 자궁 및 성주기에 미치는 영향

The effects of radioiodine (^(131)I) administration on the histology of ovary and uterus and sexual cycle of the mature guinea pigs were observed. A single dose of 4.5 mC radioiodine per ㎏ body weight was injected subcutaneously. On the 1, 7, 14, 28, 42 and 56th day after radioiodine treatment, five treated and 5 control guinea pigs were sacrificed and their ovary and uterus were examined histologically. The variations in sexual cycle were examined by vaginal smear method through the experimental period. The results obtained were as follows: 1. No histological changes in the ovary were observed on the first day after treatment. On the 7th day after treatment, however, some atretic follicles were appeared and granulosa cells were changed with pyknosis and karyorrhexis. From the 14th day onwards, primordial follicles began to be damaged with a tendency to be severe gradually. After 42 days atretic follicles .and atrophy or hypoplasia of interstitial cells wore observed. On the 56th day after treatment, these ovaries began to be restored to the normal state. 2. The atrophy of myofibrils and multiplications of interstitial connective tissue of tunica muscularis were observed in the uterus from after treatment. 3. The change in sexual cycle of the treated guinea pig was apparent. The diestrus period extended in some guinea pigs on the 10th day after treatment. From the 21st day onwards, the distinct extension of diestrus period was observed.

A Case of Reversible Parkinsonism Induced by Valproate in a Patient with Bipolar Disorder Kang-Uk Le, MD, PhD The author reports a case of reversible parkinsonism induced by valproate. The patient (61 years old male) with bipolar disorder developed typical parkinsonism from the initial period of valproate administration (dose:1500 mg, serum level:108.1 ug/mL)and disappeared rapidly after discontinuation of valproate. In psychiatric conditions such as bipolar disorder or other psychotic illnes, the possibility that the extrapyramidal sympoms be induced by valproate should be taken into careful consideration. In that case, carbamazepine can be recomended as a substitute medication. (Korean J Psychopharmacol 2004 ;15(1):107-111)
